Parameters used for risk classification have some deficiencies in the selection of patients. The objective of this study is to evaluate the impact of TAVI on carbohydrate antigen 125 (CA125) and N-terminal pro brain-type natriuretic peptide (NT-proBNP) as biomarkers that have been used frequently in recent years, and also the relationship of these biomarkers to prognosis. Methods & Results Transcatheter aortic valve implantation was practiced on 31 patients in this study. Then, CA125 and NT-proBNP levels studied in patients prior to and after the TAVI were evaluated. The patients were also grouped in accordance with their left ventrieular ejection fraction (LVEF) and CA125 levels (LVEF 〉 40% and 〈 40%; CA125 ≤ 35 U/L and 〉 35 U/L). The TAVI operation was successfully performed in all patients. There was no in-hospital mortality and substantial improvement in functional capacity was detected at follow ups. In addition, a statistically significant decrease was detected in post-TAVI CA 125 and NT-proBNP levels of all patients (CA 125 83.8 ± 18.1 U/L vs. 64.3 ±14.2 U/L, P = 0.008; NT-proBNP: 4633.6± 627.6 pg/mL vs. 2866.3±536.8 pg/mL, P 〈 0.001). In groups divided according to the CA125 levels, there was also statistically significant post-TAVI decline in CA125 levels. Within CA125 〉 35 U/L and LVEF 〈 40% groups, the permanent need for a pacemaker was required in one (3.2%) patient and mortality was observed in two (6.4%) patients after TAVI at follow up. Conclusions The results show that TAVI can be performed effectively and reliably in patients with high baseline levels of CA125 and NT-proBNP. These biomarkers are reduced substantially with TAVI, while high biomarker levels are associated with undesired events, and certainly, these biomarkers can be used for risk classifications in patient selection for TAVI.展开更多
Wetlands in the Jianghan Plain are important components of wetland types in lake area in the middle and lower reaches the Changjiang (Yangtze) River and they fulfill many uses and functions related to hydrology, waste...Wetlands in the Jianghan Plain are important components of wetland types in lake area in the middle and lower reaches the Changjiang (Yangtze) River and they fulfill many uses and functions related to hydrology, waste assimilation, ecosystem productivity and biodiversity. Owing to natural factors and human activities, especially excessive reclamation from lakes, the shrinking process of the lakes has been accelerated. Wetland ecosystem has shown the characteristics of vulnerability. According to the analysis of wetland ecological function in the Jianghan Plain, this paper presented an index system related to productivity, stability and environmental capacity. By using the method of Analytic Hierarchy Process, we computed the values of the relative weights of the indexes, and evaluated the vulnerability level of the wetland ecosystem by the method of multi-indexes. The case study showed that the fragile extent of wetland ecosystem in the Jianghan Plain is 5.6. This means that the wetland ecosystem in the Jianghan Plain is laid to the state of middle vulnerability. Therefore, the wetland conservation and eco-rehabilitation in the Jianghan Plain should be paid attention to.展开更多

Evidence of overfishing and stock depleti...In the Indian Ocean, bigeye tuna supports one of the most important fisheries in the world. This fishery mainly consists of two components: longline and purse seine fisheries. Evidence of overfishing and stock depletion of bigeye tuna calls for an evaluation of alternative management strategies. Using an age-structured operating model, parameterize dwith the results derived in a recent stock assessment,we evaluated the effectiveness of applying constant fishing mortality (CF) and quasi-constant fishing mortality (QCF) strategies to reduce fishing effort of purse seining with fish aggregating devices (FADs) at different rates. Three different levels of productivity accounted for the uncertainty in our understanding of stock productivity. The study shows that the results of CF and QCF are similar. Average SSB and catchduring simulation years would be higher if fishing mortality of FAD-associated purse seiningwas reduced rapidly. The banning or rapid reduction of purse seining with FAD resulted in a mean catch, and catch in the last simulation year, higher than that of the base case in which no change was made to the purse seine fishery. This could be caused by growth overfishing by purse seine fisheries with FADs according to the per-recruit analysis. These differences would be more obvious when stock productivity was low. Transferring efforts of FAD-associated purse seining to longline fisheries is also not feasible.Our study suggests that changes are necessary to improve the performance of the current management strategy.展开更多
