为了深入研究将针对性护理应用于反流性食管炎患者后,患者治疗依从性、心理状态及睡眠质量。方法:选取我院2019-01-2021-01这一时间段内收治的反流性食管炎患者共74例,研究组(n=37)给予护理,参照组(n=37)给予常规护理。对比两组治疗依... 为了深入研究将针对性护理应用于反流性食管炎患者后,患者治疗依从性、心理状态及睡眠质量。方法:选取我院2019-01-2021-01这一时间段内收治的反流性食管炎患者共74例,研究组(n=37)给予护理,参照组(n=37)给予常规护理。对比两组治疗依从性、心理状态及睡眠质量。结果:干预期结束后,依从性方面,研究组总的治疗依从性为94.59%,参照组总的治疗依从性为67.57%,研究组治疗依从性显著较优;睡眠质量方面,研究组睡眠质量显著较优,心理状态方面,干预后,研究组SAS为(39.87±2.79),SDS为(35.43±2.17),参照组SAS为(47.61±2.99),SDS为(42.18±2.29),干预后,研究组心理状态显著优于参照组。差异均有统计学意义(P<0.05)。结论:护理可有效改善患者治疗依从性、心理状态及睡眠质量,故方案值得推广。 展开更多

Objective:To observe the clinical efficacy of auricular needle-embedding therapy for treating primary insomnia.Methods:A total of 63 patients were randomly divided into a conventional acupuncture group and an auricular needleembedding group.The conventional acupuncture group received acupuncture at meridian points,while the auricular needle-embedding group received acupuncture at auricular points.Treatments were given once a day for 6 consecutive days,followed by a 1-day break,as a course of treatment.Both groups were treated for 2 courses.Before treatment,and after 1 course and 2 courses of treatment,the Pittsburgh sleep quality index(PSQI)score was assessed,and the efficacy was evaluated.Results:The cured and markedly effective rate and total effective rate of the auricular needle-embedding group were higher than those of the conventional acupuncture group,but there was no significant difference between the two groups(P>0.05).After 1 course of treatment,the PSQI global score and the scores of subjective sleep quality,sleep latency,sleep duration,habitual sleep efficiency,and daytime dysfunction of both groups decreased compared with those before treatment(P<0.01);there was no statistical significance in comparing the PSQI global score and individual component scores between the two groups(P>0.05).After 2 courses of treatment,the PSQI global score and the scores of sleep latency and habitual sleep efficiency of the auricular needle-embedding group decreased compared with those after 1 course of treatment(P<0.01 or P<0.05),while only the score of sleep latency of the conventional acupuncture group decreased compared with that after 1 course of treatment(P<0.05);the PSQI global score and the scores of subjective sleep quality and sleep latency of the auricular needle-embedding group were lower than those of the conventional acupuncture group(P<0.05).Conclusion:Both therapies can improve insomnia.Compared to conventional acupuncture,auricular needleembedding therapy demonstrates a therapeutic advantage in improving sleep latency and sleep quality,making it worthy of clinical promotion. 展开更多

Objective Measuring the serum concentrations of adrenocorticotropic hormone (ACTH) and cortisol in epileptic seizures during sleep to investigate their link to the EEG changes. Methods Pre-surgical evaluation was performed by videoEEG monitoring using 24 channel recording. Thirty six epilepsy patients could be attributed to two groups: 28 patients had spontaneous seizures, and the other 8 patients whose seizures were induced by bemegride. Another 11 persons with confirmed psychogenic non-epileptic seizures (PNES) served as control group. Blood samples were obtained at five points: wake (08:00 a.m.), sleep (00:00 a.m.), and shortly before, during and after an epileptic seizure. The serum ACTH and cortisol were measured and analyzed by chemiluminescent immunoassay. Results The levels of ACTH and cortisol in serum underwent significant changes: declining below the average sleep-level shortly before seizures, increasing during seizures, and far above the average wake-level after seizures (P 〈 0.001). Such changes did not occur in the control group (P 〉 0.05). The ACTH and cortisol levels had no significant difference between spontaneous group and bemegride-induced group (P 〉 0.05). Conclusion The serum concentrations of ACTH and cortisol during sleep seizures are linked with pre-ictal and ictal EEG changes in epilepsy patients. 展开更多

AIM:To examine the links between quality of sleep and the severity of intestinal symptoms in irritable bow-el syndrome(IBS).METHODS:One hundred and forty-two outpatients(110female,32 male)who met the Rome Ⅲ criteria for IBS with no psychiatric comorbidity were consecutively en-rolled in this study.Data on age,body mass index(BMI),and a set of life-habit variables were recorded,and IBS symptoms and sleep quality were evaluated using the questionnaires IBS Symptom Severity Score(IBS-SSS)and Pittsburgh Sleep Quality Index(PSQI).The associa-tion between severity of IBS and sleep disturbances was evaluated by comparing the global IBS-SSS and PSQI score(Pearson's correlation and Fisher's exact test)and then analyzing the individual items of the IBS-SSS and PSQI questionnaires by a unitary bowel-sleep model based on item response theory(IRT).RESULTS:IBS-SSS ranged from mild to severe(120-470).The global PSQI score ranged from 1 to 17(median 5),and 60 patients were found to be poor sleepers(PSQI>5).The correlation between the global IBS-SSS and PSQI score indicated a weak association(r=0.2 and 95% CI:-0.03 to 0.35,P<0.05),which becomes stronger using our unitary model.Indeed,the IBS and sleep disturbances severities,estimated as latent variables,resulted significantly high intra-subject cor-relation(posterior mean of r=0.45 and 95% CI:0.17 to 0.70,P<0.05).Moreover,the correlations between patient features(age,sex,BMI,daily coffee and alcohol intake)and IBS and sleep disturbances were also ana-lyzed through our unitary model.Age was a signif icant regressor,with patients≤50 years old showing more severe bowel disturbances(posterior mean=-0.38,P<0.05)and less severe sleep disturbances(posterior mean=0.49,P<0.05)than older patients.Higher daily coffee intake was correlated with a lower sever-ity of bowel disturbances(posterior mean=-0.31,P<0.05).Sex(female)and daily alcohol intake(modest)were correlated with less severe sleep disturbances.CONCLUSION:The unitary bowel-sleep model based on IRT revealed a strong positive correlation between the severity of IBS symptoms and sleep disturbances. 展开更多

It is not clear whether exercise can improve sleep quality and whether intensity of exercise affects any of the effects. Methods: Fifteen healthy, non-obese (body mass index = 24.4 ± 2.1 kg/m^2, mean 4- SD), sedentary (〈20 min of exercise on no more than 3 times/week) older women (66.1 ± 3.9 years) volunteered for the study. Peak oxygen consumption (VO2peak) was evaluated using a graded exercise test on a treadmill with a metabolic cart. Following a 7-day baseline period, each participant completed two exercise sessions (separated by 1 week) with equal caloric expenditure, but at different intensities (60% and 45% VO2peak, sequence randomized) between 9:00 and 11:00 am. A wrist ActiGraph monitor was used to assess sleep at baseline and two nights following each exercise session. Results: The average duration of the exercise was 54 and 72 rain, respectively at 60% (moderate-intensity) and 45% VO2peak (light-intensity). Wake time after sleep onset was significantly shorter (p = 0.016), the number of awakenings was less (p = 0.046), and total activity counts were lower (p = 0.05) after the moderate-intensity exercise compared to baseline no-exercise condition. Conclusion: Our data showed that a single moderate-intensity aerobic exercise session improved sleep quality in older women. 展开更多

During the period of reconstruction after Yushu Earthquake, a large number of sea-level or lowland workers ascended there and worked at altitudes between 3 750 m and 4 878 m which is a hypoxic environment. To investigate the sleep quality at that altitude, we performed two full polysomnographies (PSGs) in 10 volunteers, who were healthy male workers, aged 31 :i:6.6, born and living at sea level, without experience of pre-altitude expo- sure. The assessment of subjective sleep quality was performed twice in each volunteer. The first investigations were carried out at sea level in Jinan city (pB=760 torr, 1 torr=133.322 4 Pa). The second studies were performed at an altitude of 3 750 m (pB=416 tort) in Yushu Jiegu in the same 10 workers after they lived and worked at that alti- tude for 5 months. At sea level, workers presented a normal sleep structure and a higher oxygenation during sleep. However, as compared to sea-level sleep, at 3 750 m, workers had a shorter total sleep time (TST) (p 〈 0.001), a longer stage 1 non-rapid eye movement (nREM) sleep (p 〈 0.05) and a shorter 3+4 nREM and rapid eye movement (REM) sleep (p 〈 0.05) with a severe sleep hypoxemia (p 〈 0.01). Our data suggested that sea-level workers revealed a disturbed sleep and a bad sleep quality with a significant sleep hypoxemia at altitude of 3 750 m. Strengthening the prevention and treatment are thereby sorely necessary. 展开更多

This paper investigates on the base stations(BSs) sleeping control and energy saving in wireless network. The objective is to find the sleeping control and energy saving configuration between total power consumption and average video's quality. On the Software Defined Network(SDN) access network architecture, a type of sleeping control and active BSs' optimal transmitting time strategy is considered, the BS sleeps when there is no active users, and wakes up after a period of vacation time. In this paper, we study the active users grouping strategy, In order to spare more BSs into sleeping mode. Then this paper proposes an active BS transmitting time optimal strategy according to the users' Qo S. In the proposed strategy, the active BSs' transmitting time is minimized in order to save energy. This paper employs the mixed integer-programming model to present this optimization problem. Then we utilized a novel algorithm to save the energy in access networks and also meet the Qo S requirements. Both the analytical and simulation results show that the algorithm can effectively save energy in the access network BSs. 展开更多

Methods: We designed a questionnaires composed of general information, health-related factors, diseases, etc. Insomnia manifestations included regular sleep time, shorten sleep time and extent, time of falling asleep, sleep quality, difficultly returning to sleep after awakening at mid-night, insomnia time, etc. Results: A total of 4759 questionnaires (94.2%) were reclaimed. Dyssomnia was found in 1843 persons (38.7%), including 1006 cases of transient insomnia (44.0%) and 837 cases of acute insomnia (36.6%). Conclusion: About one third of the examined military retired cadres have sleep problems, so they should enhance the health care consciousness and receive proper nurse and treatment. 展开更多

Objective:To study the effect of acupuncture on depression and insomnia of malignant tumor patients.Methods:Eighty cases of cancer related depression and sleep disorders were randomly divided into 2 groups:treatmentl group(acupuncture n=40) and control group(Fluoxetione n=40).All patients were assessed by Self-rating Depression Scale(SDS),Hemilton Depression Rating Scale(HAMD) and Pittsburgh Sleep Quality Index(PSQI) after treatment for 30 days.Results:Before treatment the SDS and HAMD scores of the trial group were 64.12±5.34 and 20.92±2.38,those of the control group were 64.24±4.98 and 20.72±2.74,after treatment the SDS and HAMD scores of the trial group were 43.64±5.28 and 9.88±1.27 respectively,significantly lower than those of the control group(50.76±5.42 and 13.72±2.05 respectively,both P<0.05).Before treatment the PSQI score of the trial group and the control group were 14.48±1.71 and13.92±2.59,after treatment the PSQI score of the trial group was 7.92±1.22 after treatment,significantly lower than those of the control group(11.44±1.89,P<0.01).Conclusion:Acupuncture can effectively reduce malignant-related depression,improve sleep quality,and help to improve the quality of life of cancer patients. 展开更多

METHODS: One hundred and twenty patients were randomly divided into an experiment group and a control group. Sixty patients in the experiment group were treated once a day with acupuncture at Baihui (GV 20), Sishencong (EX-HN 1), Shenmai (BL 62), and Zhaohai (KI 6) and with moxibustion at Baihui (GV 20) and Sishencong (EX-HN 1). Sixty patients in the control group were acupunctured once a day at Shenmen (HT 7), Neiguan (PC 6), and Sanyinjiao (SP 6).The Pittsburgh Sleep Quality Index (PSQI) was used to compare sleep improvement between the two groups. RESULTS: The total effective rate was 87.7% in the experiment group and 76.3% in the control group. The PSQI scores and the total score were lower after treatment than before treatment in bothgroups. However, the reduction in the experiment group was greater than that in the control group in sleeping quality, time to fall asleep, sleeping disorder, and daytime function (P<0.05). CONCLUSION: Acupuncture and moxibustion at Baihui (GV 20), Sishencong (EX-HN 1), Shenmai (BL 62), and Zhaohai (KI 6) significantly improved insomnia symptoms in the experiment group compared with the control group. 展开更多

OBJECTIVE: To evaluate the clinical efficacy of Zhenjingdingzhi decoction in treating insomnia with Qi-deficiency of heart and gallbladder.METHODS: We conducted a double-blind,randomized,controlled trial involving 100 patients with insomnia of Qi-deficiency of heart and gallbladder.Patients were randomly divided into the treatment group(n = 50) and the control group(n = 50) according to a random number table. The treatment group was given Zhenjingdingzhi decoction,while the control group was treated with Suanzaoren decoction. the pharmacological treatment lasted for 8weeks. The clinical efficacy was assessed by using Spiegel scale,Pittsburgh sleep quality index(PSQI)and Traditional Chinese Medicine(TCM) syndrome scores.RESULTS: Comparing Spiegel scores between the two groups at 4 and 8 weeks,the differences in curative effect between the two groups were both significant(both P < 0.05). The total effective rate was46% in the treatment group and 27.7% in the control group at 4 weeks,and 80% and 53.2% at 8weeks,respectively; After 8 weeks,PSQI scores showed that the total effective rates differed significantly between the two groups(P < 0.01): 84% in the treatment group and 59.6% in the control group; In improving sleep quality and sleep duration,the curative effect of the treatment group was better than that of the control group(P < 0.05).TCM syndrome,especially insomnia and palpitation,was improved better in the treatment group after 8 weeks as compared to that in the control group(P < 0.05). The total effective rate of the two groups was 84% and 66%,respectively.CONCLUSION: Zhenjingdingzhi decoction is effective and safe for the treatment of insomnia with Qi-deficiency of heart and gallbladder,especially for improving sleep quality and sleep duration. 展开更多

Objective To observe the effects of acupoint sticking therapy of different dosages and durations on the subjective and objective sleep indicators in insomnia patients.Methods Ninety-six patients with insomnia due to liver-Qi stagnation and spleen deficiency were divided into a high-dosage 7 d group(25 cases),a high-dosage 14 d group(22 cases),a low-dosage 7 d group(21 cases),and a low-dosage 14 d group(28 cases)using the random numbers generated in a stratified and stage-by-stage manner in combination with the visiting sequence.The four groups all received the same acupuncture treatment,but acupoint sticking therapy varied in dosage and duration.Before and after treatment,the actigraphic readings(total time in bed,total sleep time,sleep efficiency,number of wake bouts,length of wakes after asleep,and sleep latency),Pittsburgh sleep quality index(PSQI)score,and symptoms score of traditional Chinese medicine(TCM)were observed.A correlation analysis was conducted among the subjective and objective indicators.Results The PSQI score was positively correlated with the total time in bed and total sleep time(P<0.05).After treatment,the sleep latency,PSQI scores,and TCM symptoms scores changed significantly in the four groups(P<0.05).The total sleep time and sleep efficiency gained improvements after treatment in the high-dosage 14 d and low-dosage 14 d groups(P<0.05).The high-dosage acupoint sticking groups had longer total sleep time compared with the low-dosage groups of the same treatment duration(P<0.05).After treatment,the length of wakes after asleep,PSQI scores,and TCM symptoms scores were better in the 14 d groups than in the 7 d groups of the same acupoint sticking dosage(P<0.05).Conclusion Given the same acupuncture treatment,acupoint sticking therapy of different treatment durations produces different effects on the length of wakes after asleep,PSQI score,and TCM symptoms score in insomnia patients,and the 14-day acupoint sticking treatment is superior to the 7-day treatment. 展开更多

Objective: To systematically evaluate the therapeutic efficacy of tuina therapy for primary insomnia. Methods: Nine Chinese and English databases were searched from the inception to May 2017 to identify randomized controlled trials (RCTs) studying tuina therapy for insomnia. The enrolled articles were all RCTs with tuina as the monotherapy or major therapy in the experiment group, with clear diagnostic criteria for primary insomnia well recognized worldwide or in China, and Pittsburgh sleep quality index (PSQI) as one of the outcome measures. Two researchers evaluated the risk of bias and quality of the enrolled studies by following Cochrane Handbook version 5.1.0. The meta-analysis was performed by RevMan version 5.3. Results: Eleven studies were included with a total of I 076 participants. The Western medication adopted in the control groups were benzodiazepine receptor agonists. The studies were all assessed as high risk of bias for blinding since blinding method was unable to be performed due to the specificity of tuina therapy; no study reported the support of fund or potential interest conflict, so they were all rated unclear for selective reporting. The meta-analysis showed that compared with other traditional Chinese medicine therapies, tuina worked more effectively in reducing the PSQI score (MD=-4.11〈O, 95% confidence interval (CI) -6.01 to -2.22, P〈0.O001); compared with oral administration of Western medication, tuina showed more significant efficacy in reducing the PSQI score (MD=-3.42〈0, 95%CI -5.19 to -1.66, P〈0.O001). Subgroup analysis showed that head tuina alone showed no significant difference compared with oral administration of Western medication regarding the change of PSQI score (MD=-4.19〈O, 95%CI -8.87 to 0.50, P〉0.05); a combination of head and back tuina could more effectively reduce the PSOJ score compared with oral administration of Western medication (MD=-2.08〈O, 95%CI -3.09 to -1.06, P〈0.O001). Conclusion: Tuina can produce more significant efficacy in treating primary insomnia compared with other traditional Chinese medicine therapies and oral administration of Western medication, especially the combination of head and back tuina. 展开更多

Methods: In the present multi-central randomly controlled clinical study, 180 cases of chronic insomnia were randomly divided into the following two groups, a treatment group (90 cases) treated by the needle-rolling therapy and a control group (90 cases) treated with clonopin. The treatment course for both the two groups was 4 weeks. The therapeutic effects were evaluated based on improvement of the TCM symptoms and the Pittsburgs's sleep-quality index (PSQI). Results: After treatment, there were significant differences between the two groups in the effective rate (P<0.05), and in the total score of PSQI and in the scores of the 4 sub-items, i.e. sleep-quality, sleep-efficiency, hypnotic and daytime function (P<0.05). Although there was no significant difference between the two groups in the effective rate after a 3-month follow-up period, significant differences still existed in the 3 sub-items of sleep-efficiency, hypnotic, and daytime function of the PSQI (P<0.05). Conclusion: As compared with hypnotics of the second generation, the needle-rolling therapy may show better therapeutic effects for chronic insomnia patients. 展开更多

Objective: To observe the influence of acupuncture on sleep disorders and anxiety in patients with heroin dependence. Methods: Sixty-two male patients with heroin dependence were divided into two groups by random number table. Thirty-five cases in the acupuncture group were treated by puncturing Baihui (GV 20), Neiguan (PC 6), and Shenmen (HT 7) and moxibustion at Zusanli (ST 36). Twenty-seven cases in the control group did not receive any therapeutic measures. The therapeutic effects were summarized after observation of 8 weeks. Results: After treatment, the global scores and standard scores of anxiety declined in comparison with those before the treatment in both two groups, but there was statistical difference only in the acupuncture group (P〈0.05). There were no statistical differences in the global score and the standard score of anxiety between the two groups, but the standard scores in the acupuncture group reached the normal level. After treatment, only the sleep latency was obviously improved in the control group (P〈0.05). The syndromes of sleep disorder were improved noticeable in the acupuncture group (P〈0.05), and the actual sleep time was improved better than those in the control group (P〈0.05), and PSQI global scores decreased below the normal level. Conclusion: Acupuncture has the effects to improve sleep disorders and anxious emotion in the patients with heroin dependence. 展开更多

Objective: To observe the clinical efficacy of acupuncture plus bloodletting therapy in treating insomnia in people with blood stasis constitution. Methods: Seventy-two patients were randomized into a treatment group and a control group by using the random number table, 36 cases each. The treatment group received acupuncture plus bloodletting therapy, while the control group was intervened by oral administration of estazolam. One-week treatment was taken as a course, for 3 successive courses in total. A follow-up study was conducted 3 months later. The Pittsburgh sleep quality index (PSQI) was evaluated before and after the intervention, as well as in the follow-up. The clinical efficacies were also compared. Results: The total effective rate was 88.9% in the treatment group versus 83.3% in the control group, and the between-group difference was statistically significant (P〈0.O5). After the intervention, the PSO.I scores dropped significantly in both groups (both P〈0.05); the between-group difference in PSQI score was statistically significant (P〈O.05). The follow-up study showed that the between-group difference in the global score of PSO, I was statistically significant (P〈0.05). Conclusion: Acupuncture plus bloodletting therapy can produce a more significant efficacy than oral administration of estazolam in treating insomnia in people with blood stasis constitution. 展开更多
