Application of video-assisted thoracic surgery in the standard operation for thoracic tumors

Objective： To evaluate the short-term outcomes of video-assisted thoracic surgery （VATS） for thoracic tumors. Methods： The data of 1,790 consecutive patients were retrospectively reviewed. These patients underwent VATS pulmonary resections, VATS esophagectomies, and VATS resections of mediastinal tumors or biopsies at the Cancer Institute ＆ Hospital, Chinese Academy of Medical Sciences between January 2009 and January 2012. Results： There were 33 patients converted to open thoracotomy （OT, 1.84%）. The overall morbidity and mortality rate was 2.79% （50/1790） and 0.28% （5/1790）, respectively. The overall hospitalization and chest tube duration were shorter in the VATS lobectomy group （n=949） than in the open thoracotomy （OT） lobectomy group （n=753）. There were no significant differences in morbidity rate, mortality rate and operation time between the two groups. In the esophageal cancer patients, no significant difference was found in the number of nodal dissection, chest tube duration, morbidity rate, mortality rate, and hospital length of stay between the VATS esophagectomy group （n=8 1） and open esophagectomy group （n=81）. However, the operation time was longer in the VATS esophagectomy group. In the thymoma patients, there was no significant difference in the chest tube duration, morbidity rate, mortality rate, and hospital length of stay between the VATS thymectomy group （n=41） and open thymectomy group （n=41）. However, the operation time was longer in the VATS group. The median tumor size in the VATS thymectomy group was comparable with that in the OT group. Conclusions： In early-stage （Ⅰ/Ⅱ） non-small cell lung cancer patients who underwent lobectomies, VATS is comparable with the OT approach with similar short-term outcomes. In patients with resectable esophageal cancer, VATS esophagectomy is comparable with OT esophagectomy with similar morbidity and mortality. VATS thymectomy for Masaoka stage I and II thymoma is feasible and safe, and tumor size is not contraindicated. Longer follow-ups are needed to determine the oncologic equivalency of VATS lobectomy, esophagectomy, and thymectomy for thymoma vs. OT.

Finding Consistency in Rousseau

Several of Rousseau＇s critics begin with the presupposition that his writings are inconsistent or incoherent ann proceed to locate the ＂essence＂ of his philosophy in some of his writings while excluding others. Ernst Cassirer is among the few philosophers who have attempted to defend Rousseau＇s claim to consistency. Despite its broad influence, Cassirer＇s interpretation has remained largely unchallenged. The aim of this paper is twofold. Firstly, it aims to show that Cassirer＇s interpretation undermines （1） the important role Rousseau assigns to pity in both the state of nature and civil society and （2） the significant role the general will plays in his political theory. Secondly, it proposes an alternative interpretation that succeeds in uniting Rousseau＇s opus.

Rhetorical Devices in Dialogues of The Big Bang Theory

This paper analyzes the effect of rhetorical devices, such as irony, sarcasm, parody, hyperbole, pun, antonomasia, etc., in dialogues of the American TV series The Big Bang Theory （2007） with an aim to help improve English learners＇ awareness and appreciation of rhetorical means in English-based literary works. Instances are cited with the use of such rhetoric means in the dialogue transcriptions. It is discovered that integrated use of a variety of rhetorical devices is a significant means for vivid and impressive characterization in literature. Such an analysis is a rewarding journey on either an academic or entertainment basis, and a process of discovery and learning that may particularly be well-received with EFL （English as a Foreign Language） learners

At the Crossroads： Examining a New Paradigm for Documentaries in the Twenty-First Century

The culture and history of traditional documentaries carries a set of expectations about types and topics, which are covered by the genre. Documentaries are designed to push social agendas with few thoughts to the audience reaction. They align more closely with the vision of the creators and producers, often delivering ideological expressions that present cultural, political, and historical content. However, this might be a voice from the past as television, film, and consumer-generated content finds easy voices for expression due to accessibility of technology in the digital era. The role of documentaries as a genre is in the process of changing. The challenges to this type of production are the result of both intrinsic and extrinsic factors. Production costs for creating documentaries have simultaneously soared and shrunk, depending on the accessibility of equipment, post-production facilities and distribution. Of course there are many exceptions to this rule given outlets such as film festivals and YouTube as well as Michael Moore-level auteurs. External influences center on the blurring of lines, caused by the proliferation of television programs under the ＂umbrella＂ of reality television. This, in turn, begs the question： Is reality programming the new documentary or is it a new separate genre？ This discussion will explore the overlap and similarities that exist between documentary programming and reality television programming. Furthermore, it will develop a paradigm of comparisons that re-examines the role and character of documentary production and aesthetics within the landscape of the reality television genre.

Study on intervention mechanism of acupuncture for neuron abnormal neural coding in visual cortex area 17 of monocular deprivation rats

Objective： To investigate the electrophysiological mechanism of acupuncture for the visual cortex plasticity during the sensitive period.Methods： Fifty 2-week-old Wistar rats were randomly divided into a blank control group, a model group, an early-stage acupuncture group, a middle-stage acupuncture group and a late-stage acupuncture group, 10 rats in each group.Monocular deprivation amblyopia models were prepared in rats except those in the blank control group by unilateral eyelid suture. After successful modeling, no treatment was applied to the rats in the model group. Rats in each acupuncture group were treated with acupuncture at bilateral Jingming（BL 1）, Cuanzhu（BL 2）, Fengchi（GB 20） and Guangming（GB 37）, started from the 3rd day, 12 th day or 21 st day after modeling separately, once a day, for a total of 9 d treatment. The neuronal discharge frequency and action potential inter-spike interval（ISI） in the rat visual cortex area 17 of each group were measured by multi-channel microelectrode array nerve signal technique.Results： The discharge number of neurons in the visual cortex of the model group was significantly lower than that in the blank control group（P〈0.05）. After treatment, the discharge numbers in the early-stage acupuncture group and the middle-stage acupuncture group were significantly higher than that in the model group（P〈0.05）. There was no significant difference in the discharge numbers between the late-stage acupuncture group and the model group（P〉0.05）. The discharge number in the middle-stage acupuncture group was lower than that in the early-stage acupuncture group（P〈0.05）, and the discharge number of the late-stage acupuncture group was lower than that in the middle-stage acupuncture group（P〈0.05）. The ISI sequences of the visual cortex neurons were mainly distributed under 0.3 s in the blank control group, under 15 s in the model group, under 1 s in the early-stage acupuncture group, under 4 s in the middle-stage acupuncture group, and under 10 s in the late-stage acupuncture group, divergent in each group.Conclusion： The neuronal coding appears abnormality in the visual cortex area 17 of monocular deprivation rats, indicating that there is a plasticity change in the visual cortex neurons during the sensitive period. Acupuncture has a significant effect on the abnormal neural coding. The therapeutic efficacy is closely related to the stage to start the treatment. Early stage treatment in the sensitive period is the key to achieving the good efficacy.
