目的:评价硬脊膜穿破硬膜外(DPE)阻滞联合程控硬膜外间隙脉冲给药(PIEB)模式在经产妇分娩镇痛中的应用效果及对新生儿的影响。方法:选择2023年3—4月江西省妇幼保健院自然分娩行椎管内分娩镇痛的孕37~42周、单胎头位、美国麻醉医师协会(... 目的:评价硬脊膜穿破硬膜外(DPE)阻滞联合程控硬膜外间隙脉冲给药(PIEB)模式在经产妇分娩镇痛中的应用效果及对新生儿的影响。方法:选择2023年3—4月江西省妇幼保健院自然分娩行椎管内分娩镇痛的孕37~42周、单胎头位、美国麻醉医师协会(ASA)分级Ⅰ或Ⅱ级的经产妇180例。采用在线随机数生成器将产妇分为三组:DPE阻滞联合PIEB模式组(DPE+PIEB组,60例)、DPE阻滞联合连续硬膜外输注(CEI)模式组(DPE+CEI组,60例)和单纯硬膜外(EP)阻滞联合CEI模式组(EP+CEI组,60例)。比较三组产妇在30 min内达“满意镇痛”的时长、30 min后无满意镇痛手动推药发生率、镇痛效果和阻滞情况、不良反应和胎儿娩出情况。结果:DPE+PIEB组和DPE+CEI组产妇达满意镇痛时长快于EP+CEI组(P<0.05)。三组产妇罗哌卡因总消耗量为:DPE+PIEB组<DPE+CEI组<EP+CEI组(P<0.05),阻滞平面上限均达T_(10)。三组产妇Bromage评分均为0分,产程总时长、导管更换率、瘙痒、头痛、神经损伤发生率和胎儿心动过缓发生率比较差异均无统计学意义(P>0.05),新生儿出生后1 min和5 min时Apgar评分≤7分的发生率均为0。结论:DPE阻滞联合PIEB模式用于经产妇的分娩镇痛可明显缩短药物起效时间,增强镇痛效果,减少罗哌卡因用量,不影响产程进展,且对新生儿无不良影响。 展开更多

目的:观察硬脊膜穿破硬膜外阻滞技术(DPE)联合程控硬膜外间歇脉冲注入模式(PIEB)在分娩镇痛中的临床应用效果及安全性。方法:选择单胎头位初产妇120例,随机分为硬膜外阻滞组(EP组)和硬脊膜穿破硬膜外阻滞组(DPE组)(n=60)。选取L2-L3间... 目的:观察硬脊膜穿破硬膜外阻滞技术(DPE)联合程控硬膜外间歇脉冲注入模式(PIEB)在分娩镇痛中的临床应用效果及安全性。方法:选择单胎头位初产妇120例,随机分为硬膜外阻滞组(EP组)和硬脊膜穿破硬膜外阻滞组(DPE组)(n=60)。选取L2-L3间隙为椎管内麻醉穿刺点,EP组穿刺成功后启动硬膜外自控镇痛(PCEA),持续输注剂量10 mL/h。DPE组硬膜外穿刺成功后,用腰麻针刺破硬脊膜,不进行蛛网膜下腔给药,退出腰麻针,置管后连接脉冲式全自动注药泵,持续剂量5 mL/h,随后每隔1 h自动脉冲式注药一次5 mL;两组镇痛泵麻醉药液均为0.08%盐酸罗哌卡因+0.4μg/mL舒芬太尼,首次剂量8 mL,自控镇痛量5 mL,锁定时间30 min。比较两组产妇镇痛前(T0)、镇痛后30 min(T1)、1h(T2)、分娩时(T3)视觉模拟评分法(VAS)评分;记录两组产妇镇痛起效时间、产程、镇痛泵患者自控镇痛(PCA)按压次数及麻醉药物使用总量;记录两组患者不良反应发生率及1 min新生儿Apgar评分。结果:两组产妇T0、T1、T2时点VAS评分差异无统计学意义(P>0.05);DPE组T3时点VAS评分小于PE组,差异有统计学意义(P<0.05);DPE组镇痛起效时间快,PCA按压次数、镇痛药液总量比PE组减少,差异有统计学意义(P<0.05);两组产妇总产程及1min新生儿Apgar评分,不良反应的发生率,差异均无统计学意义(P>0.05)。结论:DPE联合PIEP技术用于分娩镇痛,具有更好的镇痛效应,起效时间短,药物总量少,优势明显。 展开更多

S1S2和S1S2S3两种程控刺激模式诱发猪心室颤动模型的效果

目的评价S1S2S3(300/200/190 ms)程控刺激模式诱发心室颤动(VF)的效果。方法本实验研究选取16头雄性家猪随机分为两组:S1S2组(n=8)和S1S2S3组(n=8)。分别经右心室内膜给予40 V、8∶1、步长-10 ms的S1S2(300/200 ms)或S1S2S3(300/200/190... 目的评价S1S2S3(300/200/190 ms)程控刺激模式诱发心室颤动(VF)的效果。方法本实验研究选取16头雄性家猪随机分为两组:S1S2组(n=8)和S1S2S3组(n=8)。分别经右心室内膜给予40 V、8∶1、步长-10 ms的S1S2(300/200 ms)或S1S2S3(300/200/190 ms)程控刺激。比较两种程控刺激模式诱发VF的效果和除颤成功率。结果两组家猪均诱发VF成功。S1S2组诱发VF的S2配对间期主要集中在160~190 ms(6/8),S1S2S3组诱发VF的S3配对间期也集中在160~190 ms(7/8)。S1S2组诱发VF程控刺激的次数明显多于S1S2S3组[9.5(4.0,17.5)vs.3.0(1.3,6.3),P=0.040]。S1S2组诱发VF所消耗时间(s)也明显长于S1S2S3组[71.0(27.0,132.8)vs.19.5(5.0,45.5),P=0.046]。经过VF 4 min后两组猪均复苏成功。S1S2组和S1S2S3组的除颤次数差异无统计学意义(1.5±0.5 vs.1.8±0.9,P=0.506)。结论应用程控刺激方法诱发VF猪模型是稳定、可靠的。S1S2S3模式比S1S2模式诱发VF需要程控刺激的次数更少,耗时更短。 展开更多

“绝缘-导体”复合结构被广泛应用于工业各个领域,针对这类复合结构的检测需求,提出了一种切换式涡流-电容双模式无损检测技术,并研发了配套的检测系统。利用有限元仿真模型分析了双端激励双模式传感器的探测场特征,验证了双模式传感器... “绝缘-导体”复合结构被广泛应用于工业各个领域,针对这类复合结构的检测需求,提出了一种切换式涡流-电容双模式无损检测技术,并研发了配套的检测系统。利用有限元仿真模型分析了双端激励双模式传感器的探测场特征,验证了双模式传感器对导体及绝缘体试样缺陷检测的可行性。针对两种模式下检出信号的特点设计了信号处理电路,主要包括正交锁相模块和信号放大与滤波模块,通过单片机输出信号控制多路复用结构实现了硬件电路切换。开发了检测系统软件,实现了控制信号和检测信号的传输与处理。以单片机主控电路为核心进行系统集成,形成了双模式检测系统,并进行了不同材料、不同结构的模拟缺陷试样扫描试验,试验结果表明,该系统具有良好的检测性能。 展开更多

目的观察程控硬膜外间歇脉冲注入镇痛模式在产妇全产程分娩镇痛中的应用效果。方法选取2017年6月-2019年6月于该院行分娩镇痛的500例产妇为研究对象,按随机数字表法分成观察组与对照组,各250例。两组镇痛药液配置均为0.075%罗哌卡因+0.5... 目的观察程控硬膜外间歇脉冲注入镇痛模式在产妇全产程分娩镇痛中的应用效果。方法选取2017年6月-2019年6月于该院行分娩镇痛的500例产妇为研究对象,按随机数字表法分成观察组与对照组,各250例。两组镇痛药液配置均为0.075%罗哌卡因+0.5μg/ml舒芬太尼,观察组行程控硬膜外间歇脉冲注入模式(1个脉冲/h,8 ml/脉冲,给药速度8 ml/min)+自控镇痛(5 ml/次,给药速度1.5 ml/min,锁定时间15 min),对照组行持续背景输注(8 ml/h)+自控镇痛,比较两组镇痛前(T0)、镇痛后1 h(T1)、2 h(T2)、4 h(T3)、宫口全开时(T4)、分娩时(T5)疼痛VAS评分以及镇痛相关情况(镇痛时间、自控追加次数、罗哌卡因及舒芬太尼用量、感觉阻滞最高平面、运动阻滞率),记录两组各产程时间、分娩方式、催产素使用率、产后2 h出血量以及新生儿1 min Apgar评分,观察用药不良反应。结果观察组T2~5时VAS评分均显著低于对照组同时间点(P<0.05);观察组自控追加次数、罗哌卡因用量、舒芬太尼用量均显著低于对照组(P<0.05),感觉阻滞最高平面优于对照组(P<0.05),且两组均无运动阻滞发生;观察组第一产程时间及总产程时间低于对照组,差异有统计学意义(P<0.05);观察组催产素使用率(24.40%)低于对照组的32.80%,差异有统计学意义(P<0.05);两组均无明显药物不良反应发生。结论程控硬膜外间歇脉冲注入镇痛模式用于产妇全程分娩镇痛中,可提高镇痛效果、减少镇痛药用量、促进产程进展,且安全性好,值得临床推广应用。 展开更多

In order to explore the possible diffusion distance of carbon during proeutectoid ferrite transformation, a slow cooling test of low carbon steel was carried out under vacuum of the thermal simulator. The microstructure and thermal expansion curve were discussed and the carbon concentration inside the sample was measured. The ferrite layer of about 450 μm thickness was obtained without pearlite on the surface of the sample in the microstructure. The thermal expansion curve shows that the ferrite layer without pearlite is formed during the local phase transformation, which is followed by the global transformation. The carbon concentration in the core of the sample (0.061%) is significantly higher than that of the bulk material (0.054%). All results show that carbon has long-range diffusion from the outer layer to the inner layer of the sample. The transformation is predominantly interface-controlled mode during local transformation, and the interface migration rate is about 2.25 μm/s. 展开更多

A new modeling and monitoring approach for multi-mode processes is proposed.The method of similarity measure(SM) and kernel principal component analysis(KPCA) are integrated to construct SM-KPCA monitoring scheme,where SM method serves as the separation of common subspace and specific subspace.Compared with the traditional methods,the main contributions of this work are:1) SM consisted of two measures of distance and angle to accommodate process characters.The different monitoring effect involves putting on the different weight,which would simplify the monitoring model structure and enhance its reliability and robustness.2) The proposed method can be used to find faults by the common space and judge which mode the fault belongs to by the specific subspace.Results of algorithm analysis and fault detection experiments indicate the validity and practicability of the presented method. 展开更多

This technique is based on a statistical pattern(SP) framework integrated with a self-organizing map(SOM). An SP-based SOM is used as a classifier to distinguish various states on the output map, which can visually monitor abnormal states. A case study of the Tennessee Eastman(TE) process is presented to demonstrate the fault diagnosis and process monitoring performance of the proposed method. Results show that the SP-based SOM method is a visual tool for real-time monitoring and fault diagnosis that can be used in complex chemical processes.Compared with other SOM-based methods, the proposed method can more efficiently monitor and diagnose faults. 展开更多

This paper presents a transition-mode zero-voltage-switching inverter for the cooker magnetron of household microwave ovens. The inverter drives a leakage transformer to generate the required high voltage and stabilized current. For achieving zero-voltage switching, a transition-mode driver L6561 is utilized to detect the ending of transformer resonance and drive an insulated-gate-bipolar-transistor. As transistor is conducted, rectified direct-current voltage drives the transformer. While transistor is cut off, transformer resonates with a parallel capacitor. Transistor conduction time and magnetron power are controlled with a 16-bit digital signal controller dsPIC30F4011. For widening the working range, transistor conduction time is set to be inversely changed with line-frequency input voltage. To demonstrate the analysis and design of this paper, a 1 kW inverter circuit is built. Experimental results show the feasibility and usefulness of the designed magnetron power supply. 展开更多

Consistent high-quality and defect-free production is the demand of the day. The product recall not only increases engineering and manufacturing cost but also affects the quality and the reliability of the product in the eye of users. The monitoring and improvement of a manufacturing process are the strength of statistical process control. In this article we propose a process monitoring memory-based scheme for continuous data under the assumption of normality to detect small non-random shift patterns in any manufacturing or service process.The control limits for the proposed scheme are constructed. The in-control and out-of-control average run length(AVL) expressions have been derived for the performance evaluation of the proposed scheme. Robustness to non-normality has been tested after simulation study of the run length distribution of the proposed scheme, and the comparisons with Shewhart and exponentially weighted moving average(EWMA) schemes are presented for various gamma and t-distributions. The proposed scheme is effective and attractive as it has one design parameter which differentiates it from the traditional schemes. Finally, some suggestions and recommendations are made for the future work. 展开更多
