Although the treatment of achalasia has been traditionally based on a surgical approach, especially with the introduction of laparoscopic techniques, there is still some space for a medical approach. Since they have not taken time factor into eonsideration, these hypotheses can not give a satisfactory explanation of the oceurrence and development of the outburst. A creep mathematital model of gas-contaming coal,which can better expdri the phenomenon of coal and methane outbursts,has been set up through creep tests and analyses under the condition of triaxiai compression.The tests have proved that there isn’ t essential difference between the rheological properties of outburst prone coal and non-outburst prone coal. Outburst can happen in any kind of coal if the rheologicai conditions exist. The creep mathematital model provides a soild foundation for establlshing a comprebcnsive criterion of coal and gas outburst.展开更多
AIM: To investigate whether the degree of rectal distension could define the rectum functions as a conduit METHODS: Response of the rectal and anal pressure to 2 types of rectal balloon distension, rapid voluminous ...AIM: To investigate whether the degree of rectal distension could define the rectum functions as a conduit METHODS: Response of the rectal and anal pressure to 2 types of rectal balloon distension, rapid voluminous and slow gradual distention, was recorded in 21 healthy volunteers (12 men, 9 women, age 41.7 ± 10.6 years). The test was repeated with sphincteric squeeze on urgent sensation. RESULTS: Rapid voluminous rectal distension resulted in a significant rectal pressure increase (P 〈 0.001), an anal pressure decline (P 〈 0.05) and balloon expulsion. The subjects felt urgent sensation but did not feel the 1^st rectal sensation. On urgent sensation, anal squeeze caused a significant rectal pressure decrease (P 〈 0.001) and urgency disappearance. Slow incremental rectal filling drew a rectometrogram with a "tone" limb representing a gradual rectal pressure increase during rectal filling, and an "evacuation limb" representing a sharp pressure increase during balloon expulsion. The curve recorded both the ist rectal sensation and the urgent sensation. CONCLUSION: The rectum has apparently two functions: transportation (conduit) and storage, both depending on the degree of rectal filling. If the fecal material received by the rectum is small, it is stored in the rectum until a big volume is reached that can affect a degree of rectal distension sufficient to initiate the defecation reflex. Large volume rectal distension evokes directly the rectoanal inhibitory reflex with a resulting defecation.展开更多

Left paraduodenal hernia resulting from abnormal rotation of the midgut during embryonic development is the most common form of congenital internal hernia. We report our experience in the diagnosis and management of a young male with left paraduodenal hernia presenting as recurrent intestinal obstruction. Correct preoperative diagnosis of left paraduodenal hernia had been difficult due to non-specific clinical presentations, but the advent of modern imaging technology makes early and correct diagnosis possible, Due to the risk of obstruction and strangulation, surgical treatment is indicated; however, timely intervention increases the likelihood of a favorable outcome.展开更多

Regarding the clinical application of acupuncture therapy, in the acute stage of the...In the present paper, the authors review recent progresses of acupuncture treatment of cerebral hemorrhage and theoretical research. Regarding the clinical application of acupuncture therapy, in the acute stage of the disease, many doctors adopt body acupuncture and scalp-acupuncture, fewer doctors applied eye acupuncture; while in the chronic stage of the disease, many medical workers employ body acupuncture, scalp-acupuncture, combined therapies of acupuncture, functional exercise, massage, acupoint injection, etc.. Concerning studies on the mechanisms of acupuncture in the treatment of cerebral hemorrhage, abundant experience evidence show that acupuncture can raise the activity of the plasma fibrinolytic system to promote the absorption of blood clots of the foci; improve microcirculation; reduce cerebral tissue edema; regulate some chemical substances’ levels to lessen harmful effects of oxygen free radicals; and enhance the patient’s immune function. However, some problems still exist in clinical researches at present and affect the reasonable evaluation on the clinical therapeutic effect of acupuncture.展开更多
