Objective:Cancer survivors have experienced subjective cognitive impairment(SCI)when they received cancer diagnoses or treatments.Their psychosocial and emotional statuses were also impacted.With the advancement of we...Objective:Cancer survivors have experienced subjective cognitive impairment(SCI)when they received cancer diagnoses or treatments.Their psychosocial and emotional statuses were also impacted.With the advancement of web technologies,web-based cognitive interventions have been implemented in the management and the alleviation of the SCI,the psychosocial distress,and the emotional distress in cancer survivors.This review aimed to summarize the intervention contents of web-based cognitive interventions for SCI,and to explore the effects of the interventions on SCI,psychosocial status,and emotional health.Methods:Six databases(CINAHL Plus,Cochrane Library,Embase,APA PsycInfo,PubMed and CNKI)were searched from the establishment of databases up to December 2023.Literature references were also manually searched for related articles.Results:This review contained 21 studies that covered the contents of web-based cognitive interventions,such as computer-assisted cognitive training,online cognitive rehabilitation,cognitive behavior therapy with the Internet,telehealth physical exercise,and web-based mindfulness interventions.The effects of web-based cognitive interventions positively impacted SCI for cancer survivors.Also,these interventions showed varying degrees of effectiveness in alleviating psychosocial and emotional distresses.Conclusion:By summarizingfive types of cognitive intervention contents delivered via web technology,this review demonstrated that web-based cognitive interventions optimized SCI and overall psychosocial and emotional statuses for the cancer survivors.It is recommended that future research focus on the development of customized web-based cognitive interventions for individuals with SCI,along with their psychosocial and emotional statuses.展开更多

METHODS: The 20 mer ra...AIM: To select the optimal antisense accessible sites of survivin, a highly expressed gene in tumor tissues, in order to explore a novel approach to improve biological therapy of gastric cancer. METHODS: The 20 mer random oligonucleotide library was synthesized, hybridized with in vitro transcribed total survivin cRNA, then digested by RNase H. After primer extension and autoradiography, the antisense accessible sites (AAS) of survivin were selected. Then RNADraw software was used to analyze and choose the AAS with obvious stem-loop structures, according to which the complementary antisense oligonucleotides (AS-ODNs) were synthesized and transferred into survivin highly- expressing gastric cancer cell line MKN-45. Survivin expression was detected by RT-PCR and Western Blotting. Cellular growth activities were assayed by tetrazolium bromide (MTT) colorimetry. Cellular ultrastructure was observed by electronic microscopy, while apoptosis was detected by annexin V-FITC and propidium iodide staining flow cytometry. RESULTS: Thirteen AAS of survivin were selected In vitro. Four AAS with stem-loop structures were chosen, locating at 207-226 bp, 187-206 bp, 126-145 bp and 44-63 bp of survivin cDNA respectively. When compared with non-tranfection controls, their corresponding AS-ODNs (AS-ODN1, AS-ODN2, AS-ODN3 and AS-ODN4) could reduce Survivin mRNA levels in MKN-45 cells by 54.3±±1.1% (t= 6.12, P<0.01), 86.1±±1.0% (t= 5.27, P<0.01), 32.2±±1.3% (t= 7.34, P<0.01) and 56.2±±0.9% (t = 6.45, P<0.01) respectively, while survivin protein levels were decreased by 42.2±±2.5% (t = 6.26, P<0.01), 75.4±±3.1% (t= 7.11, P<0.01), 28.3±±2.0% (t= 6.04, P<0.01) and 45.8±±1.2% (t = 6.38,P<0.01) respectively. After transfection with 600 nmol/L AS-ODN1-AS-ODN4for 24 h, cell growth was inhibited by 28.12±±1.54% (t= 7.62, P<0.01), 38.42±±3.12% (t= 7.75, P<0.01), 21.46±±2.63% (t= 5.94, P<0.01) and 32.12±1.77% (t = 6.17, P<0.01) respectively. Partial cancer cells presented the characteristic morphological changes of apoptosis, with apoptotic rates being 19.31±1.16% (t= 7.16,P<0.01), 29.24±1.94% (t = 8.15,P<0.01), 11.87±0.68% (t = 6.68, P<0.01) and 21.68±2.14% (t = 7.53, P<0.01) respectively. CONCLUSION: The MS of survivin could be effectively selected in vitro by random oligonucleotide library/RNase H cleavage method combined with computer software analysis, this has important reference values for further studying survivin-targeted therapy strategies for gastric cancer.展开更多

Objectives:This study aimed to evaluate the effects of a 16-week creative expression intervention program(CrExp)on the event-related potential(ERP)and task reaction time in older individuals with mild cognitive impair...Objectives:This study aimed to evaluate the effects of a 16-week creative expression intervention program(CrExp)on the event-related potential(ERP)and task reaction time in older individuals with mild cognitive impairment(MCI).Methods:This study is a randomized controlled clinical trial conducted in the Memory Center of Fujian Provincial hospital.Thirty-six MCI patients were randomly distributed into two groups.One group underwent a 16-week creative expression program(CrExp,n=18)and the other performed as a control group(CG,n=18)by general social activities.The amplitude and latency of ERP-P300 from the central(Cz),parietal(Pz),frontal(Fz)cortices and task reaction time(RT)were assessed at baseline,postinterventi on,and 24-week follow-up.Results:The CrExp group showed greater differences than CG of P300 latency in Cz(F=4.37,P=0.015),Pz(F=2.78,P=0.009),Fz(F=6.45,P=0.031)brain area after 16 weeks of intervention and in Fz(F=3.23,P=0.028),Cz(F=3.79,P=0.024),and Pz(F=5.60,P=0.036)at 24 weeks follow-up.Also,we analyzed the task reaction time between two groups and found that a shorten reaction time at postintervention(F=4.47,P=0.011)and 24 weeks follow-up(F=3.12,P=0.007)in the CrExp group.However,there was no difference in P300 amplitude in either brain area between the two groups.Conclusion:The electrophysiological results of the creative expression cognitive therapy group were more obvious than those of the general cognitive therapy group,and the latency and task reaction time may be considered as supported parameters in diagnosing the effects during non-drug therapy intervention in clinical practice.展开更多
This paper examines the origin,compilation,and circulation of A Barefoot Doctor’s Manual(Chijiao yisheng shouce赤脚医生手册),exploring the relationship between medical politics and knowledge transmission in China,and...This paper examines the origin,compilation,and circulation of A Barefoot Doctor’s Manual(Chijiao yisheng shouce赤脚医生手册),exploring the relationship between medical politics and knowledge transmission in China,and its impact on the promotion of Chinese medicine across the world.Barefoot doctors were a special group of rural medical practitioners active in a very special socio-political context.Various editions of barefoot doctor manuals and textbooks were published across China after the first publication of the Manual in 1969.The publication of these manuals and textbooks became an indelible hallmark of the“Cultural Revolution”(1966–1976),when political publications predominated.The Manual was not only a guide for barefoot doctors in their daily study and practice,but also a primary source of medical knowledge for ordinary people.In the middle of the 1970s,the Manual was translated into many languages and published worldwide.This paper argues that the publication of A Barefoot Doctor’s Manual embodied a public-oriented mode of knowledge transmission that emerged and was adopted during a very specific era,and though it was eventually substituted by a mode of training embedded in the formal medical education system,it demonstrated the impact of politics on medicine and health in the context of resource scarcity and low literacy.Changes in China’s geopolitical status,the West’s pursuit of alternative approaches to medicine and health,and the World Health Organization’s(WHO’s)concern over health universality and equity all contributed to the translation and circulation of the Manual,facilitating the dissemination of Chinese medicine worldwide.The paper thus presents empirical and theoretical contributions to research on the relationship between medical politics and knowledge transmission in China.展开更多

The improvement of survival observed over the previous two decades is multifactorial and ...Esophageal atresia (EA) with or without tracheoesophageal fistula (TEF) is the most common congenital anomaly of the esophagus. The improvement of survival observed over the previous two decades is multifactorial and largely attributable to advances in neonatal intensive care, neonatal anesthesia, ventilatory and nutritional support, antibiotics, early surgical intervention, surgical materials and techniques. Indeed, mortality is currently limited to those cases with coexisting severe life-threatening anomalies. The diagnosis of EA is most commonly made during the first 24 h of life but may occur either antenatally or may be delayed. The primary surgical correction for EA and TEF is the best option in the absence of severe malformations. There is no ideal replacement for the esophagus and the optimal surgical treatment for patients with long-gap EA is still contro-versial. The primary complications during the postoperative period are leak and stenosis of the anastomosis, gastro-esophageal reflux, esophageal dysmotility, fistula recurrence, respiratory disorders and deformities of the thoracic wall. Data regarding long-term outcomes and follow-ups are limited for patients following EA/TEF repair. The determination of the risk factors for the complicated evolution following EA/TEF repair may positively impact long-term prognoses. Much remains to be studied regarding this condition. This manuscript provides a literature review of the current knowledge regarding EA.展开更多

Many patients with AF are elderly and may suf...Background Medication adherence is an integral part of the comprehensive care of patients with atrial fibrillation (AF) receiving oral anticoagulations (OACs) therapy. Many patients with AF are elderly and may suffer from some form of cognitive impairment. This study was conducted to investigate whether cognitive impairment affects the level of adherence to anticoagulation treatment in AF patients. Me- thods The study involved 111 AF patients (mean age, 73.5±8.3 years) treated with OACs. Cognitive function was assessed using the Mini Mental State Examination (MMSE). The level of adherence was assessed by the 8-item Mot^sky Medication Adherence Scale (MMAS-8). Scores on the MMAS-8 range from 0 to 8, with scores 〈 6 reflecting low adherence, 6 to 〈 8 medium adherence, and 8 high adherence. Re- sults 46.9% of AF patients had low adherence, 18.8% had moderate adherence, and 33.3% had high adherence to OACs. Patients with lower adherence were older than those with moderate or high adherence (76.6 ±8.7 vs. 71.3 ~ 6.4 vs. 71.1 ± 6.7 years) and obtained low MMSE scores, indicating cognitive disorders or dementia (MMSE = 22.3 ± 4.2). Patients with moderate or high adherence obtained high MMSE test results (27.5 ±1.7 and 27,5 ± 3.6). According to Spearman's rank correlation, worse adherence to treatment with OACs was determined by older age (rs = -0.372) and lower MMSE scores (rs = 0.717). According to multivariate regression analysis, the level of cognitive function was a significant independent predictor of adherence (b = 1.139). Conclusions Cognitive impairment is an independent determinant of compliance with pharmacological therapy in elderly patients with AF. Lower adherence, beyond the assessment of cognitive function, is related to the age of patients.展开更多

The sample group was composed of 31 persons, using the 4 Basics Exercise Model over three months for 30 minutes per day on Sundays, Tuesdays, and Thursdays. Data were collected on personal information, dysmenorrhea pain scores, and satisfaction in the 4 Basics Exercise Model for dysmenorrhea records. According to the research findings, the level of dysmenorrhea pain was significantly decreased in the experimental group for one, two, and three months post-exercise. In addition, the comparison of decreases in the dysmenorrhea pain of experimental group was significantly different at two and three months post-exercise at .01. The exception was at one month post-exercise when there was no statistically significant difference. According to the findings of the study, the 4 Basics Exercise Model for dysmenorrhea effectively decreased dysmenorrhea and the duration of menstrual periods with improving signs of menstruation. Therefore, nurses who provide gynecological care should learn and have knowledge regarding the 4 Basics Exercise Model to help relieve menstrual pain.展开更多

METHODS: 138 PBC patients who met the inclusion criteria were divided into 3 groups acc...OBJECTIVE: To analyze the distribution and characteristics of TCM constitution in patients with primary biliary cholangitis(PBC). METHODS: 138 PBC patients who met the inclusion criteria were divided into 3 groups according to the stage of disease development: group A(group of basic normal), group B(group of symptom stage) and group C(group of decompensation stage). Each group collected 46 cases, and every patient was reexamined, which included blood routine examination, liver function test, image ultrasound examination, autoantibody examination and immunoglobulin test. Then, the patients' clinical symptoms were observed and recorded, and their constitution of traditional Chinese medicine were classified. RESULTS:(1) For 138 patients with PBC, their TCM constitution was characterized by biased constitution, among which constitution of Qi stagnation(19.57%) was the majority, which was followed by constitution of Qi asthenia(15.94%), constitution of Yin asthenia(15.94%) and constitution of damp-heat(14.49%). There was no significant correlation between the type of constitution and the age of onset(P>0.05).(2) The constitution types of group A and B were mainly constitution of Qi stagnation, constitution of Qi asthenia and constitution of damp-heat, while group C was mainly composed of constitution of blood stasis, constitution of Yin asthenia and constitution of Yang asthenia.(3) The highest positive rate of autoantibodies in patients with PBC was constitution of Qi stagnation, which was followed by constitution of damp-heat, constitution of Yin asthenia and constitution of Qi asthenia. The most abnormal liver function and immunoglobulin were constitution of Qi stagnation, which was followed by constitution of Qi asthenia, constitution of Yin asthenia, constitution of damp-heat.CONCLUSION: Constitution of Qi stagnation, constitution of Qi asthenia, constitution of damp-heat and constitution of Yin asthenia were the susceptible constitutions of PBC patients.展开更多

Objective To analyze the intellectual structure,hotspots and trends of traditional Chinese medicine(TCM)in immune regulation research.Methods The data were extracted from the Web of Science Core Collection(WoSCC)and t...Objective To analyze the intellectual structure,hotspots and trends of traditional Chinese medicine(TCM)in immune regulation research.Methods The data were extracted from the Web of Science Core Collection(WoSCC)and the China National Knowledge Infrastructure(CNKI)and verified by two experienced TCM researchers.The time of literature retrieval is up to 2020.CiteSpace 5.7.R1 and Microsoft Excel 2016 were used for the statistical analysis and bibliometric diagrams,including the co-occurrence network of authors,institutions,countries,keywords,references,dual-map overlays of journals and citation bursts,etc.Results A total of 12270 publications related to TCM in immune regulation were included.The annual number of publications has increased in this field.There was close cooperation of countries and institutions,while the distribution of scholars was scattered.China was the core of the cooperation network.The dual-map overlays analysis of journals showed that core and marginal fields had increased.The keywords and references analysis showed that network pharmacology,metabolism and cancer were the most high-frequency keywords with high-intensity bursts.Conclusion TCM in immune regulation has attracted wider attention,with multi-country,multi-field,multi-disciplinary and multi-level research developing toward informatization.Network pharmacology,metabolism and cancer may be the focus of future research in this field.展开更多

Objective:To study the epidemiological features of pancreatic cystadenoma and cystadenocarcinoma in China during the last 10 years and to analyze the diagnosis and treatment.Methods:Reports on pancreatic cystadenoma a...Objective:To study the epidemiological features of pancreatic cystadenoma and cystadenocarcinoma in China during the last 10 years and to analyze the diagnosis and treatment.Methods:Reports on pancreatic cystadenoma and cystadenocarcinoma published from 2000 to 2009 were retrieved from various databases,such as WANFANG data,VIP web and China National Knowledge Infrastructure(CNKI).The epidemiological features of pancreatic cystadenoma and cystadenocarcinoma and its diagnosis and treatment were analyzed.Results:Totally 1 865 patients with pancreatic cystadenoma and cystadenocarcinoma were reported in China during the last 10 years.The male to female ratio was approximately 1:2.1.The accurate ages were reported in 1 536 cases,the average age of them was 50.8 years,whose average age from 40 to 60 years old accounted 75.9% of the patients.65.3% of the cases were located in East China and Central China.Abdominal pain was the main clinical manifestation and was found in 54.9% of the patients.Pancreatoduodenectomy and resection of body and tail of the pancreas were the main procedure for the treatment of pancreatic cystadenoma and cystadenocarcinoma.Conclusion:Pancreatic cystadenoma and cystadenocarcinoma were mainly found in older women in East and Central China.Preoperative diagnosis is difficult.Pancreatoduodenectomy and resection of pancreatic body and tail were the main procedure for the treatment of pancreatic cystadenoma and cystadenocarcinoma.展开更多

Methods: A descriptive-c...Objectives: The study assessed if the level of knowledge of HIV-infected about HAART and waiting-times in the PHC (primary healthcare) clinic have an influence on antiretroviral adherence. Methods: A descriptive-cross-sectional study was conducted in South Africa. Data collected uses a standardized-questionnaire and face-to-face-exit interviews. Pill-count technique was performed and a value of≥ 95% acceptable. Data were analysed using SPSS. Univariate-factors associated with poor-adherence to knowledge about HAART and waiting times were assessed using ANOVA and p ≤ 0.05 considered statistically significant. Key findings: Of 86 enrolled, 63(73.3%) were females and 23(26.7%) males, with mean-age (± SD) of 35.6(±9.6) years and on HAART for 35.5(± 31.8) months ranging from 1-137. Of these, 27(31.40%) and 25(29.07%) were on WHO stages 2 and 3 respectively. Adherence-rates computed from 32 patients, 23(71.9%) revealed poor adherence-rates. The level of knowledge about HAART in terms of names of tablets, correct-dose, frequency, adverse-effects had no influence on ARV-adherence (p _〉 0.05). Of 23 non-compliant, 10 (40%) gave the reason of drugs-unavailability, 7(30%) adverse-effects, 5(20%) drugs' complexity, and 1(10%) too busy to take them. Waiting areas associated with poor ARV-adherence were reception (p = 0.028), doctors (p = 0.027), while nurse's station (p = 0.29) and pharmacy (p = 0.43) revealed acceptable ARV-adherence.展开更多

Furthermore, I will argue, that, pace Kripke, Russellian treatment of proper names allows to capture all our semantic intuitions, and also those semantic interpretations which concern context-belief sentences. I will close the introduction by focusing on a criticism that Kripke rightly points out against an example that Russell proposes in his On Denoting. Section 2 will be devoted to Russellian solution: I will show that not only Russell's logical treatment of proper names allows to answer to Kripke's criticism to Russell's example, but also that such treatment can disambiguate and express all our semantic intuitions about Frege's puzzle sentence "Hesperus is Phosphorus." ! will then show that, contrarily, Quinian solution (discussed in section 3) and Kripkian one (see section 4) are not satisfactory to capture our semantic knowledge about Frege's sentence. Furthermore, in section 5, I will focus on Kripke's distinction between epistemic and metaphysical level to deal with identity assertions between proper names, and I will logically show that such distinction is not plausible. 文摘Objective:Cancer survivors have experienced subjective cognitive impairment(SCI)when they received cancer diagnoses or treatments.Their psychosocial and emotional statuses were also impacted.With the advancement of web technologies,web-based cognitive interventions have been implemented in the management and the alleviation of the SCI,the psychosocial distress,and the emotional distress in cancer survivors.This review aimed to summarize the intervention contents of web-based cognitive interventions for SCI,and to explore the effects of the interventions on SCI,psychosocial status,and emotional health.Methods:Six databases(CINAHL Plus,Cochrane Library,Embase,APA PsycInfo,PubMed and CNKI)were searched from the establishment of databases up to December 2023.Literature references were also manually searched for related articles.Results:This review contained 21 studies that covered the contents of web-based cognitive interventions,such as computer-assisted cognitive training,online cognitive rehabilitation,cognitive behavior therapy with the Internet,telehealth physical exercise,and web-based mindfulness interventions.The effects of web-based cognitive interventions positively impacted SCI for cancer survivors.Also,these interventions showed varying degrees of effectiveness in alleviating psychosocial and emotional distresses.Conclusion:By summarizingfive types of cognitive intervention contents delivered via web technology,this review demonstrated that web-based cognitive interventions optimized SCI and overall psychosocial and emotional statuses for the cancer survivors.It is recommended that future research focus on the development of customized web-based cognitive interventions for individuals with SCI,along with their psychosocial and emotional statuses.

文摘AIM: To select the optimal antisense accessible sites of survivin, a highly expressed gene in tumor tissues, in order to explore a novel approach to improve biological therapy of gastric cancer. METHODS: The 20 mer random oligonucleotide library was synthesized, hybridized with in vitro transcribed total survivin cRNA, then digested by RNase H. After primer extension and autoradiography, the antisense accessible sites (AAS) of survivin were selected. Then RNADraw software was used to analyze and choose the AAS with obvious stem-loop structures, according to which the complementary antisense oligonucleotides (AS-ODNs) were synthesized and transferred into survivin highly- expressing gastric cancer cell line MKN-45. Survivin expression was detected by RT-PCR and Western Blotting. Cellular growth activities were assayed by tetrazolium bromide (MTT) colorimetry. Cellular ultrastructure was observed by electronic microscopy, while apoptosis was detected by annexin V-FITC and propidium iodide staining flow cytometry. RESULTS: Thirteen AAS of survivin were selected In vitro. Four AAS with stem-loop structures were chosen, locating at 207-226 bp, 187-206 bp, 126-145 bp and 44-63 bp of survivin cDNA respectively. When compared with non-tranfection controls, their corresponding AS-ODNs (AS-ODN1, AS-ODN2, AS-ODN3 and AS-ODN4) could reduce Survivin mRNA levels in MKN-45 cells by 54.3±±1.1% (t= 6.12, P<0.01), 86.1±±1.0% (t= 5.27, P<0.01), 32.2±±1.3% (t= 7.34, P<0.01) and 56.2±±0.9% (t = 6.45, P<0.01) respectively, while survivin protein levels were decreased by 42.2±±2.5% (t = 6.26, P<0.01), 75.4±±3.1% (t= 7.11, P<0.01), 28.3±±2.0% (t= 6.04, P<0.01) and 45.8±±1.2% (t = 6.38,P<0.01) respectively. After transfection with 600 nmol/L AS-ODN1-AS-ODN4for 24 h, cell growth was inhibited by 28.12±±1.54% (t= 7.62, P<0.01), 38.42±±3.12% (t= 7.75, P<0.01), 21.46±±2.63% (t= 5.94, P<0.01) and 32.12±1.77% (t = 6.17, P<0.01) respectively. Partial cancer cells presented the characteristic morphological changes of apoptosis, with apoptotic rates being 19.31±1.16% (t= 7.16,P<0.01), 29.24±1.94% (t = 8.15,P<0.01), 11.87±0.68% (t = 6.68, P<0.01) and 21.68±2.14% (t = 7.53, P<0.01) respectively. CONCLUSION: The MS of survivin could be effectively selected in vitro by random oligonucleotide library/RNase H cleavage method combined with computer software analysis, this has important reference values for further studying survivin-targeted therapy strategies for gastric cancer.

文摘Objectives Non-invasive and low-cost virtual reality(VR)technology is important for early evaluation and intervention in mild cognitive impairment(MCI).This study aimed to demonstrate the current status of overseas and domestic research as well as the focus and frontier of VR technology among individuals with MCI through a bibliometric analysis.Methods Studies from the core collection of Web of Science™between 1995 and 2020 were used;furthermore,CiteSpace 5.7 R3 was utilized to analyse information on authors/cited authors,keywords,burst words,and cited references.Results In total,230 publications were identified.Most studies were published in the USA(45 publications)and Italy(41 publications),where Guiseppe Riva ranks first(14 publications),and Tarnanas I is the author with the highest centrality(0.44).The hot topics in VR applications in the MCI population are‘physical activity,’‘people,’‘single-blind,’‘disease,’‘walking,’‘technology,’‘working memory,’and‘risk’in recent years.The keyword‘mild cognitive impairment’has attracted extensive attention since 2012,showing the strongest citation outbreak(8.28).The clustering results of the literature show the research types and emerging trends,including‘exergame,’‘serious games,’‘spatial navigation,’‘activities of daily living,’‘exercise,’‘enriched environment’and‘wayfinding.‘Conclusions Cognitive assessment and nonpharmacological intervention research on patients with MCI have become the focus of dementia prevention in recent years.Virtual technology,combined with traditional methods such as exercise therapy,provides new ideas for innovative cognitive evaluation and cognitive intervention.

文摘Objectives:This study aimed to evaluate the effects of a 16-week creative expression intervention program(CrExp)on the event-related potential(ERP)and task reaction time in older individuals with mild cognitive impairment(MCI).Methods:This study is a randomized controlled clinical trial conducted in the Memory Center of Fujian Provincial hospital.Thirty-six MCI patients were randomly distributed into two groups.One group underwent a 16-week creative expression program(CrExp,n=18)and the other performed as a control group(CG,n=18)by general social activities.The amplitude and latency of ERP-P300 from the central(Cz),parietal(Pz),frontal(Fz)cortices and task reaction time(RT)were assessed at baseline,postinterventi on,and 24-week follow-up.Results:The CrExp group showed greater differences than CG of P300 latency in Cz(F=4.37,P=0.015),Pz(F=2.78,P=0.009),Fz(F=6.45,P=0.031)brain area after 16 weeks of intervention and in Fz(F=3.23,P=0.028),Cz(F=3.79,P=0.024),and Pz(F=5.60,P=0.036)at 24 weeks follow-up.Also,we analyzed the task reaction time between two groups and found that a shorten reaction time at postintervention(F=4.47,P=0.011)and 24 weeks follow-up(F=3.12,P=0.007)in the CrExp group.However,there was no difference in P300 amplitude in either brain area between the two groups.Conclusion:The electrophysiological results of the creative expression cognitive therapy group were more obvious than those of the general cognitive therapy group,and the latency and task reaction time may be considered as supported parameters in diagnosing the effects during non-drug therapy intervention in clinical practice.
文摘This paper examines the origin,compilation,and circulation of A Barefoot Doctor’s Manual(Chijiao yisheng shouce赤脚医生手册),exploring the relationship between medical politics and knowledge transmission in China,and its impact on the promotion of Chinese medicine across the world.Barefoot doctors were a special group of rural medical practitioners active in a very special socio-political context.Various editions of barefoot doctor manuals and textbooks were published across China after the first publication of the Manual in 1969.The publication of these manuals and textbooks became an indelible hallmark of the“Cultural Revolution”(1966–1976),when political publications predominated.The Manual was not only a guide for barefoot doctors in their daily study and practice,but also a primary source of medical knowledge for ordinary people.In the middle of the 1970s,the Manual was translated into many languages and published worldwide.This paper argues that the publication of A Barefoot Doctor’s Manual embodied a public-oriented mode of knowledge transmission that emerged and was adopted during a very specific era,and though it was eventually substituted by a mode of training embedded in the formal medical education system,it demonstrated the impact of politics on medicine and health in the context of resource scarcity and low literacy.Changes in China’s geopolitical status,the West’s pursuit of alternative approaches to medicine and health,and the World Health Organization’s(WHO’s)concern over health universality and equity all contributed to the translation and circulation of the Manual,facilitating the dissemination of Chinese medicine worldwide.The paper thus presents empirical and theoretical contributions to research on the relationship between medical politics and knowledge transmission in China.

All physiological changes of the aged kidney are the same in both genders.

文摘Esophageal atresia (EA) with or without tracheoesophageal fistula (TEF) is the most common congenital anomaly of the esophagus. The improvement of survival observed over the previous two decades is multifactorial and largely attributable to advances in neonatal intensive care, neonatal anesthesia, ventilatory and nutritional support, antibiotics, early surgical intervention, surgical materials and techniques. Indeed, mortality is currently limited to those cases with coexisting severe life-threatening anomalies. The diagnosis of EA is most commonly made during the first 24 h of life but may occur either antenatally or may be delayed. The primary surgical correction for EA and TEF is the best option in the absence of severe malformations. There is no ideal replacement for the esophagus and the optimal surgical treatment for patients with long-gap EA is still contro-versial. The primary complications during the postoperative period are leak and stenosis of the anastomosis, gastro-esophageal reflux, esophageal dysmotility, fistula recurrence, respiratory disorders and deformities of the thoracic wall. Data regarding long-term outcomes and follow-ups are limited for patients following EA/TEF repair. The determination of the risk factors for the complicated evolution following EA/TEF repair may positively impact long-term prognoses. Much remains to be studied regarding this condition. This manuscript provides a literature review of the current knowledge regarding EA.
文摘Background Medication adherence is an integral part of the comprehensive care of patients with atrial fibrillation (AF) receiving oral anticoagulations (OACs) therapy. Many patients with AF are elderly and may suffer from some form of cognitive impairment. This study was conducted to investigate whether cognitive impairment affects the level of adherence to anticoagulation treatment in AF patients. Me- thods The study involved 111 AF patients (mean age, 73.5±8.3 years) treated with OACs. Cognitive function was assessed using the Mini Mental State Examination (MMSE). The level of adherence was assessed by the 8-item Mot^sky Medication Adherence Scale (MMAS-8). Scores on the MMAS-8 range from 0 to 8, with scores 〈 6 reflecting low adherence, 6 to 〈 8 medium adherence, and 8 high adherence. Re- sults 46.9% of AF patients had low adherence, 18.8% had moderate adherence, and 33.3% had high adherence to OACs. Patients with lower adherence were older than those with moderate or high adherence (76.6 ±8.7 vs. 71.3 ~ 6.4 vs. 71.1 ± 6.7 years) and obtained low MMSE scores, indicating cognitive disorders or dementia (MMSE = 22.3 ± 4.2). Patients with moderate or high adherence obtained high MMSE test results (27.5 ±1.7 and 27,5 ± 3.6). According to Spearman's rank correlation, worse adherence to treatment with OACs was determined by older age (rs = -0.372) and lower MMSE scores (rs = 0.717). According to multivariate regression analysis, the level of cognitive function was a significant independent predictor of adherence (b = 1.139). Conclusions Cognitive impairment is an independent determinant of compliance with pharmacological therapy in elderly patients with AF. Lower adherence, beyond the assessment of cognitive function, is related to the age of patients.
文摘The purpose of this pre-experimental research (one-group pretest-posttest design) was to study the ettects ot the 4 Basics Exercise Model on dysmenorrhea for structural equilibrium changes in nursing students at Phachomklao College of Nursing, Phetchaburi Province, who were divided the 4 Basics Exercise Model for dysmenorrhea into pre- and post-test experimental groups. The sample group was composed of 31 persons, using the 4 Basics Exercise Model over three months for 30 minutes per day on Sundays, Tuesdays, and Thursdays. Data were collected on personal information, dysmenorrhea pain scores, and satisfaction in the 4 Basics Exercise Model for dysmenorrhea records. According to the research findings, the level of dysmenorrhea pain was significantly decreased in the experimental group for one, two, and three months post-exercise. In addition, the comparison of decreases in the dysmenorrhea pain of experimental group was significantly different at two and three months post-exercise at .01. The exception was at one month post-exercise when there was no statistically significant difference. According to the findings of the study, the 4 Basics Exercise Model for dysmenorrhea effectively decreased dysmenorrhea and the duration of menstrual periods with improving signs of menstruation. Therefore, nurses who provide gynecological care should learn and have knowledge regarding the 4 Basics Exercise Model to help relieve menstrual pain.
文摘Objective:To evaluate the effect of cognitive behavioral therapy on cancer-related fatigue in breast cancer patients.Methods:Computer search for CNKI,VIP,Wanfang database,CBM,PubMed,Embase,CINAHL,The Cochrane library as of 2019 randomized controlled trials on October 20 for cognitive behavioral therapy intervention for breast cancer due to fatigue.Results:A total of 6 RCTs were included,472 patients.Cognitive Behavioral Therapy has a strong effect on cancer related fatigue compared with the usual care[SMD=-1.19,95%CI(-1.86,-0.53),P=0.0004].Results:Available evidence suggests that CBT can alleviate cancer-related fatigue in breast cancer patients,and the above studies still need to continue to conduct relevant studies for validation.
文摘Acne vulgaris is a common chronic inflammatory disease of pilosebaceous units. And sex hormone level disorder has became a hot research topic in the pathogenesis of acne. Studies show that the biological clock disorder can result in the disorder of sex hormone levels which causes or aggravates acne. Biological clock disorder coincides naturally with "sleep rhythm disorder" in Traditional Chinese Medicine(TCM). In this paper, we will discuss the modern medical mechanism of the biological clock disorder resulting in acne from the angle of sex hormones, and analyze the TCM pathogenesis of the disease caused by the disorder of the biological clock.
文摘OBJECTIVE: To analyze the distribution and characteristics of TCM constitution in patients with primary biliary cholangitis(PBC). METHODS: 138 PBC patients who met the inclusion criteria were divided into 3 groups according to the stage of disease development: group A(group of basic normal), group B(group of symptom stage) and group C(group of decompensation stage). Each group collected 46 cases, and every patient was reexamined, which included blood routine examination, liver function test, image ultrasound examination, autoantibody examination and immunoglobulin test. Then, the patients' clinical symptoms were observed and recorded, and their constitution of traditional Chinese medicine were classified. RESULTS:(1) For 138 patients with PBC, their TCM constitution was characterized by biased constitution, among which constitution of Qi stagnation(19.57%) was the majority, which was followed by constitution of Qi asthenia(15.94%), constitution of Yin asthenia(15.94%) and constitution of damp-heat(14.49%). There was no significant correlation between the type of constitution and the age of onset(P>0.05).(2) The constitution types of group A and B were mainly constitution of Qi stagnation, constitution of Qi asthenia and constitution of damp-heat, while group C was mainly composed of constitution of blood stasis, constitution of Yin asthenia and constitution of Yang asthenia.(3) The highest positive rate of autoantibodies in patients with PBC was constitution of Qi stagnation, which was followed by constitution of damp-heat, constitution of Yin asthenia and constitution of Qi asthenia. The most abnormal liver function and immunoglobulin were constitution of Qi stagnation, which was followed by constitution of Qi asthenia, constitution of Yin asthenia, constitution of damp-heat.CONCLUSION: Constitution of Qi stagnation, constitution of Qi asthenia, constitution of damp-heat and constitution of Yin asthenia were the susceptible constitutions of PBC patients.

文摘Objective To analyze the intellectual structure,hotspots and trends of traditional Chinese medicine(TCM)in immune regulation research.Methods The data were extracted from the Web of Science Core Collection(WoSCC)and the China National Knowledge Infrastructure(CNKI)and verified by two experienced TCM researchers.The time of literature retrieval is up to 2020.CiteSpace 5.7.R1 and Microsoft Excel 2016 were used for the statistical analysis and bibliometric diagrams,including the co-occurrence network of authors,institutions,countries,keywords,references,dual-map overlays of journals and citation bursts,etc.Results A total of 12270 publications related to TCM in immune regulation were included.The annual number of publications has increased in this field.There was close cooperation of countries and institutions,while the distribution of scholars was scattered.China was the core of the cooperation network.The dual-map overlays analysis of journals showed that core and marginal fields had increased.The keywords and references analysis showed that network pharmacology,metabolism and cancer were the most high-frequency keywords with high-intensity bursts.Conclusion TCM in immune regulation has attracted wider attention,with multi-country,multi-field,multi-disciplinary and multi-level research developing toward informatization.Network pharmacology,metabolism and cancer may be the focus of future research in this field.

文摘Objective:To study the epidemiological features of pancreatic cystadenoma and cystadenocarcinoma in China during the last 10 years and to analyze the diagnosis and treatment.Methods:Reports on pancreatic cystadenoma and cystadenocarcinoma published from 2000 to 2009 were retrieved from various databases,such as WANFANG data,VIP web and China National Knowledge Infrastructure(CNKI).The epidemiological features of pancreatic cystadenoma and cystadenocarcinoma and its diagnosis and treatment were analyzed.Results:Totally 1 865 patients with pancreatic cystadenoma and cystadenocarcinoma were reported in China during the last 10 years.The male to female ratio was approximately 1:2.1.The accurate ages were reported in 1 536 cases,the average age of them was 50.8 years,whose average age from 40 to 60 years old accounted 75.9% of the patients.65.3% of the cases were located in East China and Central China.Abdominal pain was the main clinical manifestation and was found in 54.9% of the patients.Pancreatoduodenectomy and resection of body and tail of the pancreas were the main procedure for the treatment of pancreatic cystadenoma and cystadenocarcinoma.Conclusion:Pancreatic cystadenoma and cystadenocarcinoma were mainly found in older women in East and Central China.Preoperative diagnosis is difficult.Pancreatoduodenectomy and resection of pancreatic body and tail were the main procedure for the treatment of pancreatic cystadenoma and cystadenocarcinoma.
文摘Objectives: The study assessed if the level of knowledge of HIV-infected about HAART and waiting-times in the PHC (primary healthcare) clinic have an influence on antiretroviral adherence. Methods: A descriptive-cross-sectional study was conducted in South Africa. Data collected uses a standardized-questionnaire and face-to-face-exit interviews. Pill-count technique was performed and a value of≥ 95% acceptable. Data were analysed using SPSS. Univariate-factors associated with poor-adherence to knowledge about HAART and waiting times were assessed using ANOVA and p ≤ 0.05 considered statistically significant. Key findings: Of 86 enrolled, 63(73.3%) were females and 23(26.7%) males, with mean-age (± SD) of 35.6(±9.6) years and on HAART for 35.5(± 31.8) months ranging from 1-137. Of these, 27(31.40%) and 25(29.07%) were on WHO stages 2 and 3 respectively. Adherence-rates computed from 32 patients, 23(71.9%) revealed poor adherence-rates. The level of knowledge about HAART in terms of names of tablets, correct-dose, frequency, adverse-effects had no influence on ARV-adherence (p _〉 0.05). Of 23 non-compliant, 10 (40%) gave the reason of drugs-unavailability, 7(30%) adverse-effects, 5(20%) drugs' complexity, and 1(10%) too busy to take them. Waiting areas associated with poor ARV-adherence were reception (p = 0.028), doctors (p = 0.027), while nurse's station (p = 0.29) and pharmacy (p = 0.43) revealed acceptable ARV-adherence.
文摘In this paper, I will focus on the debate between descriptivism and anti-descriptivism theory about proper names. In the introduction, l will propose an historical reconstruction of the debate, and focus in particular on Russell and Kripke's treatments of proper names.
