包皮环切缝合器手术与包皮套环环扎手术临床疗效对比分析

目的观察包皮环切缝合器手术与包皮套环环扎手术治疗包茎及包皮过长的疗效。方法将2015年1～8月在该院门诊行包皮环切缝合器手术或包皮套环环扎手术的200例患者分为A组（94例）和B组（106例）。A组行包皮环切缝合器手术,B组行包皮套环环扎手术。结果 A组患者术中出血量（0～5 m L）、手术时间（5～7 min）、术后疼痛视觉模拟评分（0～3分）、术后并发症发生率[1.1%（1/94）]、愈合时间（5～10 d）、术后外观满意度[100.0%（94/94）]等均优于B组[分别为0～15 m L、7～15 min、6～8分、12.3%（13/106）、11～17 d、89.6%（95/106）]。结论包皮环切缝合器手术与包皮套环环扎手术均对包皮过长或包茎具有临床治疗意义,相比之下,包皮环切缝合器手术具有手术时间短、出血少、术中术后痛苦小、愈合时间短、外形美观等临床特点,真正做到了包皮微创手术。

Men＇s Perspectives on the Relationship Between Sexuality and Female Genital Mutilation in Egypt

This paper addresses men＇s perspectives on the relationship between female genital mutilation/cutting （FGM/C） and women＇s sexuality in Egypt with the purpose of studying how men and women see a relationship between FGM/C and sexual life, The study used qualitative methods and was conducted in three sites. Men saw FGM/C as the key to ensuring women＇s sexual morality, and considered FGM/C as the key to controlling sexual relationships. Men were reported to increasingly take part in the decision-making about daughters＇ FGM/C. In men＇s view, FGM/C reduces female sex drive and thereby keeps girls chaste before marriage and women faithful during marriage. Men wholeheartedly support FGM/C for these reasons. However, others are ambivalent due to a concern that the practice might have negative consequences on their own sexuality as a consequence of a negative effect on their wives＇ sexual feelings. The study concludes that men＇s role is important and that they should be involved in community activities against the practice. It also concludes that without comprehensive sexuality education, men will continue to hold on to the misconceptions that FGM/C has a positive effect on women＇s sexual morality through reducing their sexual desire.

Evaluation of the Effectiveness of Nd:YAG Laser under Urethroscope in the Treatment of Intraurethral Condyloma Acuminata (CA) in Males

Objective: To evaluate the therapeutic effect of Nd:YAGlaser under urethroscope for the treatment of intraurethralCandyloma Acuminata (CA) in males. Method: Nd:YAG laser under urethroscope was employed toremove the warts. Result: Seventeen patients out of eighteen cases with CAwere cured after the first procedure. The cure rate was 94.4%.For second treatment, the cure rate was 100% and no relapseoccurred after observation for 12-24 months. Conclusion: Nd:YAG laser by urethroscope is a very goodmethod for treatment of intraurethral CA.

Illusory Control： Female’s Sexuality and Male’s Fantasy in The Changeling

Thomas Middleton and William Rowley’s The Changeling enacts an astonishment on the stage to the audience about the protagonist Beatrice-Joanna’s down fall-the noble lady being straightly encroached by her desire and being manipulated by the obscene servant De Flores, who, by means of rape, controls her body and finally destroys her. My article attempts to explore male’s expectations and their power over female’s sexuality with Jacques Lacan’s psychoanalytical approach. Moreover, it will elaborate that Beatrice-Joanna’s tragic outcome is not only contributed by her murder and moral degradations, but also, before her transgression being committed, her body has already been regulated by the patriarchal power in the Renaissance society-either she is valuable in the marriage market with her chastity or she is condemned by losing it. Also, the science of medicine employed to test virginity represents the male’s methodological hegemony and anxiety in facing the unknown of a female’s chastity. For the fantasy always regulating the male’s power structure over female’s body, the real manipulation over female sexuality is doomed to be an illusion.

包皮环切术后商环拆除方法与疼痛的观察

目的通过对中国商环包皮环切术后不同拆环方法疼痛评分比较性研究,探索最佳拆环方法。方法按公开发表文献中介绍的3种拆环方法将284名包皮环切术后患者随机分组,采用国际通用的视觉模拟评分法(VAS)进行疼痛评分并组间比较。结果 3种方法中,温盐水浸泡法拆环疼痛评分最低,与其他2种方法比较,差异有统计学意义(P<0.01)。结论温盐水浸泡不仅能充分软化痂面、易于拆环,且降低患者疼痛的敏感性。操作简便易行,患者依从性高,减痛效果好。

中国商环包皮环切术的学习曲线

目的评估同一医师在不同阶段应用中国商环进行包皮环切术的手术效果，探讨中国商环包皮环切术的学习曲线。方法回顾分析本院为同一医师完成的30例中国商环包皮环切术的临床资料。按手术先后次序分为A、B和C3组，每组10例，以10例为一手术学习曲线阶段。比较各阶段的手术时间、术中出血量、术中和术后并发症发生率、中转传统包皮环切手术率、术后受者对外观满意率，分析不同阶段的手术效果。结果A组手术时间（10．5±2．4）min显著长于B组手术时间（5．6±1．8）min和C组手术时间（5．1±1．5）min（P〈0．01）；B组手术时间长于C组手术时间，但差异无统计学意义（P〉0．05）；各组出血量相比差异无统计学意义（P〉0．05）。A组1例在剪除过长包皮时损伤阴茎干背侧皮肤，2例改传统包皮环切术。B组和C组术中无并发症，无中转传统包皮环切手术的病例。A组有1例受者对术后外观不满意，B组和C组受者均对术后外观满意。结论中国商环包皮环切术是对传统包皮环切术的简化和改进，通过约10例的训练后即可达到较熟练程度。

外科门诊应用中国商环行包皮环切术临床分析

目的分析应用中国商环行包皮环切术与传统包皮环切手术。方法将128例包茎、包皮过长患者分两组，中国商环行包皮环切组（中国商环组）80例和传统包皮环切手术组（传统环切组）48例，比较两种术式的优缺点。结果中国商环组在手术时间、术中出血量、术后治疗优于传统组，术后不需换药，口服抗生素，伤口无出血、血肿，系带完整，阴茎外观满意，切缘整齐无瘢痕。传统环切组术后需换药输液抗炎治疗，2例切口出血，1例系带损伤，5例切缘不齐（P均〈0．05）。结论应用中国商环行包皮环切手术有明显优越性。

包皮内板环切治疗早泄疗效观察

目的探讨包皮内板环切对早泄的治疗作用。方法早泄患者54例,随机分为A、B两组,A组28例为自愿行包皮内板环切组,B组26例为综合治疗组。从射精潜伏期及性生活满意度来评价A、B两组的治疗效果,进行比较。结果 A组切口均I期愈合。A、B两组患者症状均有明显的改善,A组有效率为53.6%;B组有效率为50%,两组结果无统计学意义(P>0.05)。结论包皮内板环切能有效治疗早泄,其方法简便、易于操作、痛苦小、疗程短,值得临床应用。

商环包皮环切200例术后处理

目的探讨改进并注重商环包皮环切吻合术术后处理对降低术后并发症的发生的积极作用。方法本组选择年龄2～65岁患者200例,其中包茎28例,包皮过长172例,改进前术后处理方案组60例,改进术后处理方案后组140例,均采用一次性包皮环切吻合器行包皮环切吻合术,并于术后采用相应的术后处理方案治疗。结果采用改进方案组患者较改进前方案组患者手术的并发症有明显的降低。结论对商环包皮环切吻合术后患者的处理进行相应的改进和注重对降低手术并发症,减轻患者痛苦,提高患者满意度有明显效果。
