2008年1月7日感染管理科首次接到蒙医血液科的电话：＂蒙医血液科29床患者,住院期间出现‘高热’伴‘皮疹’疑似院内感染＂的口头通报。感染管理科接到电话后及时地给以相应的工作指导,并同时开展了相关病例医院感染的发生情况及危险因素的追踪调查分析以及事件的控制处理工作。

IMPROVED AERODYNAMIC APPROXIMATION MODEL WITH CASE-BASED REASONING TECHNIQUE FOR MDO OF AIRCRAFT

To increase the efficiency of the multidisciplinary optimization of aircraft, an aerodynamic approximation model is improved. Based on the study of aerodynamic approximation model constructed by the scaling correction model, case-based reasoning technique is introduced to improve the approximation model for optimization. The aircraft case model is constructed by utilizing the plane parameters related to aerodynamic characteristics as attributes of cases, and the formula of case retrieving is improved. Finally, the aerodynamic approximation model for optimization is improved by reusing the correction factors of the most similar aircraft to the current one. The multidisciplinary optimization of a civil aircraft concept is carried out with the improved aerodynamic approximation model. The results demonstrate that the precision and the efficiency of the optimization can be improved by utilizing the improved aerodynamic approximation model with ease-based reasoning technique.

Cervical cellulitis and mediastinitis following esophageal perforation:A case report

Chicken bone is one of the most frequent foreign bodies (FB) associated with upper esophageal perforation. Upper digestive tract penetrating FB may lead to life threatening complications and requires prompt management. We present the case of a 52-year-old man who sustained an upper esophageal perforation associated with cervical cellulitis and mediastinitis. Following CT-scan evidence of FB penetrating the esophagus, the impacted FB was successfully extracted under rigid esophagoscopy. Direct suture was required to close the esophageal perforation. Cervical and mediastinal drainage were made immediately. Naso- gastric tube decompression, broad-spectrum intravenous antibiotics, and parenteral hyperalimentation were administered for 10 d postoperatively. An esophagogram at d 10 revealed no leak at the repair site, and oral alimentation was successfully reinstituted. Conclusion: Rigid endoscope management of FB esophageal penetration is a simple, safe and effective procedure. Primary esophageal repair with drainage of all affected compartments are necessary to avoid life-threatening complications.

Acute acalculous cholecystitis in children

Acute acalculous cholecystitis(AAC) is the inflammatory disease of the gallbladder in the absence of gallstones. AAC is estimated to represent at least 50% to 70% of all cases of acute cholecystitis during childhood. Although this pathology was originally described in critically ill or post-surgical patients, most pediatric cases have been observed during several infectious diseases. In addition to cases caused by bacterial and parasitic infections, most pediatric reports after 2000 described children developing AAC during viral illnesses(such as Epstein-Barr virus and hepatitis A virus infections). Moreover, some pediatric cases have been associated with several underlying chronic diseases and, in particular, with immune-mediated disorders. Here, we review the epidemiological aspects of pediatric AAC, and we discuss etiology, pathophysiology and clinical management, according to the cases reported in the medical literature.

Congenital H-type anovestibuler fistula

The congenital H-type fistula between the anorectum and genital tract besides a normal anus is a rare entity in the spectrum of anorectal anomalies. We described a girl with an anovestibuler H-type fistula and left vulvar abscess. A 40-day-old girl presented symptoms after her parents noted the presence of stool at the vestibulum. On the physical examination, anus was in normal location and size, and had normal sphincter tone. A vestibuler opening was seen in the midline just below of the hymen. A fistulous communication was found between the vestibuler opening and the anus, just above the dentate line. There was a vulvar abscess which had a left lateral vulvar drainage opening 15 mm left lateral to the perineum. After the management of local inflammation and abscess, the patient was operated for primary repair of the fistula. A protective colostomy wasn′t performed prior the operation. A profuse diarrhea started after 5 hours of postoperation. After the diarrhea, a recurrent fistula was occurred on the second postoperative day. A divided sigmoid colostomy was performed. 2 months later, and anterior sagital anorectoplasty was reconstructed and colostomy was closed 1 month later. Various surgical techniques with or without protective colostomy have been described for double termination repair. But there is no consensus regarding surgical management of double termination.

Monitoring open water area changes in a small tarn using historical orthophotomaps and a historical bathymetric map:a case study of the Litworowy Staw lake,the High Tatras

According to scientific evidence in general, the disappearance of the Tatras tarns as a result of external geomorphological processes has been accepted since the long term analyses of lake sediments and peat bogs, geomorphological mapping, geophysical measurements(– all indirect methods). It is paradoxical that the direct cartographic evidence of such changes has not existed until now. In this paper, we evaluate shore line changes of the small mountain glacial lake in the High Tatras-Litworowy Staw lake using a multitemporal analysis of a series of historical orthophotomaps and a historical map. Over the last 100 years, the tarn has lost 46.4% of open water level area. Significant visual changes were caused by vegetation growth of Carex species(sedges) on the water's surface. The accumulation of fine sediments and dead plant residues in the tarn basin create suitable conditions for this process, all together causing the tarn to become more shallow.

English translation of ancient codes and records from the perspective of the semantic domain --Taking the two English editions of ＂Exploitation of the Works of Nature＂ as an example

The semantic domain, as the region of a word or a sememe occupied in the semantic field, can provide theoretical guidance for the word selection of the translation in the English translation of the ancient codes and records. With the two English versions of the ancient code of science and technology ＂Exploitation of the Works of Nature＂, by introducing the concept of the semantic domain, the author divides the biased errors in the translation of the English versions of ＂Exploitation of the Works of Nature＂ into three categories of the migration of the semantic domain, the reduction of the semantic domain and the expansion of the semantic domain, which are illustrated respectively. We find that the boundaries of the semantic domains of the words in the scientific and technological codes and books are usually clear. Therefore, when selecting the translation words, we must choose the foreign words with the same semantic field, to prevent the unnecessary translation errors, which will affect the overseas dissemination of the scientific and technological classics.

Recurrent inflammatory fibroid polyp of cardia:A case report

Inflammatory fibroid polyp is one of the chronic inflammatory diseases in the digestive tract, which often mimics the submucosal tumor. Precise diagnosis is possible after removal of the detected lesion. Endoscopic removal is recommended as a safe and efficient method of the treatment. In this report the authors present a case of inflammatory fibroid polyp located in the cardia, which has been removed endoscopically. Twelve months later, recurrence of the lesion was noted and the patient was referred to surgical resection.

Decadence and Mass Culture： The Novel as a Panorama. The Case of Bruges-La-Morte

Today＇s idea of the so-called fin-de-sibcle is still somewhat inexact and incomplete. This lack of precision originates from a deeply rooted methodological approach which consists of considering the respective developments of literature, art, and science in isolation, without paying attention to any interference or reciprocal contamination. This defective method also affects the overlaps and contacts between high culture and popular culture. Despite the progress made over recent years, there are few researchers who approach their objective with a real awareness of the complexity of a time period which saw the appearance of mass culture in the current sense and an unprecedented boom in scientific and technological development.1 To this end, the example of the great German philosopher Walter Benjamin and his studies on Paris during the 19th century----collected in the essays on the great French poet Charles Baudelaire and especially in the monumental and unfinished work The Arcades Project--is, without doubt, a model to follow. Taking the path suggested by Benjamin, this article sets out a revision of the novel Bruges-la-Morte （1892） by the Belgian writer Georges Rodenbach, considered one of the major exponents of thefin-de-sikcle decadence, which goes beyond the usual approach of a symbolist reading, by paying special attention to the frictions between literature and technology on one hand, and between literature and popular entertainment on the other.

Left Ventricular Myxoma： Case Report

Male, 22 years old, hospitalized in a reference hospital, was diagnosed with a left ventricular myxoma, and underwent into surgical removal of the tumor. The left ventricular myxoma is a rare condition where very few hospitals in the world have experienced in its resolution, making the advancement on its management studies difficult. This article’s goal is to promote an update about this subject.

D-Transposition of Great Arteries in a Primigravida of 35 Years Old-Case Report and Literature Review

This is a case report with discussion of the maternal-fetal outcome of pregnant women with uncorrected transposition of the great arteries （TGA） associated with pulmonary arterial hypertension and a large ventricular septal defect. This case draws attention to the severity of the pathology and maternal symptoms prior to gestation, and how an adequate clinical management of both obstetrics and cardiology can provide a favorable outcome for mother and fetus.

A case study of Chinese double-character vocabularies in modem China--Taking the completive pattern of ＂Exploitation of the Works of Nature＂ as an example

This paper focuses on the research on the vocabularies in the modem Chinese monograph with the division of history into periods and the special classes of the ancient codes and records. ＂Exploitation of the Works of Nature＂ is China＇s first encyclopedia on the production technologies of the agricultural and handicraft industries, published in 1637, with the contents covering many fields of the social production. It can fully reflect the terminologies of various industries in the late Ming Dynasty and the early Qing Dynasty and the real situation of the language at that time, which has a very high value of the linguistic research. The article extracts 5573 double-character compound words from the whole book, and uses the method of the quantitative analysis and the qualitative analysis to study the vocabularies of the whole book. Limited by the length of the text, the article carries out the investigation and analysis with the supplementary double-character words as the examples.

Reading and Writing Practices： Case Studies at the Sergio Arboleda University of Colombia

This research project analyzes the reading and writing practices used in five first and second year undergraduate courses of different disciplines at a Colombian university. During one semester, we conducted polls, class observations, and interviews with teachers. The data collected contain information about the strategies that teachers use to guide, clarify, and evaluate the readings they assigned. It also informs about the ways in which teachers classify texts and participate in the process of composing, editing, and evaluating the students＇ writing. Finally, the data inform about the role that teachers assign to the practices of reading and writing in their courses. The results of the study prove that there is a misconception based on the idea that there is a unique way of understanding and practicing reading and writing in the academia, and that there is a set of unique strategies that are transferable to any discipline or field of study. The cases analyzed in this study suggest that it would be much more effective to incorporate intensive reading and writing strategies in disciplinary courses, than to concentrate on first year writing courses.

The Manuscript Drafting Journey of a NNE Scientific Writer： An Ethnographic Case Study

Given that there is a dramatic increase of the English manuscript submission rate of doctoral students, yet little research effort is devoted to this line, the author investigates the manuscript drafting strategies of one Taiwan Residents hydraulics major doctoral student, who was required by his department to have international publications for the qualifications for the degree. The study employs Ken Hyland＇s social constructionist approach to disciplinary writing and follows an ethnographic research method for the collection/analysis of the data. The results show that the participating student, in order to establish a professional persona for the heightening of manuscript acceptance possibility, followed some prescribed procedures such as citing higher potential journals, adhering to the expert model, and formulizing the research procedures/results. A conflicting rhetorical structure of literature review writing and the favor of doing applied research for publication purpose were also found in his writing. The author addresses the implicated meanings of these writing attempts and concludes that a ＂short-sighted operation＂ of writing attempts may undermine the value of a research. This study contributes to international scientific research communities＇ understanding of this group of writers＇ disciplinary knowledge construction and writing for publication.

EDGE non-invasive radiosurgery for gastric neuroendocrine hepatic portal lymph node metastases:a case report

Treating metastatic gastric neuroendocrine neoplasms （gNENs） is challenging, especially for those with progressive disease during somatostatin analog processing. In this report, we present a case of a welldifferentiated grade 2, type 3 gNEN with metastatic hepatic portal lymph nodes. EDGE non-invasive radiosurgery （800 cGy x 5 F） was performed to radiate the metastatic hepatic portal lymph nodes. Three months after the hyperfractionated radiotherapy, no signs of metastatic hepatic portal lymph nodes were observed using 8Ga-dotatate positron emission tomography-computed tomography or magnetic resonance imaging. Therefore, EDGE non-invasive radiosurgery could be a potential option for treating local metastatic nodes.
