晚期胰腺癌的X线三维适形放射治疗 被引量:3

目的:探讨X线三维适形放疗对晚期胰腺癌的治疗可能性和治疗效果。方法:1999年9月至2003年7月,采用X线三维适形放疗治疗晚期胰腺癌28例。根据包绕计划靶区的等剂量线(80%的等剂量曲线)确定治疗剂量,5~10Gy/次,隔日1次,使计划靶区边缘剂... 目的:探讨X线三维适形放疗对晚期胰腺癌的治疗可能性和治疗效果。方法:1999年9月至2003年7月,采用X线三维适形放疗治疗晚期胰腺癌28例。根据包绕计划靶区的等剂量线(80%的等剂量曲线)确定治疗剂量,5~10Gy/次,隔日1次,使计划靶区边缘剂量达35~50Gy。结果:治疗结束8~10周后评价近期疗效,完全缓解(CR)率为32.1%,部分缓解(PR)率为39.3%,无变化(NC)率为28.6%,总有效(CR+PR)率达71.4%。1年累计局部控制率为75%,1年累计生存率为57.1%。不良反应主要为恶心、呕吐,患者均能耐受。结论:X线三维适形放射治疗胰腺癌近期疗效满意,不良反应轻,为不能手术的晚期胰腺癌患者提供了一种有效治疗手段。 展开更多

Despite the wide array of diagnostic tools, preoperative detection of small pancreatic carcinomas remains difficult. We report a case of small pancreatic carcinoma of the head of pancreas with indeterminate findings on US, ERCP, MRI and EUS which was successfully diagnosed via fusion CT-PET. This case illustrates the utility of CT-PET in the diagnosis of patients with small pancreatic carcinoma with equivocal findings on conventional diagnostic modalities. 展开更多

Radiotherapy for locally advanced pancreatic cancer is technically difficult and frequently associated with high-grade digestive toxicity.Helical tomotherapy(HT)is a new irradiation modality that combines megavoltage computed tomography imaging for patient positioning with intensity-modulated fan-beam radiotherapy.Its recent availability opens new fields of exploration for pancreatic radiotherapy as a result of its ability to tailor very well-defined dose distributions around the target volumes.Here,we report the use of HT in two patients with locally advanced pancreatic cancer. Doses to the bowel,kidneys and liver were reduced significantly,which allowed for excellent treatment tolerance without any high-grade adverse effects in either patient. 展开更多

Objective: To evaluate the clinical effect of transarterial infusion chemotherapy of gemcitabine plus three dimen- sional conformal radiotherapy on patients with local advanced pancreatic cancer. Methods: Fifty-one patients with local ad- vanced pancreatic cancer from June 2002 to February 2004 were enrolled, twenty-four patients of combined group were treat- ed with transarterial infusion chemotherapy of gemcitabine plus three dimensional conformal radiotherapy, while twenty-seven patients of control group were treated only with transarterial infusion chemotherapy of gemcitabine. Results: There were significant statistical differences between two groups in clinical benefit response (91.7% versus 74.1%, P < 0.01) and overall remission rate (70.8% versus 33.3%, P < 0.01). The 6-month survival rate, 12-month survival rate and 24-month survival rate of combined group were 83.3%, 62.5% and 37.5% respectively, while that of control group were 55.6%, 33.3% and 11.1% respectively. This showed significant difference between the two groups. Conclusion: Transarterial infusion chemotherapy of gemcitabine plus three dimensional conformal radiotherapy may be better than single transarterial infusion chemotherapy of gemcitabine in improving survival rates and elongating survival time of patients with local advanced pancreatic cancer. 展开更多
