目的分析尖吻蝮蛇血凝酶(haemocoagulase agkistrodon,HCA,商品名苏灵)在双侧慢性鼻-鼻窦炎伴鼻息肉(chronicrhinosi nusitis with nasalpolyps,CRSWNP)患者FESS手术中的止血效果和安全性。方法采取双盲和随机对照的方法,将50例手术患... 目的分析尖吻蝮蛇血凝酶(haemocoagulase agkistrodon,HCA,商品名苏灵)在双侧慢性鼻-鼻窦炎伴鼻息肉(chronicrhinosi nusitis with nasalpolyps,CRSWNP)患者FESS手术中的止血效果和安全性。方法采取双盲和随机对照的方法,将50例手术患者随机分为试验组A组(25例),术前半小时静注尖吻蝮蛇血凝酶2 U;对照组B组(25例),术前半小时静注生理盐水作为安慰剂模拟尖吻蝮蛇血凝酶。记录术中出血量、单位时间出血量,并于事前及术后第1天观察对比凝血功能、血常规和肝肾功能等安全性指标。结果实验组出血量及单位时间出血量均明显少于对照组(P<0.05);在凝血功能及肝肾功能等方面,两组结果差异无统计学意义(P>0.05);实验组、对照组血常规的变化均存在差异,但应用HCA的差异并不以血常规形式体现。结论尖吻蝮蛇凝血酶对慢性鼻窦炎手术切口有较好的止血效果和安全性。 展开更多

Inflammation and coagulation constantly influence each other and are constantly in balance.Emerging evidence supports this statement in acute inflammatory diseases,such as sepsis,but it also seems to be very important in chronic inflammatory settings,such as inflammatory bowel disease(IBD).Patients with Crohn's disease and ulcerative colitis have an increased risk of thromboembolic events,and several abnormalities concerning coagulation components occur in the endothelial cells of intestinal vessels,where most severe inflammatory abnormalities occur.The aims of this review are to update and classify the type of coagulation system abnormalities in IBD,and analyze the strict and delicate balance between coagulation and inflammation at the mucosal level.Recent studies on possible therapeutic applications arising from investigations on coagulation abnormalities associated with IBD pathogenesis will also be briefly presented and critically reviewed. 展开更多

Thermal performance is the most important factor in the development of a borehole heat exchanger utilizing geothermal energy.The thermal performance is affected by many different design parameters and different operating conditions such as bleeding.This eventually determines the operation and cost efficiency of the borehole heat exchanger system.The thermal performance of an open standing column well (SCW) type geothermal heat exchanger was assessed under the influence of bleeding.For this,a thermal response test rig was established with line-source theory.The test rig also had a bleeding function by releasing fluid while taking additional underground water through the heat exchanger.The thermal response test was performed with an additional constant input heat source.Effective thermal conductivity and thermal resistance were obtained from the measured data.From the measurement,the effective thermal conductivity is found to have 1.47 times higher value when bleeding is applied.The thermal resistance also increases by 1.58 times compared to a non-bleeding case.This trend indicates enhanced heat transfer in the SCW type heat exchanger with a bleeding function.Bleeding,therefore,could be an effective method of achieving a high heat transfer rate in the SCW type heat exchanger with sufficient underground water supply. 展开更多

Methods: The core region of fused hirudin was the C-terminal tail of hirudin(hirudin_ 53-64),which could bind to the anion binding exosite (ABE) of thrombin.Arg-Pro-Pro-Gly-Phe(RPPGF) amino acid sequence,a metabolite of bradykinin,was added to the N-terminus of hirudin_ 53-64.It bound to the active site of thrombin.Additionally,Arg-Gly-Asp(RGD)amino acid sequence,an inibitor of glycoprotein Ⅱb/Ⅲa( GP Ⅱb/Ⅲa) receptor,was linked to C-terminus of hirudin_ 53-64.This 26-animo acid-fused hirudin peptide was artificially synthesized,purified and analysed. Results: Fused hirudin peptide significantly lengthened the activated partial thromboplastin time(APTT),thrombin time(TT)and prothrombin time(PT) and inhibited the amidolytic activity of thrombin.The ADP-induced platelet aggregation was markedly inhibited by fused hirudin peptide. Conclusion: Fused hirudin peptide has activity of antithrombin as well as antiplatelet.Therefore bifunctional anticoagulation peptide has capacity to target various components of haemostatic process and may become more powerful antithrombosis agent. 展开更多

为了进一步提高伤口敷料的止血性能,文中在生物相容性良好的壳聚糖溶液中引入含有多种生长因子的人源性富血小板血浆(Humanplatelet-richplasma,hPRP),并加入不同体积比例(1∶1、1∶3、3∶1、1∶0)的丝素蛋白溶液以提高材料的多孔性与... 为了进一步提高伤口敷料的止血性能,文中在生物相容性良好的壳聚糖溶液中引入含有多种生长因子的人源性富血小板血浆(Humanplatelet-richplasma,hPRP),并加入不同体积比例(1∶1、1∶3、3∶1、1∶0)的丝素蛋白溶液以提高材料的多孔性与止血性,通过冷冻干燥法制备不同配比的hPRP-壳聚糖/丝素蛋白敷料,并将纯壳聚糖敷料作为对照组,研究hPRP和丝素蛋白对敷料的止血性能的影响以及丝素蛋白对PRP中生长因子控制释放的影响。结果表明,在壳聚糖敷料中引入hPRP对敷料的止血性有所提高,但对敷料的多孔结构及吸水率无明显改善,若在hPRP-壳聚糖溶液中按照体积比为1∶1的比例加入丝素蛋白溶液,会得到具有较为均匀的多孔结构的敷料,敷料的孔隙率与吸水率分别可达到86.83%±3.84%与1 474%±114%,且该比例的敷料在快速止血性能上表现优异。此外,加入丝素蛋白与壳聚糖比例为1∶1的PRP敷料能有效减少PRP中生长因子在初始阶段的爆裂释放。因此,含hPRP的壳聚糖/丝素蛋白复合敷料有望成为一种能快速止血且能促进伤口愈合的新型伤口敷料。 展开更多

目前,大量输血(massive transfusion,MT)仍是失血性休克病人在尚未明确止血前的主要救治手段。既往的MT要求快速输注红细胞和晶体液以维持机体有效循环血容量和组织氧供,而忽视了凝血因子和血小板的补充,导致稀释性凝血病和发生"... 目前,大量输血(massive transfusion,MT)仍是失血性休克病人在尚未明确止血前的主要救治手段。既往的MT要求快速输注红细胞和晶体液以维持机体有效循环血容量和组织氧供,而忽视了凝血因子和血小板的补充,导致稀释性凝血病和发生"出血的恶性循环(bloody viciouscycle)",使失血性休克的病死率仍处于较高水平。随着现代医学的不断进步,现在MT强调"止血性复苏(hemostaticresuscitation)"概念,增强补充凝血因子,建议以1:1:1的比例输注压积红细胞、新鲜冰冻血浆及血小板,并且在未明确止血前减少等渗晶体液输注和实施允许性低血压复苏。一旦出血得到控制,应立即减少或停止输血,以降低后期输血相关并发症的发生率。 展开更多

In the present study,we aimed to observe the effects of ethanol extract of Oxalis corniculata L.on bleeding time(BT)and coagulation time(CT)and determine its acute toxicity in mice.Firstly,the ethanol extract of O.corniculata was administered to 40 mice,which were randomly divided into the normal control group,Yunnan Baiyao control group,low-dose group,and high-dose group.The low-dose(0.6 g/kg)and high-dose(1.2 g/kg)groups received ethanol extract of O.corniculata by gavage,and the BT and CT of the mice were measured by the slide method and tail clipping method,respectively.Secondly,the median lethal dose method was used with 50 mice to observe the physiological state,poisoning reaction,and death of the mice after intragastric administration.Finally,on the 14th day of the experiment,a necropsy was performed to observe any abnormality of the organs.In conclusion,in the coagulation and hemostasis tests,there were no statistical differences between the groups(P>0.05),while there was a significant dose-response relationship,and the BT and CT were significantly shorter than those of the negative control group,which were comparable to the Yunnan Baiyao control group.Moreover,in the acute toxicity test,the median lethal dose of ethanol extract of O.corniculata was 6.0291 g/kg,with a 95%confidence interval of 5.3065–6.7829 mg/kg. 展开更多
