Traditionally, PC wa...This article is to offer a concise review on the use of cytoreductive surgery (CRS) plus intraperitoneal hyperthermic chemotherapy (IPHC) for the treatment of peritoneal carcinomatosis (PC). Traditionally, PC was treated with systemic chemotherapy alone with very poor response and a median survival of less than 6 too. With the establishment of several phase Ⅱ studies, a new trend has been developed toward the use of CRS plus IPHC as a standard method for treating selected patients with PC, in whom sufficient cytoreduction could be achieved. In spite of the need for more high quality phase Ⅲ studies, there is now a consensus among many surgical oncology experts throughout the world about the use of this new treatment strategy as standard care for colorectal cancer patients with PC. AIM: To evaluate the results of the treatment of simple liver cysts (solitary and multiple) and polycystic liver disease (PLD) using percutaneous sclerotherapy and/or surgical procedures in a single tertiary referral ...AIM: To evaluate the results of the treatment of simple liver cysts (solitary and multiple) and polycystic liver disease (PLD) using percutaneous sclerotherapy and/or surgical procedures in a single tertiary referral centre. METHODS: Retrospective analysis of 54 patients referred for evaluation and possible treatment of simple liver cysts (solitary and multiple) and PLD, from January 1997 to July 2006. RESULTS: Simple liver cysts were treated in 41 pts (76/) with a mean size of 12.6 cm. The most common reason for referral was abdominal pain or discomfort (85/). Percutaneous sclerotherapy was performed as initial treatment in 30 pts, showing cyst recurrence in 6 pts (20/). Surgical treatment was initially performed in 11 pts with cyst recurrence in 3 pts (27/). PLD was treated in 13 pts (24/) with a mean size of the dominant cyst of 13 cm. Percutaneous sclerotherapy for PLD was performed in 9 pts with recurrence in 7 pts (77.8/). Surgical treatment for PLD was undertaken in 4 pts (30.8/) with recurrence in all. Eventually, 2 pts with PLD in the presence of polycystic kidney disease underwent liver-and kidney transplantation because of deterioration of liver and kidney function. CONCLUSION: The majority of patients with simple liver cysts and PLD are referred for progressive abdominal pain. As initial treatment, percutaneous sclerotherapy is appropriate. Surgical deroofing is indicated in caseof cyst recurrence after percutaneous sclerotherapy. However, the results of percutaneous sclerotherapy and surgical treatment for PLD are disappointing. Partial liver resection is indicated when there is suspicion of a pre-malignant lesion.展开更多

Methods: Aging mouse model was established by subcutaneous injection of D-galactose, and treated by auricular acupuncture, auricular acupuncture plus moxibustion, and auricular acupuncture plus Chinese herbs respectively for 6 weeks and then serum interleukin-2 (IL-2) and IL-6 contents and splenic lymphocyte transformation rate were compared among the groups. Results: Compared with the young group, the serum IL-2 level and the splenic lymphocyte transformation rate significantly decreased and the IL-6 level significantly increased in the aging mouse. After treatment by auricular acupuncture, auricular acupuncture plus Chinese herbs, especially auricular acupuncture plus moxibustion, the serum IL-2 level and the splenic lymphocyte transformation rate significantly increased, and the IL-6 level significantly decreased as compared with the model group. Conclusions: Combined application of auricular acupuncture, moxibustion and Chinese herbs can improve the decline or dysfunction of immunological function in the aging organism to a certain extent.展开更多

This is accomplished by reviewing the impact tha...This paper highlights the role of automation technologies for improving the safety, productivity, and environmental sustainability of underground coal mining processes. This is accomplished by reviewing the impact that the introduction of automation technology has made through the longwall shearer automation research program of Longwall Automation Steering Committee(LASC). This result has been achieved through close integration of sensing, processing, and control technologies into the longwall mining process. Key to the success of the automation solution has been the development of new sensing methods to accurately measure the location of longwall equipment and the spatial configuration of coal seam geology. The relevance of system interoperability and open communications standards for facilitating effective automation is also discussed. Importantly, the insights gained through the longwall automation development process are now leading to new technology transfer activity to benefit other underground mining processes.展开更多

The proposed algorithm is an improved Kalman filter— multiple model adaptive estimation unscented Kalman filter(MMAE-UKF) rather than conventional Kalman filter methods,like the extended Kalman filter(EKF) and the unscented Kalman filter(UKF). UKF is used as a subfilter to obtain the system state estimate in the MMAE method. Single model filter has poor adaptability with uncertain or unknown system parameters,which the improved filtering method can overcome. Meanwhile,this algorithm is used for integrated navigation system of strapdown inertial navigation system(SINS) and celestial navigation system(CNS) by a ballistic missile's motion. The simulation results indicate that the proposed filtering algorithm has better navigation precision, can achieve optimal estimation of system state, and can be more flexible at the cost of increased computational burden.展开更多

METHODS: A retrospective review of the medical records of 55 patients diagnosed with cholangio- carcinoma at the American University of Beirut between 1990 and 2005 was conducted. Univariate and multivariate analyses were performed to determine the impact of surgery, chemotherapy, body mass index, bilirubin level and other factors on survival. RJ^SULTS: The median survival of all patients was 8.57 mo (0.03-105.2). Univariate analysis showed that low bilirubin level (〈 10 mg/dL), radical surgery and chemotherapy administration were significantly associated with better survival (P = 0.012, 0.038 and 0.038, respectively), in subgroup analysis on patients who had no surgery, chemotherapy administration prolonged median survival significantly (17.0 mo vs 3.5 mo, P = 0.001). Multivariate analysis identified only low bilirubin level 〈 10 mg/dL and chemotherapy administration as independent predictors associated with better survival (P 〈 0.05). CONCLUSION: Our data show that palliative and postoperative chemotherapy as well as a bilirubin level 〈 10 mg/dL are independent predictors of a significant increase in survival in patients with cholangiocarcinoma.展开更多

The main purpose of the present study is to provide a preliminary idea that how the nonhomogeneity influences the stability of slopes under four different water drawdown regimes. Two typical categories of nonhomogeneity, identified as layered profile and strength increasing with depth profile, are included in the paper, and a nonhomogeneity coefficient is defined to quantify the degree of soil properties nonhomogeneity. With a modified discretization technique, the safety factors of nonhomogeneous slopes are calculated. On this basis, the variation of safety factor with the nonhomogeneity coefficient of friction angle and the water table level are investigated. In the present example, safety factor correlates linearly with friction angle nonhomogeneity coefficient from a whole view and the influences of the water table level on safety factor is basically similar with that in homogeneous condition.展开更多
AIM: To assess the efficacy of metoclopramide (Met) for prevention of prolonged post-operative ileus in advanced gastric cancer patients undergoing D2 gastrectomy and intra-peritoneal chemotherapy (IPC). METHODS...AIM: To assess the efficacy of metoclopramide (Met) for prevention of prolonged post-operative ileus in advanced gastric cancer patients undergoing D2 gastrectomy and intra-peritoneal chemotherapy (IPC). METHODS: Thirty-two advanced gastric cancer patients undergoing D2 gastrectomy and IPC were allocated to two groups. Sixteen patients received Met immediately after operation (group A), and 16 did not (group B). Another 16 patients who underwent D2 gastrectomy without IPC were enrolled as the control group (group C). All patients had received epidural pain control. The primary endpoints were time to first post-operative flatus and time until oral feeding with a soft diet without discomfort. Secondary endpoints were early complications during hospitalization. RESULTS: Gender, the type of resection, operating time, blood loss, tumor status and amount of narcotics were comparable in the three groups. However, the group C patients were older than those in groups A and B (67.5±17.7 vs 56.8±13.2, 57.5±11.7 years, P= 0.048). First bowel flatus occurred after 4.35±0.93 d in group A, 4.94±1.37 d in group B, and 4.71±1.22 d in group C (P〉0.05). Oral feeding of a sore diet was tolerated 7.21±1.92 d after operation in group A, 10.15±2.17 d in group B, and 7.53±1.35 d in group C (groups A and C vsgroup B, P〈0.05). There was no significant difference in respect to the first flatus among the three groups. However, the time of tolerating oral intake with soft food in groups A and C patients was significantlyshorter than that in group B patients. Levels of C-reactive protein (CRP) were significantly lower in group C and there was a more prominent and prolonged response in CRP level in patients undergoing IPC. The incidence of post-operative complications was similar in the three groups except for prolonged post-operative ileus. There was no increased risk of anastomotic leakage in patients receiving Met. CONCLUSION: The results suggest that a combination of intravenous Met and epidural pain control may be required to achieve a considerable decrease in time to resumption of oral soft diet in advanced gastric cancer patients who underwent gastrectomy and IPC. Furthermore, the administration of Met did not increase anastomotic leakage. Met has a role in the prevention of prolonged post-operative ileus.展开更多

In this paper, a model predictive control based energy efficient control strategy which aims at maximizing the system efficiency is proposed. Firstly, according to the mass and energy conservation law, an analysis on the nonlinear relationship between superheat and cooling load is carried out, which can produce the maximal effect on the system performance. Then a model predictive control (MPC) based controller is developed for tracking the calculated setting curve of superheat degree and pressure difference based on model identified from data which can be obtained from an experimental rig. The proposed control strategy maximizes the coefficient of performance (COP) which depends on operating conditions, in the meantime, it meets the changing demands of cooling capacity. The effectiveness of the control performance is validated on the experimental rig. X-ray diffraction and scanning electron microscopy revealed that the films mainly consisted of anatase phase with a porous surface morphology. The films have an excellent photocatalytic effect for degradation of methylene blue and decomposition of water under visible light illumination. This arises from the formation of Cr3+/Cr4+ and oxygen vacancy energy levels owing to Cr doping. The former reduces the electron-hole recombination chance, while the latter generates a new gap between the conduction band (CB) and valence band (VB) of TiO2, which lowers the photo energy of the excited electron in the VB to the oxygen vacancy states. The mechanisms for film synthesis during the MAO process are also presented.展开更多

On this basis, this paper uses the graph partitioning theory to build the fog computing's load balancing algorithm based on dynamic graph partitioning. AIM:To investigate if contrast-enhanced ultrasonography(CE-US) is useful for determining treatment efficacy and outcome in the early stages of pancreatic cancer chemotherapy by assessing changes in intratumor hemodyna...AIM:To investigate if contrast-enhanced ultrasonography(CE-US) is useful for determining treatment efficacy and outcome in the early stages of pancreatic cancer chemotherapy by assessing changes in intratumor hemodynamics using CE-US with a contrast agent.METHODS:The subjects were 34 patients with unresectable advanced pancreatic cancer treated by chemotherapy.CE-US was assessed after every treatment(course) completion under the same conditions,and patients were divided into two groups according to the intratumor enhancement pattern:Vascular rich(R) group and vascular poor(P) group.RESULTS:After the second course of treatment,R group in intratumor hemodynamics had 18 patients,and P group had 16 patients.The reduction rates of serum CA19-9 level after chemotherapy which decreased to half or less of the baseline level were 2/15(0.1%) in P group,but 11/16(69%) in R group(P = 0.006).When the mean number of courses of chemotherapy and outcome were compared,P group had a mean number of courses of 4.9(R group,10.2) and mean survival time(MST) of 246 d(R group,402 d),showing that outcome was significantly better in R group(P=0.006).CONCLUSION:CE-US revealed that the change in intratumor blood flow correlated with both serum CA19-9 level and outcome.Patients with serum CA19-9 that decreased to less than half the baseline level,and patients with an abundant intratumor blood flow,had a significantly better outcome.Thus,CE-US is potentially useful for evaluating treatment efficacy and outcome in the early stages of pancreatic cancer chemotherapy.展开更多
AIM: To develop a technique of combined endoscopic sclerotherapy and ligation (ESL) in which both techniques of endoscopic sclerotherapy (ES) and endoscopic variceal ligation (EVL) can be optimally used.METHODS: ESL w...AIM: To develop a technique of combined endoscopic sclerotherapy and ligation (ESL) in which both techniques of endoscopic sclerotherapy (ES) and endoscopic variceal ligation (EVL) can be optimally used.METHODS: ESL was performed in 10 patients (age 46.4±7.9;9 males, 1 female) with cirrhosis of liver using sclerotherapy needle and Speedband, Superview multiple band ligater (Boston Scientific, Microvasive, Watertown, MA). A single band was placed 5-10 cm proximal to the gastro-esophageal junction over each varix from proximal to distal margin,followed by intravariceal injection of 1.5 % ethoxysclerol (4 ml each) 2 to 3 cm proximal to the gastroesophageal junction on the ligated varices distal to deployed band. EVL was then performed at the injection site. Similarly other varices were also injected and ligated from distal to proximally. In the subsequent sessions, ES alone was performed to sclerose small varices at the gastroesophageal junction.RESULTS: ESL was successfully performed in all patients.A median of 3 (ESL 1, ES 2) sessions (ranged 1-4) were required to eradicate the varices in 9 (90 %) of 10 patients.Recurrence of varices without bleed was seen in 1 patient during a mean follow-up of 10.3 months (ranged 6-15).Two patients died of liver failure. None died of variceal bleeding. None of the patients had procedure related complications.CONCLUSION: ESL may be useful in the fast eradication of esophageal varices. However, randomised controlled trials are required to find out its relative efficacy and impact on variceal recurrence in comparison to ES or EVL.展开更多

One of the main characteristics of SGA is its searching space can be limited in feasible regions rather than infeasible regions. The superiority of SGA to other GAs lies in that some supervised search rules in which the information comes from the problems are incorporated into SGA. The simulation results show that SGA improves the ability of searching an optimum solution and accelerates the convergent process up to 20 times.展开更多

On the basis, the influencing factors of support stability were analyzed and the technical measures of controlling support and surrounding rock stability were put forward. Then the loose particles simulation experiment was conducted to analyze the impacts of caving directions and methods on the top-coal recovery in large dip angle fully-mechanized caving face. Finally, the ‘‘upward sequence and double-openings doublerounds" caving technology was determined. CONCLUSION: The majority of patients with simple liver cysts and PLD are referred for progressive abdominal pain. As initial treatment, percutaneous sclerotherapy is appropriate. Surgical deroofing is indicated in caseof cyst recurrence after percutaneous sclerotherapy. However, the results of percutaneous sclerotherapy and surgical treatment for PLD are disappointing. Partial liver resection is indicated when there is suspicion of a pre-malignant lesion.
文摘Objective:To observe the regulative effect of auricular acupuncture, moxibustion and Chinese herbs on immunologic function in the D-galactose-induced aging mouse and to probe the feasibility and possible mechanisms for delaying aging, so as to provide experimental basis for acupuncture, moxibustion and Chinese herbs for delaying aging. Methods: Aging mouse model was established by subcutaneous injection of D-galactose, and treated by auricular acupuncture, auricular acupuncture plus moxibustion, and auricular acupuncture plus Chinese herbs respectively for 6 weeks and then serum interleukin-2 (IL-2) and IL-6 contents and splenic lymphocyte transformation rate were compared among the groups. Results: Compared with the young group, the serum IL-2 level and the splenic lymphocyte transformation rate significantly decreased and the IL-6 level significantly increased in the aging mouse. After treatment by auricular acupuncture, auricular acupuncture plus Chinese herbs, especially auricular acupuncture plus moxibustion, the serum IL-2 level and the splenic lymphocyte transformation rate significantly increased, and the IL-6 level significantly decreased as compared with the model group. Conclusions: Combined application of auricular acupuncture, moxibustion and Chinese herbs can improve the decline or dysfunction of immunological function in the aging organism to a certain extent.
文摘This paper highlights the role of automation technologies for improving the safety, productivity, and environmental sustainability of underground coal mining processes. This is accomplished by reviewing the impact that the introduction of automation technology has made through the longwall shearer automation research program of Longwall Automation Steering Committee(LASC). This result has been achieved through close integration of sensing, processing, and control technologies into the longwall mining process. Key to the success of the automation solution has been the development of new sensing methods to accurately measure the location of longwall equipment and the spatial configuration of coal seam geology. The relevance of system interoperability and open communications standards for facilitating effective automation is also discussed. Importantly, the insights gained through the longwall automation development process are now leading to new technology transfer activity to benefit other underground mining processes.

文摘This paper explores multiple model adaptive estimation(MMAE) method, and with it, proposes a novel filtering algorithm. The proposed algorithm is an improved Kalman filter— multiple model adaptive estimation unscented Kalman filter(MMAE-UKF) rather than conventional Kalman filter methods,like the extended Kalman filter(EKF) and the unscented Kalman filter(UKF). UKF is used as a subfilter to obtain the system state estimate in the MMAE method. Single model filter has poor adaptability with uncertain or unknown system parameters,which the improved filtering method can overcome. Meanwhile,this algorithm is used for integrated navigation system of strapdown inertial navigation system(SINS) and celestial navigation system(CNS) by a ballistic missile's motion. The simulation results indicate that the proposed filtering algorithm has better navigation precision, can achieve optimal estimation of system state, and can be more flexible at the cost of increased computational burden.
文摘AIM: To study the factors that may affect survival of cholangiocarcinoma in Lebanon. METHODS: A retrospective review of the medical records of 55 patients diagnosed with cholangio- carcinoma at the American University of Beirut between 1990 and 2005 was conducted. Univariate and multivariate analyses were performed to determine the impact of surgery, chemotherapy, body mass index, bilirubin level and other factors on survival. RJ^SULTS: The median survival of all patients was 8.57 mo (0.03-105.2). Univariate analysis showed that low bilirubin level (〈 10 mg/dL), radical surgery and chemotherapy administration were significantly associated with better survival (P = 0.012, 0.038 and 0.038, respectively), in subgroup analysis on patients who had no surgery, chemotherapy administration prolonged median survival significantly (17.0 mo vs 3.5 mo, P = 0.001). Multivariate analysis identified only low bilirubin level 〈 10 mg/dL and chemotherapy administration as independent predictors associated with better survival (P 〈 0.05). CONCLUSION: Our data show that palliative and postoperative chemotherapy as well as a bilirubin level 〈 10 mg/dL are independent predictors of a significant increase in survival in patients with cholangiocarcinoma.

文摘Comparing with the homogeneous slope, the nonhomogeneous slope has more significance in practice. The main purpose of the present study is to provide a preliminary idea that how the nonhomogeneity influences the stability of slopes under four different water drawdown regimes. Two typical categories of nonhomogeneity, identified as layered profile and strength increasing with depth profile, are included in the paper, and a nonhomogeneity coefficient is defined to quantify the degree of soil properties nonhomogeneity. With a modified discretization technique, the safety factors of nonhomogeneous slopes are calculated. On this basis, the variation of safety factor with the nonhomogeneity coefficient of friction angle and the water table level are investigated. In the present example, safety factor correlates linearly with friction angle nonhomogeneity coefficient from a whole view and the influences of the water table level on safety factor is basically similar with that in homogeneous condition.
文摘AIM: To assess the efficacy of metoclopramide (Met) for prevention of prolonged post-operative ileus in advanced gastric cancer patients undergoing D2 gastrectomy and intra-peritoneal chemotherapy (IPC). METHODS: Thirty-two advanced gastric cancer patients undergoing D2 gastrectomy and IPC were allocated to two groups. Sixteen patients received Met immediately after operation (group A), and 16 did not (group B). Another 16 patients who underwent D2 gastrectomy without IPC were enrolled as the control group (group C). All patients had received epidural pain control. The primary endpoints were time to first post-operative flatus and time until oral feeding with a soft diet without discomfort. Secondary endpoints were early complications during hospitalization. RESULTS: Gender, the type of resection, operating time, blood loss, tumor status and amount of narcotics were comparable in the three groups. However, the group C patients were older than those in groups A and B (67.5±17.7 vs 56.8±13.2, 57.5±11.7 years, P= 0.048). First bowel flatus occurred after 4.35±0.93 d in group A, 4.94±1.37 d in group B, and 4.71±1.22 d in group C (P〉0.05). Oral feeding of a sore diet was tolerated 7.21±1.92 d after operation in group A, 10.15±2.17 d in group B, and 7.53±1.35 d in group C (groups A and C vsgroup B, P〈0.05). There was no significant difference in respect to the first flatus among the three groups. However, the time of tolerating oral intake with soft food in groups A and C patients was significantlyshorter than that in group B patients. Levels of C-reactive protein (CRP) were significantly lower in group C and there was a more prominent and prolonged response in CRP level in patients undergoing IPC. The incidence of post-operative complications was similar in the three groups except for prolonged post-operative ileus. There was no increased risk of anastomotic leakage in patients receiving Met. CONCLUSION: The results suggest that a combination of intravenous Met and epidural pain control may be required to achieve a considerable decrease in time to resumption of oral soft diet in advanced gastric cancer patients who underwent gastrectomy and IPC. Furthermore, the administration of Met did not increase anastomotic leakage. Met has a role in the prevention of prolonged post-operative ileus.

文摘Abstract--Vapor compression refrigeration cycle (VCC) system is a high dimensional coupling thermodynamic system for which the controller design is a great challenge. In this paper, a model predictive control based energy efficient control strategy which aims at maximizing the system efficiency is proposed. Firstly, according to the mass and energy conservation law, an analysis on the nonlinear relationship between superheat and cooling load is carried out, which can produce the maximal effect on the system performance. Then a model predictive control (MPC) based controller is developed for tracking the calculated setting curve of superheat degree and pressure difference based on model identified from data which can be obtained from an experimental rig. The proposed control strategy maximizes the coefficient of performance (COP) which depends on operating conditions, in the meantime, it meets the changing demands of cooling capacity. The effectiveness of the control performance is validated on the experimental rig. Index Terms--Cooling load, model predictive control (MPC), superheat, vapor compression refrigeration cycle (VCC).
文摘A series of Cr doped TiO2 films were prepared by micro arc oxidation (MAO) using an electrolyte of Na3PO4+K2Cr2O7. X-ray diffraction and scanning electron microscopy revealed that the films mainly consisted of anatase phase with a porous surface morphology. The films have an excellent photocatalytic effect for degradation of methylene blue and decomposition of water under visible light illumination. This arises from the formation of Cr3+/Cr4+ and oxygen vacancy energy levels owing to Cr doping. The former reduces the electron-hole recombination chance, while the latter generates a new gap between the conduction band (CB) and valence band (VB) of TiO2, which lowers the photo energy of the excited electron in the VB to the oxygen vacancy states. The mechanisms for film synthesis during the MAO process are also presented.

文摘Because of cloud computing's high degree of polymerization calculation mode, it can't give full play to the resources of the edge device such as computing, storage, etc. Fog computing can improve the resource utilization efficiency of the edge device, and solve the problem about service computing of the delay-sensitive applications. This paper researches on the framework of the fog computing, and adopts Cloud Atomization Technology to turn physical nodes in different levels into virtual machine nodes. On this basis, this paper uses the graph partitioning theory to build the fog computing's load balancing algorithm based on dynamic graph partitioning. The simulation results show that the framework of the fog computing after Cloud Atomization can build the system network flexibly, and dynamic load balancing mechanism can effectively configure system resources as well as reducing the consumption of node migration brought by system changes.

文摘The poor quality of crude oil obviously leads to high sulfur contents of oil products, and the technology for desulfurization of crude oil is urgently needed so that the sulfur contents in petroleum product could be reduced from the root. This paper describes the progress in technology for desulfurization of crude oil. The present technologies for desulfurization of crude oil include caustic washing, dry gas desulfurization, hydrodesulfurization (HDS), etc. The new combined technologies for desulfurization of crude oil being studied are: biodesulfurization (BDS), hydrogenationbacterial catalysis, the microwave-catalytic hydrogenation, the BDS-OD-RA desulfurization and oxidative desulfurization in electrostatic fields, and the ultrasonic/microwave-catalytic oxidation applied in our lab, with their development trends being also discussed.

文摘This paper describes the deployment optimization technology and the cross-layer design of a surveil-lance WSN system applied in relic protection.Facing the typical technical challenges in the applicationcontext of relic protection,we firstly propose a deployment technology based on ant colony optimization al-gorithm(DT-ACO)to overcome the difficulties in communication connectivity and sensing coverage.Meanwhile,DT-ACO minimizes the overall cost of the system as much as possible.Secondly we proposea novel power-aware cross-layer scheme(PACS)to facilitate adjustable system lifetime and surveillanceaccuracy.The performance analysis shows that we achieve lower device cost,significant extension of thesystem lifetime and improvement on the data delivery rate compared with the traditional methods.
文摘A 68-year-old man underwent laparoscopic low anterior resection for rectal carcinoma in December 2006.Nearly 19 mo after the operation,he developed recurrent rectal cancer with peritoneal metastasis. In September 2008,he subsequently underwent a laparotomy with a peritonectomy,omentectomy, splenectomy,and a Hartmann procedure.Hyperthermic intraperitoneal oxaliplatin 750 mg was administered. The patient was discharged with no postoperative complications and has been well with postoperative FOLFOX chemotherapy.

文摘AIM:To investigate if contrast-enhanced ultrasonography(CE-US) is useful for determining treatment efficacy and outcome in the early stages of pancreatic cancer chemotherapy by assessing changes in intratumor hemodynamics using CE-US with a contrast agent.METHODS:The subjects were 34 patients with unresectable advanced pancreatic cancer treated by chemotherapy.CE-US was assessed after every treatment(course) completion under the same conditions,and patients were divided into two groups according to the intratumor enhancement pattern:Vascular rich(R) group and vascular poor(P) group.RESULTS:After the second course of treatment,R group in intratumor hemodynamics had 18 patients,and P group had 16 patients.The reduction rates of serum CA19-9 level after chemotherapy which decreased to half or less of the baseline level were 2/15(0.1%) in P group,but 11/16(69%) in R group(P = 0.006).When the mean number of courses of chemotherapy and outcome were compared,P group had a mean number of courses of 4.9(R group,10.2) and mean survival time(MST) of 246 d(R group,402 d),showing that outcome was significantly better in R group(P=0.006).CONCLUSION:CE-US revealed that the change in intratumor blood flow correlated with both serum CA19-9 level and outcome.Patients with serum CA19-9 that decreased to less than half the baseline level,and patients with an abundant intratumor blood flow,had a significantly better outcome.Thus,CE-US is potentially useful for evaluating treatment efficacy and outcome in the early stages of pancreatic cancer chemotherapy.
文摘AIM: To develop a technique of combined endoscopic sclerotherapy and ligation (ESL) in which both techniques of endoscopic sclerotherapy (ES) and endoscopic variceal ligation (EVL) can be optimally used.METHODS: ESL was performed in 10 patients (age 46.4±7.9;9 males, 1 female) with cirrhosis of liver using sclerotherapy needle and Speedband, Superview multiple band ligater (Boston Scientific, Microvasive, Watertown, MA). A single band was placed 5-10 cm proximal to the gastro-esophageal junction over each varix from proximal to distal margin,followed by intravariceal injection of 1.5 % ethoxysclerol (4 ml each) 2 to 3 cm proximal to the gastroesophageal junction on the ligated varices distal to deployed band. EVL was then performed at the injection site. Similarly other varices were also injected and ligated from distal to proximally. In the subsequent sessions, ES alone was performed to sclerose small varices at the gastroesophageal junction.RESULTS: ESL was successfully performed in all patients.A median of 3 (ESL 1, ES 2) sessions (ranged 1-4) were required to eradicate the varices in 9 (90 %) of 10 patients.Recurrence of varices without bleed was seen in 1 patient during a mean follow-up of 10.3 months (ranged 6-15).Two patients died of liver failure. None died of variceal bleeding. None of the patients had procedure related complications.CONCLUSION: ESL may be useful in the fast eradication of esophageal varices. However, randomised controlled trials are required to find out its relative efficacy and impact on variceal recurrence in comparison to ES or EVL.

文摘A supervised genetic algorithm (SGA) is proposed to solve the quality of service (QoS) routing problems in computer networks. The supervised rules of intelligent concept are introduced into genetic algorithms (GAs) to solve the constraint optimization problem. One of the main characteristics of SGA is its searching space can be limited in feasible regions rather than infeasible regions. The superiority of SGA to other GAs lies in that some supervised search rules in which the information comes from the problems are incorporated into SGA. The simulation results show that SGA improves the ability of searching an optimum solution and accelerates the convergent process up to 20 times.

文摘When mining the fully-mechanized longwall caving face along strike, the unstable equipment, the low top-coal recovery ratio and the difficulty in controlling surrounding rock may occur due to large dip angle. Considering the effects of strike angle on support stability, the ‘‘support-surrounding rock"mechanical models of support topple and support slip were established in this paper. On the basis, the influencing factors of support stability were analyzed and the technical measures of controlling support and surrounding rock stability were put forward. Then the loose particles simulation experiment was conducted to analyze the impacts of caving directions and methods on the top-coal recovery in large dip angle fully-mechanized caving face. Finally, the ‘‘upward sequence and double-openings doublerounds" caving technology was determined. The research results are of great scientific significance and practical values to improve large dip thick seam mining technology.
文摘Linux has a special feature of automount in the Linux kernel. Filesystem can be mounted and unmounted automatically. Its performance is similar to MS Windows. It works well under demands of more saving resources and automatization, like memory of system. An approach to install/update software automatically on embedded platform was proposed. This approach derives from above feature based on embedded environment with Linux OS. Configurations of files related to this feature were introduced. An example in practice was given to realize this approach from universal serial bus (USB) memory disk when USB disk is available.
