In inflammatory conditions, the number, diameter and density of isolated lymphoid follicles (ILFs) increases. They are not only involved in immune surveillance, but their presence is also indispensable in normal mucosal regeneration of the colon. In carcinogenesis, ILFs may play a dual role. On the one hand they may support tumor growth and the metastatic process by vascular endothelial growth factor receptor signaling and producing a specific cytokine and cellular milieu, but on the other hand their presence is sometimes associated with a better prognosis. The relation of ILFs to bone marrow derived stem cells, follicular dendritic cells, subepithelial myofibroblasts or crypt formation, which are all involved in mucosal repair and carcinogenesis, has not been directly studied. Inflammation of axial and/or peripheral joints is one of the most frequent extra-intestinal manifestations complicating the clinical course and therapeutic approach in inflammatory bowel diseases(IBD).The frequency of...Inflammation of axial and/or peripheral joints is one of the most frequent extra-intestinal manifestations complicating the clinical course and therapeutic approach in inflammatory bowel diseases(IBD).The frequency of these complications seems to be similar for both diseases, Crohn's disease and ulcerative colitis.Arthritis associated with IBD belongs to the category of spondyloarthropathies.Axial involvement ranges from isolated inflammatory back pain to ankylosing spondylitis, whereas peripheral arthritis is noted in pauciarticular and in polyarticular disease.Asymptomatic radiological involvement of the sacroiliac joints is reported to occur in up to 50% of patients.Other musculoskeletal manifestations such as buttock pain, dactylitis, calcaneal enthesitis, and thoracic pain are frequently underdiagnosed and, consequently, are not treated appropriately.Several diagnostic approaches and criteria have been proposed over the past 40 years in an attempt to correctly classify and diagnose such manifestations.The correct recognition of spondylarthropathies needs an integrated multidisciplinary approach in order to identify common therapeutic strategies, especially in the era of the new biologic therapies.展开更多
Objective The aim of this research was to study the prognostic significance of the number of pelvic lymph nodes removed in patients with early cervical cancer.Methods We searched the Pub Med database using the terms &...Objective The aim of this research was to study the prognostic significance of the number of pelvic lymph nodes removed in patients with early cervical cancer.Methods We searched the Pub Med database using the terms "cervical cancer" and "lymph nodes" or "lymphadenectomy". Studies on the association between number of lymph nodes removed and prognosis or survival were identified. We retrospectively studied the relevant research.Results Ten retrospective studies were included. Two studies indicated that the number of lymph nodes had no association with prognosis whereas three studies found a positive relationship. Five studies indicated some factors that could influence the relationship between number of lymph nodes and prognosis.Conclusion The number of lymph nodes removed may positively influence the prognosis of patients with cervical cancer. Some factors may influence the relationship between the extent of lymph nodes removed and patient prognosis. Additional multicenter, prospective studies with large samples are required to confirm the study findings.展开更多
