经食道超声心动图右心声学造影中应用手振加血激活生理盐水显影对卵圆孔未闭的诊断价值

目的 探讨经食道超声心动图右心声学造影(TEE-cTTE)中应用手振加血激活生理盐水(ASB)显影对卵圆孔未闭(PFO)的诊断价值。方法 选取2020年7月至2023年1月临汾市人民医院疑似PFO患者106例,根据相关诊断标准分为PFO组(78例)与非PFO组(28例)... 目的 探讨经食道超声心动图右心声学造影(TEE-cTTE)中应用手振加血激活生理盐水(ASB)显影对卵圆孔未闭(PFO)的诊断价值。方法 选取2020年7月至2023年1月临汾市人民医院疑似PFO患者106例,根据相关诊断标准分为PFO组(78例)与非PFO组(28例),均行TEE-cTTE检查,根据造影剂类型分为ASB、手振激活50%葡萄糖溶液(AGB)、手振激活生理盐水(AS),对比3种造影剂右心开始显影到微气泡逐渐减少到每帧图像低于10个微气泡的时间(T-R)、两组卵圆孔内径和隧道长度,分析3种造影剂行TEE-cTTE检查对PFO、PFO类型及右向左分流(RLS)的诊断价值。结果 ASB静息状态下、Valsalva动作下T-R长于AGS、AS,AGS静息状态下、Valsalva动作下T-R长于AS(P<0.05);3种造影剂检测PFO组卵圆孔内径长于非PFO组(P<0.05);ASB、AGS检测的PFO组卵圆孔内径长于AS检测(P<0.05);ROC曲线分析,ASB行TEE-cTTE诊断PFO的AUC为0.872(95%CI 0.793~0.929),敏感度为92.31%(72/78),特异度为82.14%(23/28);AGS行TEE-cTTE诊断PFO的AUC为0.842(95%CI 0.758~0.905),敏感度为89.74%(70/78),特异度为78.57%(22/28);AS行TEE-cTTE诊断PFO行TEE-cTTEAUC为0.672(95%CI 0.574~0.760),敏感度为80.77%(63/78),特异度为53.57%(15/28),提示ASB、AGS行TEE-cTTE诊断PFO的价值优于AS;ASB行TEE-cTTE检查诊断PFO类型、RLS分级与临床诊断结果的一致性较高(Kapp值=0.819、0.838);AGS行TEE-cTTE检查诊断PFO类型、RLS分级与临床诊断结果的一致性一般(Kapp值=0.653、0.655);AS行TEE-cTTE检查诊断PFO类型与临床诊断结果的一致性较差(Kapp值=0.393),诊断RLS分级与临床诊断结果的一致性一般(Kapp值=0.480)。结论 TEE-cTTE检查中应用ASB显影能延长T-R时间,提高PFO诊断价值,且在诊断PFO类型、RLS分级方面表现出与临床诊断结果较高的一致性,有利于开展具有针对性、科学性的临床诊治工作。 展开更多

目的探讨心腔内声学造影(ICCE)在单纯经胸超声心动图(TTE)引导下行心房水平缺损介入封堵术中应用的安全性及临床价值。方法回顾性分析我院2021年4月至2022年7月单纯TTE引导下经皮行心房水平缺损介入封堵术且术中予以含血激活生理盐水行I... 目的探讨心腔内声学造影(ICCE)在单纯经胸超声心动图(TTE)引导下行心房水平缺损介入封堵术中应用的安全性及临床价值。方法回顾性分析我院2021年4月至2022年7月单纯TTE引导下经皮行心房水平缺损介入封堵术且术中予以含血激活生理盐水行ICCE的病例资料,共148例,诊断房间隔缺损9例,诊断卵圆孔未闭139例,平均年龄(47.53±14.52)岁。统计患者术前基线资料,比较ICCE前后基础生命体征变化等。统计经ICCE校正后单纯TTE判断端孔导管通过卵圆孔进入左心系统正确率。结果直接予以含血激活生理盐水行ICCE前后基础生命体征结果比较,差异无统计学意义(P>0.05);单纯TTE判断端孔导管通过卵圆孔未闭的正确率,经ICCE校正为73.77%,单纯TTE判断与ICCE辅助下判断结果比较,差异有统计学意义(P<0.05)。结论ICCE以含血激活生理盐水作为造影剂是安全、可行的,辅助判断端孔导管的位置,可以极大降低单纯TTE引导下心脏介入封堵手术的难度,尤其是卵圆孔未闭封堵,并促进此术式的推广。 展开更多

AIM: To investigate the effects of moxibustion on the morphology and function of mast cells (MC) at Tianshu (ST25) in rats with trinitro-benzene-sulfonic acid (TNBS)-induced colitis. METHODS: A total of 53 male Sprague-Dawley rats were randomly divided into a normal group and experimental group. In the experimental group, a rat model of TNBS-induced colitis was established, and the rats were then randomly divided into a model group, moxi-bustion group, moxibustion plus disodium cromoglycate (M + DC) group and moxibustion plus normal saline (M+ NS) group. Rats in the moxibustion group received suspended moxibustion at bilateral ST25 for 10 min, once a day for 7 d. Rats in the M + DC and M + NS groups were pretreated with disodium cromoglycate and normal saline at bilateral ST25, respectively, and were then concurrently subjected to the same treatment as rats in the moxibustion group. The hematoxy- lin-eosin staining method was used to observe histology of the colon and the toluidine blue-improved method was used to observe mast cells at ST25 acupoint areas. RESULTS: An improvement in colonic injury in the moxibustion group was observed and the degranulation ratio of MC at ST25 acupoint was markedly higher in the moxibustion group than in the model group (45.91 ± 11.41 vs 32.58 ± 8.28, P < 0.05). After inhibition of degranulation of MC at ST25 by disodium cromoglycate, no improvement in colon tissue injury was observed. CONCLUSION: Moxibustion exerted its effect on healing impaired colonic mucosa in rats with TNBS-induced colitis by increasing the degranulation ratio of local MC, but had little effect on the morphology of MC at ST25 acupoint. 展开更多

Tetrandrine inhibits activation of rat hepatic stellate cells in vitro via transforming growth factor-β signaling 被引量:11

AIM: To investigate the effect of various concentrations of tetrandrine on activation of quiescent rat hepatic stellate cells (HSCs) and transforming growth factor-β (TGF-β) signaling in vitro.METHODS: HSCs were isolated from rats by in situperfusion of liver and 18% Nycodenz gradient centrifugation, and primarily cultured on uncoated plastic plates for 24 hwith DMEM containing 20% fetal bovine serum (FBS/DMEM) before the culture medium was substituted with 2% FBS/DMEM for another 24 h. Then, the HSCs were cultured in 2% FBS/DMEM with tetrandrine (0.25, 0.5, 1,2 mg/L, respectively). Cell morphological features were observed under an inverted microscope, smooth muscleα-actin (α-SMA) was detected by immunocytochemistry and image analysis system, laminin (LN) and type Ⅲprocollagen (PCⅢ) in supernatants were determined byradioimmunoassay. TGF-β1 mRNA, Smad 7 mRNA and Smad 7 protein were analyzed with RT-PCR and Western blotting, respectively.RESULTS: Tetrandrine at the concentrations of 0.25-2 mg/L prevented morphological transformation of HSC from the quiescent state to the activated one, while α-SMA, LN and PCⅢ expressions were inhibited. As estimated by gray values, the expression of α-SMA in tetrandrine groups (0.25, 0.5, 1, 2 mg/L) was reduced from 21.3% to 42.2%(control: 0.67, tetrandrine groups: 0.82, 0.85, 0.96, or 0.96, respectively, which were statistically different from the control, P<0.01), and the difference was more significant in tetrandrine at 1 and 2 mg/L. The content of LN in supernatants was significantly decreased in tetrandrine groups to 58.5%, 69.1%, 65.8% or 60.0% that of the control respectively, and that of PCⅢ to 84.6%, 81.5%,75.7% or 80.7% respectively (P<0.05 vs control), with no significant difference among tetrandrine groups. RTPCR showed that TGF-β1 mRNA expression was reduced by tetrandrine treatments from 56.56% to 87.90% in comparison with the control, while Smad 7 mRNA was increased 1.4-4.8 times. The TGF-β1 mRNA and Smad 7 mRNA expression was in a significant negative correlation (r= -0.755, P<0.01), and both were significantly correlated with α-SMA protein expression (r = -0.938, P<0.01;r = 0.938, P<0.01, respectively). The up-regulation of Smad 7 protein by tetrandrine (1 mg/L)was confirmed by Western blotting as well.CONCLUSION: Tetrandrine has a direct inhibiting effect on the activation of rat HSCs in culture. It up-regulates the expression of Smad 7 which in turn blocks TGF-β1 expression and signaling. 展开更多

In particular, the risk ofHBV reactivation is heightened by the use monoclonalantibodies, such as rituximab (anti-CD20) and alemtuzumab (anti-CD52) that cause profound and longlasting immunosuppression. Emerging data indicatethat HBV reactivation could also develop following theuse of other biologic agents, such as tumor necrosis factor (TNF)-α inhibitors. When HBV reactivation is di-agnosed, it is mandatory to suspend biologic treatmentand start antiviral agents immediately. However, preemptive antiviral therapy prior to monoclonal antibodyadministration is crucial in preventing HBV reactivationand its clinical consequences. Several lines of evidencehave shown that risk of HBV reactivation is greatlyreduced by the identifi cation of high-risk patients andthe use of prophylactic antiviral therapy. Gene editing nucleases including zinc-finger nucleases(ZFNs), transcription activator like effector nucleases(TALENs) and clustered regularly interspaced short palindromic repeats(CRISPR)–CRISPR-associated(Cas) system(CRISPR/Cas9) provide powerful tools that improve our ability to understand the physiological processes and their underlying mechanisms. To date, these approaches have already been widely used to generate knockout and knockin models in a large number of species. Fishes comprise nearly half of extant vertebrate species and provide excellent models for studying many aspects of biology. In this review, we present an overview of recent advances in the use of gene editing nucleases for studies of fish species. We focus particularly on the use of TALENs and CRISPR/Cas9 genome editing for studying sex determination in tilapia. 展开更多
