Idiopathic pulmonary arterial hypertension(IPAH) is a rare disease of unknown etiology.The exact pathogenesis of pulmonary arterial hypertension is still not well known.In the past decades,many protein molecules have been found to be in-volved in the development of IPAH.With proteomic techniques,profiling of human plasma proteome becomes more feasible in searching for disease-related markers.In present study,we showed the protein expression profiles of the serum of IPAH and healthy controls after depleting a few high-abundant proteins in serum.Thirteen spots had changed significantly in IPAH com-pared with healthy controls and were identified by LC-MS/MS.Alpha-1-antitrypsin and vitronectin were down-regulated in IPAH and may be valuable candidates for further explorations of their roles in the development of IPAH. 展开更多

Based on the study of the harmonic suppression on DC side of the multi-pulse rectification system,a software platform is established in Matlab environment.The phase-shift angle is studied from the aspects of system stability and economy,analyzing the effects of phase-shift angle on the input-side line current,the output-side voltage ripple,the equivalent capacity of autotransformer and other auxiliary devices in 12-pulse rectifier system.The software platform can complete the analysis only by inputting the initial conditions,eliminating the derivation of the intermediate formula and reducing the complexity of the system analysis,and have good scalability.The simulation results show that the system can effectively analyze the influence of phase angle on 12-pulse star-connected transformer. 展开更多

Under the inflammable or explosive environment, the direct measurement methods by opening up the explo- sion-proof shell of electrical installations were not adopted. So, it's impossible to have a quantitative analysis on the limit of conducted disturbance for electrical fast transient burst (EFT/B) in such dangerous environments. Transient conducted coupling model, which using EFT/B as its excitation source, can be built based on circuit and electromagnetic field theory. Furthermore, numerical analysis was performed. The results indicate that the capacitive coupling voltage is the same polarity as EFT/B, and is the main disturbance form of conducted coupling in mines. The inductive coupling voltage is reversed polarity with the ca- pacitive coupling voltage, and both peaks appear only in the rising time of EFT/B, which increase with the rising of load resistance. Moreover, the cable coupling voltage on the side of disturbance source is higher than the one on the other side in tunnel. To reduce the common resistance can suppress the resistive coupling disturbance. 展开更多

Assessment of Heart Rate Variability and Heart Rate Turbulence Parameters in the Patients with Arterial Hypertension of the II Degree

Background: AH (Arterial hypertension) due to the extremely high incidence is the most important risk factor for development of myocardial infarctions, strokes and lethal outcomes. It is very important to identify a group of patients with increased risk of adverse outcomes to modify treatment and prevention. Significant assistance in resolving this problem is to provide a method of HM (Holter monitoring) which allows estimating the HRV (heart rate variability) and HRT (heart rate turbulence) parameters that are independent risk predictors of sudden cardiac death. The aim of our study was to assess HRV and HRT parameters in patients with AH of the II degree. Materials and methods: We have studied parameters of HRV and HRT in 214 patients with AH of the II-nd degree aged 35 to 70 (57.7 ± 7.6) years: 121 women (56.5%) and 93 men (43.5%). All patients underwent electrocardiography and echocardiography. Statistical analysis of the results has been made using the analytical package Statistics 10.0. Results: The parameters of HRV and HRT in patients with AH of the II degree were studied. Statistically significant correlations of the HRV parameters with the age of patients, average HR, circadian index, and EF (ejection fraction) were revealed. Statistically significant correlations of the HRT parameters with the LVMI (left ventricular mass index), EF, HR average were received. High level of the statistical significance of the interrelation of HRT parameters (TO and TS) has been revealed. Conclusions: The received results can be used during the selection of the group of patients with AH having high risk for unfavorable outcomes. Taking into account the features of the HRV and turbulence parameters and factors associated with them this will enable the individualization of the assessment of the risk in AH as much as possible and prescription of adequate treatment for each patient. 展开更多

目的 探讨以脉搏轮廓动脉压波形分析法(PiCCO)指标指导感染性休克患者液体复苏及血管活性药应用及与预后的关系.方法 2006年12月至2008年6月感染性休克患者为常规治疗组,共纳入30例;2008年7月至2009年10月为PiCCO组,共纳入26例.排除... 目的 探讨以脉搏轮廓动脉压波形分析法(PiCCO)指标指导感染性休克患者液体复苏及血管活性药应用及与预后的关系.方法 2006年12月至2008年6月感染性休克患者为常规治疗组,共纳入30例;2008年7月至2009年10月为PiCCO组,共纳入26例.排除标准为既往有慢性心肺及肝肾功能不全病史.常规组给予EDGT方案复苏.PiCCO组根据每搏变异量及全心舒张末期容积指数进行液体复苏,使全心舒张末期容积指数达到600~750 mL/m2,无房颤时同时考虑每搏变异量<10%;并根据体循环外周阻力指数监测结果应用去甲肾上腺素调整在1300~1500 d·s·cm-5·m-2,根据全心射血分数及心脏指数应用多巴酚丁胺调节心功能,维持平均动脉压≥65 mmHg,同时监测血管外肺水指导液体选择和利尿剂应用.观察患者6 h后中心静脉氧饱和度和乳酸值、3 d的液体平衡状态、28 d生存率、28 d脱离呼吸机时间、住ICU时间和无脏器衰竭时间.结果 两组患者的年龄、性别、病因、APACHEⅡ及SOFA评分、血流动力学状态在液体复苏前差异无统计学意义.PiCCO组与常规组液体复苏6 h后中心静脉氧饱和度和乳酸值差异无统计学意义,28 d生存率和无脏器衰竭时间比较差异无统计学意义(P>0.05).PiCCO组6 h和1 d血管活性药物多巴酚丁胺量[(145.4±24.5)mg vs.(104.2±46.3)mg;(330.2±30.3)mg vs.(202.4±40.3)mg]及去甲肾上腺素用量[(14.5±3.8)mg vs.(10.2±5.6)mg;(38.2±4.2)mg vs.(20.1±6.2)mg]较常规组明显要多(P<0.05);但2d与3 d血管活性药量差异无统计学意义(P>0.05).PiCCO组6 h及3 d液体复苏的量明显减少[(2121±578)mL vs.(2910±987)mL;(3845±435)mL vs.(4545±765)mL;(2467±510)mL vs.(2867±618)mL;(951±332)mL vs.(1472±533)mL],28 d内脱离呼吸机时间[(19.7±8.3)d vs.(15.1±9.1)d]明显增加,住ICU时间明显缩短[(7.5±3.5)d vs.(9.5±3.2)d](P<0.05).结论 以PiCCO的指标指导感染性休克患者液体复苏及血管活性药应用,能更精确进行液体管理,减少盲目补液,减少机械通气时间,缩短住ICU时间. 展开更多
