目的 :探讨各种完全胃肠外营养 (TPN)对胆源性胰腺炎胆囊收缩及胆汁酸成分改变的作用。 方法 :选择胆源性胰腺炎住院病人共 86例 ,在常规治疗相同的基础上 ,随机分成四组 ,TPN支持分别为仅用糖盐水 (GNS组 )、糖盐水加用 7%凡命 (VM组 ... 目的 :探讨各种完全胃肠外营养 (TPN)对胆源性胰腺炎胆囊收缩及胆汁酸成分改变的作用。 方法 :选择胆源性胰腺炎住院病人共 86例 ,在常规治疗相同的基础上 ,随机分成四组 ,TPN支持分别为仅用糖盐水 (GNS组 )、糖盐水加用 7%凡命 (VM组 )、糖盐水加用 2 0 %英脱利匹特 (IL组 )、糖盐水加用 7%凡命及 2 0 %英脱利匹特(VM +IL组 )。B超测量胆囊体积及计算最大胆囊排空率 (MGER) ,气相色谱法检测胆汁游离脂肪酸 (FFA)。 结果 :发现VM +IL组及VM组与GNS及IL组比较MGER显著升高 ,差异显著 (P <0 .0 5 ) ;VM +IL组及IL组与GNS组比较 ,各种胆汁FAA构成比较差异显著 (P <0 .0 5 )。 结论 :TPN中加入VM +IL或VM会促进胆囊排空 ,加入VM +IL或IL会减轻胆汁FFA变化程度 。 展开更多

目的探讨外源性八肽胆囊收缩素(CCK-8)对谷氨酸诱导体外大鼠大脑皮层神经元损伤模型中神经细胞凋亡的保护作用及其可能作用机制。方法不同浓度CCK-8处理体外培养的大鼠大脑皮层神经元,MTT法检测细胞存活情况并筛选合适浓度。培养大鼠大... 目的探讨外源性八肽胆囊收缩素(CCK-8)对谷氨酸诱导体外大鼠大脑皮层神经元损伤模型中神经细胞凋亡的保护作用及其可能作用机制。方法不同浓度CCK-8处理体外培养的大鼠大脑皮层神经元,MTT法检测细胞存活情况并筛选合适浓度。培养大鼠大脑皮层神经细胞并用谷氨酸(Glu)处理建立神经元损伤模型,分为模型组、CCK-8组,另设对照组。采用流式细胞术检测细胞周期与凋亡率;Flou-4实验检测各组细胞Ca^(2+)水平;实时定量PCR法检测B细胞淋巴瘤因子2(Bcl-2)、Bcl-2相关蛋白(Bax)mRNA表达情况;Western blot检测Bcl-2、Bax、cleaved caspase-3蛋白表达情况。结果与对照组比较,模型组神经元细胞存活率、G_(1)期细胞分布比例、Bcl-2 mRNA和蛋白表达水平降低,S期、G_(2)/M期细胞分布比例、细胞凋亡率、Ca^(2+)水平、Bax和cleaved caspase-3蛋白表达水平升高(P<0.05)。与模型组比较,CCK-8组神经元细胞存活率、G_(1)期细胞分布比例、Bcl-2 mRNA和蛋白表达水平升高,S期、G_(2)/M期细胞分布比例、细胞凋亡率、Ca^(2+)水平、Bax和cleaved caspase-3蛋白表达水平降低(P<0.05)。结论CCK-8对谷氨酸诱导的神经元细胞凋亡有一定的抑制作用,可能是通过抑制钙离子内流,调节细胞内Bax/Bcl-2表达水平发挥作用。 展开更多

AIM:It is difficult to differentiate gallstone dyspepsia and functional dyspepsia by clinical symptoms and signs. We hypothesized that gallstone dyspepsia was related to abnormal gallbladder motility. We aimed to differentiate gallstone dyspepsia from functional dyspepsia by measuring gallbladder motility.METHODS: We measured gallbladder volume changes in response to gastric distension (saline 500mL) and fatty meal in 10normal volunteers (controls) and 62 patients with gallstones and dyspepsia before cholecystectomy. Forty cholecystectomized patients were symptom free or had improvement (group I), while the remaining 22 patients had persistent dyspepsia (group Ⅱ). Gallbladder volume change and ejection fraction were analyzed and compared among the three groups.RESULTS:In group I, there were significant decreases in gallbladder volumes 5-25 rain after gastric distension,compared to fasting volumes. Compared to normal volunteers and group Ⅱ, group I had significantly decreased gallbladder volumes 10-20min after drinking 500mL of normal saline and 10 to 50min after eating fatty meal.CONCLUSION:Our results support the hypothesis that increased gallbladder contraction after gastric distension or fatty meal may be related to dyspeptic symptoms in uncomplicated gallstone disease. These findings may be useful in differentiating functional dyspepsia from gallstone dyspepsia, patients with the latter disease may benefit from laparoscopic cholecystectomy. 展开更多

Inhibitory effects of genistein and resveratrol on guinea pig gallbladder contractility in vitro 被引量:3

AIM: To observe and compare the effects of phytoestrogen genistein, resveratrol and 17β-estradiol on the tonic contraction and the phasic contraction of isolated gallbladder muscle strips and to study the underlying mechanisms. METHODS: Isolated strips of gallbladder muscle from guinea pigs were suspended in organ baths containing Kreb’s solution, and the contractilities of strips were measured before and after incubation with genistein, resveratrol and 17β-estradiol respectively. RESULTS: Similar to 17β-estradiol, genistein and resveratrol could dose-dependently inhibit the phasic contractile activities, they decreased the mean contractile amplitude and the contractile frequencies of gallbladder muscle strips, and also produced a marked reduction in resting tone. The blocker of estrogen receptor ICI 182780 failed to alter the inhibitory effects induced by genistein and resveratrol, but potassium bisperoxo (1, 10 phenanthroline) oxovanadate bpV (phen), a potent protein tyrosine phosphatase inhibitor, markedly attenuated the inhibitory effects induced by genistein and resveratrol. In calcium-free Kreb’ssolution containing 0.01 mmol/L egtazic acid (EGTA), genistein and resveratrol inhibited the first phasic contraction induced by acetylcholine (ACh), but did not affect the second contraction induced by CaCl_2. In addition, genistein, resveratrol and 17β-estradiol also could reduce the contractile responses of ACh and KCl, and shift their cumulative concentration-response curves rightward. CONCLUSION: Phytoestrogen genistein and resveratrol can directly inhibit the contractile activity of isolated gallbladder muscle both at rest and in response to stimulation. The mechanisms responsible for the inhibitory effects probably due mainly to inhibition of tyrosine kinase, Ca^(2+) influx through potential-dependent calcium channels (PDCs) and Ca^(2+) release from sarcoplasmic reticulum (SR), but were not related to the estrogen receptors. 展开更多
