It is reasonable to anticipate a continued rise in the incidence of diabetes and its complications along with the aging of the population, increase in adult obesity rate, and other risk factors. Diabetic en- cephalopathy is one of the severe microvascular complications of diabetes, characterized by impaired cogni- tive functions, and electrophysiological, neurochemical, and structural abnormalities. It may involve direct neuronal damage caused by intracellular glucose. However, the pathogenesis of this disease is complex and its diagnosis is not very clear. Previous researches have suggested that chronic metabolic alterations, vascular changes, and neuronal apoptosis may play important roles in neuronai loss and damaged cognitive functions. Multiple factors are responsible for neuronal apoptosis, such as disturbed insulin growth factor (IGF) system, hyperglycemia, and the aging process. Recent data suggest that insulin/C-peptide deficiency may exert a primary and key effect in diabetic encephalopathy. Administration of C-peptide partially improves the condition of the IGF system in the brain and prevents neuronal apoptosis in the hippocampus of diabetic patients. Those findings provide a basis for application of C-peptide as a potentially effective therapy for diabetes and diabetic encephalopathy.展开更多

The author addresses the inadequacy of consolation as mere verbiage, and then brings to mind an expanded understanding of discourse-as-comportment to offer the possibility of deep consolation for the suffering other in the groundlessness of the tragic situation, which the author calls "abysmal consolation". In the realization that discourse is also the embodied consolation of being-with the suffering other in a shattered lifeworld, both sufferer and carer of souling are freed up to meet each other in a place that was heretofore presumed to be an alienated and speechless place.展开更多
AIM To retrospectively review patients with chronic pancreatitis(CP) treated with Frey's procedures between January 2009 and January 2014.METHODS A retrospective review was performed of patients with CP treated wi...AIM To retrospectively review patients with chronic pancreatitis(CP) treated with Frey's procedures between January 2009 and January 2014.METHODS A retrospective review was performed of patients with CP treated with Frey's procedures between January 2009 and January 2014 in the Department of Pancreatic Surgery. A cross-sectional study of postoperative pain relief, quality of life(Qo L), and alcohol and nicotine abuse was performed by clinical interview, letters and telephone interview in January 2016. Qo L of patients was evaluated with the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire(EORTC QLQ-C30) version 3.0. The patients were requested to fill in the questionnaires by themselves via correspondence or clinical interview.RESULTS A total of 80 patients were enrolled for analysis, including 44 who underwent the original Frey's procedure and 36 who underwent a modified Frey's procedure. The mean age was 46 years in the original group and 48 years in the modified group. Thirtyfive male patients(80%) were in the original group and 33(92%) in the modified group. There were no differences in the operating time, blood loss, and postoperative morbidity and mortality between the two groups. The mean follow-up was 50.3 mo in the original group and 48.7 mo in the modified group. There were no differences in endocrine and exocrine function preservation between the two groups. The original Frey's procedure resulted in significantly betterpain relief, as shown by 5-year follow-up(P = 0.032), better emotional status(P = 0.047) and fewer fatigue symptoms(P = 0.028). When stratifying these patients by the M-ANNHEIM severity index, no impact was found on pain relief after the two types of surgery.CONCLUSION The original Frey's procedure is as safe as the modified procedure, but the former yields better pain relief. The severity of CP does not affect postoperative pain relief.展开更多

Under this regulation, the airplane must be designed and certified about static strength of the fuselage and seats, horizontal and vertical dynamic strength of seats. Human injuries in dynamic crash tests and the survivability in the turnover condition should be also evaluated. The most important part among injuries of human is a head and the head injury is evaluated by Head Injury Criterion (HIC). KC-100, 4 seats airplane that is manufactured by KAI, was conducted seat dynamic tests. The HIC from tests was well within criteria limits 1,000 and the occupant compartment for head protection provided hazard-free spaces in the occupant's head strike envelope for the frontal-horizontal crash impacts. The airplane also was conducted a turnover static test and analysis to especially show living spaces of the head. The airframe could sustain living spaces for occupants in the turnover condition and occupants could escape and be protected. The results of tests and analysis corresponded to the criteria and proved the retention of survivable space in emergency landing condition.展开更多

Our study was conducted on Swiss albino mice in order to know the effect of probiotics or biotherapeutic agents, and suggest the different therapeutic possibilities of intestinal complications related to the presence of Salmonella enterica serovar typhimurium; Reference strains, Lactobacillus delbrueckii subsp, bulgaricus, Lactobacillus acidophilus and Lactobacillus paracasei in combination with Bifidobacterium sp. were tested for their health benefits. The number of Salmonella enterica serovar typhimurium decreases in intestine, in the stomach and in the liver for group 2, 3 in case of preventive treatment and group 4, 5 in case of a therapeutic treatment due to the use of a cuRure of probiotics, in comparison with group 1 where we recorded a high number. The bio-therapeutic treatment was finally confirmed by macroscopic and microscopic observations of different target organs before and after treatment for the experiment period.展开更多
OBJECTIVE:To observe the effects of Liandouqingmai Recipe on life quality and vascular endothelial injury in patients with coronary heart disease.METHODS:Capitalized 101 patients with coronary heart disease were rando...OBJECTIVE:To observe the effects of Liandouqingmai Recipe on life quality and vascular endothelial injury in patients with coronary heart disease.METHODS:Capitalized 101 patients with coronary heart disease were randomly divided into a treatment group(n=45) treated with Liandouqingmai Recipe and a standard treatment group(control group,n=56).A normal group of 16 healthy persons was additionally set up.Changes in ET-1 and NO levels were measured and Seatle Angina Questionnaire(SAQ) was adopted in studying life quality before and after treatment for two weeks.The data were analyzed with SPSS 16.0 statistic software.RESULTS:The average level of ET-1 in the normal group was lower and NO higher than that of patients with coronary heart disease.There was no significant difference in the average level of ET-1 and NO and in the scores of SAQ [physical limitation(PL),pngina stability(AS),apngina frequency(AF),treatment satisfaction(TS) and disease perception(DP)] between the two groups before treatment(P>0.05).But after treatment,the scores of SAQ(PL,AS,AF,TS,DP) and NO level were higher than those in the control group,and ET-1 average level in the treatment group was lower than that in the control group.The negative relations between PL and ET-1 and between AF and ET-1 were found in this study.CONCLUSION:Liandouqingmai Recipe can raise scores of SAQ and NO level and decline ET level in patients with coronary heart disease on the basis of convertional standard treatment,thus improving vascular endothelial function and life quality.Life quality is related to vascular endothelial function.展开更多

Methods: We assessed 105 patients with post-traumatic epilepsy and 100 healthy people as control using Quality of Life Scale-31 (QOL-31), Self-rating Depressing Scale (SDS) and Self-rating Anxiety Scale (SAS), and conducted retrospective analysis on the depression, anxiety, site of trauma, control of seizure, EEG and therapeutic compliance. Results: Patients with post-traumatic epilepsy scored much lower than the control group on QOL-31 (P〈0.01), but higher than the control group on SDS and SAS (P〈0.01). Multiple regression analysis indicated that major influencing factors on the quality of life were anxiety, therapeutic compliance, depression, poor control of epileptic seizure and site of trauma. Conclusions: The quality of life in patients with posttraumatic epilepsy has significantly declined. Doctors should pay attention to psychological and mental problems of patients with epilepsy, such as depression and anxiety, enhancing therapeutic compliance and controlling epileptic seizure, which are the keys to improving prognosis.展开更多

This research was conducted to determine the fall incidence among female population of Kashan city from the years 2005 to 2008. Methods: This was a retrospective research using existing data from the data bank of trauma center of Kashan University of Medical Sciences. Records of all the female patients treated at local hospitals with complete hospitalisation kept at the center were examined for 4 con- secutive years from 2005 to 2008. Results: A total of 2 094 female patients' records were examined. A significantly higher incidence of injuy occurred in 2008 compared to 2005 (P〈0.0001). In addition, the highest frequency of injury occurred in age group above 65 years (31.9%) and in group with elementary education level (42.8%). Conclusion: The results showed that fall incidences occurred in the old age group above 65 years. Fall injuries at this age may cause disability. Therefore, preventive measures should be taken, such as increasing the awareness of the aging population about the seriousness of fall incidence and encouraging the aged individuals to get involved in fitness program to remain physical fit and healthy.展开更多

Methods: Retrospective analysis of 68 patients with very severe traumatic brain injuries treated in our hospital from 1997 to 2002 ...Objective: To sum up the experience in treating very severe traumatic brain injuries. Methods: Retrospective analysis of 68 patients with very severe traumatic brain injuries treated in our hospital from 1997 to 2002 was done. Results: Forty one (60%) patients died. In the 50 patients treated surgically 27 (40%) survived, 8 recovered well, 9 had moderate disability and 10 had sever deficits. The 18 patients treated non operatively all died. Conclusions: Much attention should be given to the observation of the changes of severe brain injuries with cranial base injury. Timely operative decompression, basic life support, keeping effective brain blood perfusion and effective oxygen supply, improving cerebral microcirculation and preventing or controlling complications are the main methods to raise the successful rate of treating very severe brain injuries and the life quality of the patients.展开更多
