精细化综合器械管理在肝胆胰癌腹腔镜手术中的应用效果探讨

目的探讨精细化综合器械管理在肝胆胰癌腹腔镜手术中的应用效果。方法选取394台精细化综合管理前肝胆胰癌腹腔镜手术所使用的器械管理资料(对照组)和396台精细化综合管理后肝胆胰癌腹腔镜手术所使用的器械管理资料(观察组),统计分析两组手术器械的准备情况(缺失率、损坏率、不适用率)、抽查情况(遗失率、损坏率、消毒不合格率)和手术医师的满意度。结果术前准备中,观察组手术器械的缺失率、损坏率、不适用率均低于对照组,差异均有统计学意义(χ~2=6.80、7.33、4.73,P﹤0.05);随机抽查时,观察组手术器械的遗失率、损坏率、消毒不合格率均低于对照组,但差异均无统计学意义(P﹥0.05)。观察组手术医师的满意度高于对照组,差异有统计学意义(χ~2=5.06,P﹤0.05)。结论精细化综合器械管理可以优化肝胆胰癌腹腔镜手术的器械管理工作,提高手术医师的满意度,值得临床推广应用。

精细化综合器械管理在肝胆胰癌腹腔镜手术患者中的应用价值

目的探讨精细化综合器械管理在肝胆胰癌腹腔镜手术患者中的应用价值。方法将2017年12月至2018年12月应用精细化综合器械管理前收治的62例肝胆胰癌腹腔镜手术患者作为对照组,将2019年1月至2020年1月应用精细化综合器械管理后收治的62例肝胆胰癌腹腔镜手术患者作为观察组。比较两组患者一般手术指标、手术器械不良情况、负性情绪[焦虑自评量表(SAS)、抑郁自评量表(SDS)]、患者和医师满意度。结果观察组患者手术时间、手术器械准备时间、衔接时间均明显短于对照组(P﹤0.01)。观察组患者手术器械不良总发生率为4.84%,明显低于对照组患者的22.58%(P﹤0.01)。观察组患者SAS评分明显低于对照组(P﹤0.01),但两组患者SDS评分无明显差异(P﹥0.05)。观察组医师满意度为98.39%,明显高于对照组医师的83.87%(P﹤0.01);观察组患者的满意度为98.39%,高于对照组患者的87.10%(P﹤0.05)。结论精细化综合器械管理应用于腹腔镜肝胆胰癌根治术患者中,能够全面提高手术器械的管理水平,缓解患者的负性情绪,提高手术医师和患者的满意度。

HER2 aberrations and heterogeneity in cancers of the digestive system: Implications for pathologists and gastroenterologists

Management of cancers of the digestive system has progressed rapidly into the molecular era. Despite the significant recent achievements in the diagnosis and treatment of these patients, the number of deaths for these tumors has currently plateaued. Many investigations have assessed the role of HER2 in tumors of the digestive system in both prognostic and therapeutic settings, with heterogeneous results. Novel testing and treatment guidelines are emerging, in particular in gastric and colorectal cancers. However, further advances are needed. In this review we provide a comprehensive overview of the current state-ofknowledge of HER2 alterations in the most common tumors of the digestive system and discuss the operational implications of HER2 testing.
