自身免疫性溶血性贫血对血型鉴定和交叉配血的影响

目的 研究自身免疫性溶血性贫血(自免溶贫)对于血型鉴定、交叉配血的临床影响。方法 选取2018年1月-2022年12月在本院进行配血的患者共364例为本次研究的试验对象,均进行抗人球蛋白试验、血型鉴定及交叉配血试验,统计自免溶贫发生率,分... 目的 研究自身免疫性溶血性贫血(自免溶贫)对于血型鉴定、交叉配血的临床影响。方法 选取2018年1月-2022年12月在本院进行配血的患者共364例为本次研究的试验对象,均进行抗人球蛋白试验、血型鉴定及交叉配血试验,统计自免溶贫发生率,分析自免溶贫患者的直接抗人球蛋白试验(DAT)及间接抗人球蛋白试验(IAT)强度情况,计算自免溶贫患者的抗体类型和不同DAT强度、不同抗体类型的ABO血型正反定型相符率、交叉配血相合率。结果 本组364例配血患者中自免溶贫患者38例,发生率10.44%。38例自免溶贫患者的DAT与IAT试验结果均为阳性,强度均集中在++~+++之间;抗体类型以温抗体为主。在DAT试验阳性程度不断上升的情况下,患者ABO血型正反定型相符率随之下降,且交叉配血相合率也有显著降低,数据差异有统计学意义(P<0.05)。不同抗体类型患者的ABO血型正反定型相符率、交叉配血相合率数据差异无统计学意义(P>0.05)。结论 自免溶贫患者的常见抗体为温抗体,且DAT强度同交叉配血相合与否、血型相符与否有一定相关性,在发生交叉配血不相合及血型不相符的情况时需要通过吸收放散试验进行改善,以保证效果。 展开更多

目的在自身免疫性溶血性贫血(自免溶贫)患者血样的检测中,自身抗体往往易被同种抗体掩盖,本研究试图通过对流式细胞术与传统血清学方法的比较研究,分析两者对自身抗体中混合同种抗体的检测能力差异,为自免溶贫患者输血前检测提供更敏感... 目的在自身免疫性溶血性贫血(自免溶贫)患者血样的检测中,自身抗体往往易被同种抗体掩盖,本研究试图通过对流式细胞术与传统血清学方法的比较研究,分析两者对自身抗体中混合同种抗体的检测能力差异,为自免溶贫患者输血前检测提供更敏感可靠的方法。方法随机选择使用5例人源抗-D及4例抗-E分别与经验证不具血型特异性的自身抗体按比例梯度混合,配制模拟自免溶贫患者血浆,使用凝胶卡法与流式细胞术分别对上述模拟血浆进行检测。结果三个效价组中(16,32,64),流式方法可检测出效价为自身抗体1/8~1/16的同种抗体,灵敏度均明显强于凝胶卡方法,且检出下限为凝胶卡方法的4倍(P<0.05)。结论流式细胞术较传统血清学方法对自身抗体掩盖下同种抗体的检测具有更高的灵敏度,采用该方法用于自免溶贫患者输血前检查可降低已有同种抗体介导的输血免疫风险,提高此类患者输血安全。 展开更多

To investigate the therapeutic effects of angelica sinensis polysaccharide-iron complex (APIC) on hemolytic anemia and bone marrow injury in mice models. The hemolytic anemia mouse model was established by i.p. of phenylhydrazine (PHZ). Changes of the indices including red blood cell count (RBC), hemoglobin (Hb) and hematocrit (HCT) were determined by blood analyzer, and reticulocytes were observed by brilliant cresol blue staining during administration. Bone marrow injured mouse model was established by i.p. of cytoxan (CY) and chloramphenicol (CH), and the therapeutic effect was observed by H-E staining. The indices of APIC treated groups with the medium and high doses were higher than those of the model group significantly. Moreover, the Hb and HCT were restored to the normal level after drug treatments. In addition, APIC can promote the proliferation and differentiation of reticulocytes obviously in the early stage of anemia mice, decrease adipose cell proliferation in bone marrow of injured mice and hasten the recuperation. In conclusion, APIC has therapeutic efficacy on hemolytic anemia and bone marrow injury caused by chemicals, which is reported for the first time. 展开更多

The enzyme a... Objective: To further explore the mechanism of congenital pyrimidine 5'-nucleotidase I deficiency. Methods; The samples were collected from the family members of a patient with P5'N- I deficiency. The enzyme activities were measured by UMP method and the enzyme proteins were quantified by ELISA while the morphology of peripheral blood cells was observed. Results: The enzyme contents reduced as their enzyme activities decreased in the family especially in four members. There was a significant positive correlation(r =0. 955) between the activity and the content of P 5'N- I . The count of the stippling cell was varied in the family. Conclusion.- One of the reasons for congenital P5' N- I deficiency might be the deficiency in the enzyme content. The morphology of peripheral blood erythrocyte may be an assistant diagnotic index. The P5'N- I activities and contents were measured simultaneously may be a effective method in clinic diagnosis. 展开更多
