目的:探讨慢性泪囊炎合并睑板腺功能障碍患者通过泪道激光成型并置管术及术后联合睑板腺治疗的疗效。方法:收集我院2021-03/2022-12慢性泪囊炎合并睑板腺功能障碍的患者128例,均行泪道激光成型联合置管术;术后随机分为2组,A组64例(无睑...目的:探讨慢性泪囊炎合并睑板腺功能障碍患者通过泪道激光成型并置管术及术后联合睑板腺治疗的疗效。方法:收集我院2021-03/2022-12慢性泪囊炎合并睑板腺功能障碍的患者128例,均行泪道激光成型联合置管术;术后随机分为2组,A组64例(无睑板腺治疗)、B组64例(睑板腺治疗),术后3 mo取出泪道置管后评价两组泪道冲洗通畅率;并对比两组眼表疾病指数(OSDI)评分、非侵袭性泪膜破裂时间、泪河高度、眼红分析、睑板腺分析、泪液脂质层厚度、泪液蕨类试验、结膜印记细胞检查的变化情况。结果:A组与B组泪道冲洗通畅率分别为78.1%、81.2%,两者比较无差异(P>0.05);与A组比较,术后3 mo B组患者非侵袭性泪膜破裂时间延长,OSDI评分、眼红分析、泪液蕨类试验及结膜印记细胞学分级降低(均P<0.05),而泪河高度、泪液脂质层厚度、睑板腺缺失评分两者无明显差异(均P>0.05)。结论:慢性泪囊炎合并睑板腺功能障碍患者综合治疗后其舒适度、泪膜稳定性、局部炎症反应有所好转,这类患者手术治疗的同时应同步重视眼表微环境异常的改善,以期达到满意的疗效。展开更多

Methods: 62 patients diagnosed with meibomian gland dysfunction dry eye syndrome in our hospital from October 2022 to December 2023 were randomly divided into a control group (31 cases) and an observation group (31 cases). Both the control group and the observation group received traditional basic treatment, while the observation group received IPL treatment on top of the control group. The effects of the two groups before and after treatment were compared. Results: After 2 months of treatment, the eye surface disease index score and meibomian gland function scores of both groups decreased compared to before treatment, and the observation group was lower than the control group (P < 0.05). Both groups showed improvement in tear film breakup time (TBUT), tear secretion test (SIT), and corneal fluorescein sodium staining (CFS) compared to before treatment, and the observation group had significantly longer TBUT and SIT than the control group, while CFS was lower than the control group (P < 0.05). Conclusion: The treatment of meibomian gland functional dry eye with strong pulsed light has a significant effect.展开更多
目的:系统评价中药熏蒸结合睑板腺按摩对睑板腺功能障碍性干眼患者的临床疗效。方法:对中国知网、万方数据库、维普中文科技期刊全文数据库和中国生物医学文献服务系统数据库以及PubMed、Web of Science、The Cochrane Library等外文数...目的:系统评价中药熏蒸结合睑板腺按摩对睑板腺功能障碍性干眼患者的临床疗效。方法:对中国知网、万方数据库、维普中文科技期刊全文数据库和中国生物医学文献服务系统数据库以及PubMed、Web of Science、The Cochrane Library等外文数据库进行检索。纳入采用中药熏蒸结合睑板腺按摩,对睑板腺功能障碍性干眼患者进行随机对照试验。参照Cochrane手册来评价研究有无偏倚,并采用RevMan 5.4软件对总有效率、泪膜破裂时间、泪液分泌试验和眼表疾病指数得分等结局指标进行Meta分析。结果:纳入7个研究共559人。Meta分析结果显示,与对照组相比,治疗组可显著提高干眼患者的临床治愈率[RR=1.31,95%CI(1.19,1.44),P<0.01],延长泪膜破裂时间[MD=1.36,95%CI(1.08,1.65),P<0.01],增加泪液分泌量[MD=2.18,95%CI(1.82,2.54),P<0.01],缓解临床症状[MD=-5.24,95%CI(-7.03,-3.54),P<0.01]。结论:采用中药熏蒸结合睑板腺按摩的方法对睑板腺功能障碍性干眼患者进行治疗,在一定程度上能提高患者的临床总有效率、延长泪膜破裂的时间、增加泪液的分泌量、改善患者的临床症状。与常规疗法相比,能提高总体疗效,更好地改善患者的症状。展开更多
