AIM: To determine whether routine nasogastric (NG) decompression benefitted patients undergoing radical gastric surgery. METHODS: Between January 1998 and December 2008, 519 patients who underwent distal gastrectomy f...AIM: To determine whether routine nasogastric (NG) decompression benefitted patients undergoing radical gastric surgery. METHODS: Between January 1998 and December 2008, 519 patients who underwent distal gastrectomy for gastric cancer were retrospectively divided into 2 time-period cohorts; those treated with Billroth Ⅱ (BⅡ) reconstruction in the first 6 years and those with Roux-en-Y (RY) reconstruction in the last 5 years. In the latter group, the patients were further divided into 2 subgroups; with and without nasogastric decompression.RESULTS: Postoperatively, there were no significant differences in the number of anastomotic leaks between the 3 groups. In the tubeless RY group, time to semiliquid diet was significantly shorter than in the other 2 groups (4.4 d ± 1.4 d vs 7.2 d ± 1.3 d and 5.9 d ± 1.2 d, P = 0.005). The length of postoperative stay was significantly increased in patients with BⅡ reconstruction compared with patients with RY reconstruction with/without NG decompression (15.4 d ± 4.3 d in BⅡ group vs 12.6 d ± 3.1 d in decompressed RY and 11.4 d ± 3.4 d in the tubeless RY group, P = 0.035). The postoperative pneumonia rate was lowest in the tubeless group and highest in the BⅡ group (1.4% vs 4.6%, P = 0.01). Severe sore throat was noted in 59 (20.7%) members of the BⅡ group, 18 (17.4%) members of the decompressed RY group and 6 (4.2%) members of the tubeless RY group. Fewer patients in the tubeless group complained of severe sore throat (P = 0.001). CONCLUSION: This study provides support for abandoning routine NG decompression in patients undergoing subtotal gastrectomy with Roux-en-Y gastrojejunostomy.展开更多
This paper proposes the architectural communication as a socio-spatial identification, and a channel for the politico-patrimonial dialogue in a determined territory which is in this case the south Moroccan city of Aga...This paper proposes the architectural communication as a socio-spatial identification, and a channel for the politico-patrimonial dialogue in a determined territory which is in this case the south Moroccan city of Agadir. By analyzing Agadir's urbanistic and architectural content thorough ages, we find ourselves involved in an enthralling and passionate debate concerning two important aspects. The first is the historical temporality of the city's architectural and urbanistic changes. In other words, it is the different architectural transformation related to the political transmutations that Agadir has witnessed since the Portugal settlement until the after earthquake of 1960. While the second is about the perception toward the architectural oeuvre deeply settled in the territory and the memory of the city's designers. This aspect is linked to the society's perception toward the architectural-urban transformations in their territory. In our paper, we are going to focus on two major architectural and urbanistic ages in the history of Agadir. The first period we are going to approach is the architectural and urbanistic features of Agadir during French settlement. The second period is Agadir's architectural renaissance after 1960s earthquakes. Since the core of our study is the architectural acts, it is definitely a matter of interpretation related to the philosophical, mental and ideological representation of the city's architecture and urbanism either by those who artistically invented Agadir during colonialism or those who reinvented Agadir after 1960s earthquakes. Do urbanism and architecture represent for Agadir, a power, a doctrine or a savoir-faire? Can we affirm that Agadir's architecture is truly reflecting the image of its society? Is this society in itself immersed sufficiently in its architecture? Several hypotheses are possible in this research. Nevertheless, the fact that in this paper we use communication as a vehicle to establish a dialogue between arts, politics and socio-ideologic and territorial governance makes us recognize the different bridging relationships between the architectural action, the political and urbanistic content represented in the architectural expression. It also helps us to discover and analyze the ideologies that lead the architects during their conception. Finally, is this architecture which normally must represent the society in tempo-spatial territory admitted by the local citizens? All these are lines of research we are going to shed light on in this paper.展开更多

However, the temporal and spatial expression pattern of DNRX still remains unclear. For this study, we generated a novel Drosophila transgenic strain termed the DNRX-Gal4 transgenic line, with characteristic features in agreement with the endogenous DNRX expression pattern. DNRX expression was examined by driving the expression of a GFP reporter(nuclear-localized and membrane-localized GFP) using the DNRX-Gal4 promoter. We found that DNRX was expressed preferentially in central and motor neurons in embryos, larvae and adults, but not in glial cells. DNRX was expressed in pre- and post-synaptic areas in third instar larvae neuromuscular junctions(NMJs). Reporter expression was also observed in the salivary glands, guts, wings and legs of adult flies. In the adult brain, reporter expression was observed throughout several brain regions, including the mushroom body(MBs), antennal lobe(AL) and optic lobe neurons, which is consistent with endogenous DNRX expression via antibody staining. Motivated by the recent work that the periodicity of a black hole is responsible for the area spectrum,we exclusively utilize the period of motion of an outgoing wave,which is shown to be related to the vibrational fr...Motivated by the recent work that the periodicity of a black hole is responsible for the area spectrum,we exclusively utilize the period of motion of an outgoing wave,which is shown to be related to the vibrational frequency of the perturbed black hole,to study area spectra of a non-rotating BTZ black hole and a rotating BTZ black hole.It is found that the area spectra and entropy spectra for both space times are equally spaced.In addition,we find that though the entropy spectra of the 3-dimensional BTZ black holes take the same form as those of the 4-dimensional black holes,the area spectra depend on the dimension of space times.Our result confirms that the entropy spectrum of a black hole is more fundamental than the area spectrum.展开更多
