目的探讨吸入糖皮质激素对咳嗽变异性哮喘患者AHR、BPT转阴率及LCQ评分的影响。方法将48例咳嗽变异性哮喘患者按就诊顺序随机分为布地奈德组、孟鲁司特钠组、沙丁胺醇组三组,每组各16例,三组患者均予吸氧,清淡饮食、盐酸溴已新片8mg po ... 目的探讨吸入糖皮质激素对咳嗽变异性哮喘患者AHR、BPT转阴率及LCQ评分的影响。方法将48例咳嗽变异性哮喘患者按就诊顺序随机分为布地奈德组、孟鲁司特钠组、沙丁胺醇组三组,每组各16例,三组患者均予吸氧,清淡饮食、盐酸溴已新片8mg po tid及对症治疗,在此基础上,布地奈德组予布地奈德100μg吸入bid,孟鲁司特钠组予孟鲁司特钠10mg po qn,沙丁胺醇组予沙丁胺醇100μg吸入bid。疗程均为8周。比较BPT转阴率、抗透明质酸酶反应(AHR)、LCQ评分之间的差异。结果治疗后三组BPT转阴率比较,布地奈德组优于孟鲁司特组(P<0.05),并明显优于沙丁胺醇组P<0.01);治疗后三组AHR均有改善,布地奈德组作用最显著,较孟鲁司特组更具优势(P<0.05)。三组LCQ评分前后差值,布地奈德组较孟鲁司特组及沙丁胺醇组分值减少更显著(P<0.01)。结论吸入糖皮质激素能降低AHR及增加BPT的转阴率,还能有效减少LCQ评分。 展开更多

The volume of fluid (VOF) method was used to track the free surface of sloshing. External excitation was imposed through the motion of the tank by using the dynamic mesh technique. The study shows that if the tank is subjected to multiple coupled excitations and resonant excitation frequencies, liquid sloshing will become violent and sloshing loads, including impact on the top wall, will be intensified. 展开更多

Objective:To treat and prevent auricular keloid so as to imporve the cure rate and lower the rate of recurrence. Methods:Taking 156 sufferers of auriclar keloid at the orthopaedics department of the Southwest Hospital under the Third Military Medical University from June, 2008 to June, 2011 as the research subject, this research carried out retrospective analysis and summary of different surgical methods with the clinical data of subseque nt treatments, including medicament, radiotheropy, pressure, etc. Results:There was no auricular cartilage necrosis and deformation in the total of 156 cases. It was found that 134 cases were cured in a year of postoperative follow-up, getting the care rate to 85.9%. The treatment was effective on 20 cases, with the effective rate of 12.8%, while it was ineffective on 2 cases, with the ineffective rate being 1.3%. However, two cases showed tendency towards recurrence, which were treated effectively by non-surgical methods like local hormone injection. There was neither recurrence nor apparent hyperplasia of hypertrophic scars on the rest cases, with the auricle and the earlobe in good shape. Conclusion:By auricular keloid excision and stripping surgery, surgical suture tension was reduced. Glucocorticoid and radiotherapy were instantly applied to inhibit pyperplasia of cicatrical tissue. Subsequently, pressure, anti-scarring drugs and silicone membrane were adopted to ensure fine postoperative apperance and reduce recurrence. These methods were proved to be effectiveand they provided systematic and effective treatment for auricular keloid. 展开更多

Therefore, the probability of ... The kinetic processes of Xe(6p[1/2]0, 6p[3/2]2, and 6p[5/2]2) atoms under the focused condition were investigated. The atomic density of the laser prepared state significantly increases. Therefore, the probability of the energy-pooling between two high-lying atoms increases. There are three major types of the energy-pooling collisions. The first type is the energy-pooling ionization. Once the excitation laser is focused, the obvious ionization can be observed from the side window whenever the laser prepared state is 6p[1/2]0, 6p[3/2]2, or 6p[5/2]2 state. Ionization of Xe is attributed to the energy-pooling ionization or a Xe* atom reabsorbing another excitation photon. The second type is energy-pooling with big energy difference. When the 6p[1/2]0 state is the laser prepared state, the energy-pooling collision between two 6p[1/2]0 atoms can produce one 5d[3/2]1 atom and one 6s'[1/2]0 atom. The third type is energy-pooling with small energy difference. The intensities of fluorescence lines are much stronger that five secondary 6p states act as the upper states, and the rising edges of these fluorescence lines are much steeper. The primary mechanism of generating the secondary 6p atoms is energy-pooling collision instead of collision relaxation. Based on the collision probability, the rate of energy-pooling between two 6p[1/2]0 atoms is deduced (6.39x10^8s-1). In addition, the 6s atoms also increase under the focused condition. Therefore, all the fluorescence lines are serious trailing by radiation trapping. 展开更多

目的:观察用麻黄汤合止嗽散联合西药治疗风寒犯肺证咳嗽变异性哮喘的临床效果。方法:将86例风寒犯肺证咳嗽变异性哮喘患者随机分为对照组和治疗组各43例。对照组给予沙美特罗替卡松粉吸入剂治疗,治疗组在对照组治疗基础上以麻黄汤合止... 目的:观察用麻黄汤合止嗽散联合西药治疗风寒犯肺证咳嗽变异性哮喘的临床效果。方法:将86例风寒犯肺证咳嗽变异性哮喘患者随机分为对照组和治疗组各43例。对照组给予沙美特罗替卡松粉吸入剂治疗,治疗组在对照组治疗基础上以麻黄汤合止嗽散加减治疗。2组均以10天为1疗程,治疗2疗程。观察2组患者临床症状的改善情况,比较患者治疗后肺功能激发试验转阴率及临床疗效。结果:临床疗效总有效率对照组为72.1%,治疗组为93.0%,2组比较,差异有显著性意义(P<0.05)。治疗前,2组惠者肺功能激发试验均显示阳性,治疗后,治疗组肺功能激发试验显示转阴者为9例(20.9%),对照组肺功能激发试验显示转阴者为3例(7.0%),2组转阴率比较,差异有显著性意义(P<0.05)。结论:麻黄汤合止嗽散联合西药治疗风寒犯肺证咳嗽变异性哮喘能有效改善患者的临床症状,提高患者肺功能激发试验转阴率,凸显出中西医结合治疗的优势与特色,疗效显著。 展开更多

Endoscopic submucosal dissection (ESD) with a knife is a technically demanding procedure a ssociated with ahigh complication rate. The shortcomings of this meth-od are the inability to fix the knife to the target lesion,and compression of the lesion. These can lead to major complications such as perforation and bleeding. To reduce the risk of complications related to ESD, we developed a new grasping type scissors forceps (GSF),which can grasp and incise the targeted tissue using electrosurgical current. Colonoscopy on a 55-year-old woman revealed a 10-ram rectal submucosal nodule.The histological diagnosis of the specimen obtained by biopsy was carcinoid tumor. Endoscopic ultrasonog-raphy demonstrated a hypoechoic solid tumor limitedto the submucosa without lymph node involvement. Itwas safely and accurately resected without unexpectedincision by ESD using a GSF. No delayed hemorrhage or perforation occurred. Histological examination confirmed the carcinoid tumor was completely excisedwith negative resection margin. 展开更多

Based on an analytical solution for the current point source in an anisotropic half-space,we study the apparent resistivity and apparent chargeability of a transversely isotropic medium with vertical and horizontal axes symmetry,respectively.We then provide a simple derivation of the anisotropy paradoxes in direct current resistivity and time-domain induced polarization methods.Analogous to the mean resistivity,we propose a formulation for deriving the mean polarizability.We also present a three-dimensional finite element algorithm for modeling the direct current resistivity and time-domain induced polarization using an unstructured tetrahedral grid.Finally,we provide the apparent resistivity and apparent chargeability curves of a tilted,transversely isotropic medium with diff erent angles,respectively.The subsequent results illustrate the anisotropy paradoxes of direct current resistivity and time-domain induced polarization. 展开更多

The ground-state and lowest excited-state binding energies of a hydrogenic impurity in GaAs parabolic quantum-well wires (Q WWs) subjected to external electric and magnetic fields are investigated using the finite-difference method within the quasi-one-dimensional effective potential model. We define an effective radius Pen of a cylindrical QWW, which can describe the strength of the lateral confinement. For the ground state, the position of the largest probability density of electron in x-y plane is located at a point, while for the lowest excited state, is located on a circularity whose radius is Pen. The point and circularity are pushed along the left haft of the center axis of the quantum-well wire by the electric field dire ted along the right half. When an impurity is located at the point or within the circularity, the ground-state or lowest excited-state binding energies are the largest; when the impurity is apart from the point or circularity, the ground-state or lowest excited-state binding energies start to decrease. 展开更多

The model can be implemented in conventional SPICE-like circuit simulators, including HSPICE, and be used to simulate the key features of VCSEL. The results compare favorably with experimental data from a device reported in the literature. TFCCE adopts the chirp signal excitation scheme and strikes a balance in the selection of sub-signal bandwidth, the bandwidth overlap and the number of sub-strain image based on theoretical derivation, so as to further improve the quality of elastic image. Experiments have proved that, compared with the other optimizing methods, the elastographyic signal-to-noise ratio(Re-SN) and contrast-to-noise ratio(Re-CN) are improved significantly with different echo signal-to-noise ratios (ReSN) and attenuation coefficients. When ReSN is 50 dB, compared with short pulse, Rc-SN and Re-CN obtained by TFCCE increase by 53% and 143%, respectively. Moreover, in a deeper investigation (85-95 mm), the image has lower strain noise and clear details. When the attenuation coefficient is in the range of 0-1 dB/(cm.MHz), Re-SN and Re-CN obtained by TFCCE can be kept in moderate ranges of 5〈Re-SN〈6.8 and 11.4〈Re-CN〈15.2, respectively. In particular, for higher tissue attenuation, the basic image quality cannot be ensured with short pulse excitation, while mediocre quality strain figure can be obtained by TFCCE.
