The prevalence of obesity is growing to epidemic proportions,and there is clearly a need for minimally invasive therapies with few adverse effects that allow for sustained weight loss.Behavior and lifestyle therapy are safe treatments for obesity in the short term,but the durability of the weight loss is limited.Although promising obesity drugs are in development,the currently available drugs lack efficacy or have unacceptable side effects.Surgery leads to long-term weight loss,but it is associated with morbidity and mortality.Gastric electrical stimulation(GES) has received increasing attention as a potential tool for treating obesity and gastrointestinal dysmotility disorders.GES is a promising,minimally invasive,safe,and effective method for treating obesity.External gastric pacing is aimed at alteration of the motility of the gastrointestinal tract in a way that will alter absorption due to alteration of transit time.In addition,data from animal models and preliminary data from human trials suggest a role for the gut-brain axis in the mechanism of GES.This may involve alteration of secretion of hormones associated with hunger or satiety.Patient selection for gastric stimulation therapy seems to be an important determinant of the treatment's outcome.Here,we review the current status,potential mechanisms of action,and possible future applications of gastric stimulation for obesity. 展开更多

Chronic diseases have become a focal point of public health worldwide with estimates of trillions of dollars in annual health care cost and causing more than 36 million deaths a year. Lifestyle factors such as physica... Chronic diseases have become a focal point of public health worldwide with estimates of trillions of dollars in annual health care cost and causing more than 36 million deaths a year. Lifestyle factors such as physical inactivity are heavily correlated with the development of many chronic diseases. New strategies for primary and secondary disease prevention are desperately needed to aid in blunting the negative economic and social impact of these diseases. Physical activity (PA) and exercise are now considered principal interventions for use in primary and secondary prevention of chronic diseases. Currently, more emphasis in primary prevention of disease is necessary to reduce disease risk in youth and adults; however with chronic disease prevalence so high, similar emphasis is also necessary for secondary prevention in those children and adults already inflicted with chronic diseases. Conditions such as cardiovascular disease, type 2 diabetes, obesity, and cancer are drastically improved when PA and exercise are part of a medical management plan. In addition, the national PA guidelines in conjunction with PA promotion tools like Exercise is MedicineTM are needed to promote increased PA and exercise levels worldwide. 展开更多

The advent of the Internet age also become more virtual to business models, and now the development of enterprises has involved the application of e-commerce technology, the company' s business model combines network through electronic technology to some extent, and it made great adjustments and changes in order to deal with the new face of increasingly fierce competition in the market. Thus a new term: "E-commerce" is well known and accepted as more and more people. A regional economic development need of the residents' daily consumption is driven, so local governments need to focus on the various steady development of local small and medium enterprises. AIM: To evaluate the impact of the diagnosis of hepatitis C infection on lifestyle habits such as smoking, drinking, sports activity and diet.METHODS: A self-administered, anonymous question-naire was offered to out-patients with HCV infection consecutively attending three clinical centres in Italy.RESULTS: Of the 275 respondents, 62.2% (171) were male. Mean age was 51 (range 20-80) years. Overall, after the diagnosis of hepatitis C, 74.5% of drinkers had modified (giving up or reducing) alcohol consumption, 21.3% of smokers had modifi ed their habits and 32.3% of subjects who reported sports activity had either stopped or reduced frequency of activity or chose a less fatiguing sport. Sixty-four percent of the respondents reported having modified their diet, most of them on physician's advice. CONCLUSION: After the diagnosis of hepatitis C many patients correctly modif ied their alcohol consumption and a minority their smoking habits. The reason for reported changes in diet and physical activity needs further investigations. 展开更多

Urban Transformation and Nostalgia in Merdivenkoy, Istanbul

In this paper the author will try to discuss the experiences and narratives of three different categories of families living in Merdivenkoy, Fikirtepe, the oldest settlement and suburban area of the Anatolian side of Istanbul, on the backdrop of urban transformation that will change the area soon. In life story narratives, these three examples give us information about the history, economic linkage, and life styles of neighboring families. The first category basically consists of the families that live in the area for a very long time, over some decades or more. Even one of them mentioned that, their family is living there for hundreds of years. The second category, which represents the biggest group living in the area, consists of families migrated from Sivas, a central Anatolian city, in the 1950s. Third category consist of Kurds migrated from eastern Anatolian villages and cities such as Mardin. Each category has different kinds of nostalgia about the past. Interviewees from the first category talk about family narratives related to the small village Merdivenk6y, Fikirtepe and the area. Mostly, they have a story of a "well off" family, structured and detailed narratives about the family history, including relations with the Ottoman Palace such as doing some sort of production for it. They seem very sensitive about the local cultural characteristics and what's going on in the area; realistic about and reconciled with what will happen as a result of slum clearance. Interviewees from the second category mostly emphasize the hardship of settling in a muddy suburban area, where once "muddy lands" now turned out to become valuable urban properties on which new and modem neighborhoods, shopping malls and office buildings will be built. They are expecting to get maximum gain out of the exchange of their small property, since they are desperately in need, because of the financial crisis situation. Although their houses with small gardens near the small industrial area are small, old, and neglected for years, they expect the most. Relatively newcomers, Kurdish people in the area talk about the good old days on the one hand and the painful stories about migration and settling on the other hand. They do not think about going back to their native lands, since here it is easier for them to find jobs in the informal economy as they live very close to central parts of the city. As a part of a larger research study focused on the experience of urban transformation, we made more than 50 interviews, trying to make a general outlook of the area, working on the dynamics of change before and after the urban transformation that had been started in June 2010. By using oral history research techniques and social science methodology, we conducted in-depth interviews with the people living in the neighborhood, made observations in the neighborhood and took photographs. The author's work is about the reflections of nostalgia in their family histories and experiences of different categories of families living in the area referring life story narratives. As far as the author can see in each group he works on, there are different contradictions related to their position in the city and conception of the area. The urban transformation presented and is perceived as a fortune for all of them, but it will have contradictory outcomes in reality for each as indicated in other examples in the world. 展开更多

Beyond Green Capitalism: Social-Ecological Transformation and Perspectives of a Global Green-Left 被引量:5

Given certain reactions to the ecological crisis as part of the multiple crisis of capitalism (like the so-called energy transition in Germany), a new mode of development might emerge which can be called green capitalism. This would shift the terrain of social critique and emancipatory social struggles. The paper introduces the debate on "social-ecological transformation" which emerged as a radical part of a more comprehensive debate on "great" or "societal transformation" and high- lights its core issues: the issue of a necessary attractive mode of production and living for all, the role of pioneers of change and changing political and economic institutions, the acknowledgment of shifting social practices, the requirement for alternative imaginaries or "stories" of a good life as part of a contested process which is called "futuring." As an example for alternative imaginaries, the current debate on "degrowth" is outlined and evaluated. The second part of the paper focuses more concretely on issues around the formation and existence of a global green-left. After mentioning a crucial problem for any global alternative--i.e., the structural feature of economic and geopolitical competition which historically divided the global Left and pulled it into compromises at national or regional scales--four requirements or characteristics of a global green-left are highlighted: to weaken and change capitalistically driven competition and competitiveness, to push a social-ecological transformation in democratic ways and not at the back ofordinary people (like conservative and liberal proposals for transformation tend to do), to link more systematically green issues with labor issues and, finally, to transform the overall dispositive of political action from a "distributive" to a "transformative Left." One dimension of such an enhancement, it is concluded, is a broader understanding of the "economy" itself by acknowledging the demands and achievements of a "care revolution" which will be crucial for an alternative mode of production and living. 展开更多

Most of life maintains itself through turnover, namely cell proliferation, movement and elimination. Hydra's cells, for example, disappear continuously from the ends of tenta- cles, but these cells are replenished by cell proliferation within the body. Inspired by such a biological fact, and together with various operations of polynomials, I here propose polynomial-life model toward analysis of some phenomena in multicellular organisms. Polynomial life consists of multicells that are expressed as multivariable polynomials. A cell is expressed as a term of polynomial, in which point (m, n) is described as a term zmy~ and the condition is described as its coefficient. Starting with a single term and following reductions by set of polynomials, I simulate the development from a cell to a multicell. In order to confirm uniqueness of the eventual multicell-pattern, GrSbner base can be used, which has been conventionally used to ensure uniqueness of normal form in the mathematical context. In this framework, I present various patterns through the polynomial-life model and discuss patterns maintained through turnover. Cell elimina- tion seems to play an important role in turnover, which may shed some light on cancer or regenerative medicine. 展开更多
