目的探究身痛逐瘀汤合活络效灵丹治疗糖尿病合并肩周炎效果。方法该研究纳入88例糖尿病合并肩周炎患者(2019年11月-2021年4月收治),将患者以随机数字表法分成两组,各44例,对照组患者根据自身情况服药控制血糖水平,并采取发散式冲击波结... 目的探究身痛逐瘀汤合活络效灵丹治疗糖尿病合并肩周炎效果。方法该研究纳入88例糖尿病合并肩周炎患者(2019年11月-2021年4月收治),将患者以随机数字表法分成两组,各44例,对照组患者根据自身情况服药控制血糖水平,并采取发散式冲击波结合康复锻炼治疗,观察组患者在对照组治疗基础上联合身痛逐瘀汤合活络效灵丹治疗,比较两组患者治疗效果、治疗前后Constant-Murley肩关节功能量表评分变化、美国加州大学肩关节评分(UCLA)变化、肩关节主动活动度(AROM)变化、视觉模拟疼痛评分(VAS)变化、中医证候各项积分(关节刺痛、活动不利、局部僵硬感、痛处固定等)变化、不良反应及治疗满意度情况。结果两组患者血糖水平均正常水平;与对照组(84.09%,37/44)相比,观察组总有效率(97.73%,43/44)更高,P<0.05;治疗前,两组患者Constant-Murley肩关节功能量表评分、肩关节主动活动度(AROM)、VAS评分、UCLA评分及中医证候各项积分(关节刺痛、活动不利、局部僵硬感、痛处固定等)等指标比较,P>0.05,治疗后各组患者Constant-Murley肩关节功能量表评分、肩关节主动活动度(AROM)、VAS评分、UCLA评分及中医证候各项积分(关节刺痛、活动不利、局部僵硬感、痛处固定)等指标均显著改善,观察组患者治疗后Constant-Murley肩关节功能量表评分、肩关节主动活动度(AROM)、VAS评分、UCLA评分及中医证候各项积分(关节刺痛、活动不利、局部僵硬感、痛处固定等)等指标优于对照组,P<0.05;两组患者治疗过程中均未见心慌、头晕、恶心等不良反应,P>0.05;对照组患者治疗满意率(79.55%,35/44)明显比观察组患者(97.73%,43/44)低,P<0.05。结论身痛逐瘀汤合活络效灵丹治疗糖尿病合并肩周炎效果显著,安全可靠,患者肩关节功能恢复较好,值得应用。 展开更多

目的基于网络药理学方法探讨活络效灵丹治疗冠心病的有效活性成分及靶点的作用机制。方法本研究运用TCMSP数据库结合筛选条件确定活络效灵丹的主要有效活性成分及其作用靶点。利用GeneCards数据库和OMIM数据库获取冠心病作用靶点,取活... 目的基于网络药理学方法探讨活络效灵丹治疗冠心病的有效活性成分及靶点的作用机制。方法本研究运用TCMSP数据库结合筛选条件确定活络效灵丹的主要有效活性成分及其作用靶点。利用GeneCards数据库和OMIM数据库获取冠心病作用靶点,取活络效灵丹和冠心病作用靶点的交集,对交集靶点进行蛋白质互相作用网络分析、基因功能富集分析和信号通路富集分析。结果筛选主要有效活性成分118种,并获得药物靶点197个。通过GeneCards数据库和OMIM数据库共获得7266靶点,药物作用靶点与疾病作用靶点交集基因172个,PPI蛋白互作网络分析得出STAT3、AKT1、JUN、MAPK1、APP、ERLA、CXCL8、IL6、MAPK14、EGFR等可能是活络效灵丹治疗冠心病的核心靶点。GO功能富集分析得到163个条目,KEGG通路富集分析获得69条信号通路,其中富集排名靠前通路主要有IL-17信号通路TNF信号通路、HIF-1信号通路、内分泌抵抗、EGFR酪氨酸激酶抑制剂耐药、PI3K-Akt信号通路、铂耐药、细胞凋亡、C型凝集素受体信号通路、Th17细胞分化、Toll样受体信号通路等。结论活络效灵丹治疗动冠心病具有多途径、多靶点的作用特点,为探析中医治疗冠心病提供参考。 展开更多

During seismic data acquisition, a high-sensitivity geophone with a high inherent frequency can increase high frequency energy by suppressing low frequency signals. This could cause a worse response at low frequencies. If the advantages of high-sensitivity data and conventional data are combined, the effective bandwidth will be broadened. Considering this, we propose a partial frequency band match filtering method which can combine the advantages of both high frequency and conventional frequency ranges. By introducing Ricker wavelets with different dominant frequencies and amplitudes, we established a theoretical model which possesses characteristics of both types of seismic data and demonstrates the feasibility of the partial frequency band match filtering method. A test using single shot records shows the effectiveness of this method for widening the effective frequency band. 展开更多

Especially when GA is used in complex structural optimization problems, if the structural reanalysis technique is not adopted, the more the number of finite element analysis (FEA) is, the more the consuming time is. In the conventional structural optimization the number of FEA can be reduced by the structural reanalysis technique based on the approximation techniques and sensitivity analysis. With these techniques, this paper provides a new approximation model-segment approximation model, adopted for the GA application. This segment approximation model can decrease the number of FEA and increase the convergence rate of GA. So it can apparently decrease the computation time of GA. Two examples demonstrate the availability of the new segment approximation model. 展开更多

Rifaximin vs conventional oral therapy for hepatic encephalopathy:A meta-analysis 被引量:9

AIM: To characterize the efficacy of rifaximin in the management of hepatic encephalopathy (HE) as several randomized controlled studies have shown contradictory results on its effectiveness in comparison to other ora... AIM: To characterize the efficacy of rifaximin in the management of hepatic encephalopathy (HE) as several randomized controlled studies have shown contradictory results on its effectiveness in comparison to other oral agents. METHODS: We performed a systematic review and random effects meta-analysis of all eligible trials identifi ed through electronic and manual searches. Twelve randomized controlled trials met the inclusion criteria with a total of 565 patients. RESULTS: The clinical effectiveness of rifaximin was equivalent to disaccharides or other oral antibiotics[odds ratio (OR) 0.96; 95% CI: 0.94-4.08] but with a better safety profi le (OR 0.27; 95% CI: 0.12-0.59). At the completion of treatment protocols, patients receiving rifaximin showed lower serum ammonia levels [weighted mean difference (WMD) = -10.65; 95% CI: -23.4-2.1; P = 0.10], better mental status (WMD = -0.24; 95% CI: -0.57-0.08; P = 0.15) and less asterixis (WMD -0.1; 95% CI -0.26-0.07; P = 0.25) without reaching statistical signifi cance. On the other hand, other psychometric outcomes such as electroencephalographic response and grades of portosystemic encephalopathy were superior in patients treated with rifaximin in comparison to the control group (WMD = 0.21, 95% CI: -0.33-0.09, P = 0.0004; and WMD = -2.33, 95% CI: -2.68-1.98, P = 0.00001, respectively). Subgroup and sensitivity analysis did not show any signifi cant difference in the above fi ndings. CONCLUSION: Rifaximin appears to be at least as effective as other conventional oral agents for the treatment of HE with a better safety profi le. 展开更多

目的观察五首活血化瘀方对新西兰兔血瘀证血常规、血沉及心、肝脏器系数的影响,对比其作用差异,为临床选方用药提供实验依据。方法70只新西兰兔随机分为正常组10只,造模组60只。采用“饥饿+高脂饲料+肾上腺素”方法复制血瘀证模型。造... 目的观察五首活血化瘀方对新西兰兔血瘀证血常规、血沉及心、肝脏器系数的影响,对比其作用差异,为临床选方用药提供实验依据。方法70只新西兰兔随机分为正常组10只,造模组60只。采用“饥饿+高脂饲料+肾上腺素”方法复制血瘀证模型。造模成功后将造模组随机分为模型组、血府组、丹参组、失笑组、活络组、桃红组。各复方组分别给予相应复方灌胃治疗,正常组和模型组予以同等剂量蒸馏水灌胃作为对照。治疗结束后腹主动脉取血检测血常规及血沉,空气栓塞处死后取心脏、肝脏计算脏器系数。结果与正常组比较,模型组红细胞计数(RBC)、红细胞压积(HCT)、血红蛋白(HGB)显著降低(P<0.01),平均红细胞体积(MCV)、平均红细胞血红蛋白含量(MCH)、平均红细胞血红蛋白浓度(MCHC)、白细胞计数(WBC)、中间细胞计数(Mid)、粒细胞计数(Gran)、肝脏系数显著升高(P<0.05或P<0.01),红细胞体积分布宽度变异系数(RDW-CV)、淋巴细胞计数(Lymph)、血小板计数(PLT)、血小板压积(PCT)、平均血小板体积(MPV)、血小板分布宽度(PDW)、血沉(ESR)、心脏系数差异无统计学意义。与模型组比较,丹参组、血府组、桃红组RBC显著升高(P<0.05或P<0.01);与模型组比较,桃红组HCT显著升高(P<0.05);与模型组比较,丹参组、血府组、活络组、桃红组MCV显著降低(P<0.05或P<0.01);与模型组比较,丹参组、血府组、失笑组、桃红组MCH显著降低(P<0.01);与模型组比较,丹参组、血府组、活络组MCHC显著降低(P<0.05);与模型组比较,血府组Mid、Gran显著降低(P<0.05);与模型组比较,活络组肝脏系数显著升高(P<0.05);与模型组比较,各复方组RDW-CV、HGB、PLT、PCT、MPV、PDW、WBC、Lymph、ESR、心脏系数差异无统计学意义。结论五首活血化瘀方调节红细胞、血红蛋白相关指标RBC、HCT、MCV、MCH、MCHC的作用力大小不同,桃红四物汤疗效最佳,其次为血府逐瘀汤;调节炎症反应相关指标Mid、Gran效果最优的是血府逐瘀汤。 展开更多

Objective This study aimed to compare the mechanisms and efficacies of five formulas that improve blood circulation and remove blood stasis.Methods(1)A network pharmacology method was used to determine the targets of five formulas that promote circulation and remove blood stasis.Compounds of the five formulas,namely Danshen Yin(丹参饮,DSY),Huoluo Xiaoling Dan(活络效灵丹,HLXLD),Shixiao San(失笑散,SXS),Taohong Siwu Tang(桃红四物汤,THSWT),and Xuefu Zhuyu Tang(血府逐瘀汤,XFZYT),were retrieved from the Traditonal Chinese Medicine System Pharmacology Database(TCMSP),the Shanghai Institute of Organic Chemistry of CAS,and the TCM Integrated Database.Drug target network was constructed by searching the Swiss Target Prediction database and the STITCH database.The target network of stasis was extracted from the PharmGKB database,the Online Mendelian Inheritance in Man(OMIM)database,the Genetic Association Database(GAD),and the Therapeutic Target Database(TTD).Candidate targets were determined using protein-protein interaction(PPI)network extension and topology selection.Thereafter,Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis was used to determine the differentiation of the mechanism of the five formulas.(2)Animal experiments were conducted to explore the efficacies of the five formulas in treating blood stasis.Seventy New Zealand rabbits were exposed to high-fat feeding+epinephrine injection to construct a blood stasis syndrome model.The rabbits were evenly divided into control,model,DSY,HLXLD,SXS,THSWT,and XFZYT groups.The latter five groups were orally administered the corresponding formulas[DSY:3.92 g/(kg·d),XFZYT:7.10 g/(kg·d),SXS:1.12 g/(kg·d),HLXLD:5.60 g/(kg·d),THSWT:4.48 g/(kg·d)].Serum lipid and blood rheology were analyzed,and pathology slices were observed.Results(1)A total of 269,358,288,370,and 376 candidate targets of DSY,HLXLD,SXS,THSWT,and XFZYT were obtained among which were 232 shared candidate targets.Fluid shear stress and atherosclerosis were the biological processes common to the five formulas.HLXLD,SXS,DSY,and THSWT regulated lipolysis in adipocytes,and XFZYT,HLXLD,SXS,and THSWT regulated the AGE-RAGE signaling pathway and complement and coagulation cascades.HLXLD,SXS,and XFZYT regulated the HIF-1 signaling pathway,DSY regulated the cGMP-PKG signaling pathway,HLXLD reduced platelet activation,SXS regulated the calcium signaling pathway,and XFZYT regulated the PPAR signaling pathway.(2)In the animal experiments,the values of total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL),high density lipoprotein cholesterol(HDL),TC/HDL,and TG/HDL in each group decreased,among which the ones seen in XFZYT,HLXLD,and SXS groups were statistically significant(P<0.05).XFZYT presented the best effect,followed by HLXLD and SXS.XFZYT and HLXLD decreased apolipoprotein B100(apoB100)and increased apolipoprotein A1/apoB100(P<0.05).XFZYT decreased all the values of hematocrit(HCT),plasma viscosity,whole blood viscosity(WBV);HLXLD and SXS affected HCT;and DSY and THSWT regulated WBV(P<0.05).All the five formulas decreased the values of optical density and area of plaque,among which XFZYT and HLXLD showed statistical significance(P<0.05).Conclusion Adjusting fluid shear stress and alleviating the injury of endothelial cells might be the common mechanisms by which the five formulas promote blood circulation and remove blood stasis.Different formulas also have unique targets,which may provide guidance for clinical drug selection.By regulating different indices,the five formulas can regulate blood lipid and hemorheology,improve the state of blood stasis,and decrease the degree of aortic plaque in the blood stasis model rabbits.XFZYT and HLXLD had higher efficacies than DSY,THSWT,and SXS. 展开更多

Numerical method is popular in analysing the blast wave propagation and interaction with structures.However,because of the extremely short duration of blast wave and energy trans-mission between different grids,the numerical results are sensitive to the finite element mesh size.Previous numerical simulations show that a mesh size acceptable to one blast scenario might not be proper for another case,even though the difference between the two scenarios is very small,indicating a simple numerical mesh size convergence test might not be enough to guarantee accu-rate numerical results.Therefore,both coarse mesh and fine mesh were used in different blast scenarios to investigate the mesh size effect on numerical results of blast wave propagation and interaction with structures.Based on the numerical results and their comparison with field test re-sults and the design charts in TM5-1300,a numerical modification method was proposed to correct the influence of the mesh size on the simulated results.It can be easily used to improve the accu-racy of the numerical results of blast wave propagation and blast loads on structures. 展开更多

Taking a concrete frame supporting space structure as the research object,we systematically studied its lateral-torsion coupling effect of reverse problems in consistent earthquake excitation.Firstly,based on its reverse forms and features,we put forward a mechanical analysis model(flexibility layer model) and a calculation method using the response spectrum method and the weighted average method,and verified their validity and feasibility using case analysis.The result shows that the translation displacement change trend of the space structure is basically the same whether reverse exists in the supporting structure or not,but the supporting structure torsion has an effect on the displacement with a relative increase of 10%. 展开更多

Experimental approach to investigation of NLCI (Non-local Consciousness Influence) effects is based on sensors of different design, and most of it based on transitional effects in gas-discharge plasma with several quasi-stable states is also presented. Computerized device for detecting NLCI is based on commercially available Gas Discharge Camera (www.ktispb.ru) and allows following time dynamics of several sensors in NLCI conditions. The typical sensors are: water, air, earth, and wood, this instrument was named "The 5th Element". Readings are taken continuously by special software every five or ten seconds in automatic mode. Sensitivity of the device was tested by detecting the influence of total sun eclipse in Siberia in 2008 and by detecting sunrise and sunset. Several experimental modalities have been developed: (1) directed NLCI of a person; (2) directed NLCI of a group of people; and (3) non-directed NLCI of a group of people. In this study, three hymexazol-linked chitosan derivatives (HML-CS) were synthesized and their structures confirmed by Fourier transform infrared and elemental analysis. Linkage ratios were measured by high performance liquid chromatography. The derivatives' antifungal activity against the plant pathogenic fimgi Rhizoctonia solani CGMCC 3.28 and Gibberella zeae CGMCC 3.42 were investigated at concentrations of 100, 200, and 400 mg/L. These HML-CS derivatives exhibited stronger antifungal activity than CS alone. HML-CS-1 showed the best antifizngal activity against G. zeae, whose antifimgal index was 65.9% at 400 mg/L, and also showed the best antifungal activity against R. solani, whose antifimgal index was 52.7% at 400 mg/L. This conjugation of CS and HML suggested the presence of synergistic effects between the moieties and indicated that these derivatives possessed great potential as novel fungicides and require further research for the development of applications in crop protection. 展开更多

We employed high performance liquid chromatography (HPLC) method coupled with diode array detector (DAD) for the determination of seven major bioactive compounds (forsythoside A, specnuezhenide, phillyrin, schisandrin, schisantherin A, deoxyschizandrin and schisandrin B) in Liuwei Wuling tablet (LWWLT). The HPLC baseline separation was carried out on an Inertsil ODS-3 column (250 mm×4.6 mm, 5 μm) at 25 ℃ with a gradient elution system composed of 0.1% phosphoric acid solution and acetonitrile. The detection wavelength was set at 230 nm. Calibration curves for seven compounds showed good linear regressions, with correlation coefficients exceeding 0,999 within the tested concentration range. The limits of detection and quantification of each compound were in the range of 0.03-0.28 μg/mL and 0.11-0.87μg/mL, respectively. The relative standard deviation values of precision, stability and repeatability were less than 2.34%. The average recovery of all seven constituents ranged from 96.71% to 103.9%. The validated quantification method was successfully applied for the analysis of 10 batches of commercial LWWLT from Shandong Shibo Jindu Pharmaceutical Co., Ltd. These results would make foundations for quality control and the further pharmacology study of LWWLT. 展开更多
