There still a paucity of data on the proportional distribution of precipitating factors specifically in elderly patients. The aim of this study was to examine prospectively the precipitating factors leading to hospitalization in elderly patients with heart failure in our community hospital. Methods We evaluate elderly patients who need admissions for decompensate heart failure. All patients were reviewed daily by the study investigators at the first 24 h and closely followed-up. Decompensation was defined as the worsening in clinical NYHA class associated with the need for an increase in medical treatment (at minimum intravenously diuretics). Results We included 102 patients (mean age 79 ± 12 years). Precipitating factors were identified in 88.5%. The decompensation was sudden in 35% of the cases. Noncompliance with diet was identified in 52% of the patients, lack of adherence to the prescribed medications amounted to 30%. Others precipitating factors were infections (29%), arrhythmias (25%), acute coronary ischemia (22%), and uncontrolled hypertension (15%), miscellaneous causes were detected in 18% of the cases (progression of renal disease 60%, anemia 30% and iatrogenic factors 10%). Concomitant cause was not recognizable in 11.5%. Conclusions Large proportion heart failure hospitalizations are associated with preventable precipitating factors. Knowledge of potential precipitating factors may help to optimize treatment and provide guidance for patients with heart failure. The presence of potential precipitating factors should be routinely evaluated in patients presenting chronic heart failure.展开更多

Aiming to promote better standards of living to the Latin American rural population, AIA is merged in a historical context of increasing US economic and politic influence over Latin America and Brazil, diffusing and adapting US technical assistance programs to local contexts. However, this research developed the idea of an intense process of political negotiation, including resistance from Brazilian political staff. Also, there were many difficulties in adapting these programs to the local context, leading to constant reorganization of the AIA's work in Brazil.展开更多

This research joins the discussion by exploring how Black hair magazine advertisements influence Black women's hair decisions and their percepfious of what constitutes beautiful Black hair. Findings reveal that the images African-American women consume from Black hair magazine advertisements do impact what they consider to be beautiful and, subsequently, influence their day-to-day styling and hair care mechanisms, lending further support to the idea that the media serve as powerful sources of knowledge. This study culminates by theorizing about the constitutive meanings and importance of Black hair as conveyed through magazine advertisements and considers how Black women use social comparisons to make everyday hair decisions to achieve what they identify as good hair.展开更多

However, the two different ways are mutually influenced and connected along with the social economic development and globalization. This requires the absorption of the excellent western singing styles on the basis of developing Chinese national vocal music, so as to promote a common development. In this paper, Chinese national vocal music and bel canto are studied and analyzed according to the historical origins, different characteristics, effects, etc.展开更多

In 1954, Brown v. Board of Education fundamentally changed the education system in the United States and was a turning point in the evolution of U,S. democracy that made the beautiful American dream available for each individual. However, six decades later, the debate over educational inequality continues. People are still fighting for their equality and well-being. Along with dramatically influential political, economic, and social factors in the United States, educational inequality destroys the commitment of this democratic nation to offer equal opportunity, care, and social justice to younger members of the community. This paper discusses the current problem of educational inequality as it relates to racial and socioeconomic discrimination in urban schools. The aim of this study is to explore knowledge of inequality in the U.S. school system and to raise social and educational awareness to help American children to have equal chances in education and to achieve future success.展开更多

The label "Asian American literature" is so instrumental in instituting the literature, can also regulate creativity. As a strategic term and group identification, "Asian American" lends itself to the illusion of unity and homogeneity. Scholars differ in the way they elect to demarcate Asian American literature. One of the fundamental barriers to understanding and appreciating Asian American literary expression has been the existence of race stereotypes about Asians in American popular culture. For many Asian American writers, ethnic authenticity is a necessary but weak choice Most early immigrant writers write to challenge the negative view of Asia in the West and see themselves as ambassadors of goodwill to the West. The purpose is to mark difference as exotica and appeal to the benign curiosity of the Western readers while appeal to notions of universal humanity. This type of strong ethnic quotient writing continues to exist in various guises even today. The trade publishers have a predilection for Asian American personal narratives that stress the "model minority" discourse, "good workers" image, and Asian Americans' ability to accommodate to the basic rules of American society. Asian American writers never stop attempting to build bridges. But we should remind ourselves that boundary crossing must not be merely an aesthetic and intellectual exercise .展开更多

Using an interview study, the author focuses on understanding the students' perspectives in relation to the label of model minority. The purpose of the study is to investigate how social, academic, and economic factors affect these students, including different outcomes in terms of school achievement and self-identity formation. The findings aim to help urban educators approach complex factors regarding minority students' educational opportunities. Comprehensive results identified that: (1) The concept of model minority significantly affects Asian American students at all levels of daily life; (2) Urban schools continue reinforcing social reproduction and producing perceptions based on socioeconomic background and intersectional discrimination; (3) The evidence in the study shows that school environments do not aid students in valuing their cultural capital; (4) Students from different social classes present different linguistic and behavioral patterns; and (5) Social stratification significantly influences students' perspectives in response to the Asian stereotype, self-identity, and racial hierarchy in school and society.展开更多

We are interested in these theoretical approaches (Foucault's biopolitics and Dussel's Liberation Politics) given the political implications and prominence they grant to the notions of body and life in contemporary societies. The works we are interested in to contrast present different standpoints: In the first one, life is related to the exercise of political power, whereas in the second one its approach concentrates on political emancipation processes. We believe, however, that it is possible to find convergence points between them that allow us to explain, to a certain extent, the importance of the notion of life in contemporary societies. For this purpose, we will carry out an analysis of the notion of "counter behaviors," a concept that Foucault briefly develops to explain how life has not been thoroughly integrated to technologies that dominate or run it but instead escape them ceaselessly.展开更多

The so-called aesthetic, simply put, is to feel, and understand the objective things or phenomena presented by the United States itself: specifically, it refers to the people in their social practice and objective process of historical things or phenomena occur and construct a special performance relationship. There are basically three kinds of relationships of man and objective things or phenomena: firstly, cognitive relation of science; Secondly, ethical norms relations; thirdly, aesthetic performance relationship. Focus on the relationship between the performances of the aesthetic object vivid sense of the expressive form of regularity is the unity and purpose, with freedom. The essence of aesthetic is liberty. Art aesthetic and general aesthetic are different, mainly because of its aesthetic objects, which is based on works of art as an aesthetic object. General aesthetic contemplation of beauty of reality, the reality of objective things or phenomena, not as people practice the original object mad the object of knowledge exists. In social practice, they first form a rational understanding of relationships and will practice relationship. On this basis, when the society develops to the degree of no direct utilitarian attitude, practical attitude towards the object of an object only when there has been more mature, purely aesthetic relationship. The beauty of art is different from the reality beauty, which is produced and existed as an aesthetic object.展开更多
