Transit time and balloon distension test were performed.RESULTS: 34.5% patients were classified as slow transit type of constipation. The total colonic transit time (56 h vs 10 h, P<0.0001) and rectal sensation including urge sensation (79 mL vs 63 mL, P = 0.019) and maximum tolerable volume (110 mL vs95 mL, P = 0.03) differed in patients and controls. Constipated subjects had significantly higher anxiety and depression scores and lower SF-36 scores in all categories. They also demonstrated higher scores of'monitoring' coping strategy (14+6 vs9+3, P = 0.001),which correlated with the rectal distension sensation (P = 0.005), urge sensation (P=0.002), and maximum tolerable volume (P = 0.035). The less use of blunting strategy predicted slow transit constipation in both univariate (P = 0.01) and multivariate analysis (P = 0.03).CONCLUSION: Defective or ineffective use of coping strategies may be an important etiology in functional constipation and subsequently reflected in abnormal anorectal physiology.展开更多
AIM:To define the topography of mast cells and their numbers in cases of Hirschsprung's disease(HD)and non-HD,assess neural hypertrophy using imaging software and to study the relationship between mast cells and n...AIM:To define the topography of mast cells and their numbers in cases of Hirschsprung's disease(HD)and non-HD,assess neural hypertrophy using imaging software and to study the relationship between mast cells and nerve fibers.METHODS:HE stained sections of 32 cases of chronic constipation in the age group of 0-14 years were reviewed for ganglion cells.AChE staining was performed on frozen sections of colonic and rectal biopsies.Based on their findings cases were divided into HD and non-HD and mast cells stained by toluidine blue were evaluated.Image analysis by computerized software was applied to S-100 stained sections for assessment of neural hypertrophy.RESULTS:Difference between number of mast cells in HD group(mean=36.44)and in non-HD group(mean =14.79)was statistically significant.Image analysis morphometry on S-100 stained sections served as a useful adjunct.The difference between number,size,and perimeter of the nerve fibers between HD and non-HD group was statistically significant.CONCLUSION:Mast cells are significantly increased in HD and their base line values are much higher in Indian children than that reported in Western literature.Their role in HD needs further research.Morphometry of S-100 stained nerve fibers is a useful adjunct to conventional methods for diagnosis of HD.展开更多

The main feature of a high-fiber diet is its poor digestibili...Whilst fruits and vegetables are an essential part of our dietary intake, the role of fiber in the prevention of colorectal diseases remains controversial. The main feature of a high-fiber diet is its poor digestibility. Soluble fiber like pectins, guar and ispaghula produce viscous solutions in the gastrointestinal tract delaying small bowel absorption and transit. Insoluble fiber, on the other hand, pass largely unaltered through the gut. The more fiber is ingested, the more stools will have to be passed. Fermentation in the intestines results in build up of large amounts of gases in the colon. This article reviews the physiology of ingestion of fiber and defecation. It also looks into the impact of dietary fiber on various colorectal diseases. A strong case cannot be made for a protective effect of dietary fiber against colorectal polyp or cancer. Neither has fiber been found to be useful in chronic constipation and irritable bowel syndrome. It is also not useful in the treatment of perianal conditions. The diverticulosis theory should also be challenged. The authors urge clinicians to keep an open mind about fiber. One must be aware of the truths and myths about fiber before recommending it.展开更多

However, the definition of megarectum is variable, and a few data exist for Italy.We studied anorectal manometric variables and sensation in a group of constipated children with megarectum defined by radiologic criteria. Data from this group were compared with those obtained in a similar group of children with recurrent abdominal pain.METHODS: Anorectal testing was carried out in both groups by standard manometric technique and rectal balloon expulsion test.RESULTS: Megarectum patients displayed discrete abnormalities of anorectal variables and sensation with respect to controls. In particular, the pelvic floor function appeared to be impaired in most patients.CONCLUSION: Constipated children with megarectum have abnormal anorectal function and sensation. In this scheme, the secret messages can be encoded by employing four two-particle unitary operatio...A two-step deterministic secure quantum communication (DSQC) scheme using blocks of three-qubit W state is proposed. In this scheme, the secret messages can be encoded by employing four two-particle unitary operations and directly decoded by utilizing the corresponding measurements in Bell basis or single-particle basis. Comparing with most previous DSQC protocols, the present scheme has a high total efficiency, which comes up to 50%. Apart from this, it has still the advantages of high capacity as each W state can carry two bits of secret information, and high intrinsic efficiency because almost all the instances are useful. Furthermore, the security of this communication can be ensured by the decoy particle checking technique and the two-step transmitting idea.展开更多

The scheme is devised...In this paper, we show that a(2, 3) discrete variable threshold quantum secret sharing scheme of secure direct communication can be achieved based on recurrence using the same devices as in BB84. The scheme is devised by first placing the shares of smaller secret pieces into the shares of the largest secret piece, converting the shares of the largest secret piece into corresponding quantum state sequences, inserting nonorthogonal state particles into the quantum state sequences with the purpose of detecting eavesdropping, and finally sending the new quantum state sequences to the three participants respectively. Consequently, every particle can on average carry up to 1.5-bit messages due to the use of recurrence. The control codes are randomly prepared using the way to generate fountain codes with pre-shared source codes between Alice and Bob, making three participants can detect eavesdropping by themselves without sending classical messages to Alice. Due to the flexible encoding, our scheme is also dynamic, which means that it allows the participants to join and leave freely.展开更多

Methods One hundred cases of constipation were randomly divided into an acupuncture-moxibustion group and a simple acupuncture group with 50 cases for each group. Two groups of acupoints were adopted every other day alternatively for both groups. Point group 1: Tiansha (天枢ST 25), Daheng (大横 SP 15), Qihai (气海 CV 6), Guanyuan (关元 CV 4), Ztisanli (足三里 ST 36), Shangjhxu (上巨虚 ST 37) and Sanyinjiao (三阴交 SP 6). Point group 2: Zhsngliao (中髎 BL 33), Xiatliao (下髎 BL 34), Dachangsha (大肠俞 BL 25), Shenshu (肾俞 BL 23) and Pisha (脾俞 BL 20). Acupuncture was the only therapy adopted for acupuncture group on all the above mentioned points. While, for acupuncture-moxibustion group, moxibustion with grain-shaped moxa cones was applied on Qihai (所海 CV 6), Zusatnli (足三里 ST 36), Datchangshu (大肠俞 BL 25) and Pishu (脾俞 BL 20), and acupuncture was applied on the rest points. Clinical therapeutic effect, clinical score of constipation, quality of life and score of psychological symptoms were observed before and after the treatment. Results The total effective rate in acupuncture-moxihustion group was 74.0%(37/50), which was superior to that (52.0%, 26/50) in acupuncture group (P〈0.05). The clinical scores of constipation, quality of life and scores of psychological symptoms of both groups improved after the treatment. For clinical scores of constipation, degree of difficulty in bowel movement, duration of defecation, abdominal pain, difficulty in exsufflation, incomplete sensation after defecation, obstruction sense of anus, purgative prescription dependence, score of life quality and psychological symptoms of acupuneture-moxibustion group were all better than those of simple acupuncture group (P〈0.05, P〈0.01). Conclusion The combined therapy of both moxibustion with grain-shaped moxa cones and acupuncture is safe and effective in treatment of chronic functional constipation, and the effect is superior to simple therapy of acupuncture.展开更多
