心向量图对陈旧性下壁心肌梗死与左后分支阻滞的鉴别

心电图鉴别下壁陈旧性心肌梗死（OMI）与左后分支阻滞（LPH）非常困难,起始向量都向上,在Ⅱ、Ⅲ、aVF导联恒定出现Q波。至今还没有一个确切的鉴别方法。现结合一例曾诊断为下壁OMI,实为LPH的心电图（ECG）、心向量图（VCG）,分析如下。 心电图特点:Ⅲ、aVF呈QR波,Q波时限为0.04s,深度为1/2R;Ⅱ导呈qr形,Tv5低平、TⅠ、aVL倒置;
