不同灸次对原发性痛经患者血清PGE_2、PGF_(2α)影响的临床研究 被引量:10

目的比较不同灸次艾灸关元穴治疗原发性痛经(Primary Dysmenorrhea,PD)的疗效,确定灸法治疗PD的较佳灸次,并从生殖内分泌角度探讨灸法治疗PD的作用机制。方法将符合诊断标准的60例患者随机分为A组、B组,每组各30例。两组均以清艾条温和... 目的比较不同灸次艾灸关元穴治疗原发性痛经(Primary Dysmenorrhea,PD)的疗效,确定灸法治疗PD的较佳灸次,并从生殖内分泌角度探讨灸法治疗PD的作用机制。方法将符合诊断标准的60例患者随机分为A组、B组,每组各30例。两组均以清艾条温和灸关元穴,1次/日,A组从月经来潮前5天灸至月经来潮前1天(5次/周期),B组从月经来潮前5天灸至经期第2天(7次/周期),连续治疗3个周期,观察两组腹痛程度变化,并采集治疗前后肘静脉血(经期第2天),比较治疗前后前列腺素F2α(PGF2α)和前列腺素F2α(PGF2α)/前列腺素E2(PGE2)。结果两组腹痛评分(VAS平均分)均有良好改善作用(P<0.05),组间比较差异无统计学意义(P>0.05),两组治疗前后外周血中PGF2α含量及PGF2α/PGE2比值差异均有统计学意义(P<0.05);两组治疗前后外周血PGE2含量比较差异无统计学意义(P>0.05),两组临床疗效相比差异无统计学意义(P>0.05)。结论艾灸关元穴治疗原发性痛经有效,临床治疗可选择从经前5天至月经来潮前1天施灸治疗,即5次/周期,两组均能降低外周血中的PGF2α及PGF2α/PGE2比值,这可能是艾灸治疗原发性痛经有效的机理。 展开更多

[目的]系统评价艾条灸治疗原发性痛经(PD)的总有效性以及艾条灸不同灸次之间对治疗PD临床疗效影响的差异性。[方法]计算机检索CENTRAL、MEDLINE、EMbase、AMED、CINAHL、Web of Science、中国生物医学文献数据库(CBM)、中国知网(CNKI)... [目的]系统评价艾条灸治疗原发性痛经(PD)的总有效性以及艾条灸不同灸次之间对治疗PD临床疗效影响的差异性。[方法]计算机检索CENTRAL、MEDLINE、EMbase、AMED、CINAHL、Web of Science、中国生物医学文献数据库(CBM)、中国知网(CNKI)、维普(VIP)和万方数据库(Wan Fang Data),检索时限均从建库至2015年12月31日,收集国内外公开发表的关于艾条灸治疗PD的临床随机对照试验。由两名研究者按照纳入排除标准独立筛选文献及提取数据,参考Cochrane Handbook质量评价标准进行质量评价,采用Rev Man 5.2软件进行Meta分析,Stata 12.0软件进行文献发表偏倚的定量分析。[结果]纳入10项随机对照实验(RCT),共586例病人。Meta分析结果显示:与非艾条灸组相比,艾条灸组能提高治疗PD病人的总有效率[RR=1.13,95%CI(1.03~1.23),P=0.007],降低血前列腺素F2α(PGF2α)水平[MD=-4.65,95%CI(-8.42^-0.88),P=0.02];在改善PD病人的疼痛方面差异无统计学意义[MD=0.14,95%CI(-0.11~0.39)];将纳入研究每个疗程(1个月经周期)施灸5次、6次、7次进行比较,在提高总有效率方面,对疗效影响的差异无统计学意义(P>0.05)。[结论]本系统评价显示:艾条灸在提高治疗PD病人的总有效率和降低血PGF2α水平方面有一定优势;不同灸次之间对疗效的影响无明显差异,从节约医疗资源、合理安排和节约医生及病人时间精力方面考虑,可选择每个疗程灸5次。因纳入研究的总体方法学质量一般,结论尚需更多高质量研究进一步验证。 展开更多

Objective To explore clinical efficacy and safety of acupuncture in treatment of functional constipation.Methods One hundred and twenty cases were randomly divided into an observation group and a control group,60 cases in each group.The patients in the observation group were treated with acupuncture at Tiānshū(天枢 ST 25),Shàngjùxū(上巨虚 ST 37),Zúsānlǐ(足三里 ST 36),Dàchángshū(大肠俞 BL 25),etc.,and the control group was treated with oral administration of Macrogol 4000 and Mosapride.Defecation frequency,stool property,constipation symptom score,accompanying symptom score,gastrointestinal transit time and adverse reaction were observed before treatment,at the end of treatment and 4 weeks after treatment in the two groups.Results Compared with those before treatment,defecation frequency was increased significantly at the end of treatment in the two groups,while stool property,constipation symptom score,accompanying syndrome score and gastrointestinal transit time all were decreased significantly(all P0.01),with no significant difference in these indexes between the two groups at the end of treatment (all P0.05).Compared with the end of treatment,the above-mentioned indexes did not significantly recur in the observation group 4 weeks after the treatment (all P0.05),but the therapeutic effects were unstable with decreased to some extent in the control group (all P0.05).No serious adverse reaction was found in the two groups.Conclusion In the patients with functional constipation,acupuncture can significantly increase defecation frequency,change stool property,alleviate constipation and accompanying symptoms,and shorten gastrointestinal transit time with better safety and tolerance,and the therapeutic effect is more stable than that of Macrogol 4000 and Mosapride. 展开更多

Objective: To observe the clinical efficacy of acupuncture plus moxibustion at Ciliao (BL 32) for primary dysmenorrhea (PD) due to cold-dampness. Methods: A total of 58 cases with PD due to cold-dampness were randomly allocated into an observation group (n=29) and a control group (n=29). Patients in the observation group were treated with acupuncture and suspended moxibustion at Ciliao (BL 32), whereas patients in the control group were treated with routine acupuncture alone. After three courses of treatment, the clinical efficacy was evaluated and compared between the two groups. Results: After treatment, the symptom scores were significantly reduced in both groups (P〈0.05), but there were no significant between-group differences in clinical efficacy and symptom scores (P〉0.05). Conclusion: With fewer points and less pain, acupuncture-moxibustion at Ciliao (BL 32) can obtain similar effect as routine acupuncture therapy for PD due to cold-dampness. 展开更多
