目的 探讨聚乙二醇干扰素α-2a与替诺福韦酯对高病毒载量慢性乙肝初治患者血清单核因子(monokine induced by IFN-γ,Mig)、调节活化正常T细胞表达和分泌的趋化因子(egulated upon activation normal T cell expressed and secreted,RAN...目的 探讨聚乙二醇干扰素α-2a与替诺福韦酯对高病毒载量慢性乙肝初治患者血清单核因子(monokine induced by IFN-γ,Mig)、调节活化正常T细胞表达和分泌的趋化因子(egulated upon activation normal T cell expressed and secreted,RANTES)以及肝纤维化指标表达的影响。方法 对赣州市人民医院2020年5月—2021年5月收治的129例慢性乙肝患者展开回顾性分析,按照治疗方案分组,将使用替诺福韦酯治疗的64例患者归为常规组,将使用聚乙二醇干扰素α-2a与替诺福韦酯联合治疗的65例患者纳入联合组。2组治疗时间均为6个月,治疗后比较2组肝功能指标[总胆红素(total bilirubin,TBIL)、谷丙转氨酶(alanine aminotransferase,ALT)以及谷草转氨酶(aspartate aminotransferase,AST)],对比2组血清Mig、RANTES水平,记录2组肝纤维化指标[层连黏蛋白(laminin,LN)、Ⅲ型前胶原蛋白(procollagen typeⅢ,PCⅢ)、Ⅳ型肽原(CollagenⅣ,Ⅳ-C)、透明质酸酶(hyaluronidase,HA)],观察2组乙肝病毒脱氧核糖核酸(hepatitis B virus deoxyribonucleic acid,HBV-DNA)转阴率与乙型肝炎病毒e抗原(hepatitis B e antigen,HBeAg)转阴率。结果 治疗后,联合组TBIL、ALT以及AST均比常规组低(P<0.05)。治疗后,联合组血清Mig、RANTES水平低于常规组(P<0.05)。治疗后,联合组LN、PCⅢ、Ⅳ-C和HA水平均比常规组低(P<0.05)。联合组HBV-DNA、HBeAg转阴率为分别86.15%、75.38%,高于常规组的65.63%、54.69%(P<0.05)。结论 对高病毒载量慢性乙肝初治患者采用聚乙二醇干扰素α-2a与替诺福韦酯联合治疗,可改善患者肝功能与肝纤维化指标,降低血清Mig、RANTES水平,有效促进HBV-DNA、HBeAg转阴。该方法可作为治疗慢性乙肝的优选方案。展开更多
