目的:比较早期肠内营养(EEN)与肠外营养(PN)对胃、胰腺肿瘤术后免疫和营养状况的影响。方法:胃、胰腺肿瘤病人96例,分为EEN组52例、PN组24例和对照组(常规补液)20例。术后24 h开始给予肠内或肠外营养,检测术前和术后第9 d营养以及免疫指... 目的:比较早期肠内营养(EEN)与肠外营养(PN)对胃、胰腺肿瘤术后免疫和营养状况的影响。方法:胃、胰腺肿瘤病人96例,分为EEN组52例、PN组24例和对照组(常规补液)20例。术后24 h开始给予肠内或肠外营养,检测术前和术后第9 d营养以及免疫指标,观察术后严重并发症及肠功能的恢复。结果:EEN组和PN组术后体重、前清蛋白、清蛋白、IgG、IgM及IgA均显著高于对照组(P<0.05),而EEN组和PN组之间差异无显著性意义;EEN组和PN组术后CD3+、CD4+、CD4+/CD8+均显著高于对照组(P<0.01),并且EEN组术后CD4+、CD4+/CD8+亦显著高于PN组(P<0.05)。结论:EEN可明显改善胃、胰腺肿瘤术后近期的营养和免疫状况,在术后细胞免疫和肠功能恢复方面明显优于肠外营养。 展开更多

Gastro-entero-pancreatic (GEP) neuroendocrine tumors (NETs) are rare neoplasms, although their prevalence has increased substantially over the past three decades. Moreover, there has been an increased clinical recognition and characterization of these neoplasms. They show extremely variable biological behavior and clinical course. Most NETs have endocrine function and secrete peptides and neuroamines that cause distinct clinical syndromes, including carcinoid syndrome; however, many are clinically silent until late presentation with mass effects. Investigation and management should be individualized for each patient, taking into account the likely natural history of the tumor and general health of the patient. Management strategies include surgery for cure or palliation, and a variety of other cytoreductive techniques, and medical treatment including chemotherapy, and biotherapy to control symptoms due to hormone release and tumor growth, with somatostatin analogues (SSAs) and alphainterferon. New biological agents and somatostatintagged radionuclides are under investigation. Advances in the therapy and development of centers of excellence which coordinate multicenter studies, are needed to improve diagnosis, treatment and therefore survival of patients with GEP NETs. 展开更多

We report a rare case of a cystic colonic duplication in a female adult. Preoperative investigations were suggestive of pancreatic tumor. The diagnosis was established based on the histopathological examination of the resected specimen. AIM: To investigate the effects of gastrin and cholecystokinin (CCK) and their specific antagonists on the growth of pancreatic and biliary tract cancer cell lines. METHODS: Five pancreatic and 6 biliary cancer cell lines with 2 conrtol cells were used in this study. Cell proliferation study was done using 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyl tetrazolium bromide (MTT) test and direct cell count method. Reverse transcription-polymerase chain reaction (RT-PCR) and slot blot hybridization were performed to examine and quantify the expression of hormonal receptors in these cell lines. RESULTS: SNU-308 showed a growth stimulating effect by gastrin-17, as did SNU-478 by both gastrin-17 and CCK-8. The trophic effect of these two hormones was completely blocked by specific antagonists (L-365, 260 for gastrin and L-364, 718 for CCK). Other cell lines did not respond to gastrin or CCK. In RT-PCR, the presence of CCK-A receptor and CCK-B/gastrin receptor mRNA was detected in all biliary and pancreatic cancer cell lines. In slot blot hybridization, compared to the cell lines which did not respond to hormones, those that responded to hormones showed high expression of receptor mRNA. CONCLUSION: Gastrin and CCK exert a trophic action on some of the biliary tract cancers. 展开更多

Gastroscopic examination of this 63-year-old male patient showed multiple protrusions in gastric corpus near the greater curvature, identified by subsequent biopsy as carcinoid, Distal subtotal gastrectomy was performed, Histological and immunohistochemical examinations showed a carcinoid tumor in gastric corpus near the greater curvature, an intramucosal SRC at the lesser curvature of corpus and heterotopic pancreatic tissue in muscularis propria of the antrum at the lesser curvature with hyperplasia of peripheral endocrine cells producing multiple pancreatic hormones, We reviewed the literatures on clinicopathological characteristics and the differential diagnosis of the above three abnormalities, and concluded that the carcinoid in corpus near the greater curvature and SRC in the lesser curvature are independent lesions; the loci of endocrine cells in the muscularis propria and serosa are hyperplastic lesions from the heterotopic pancreatic tissue, rather than dissemination of carcinoid in corpus. 展开更多
