目的探讨生酮饮食(KD)方案结合脑功能区定位电刺激对癫痫患儿的影响。方法选取2018年1—12月我院收治的94例癫痫患儿为研究对象,随机将其等分为对照组与观察组,对照组实施常规护理,观察组在对照组基础上实施生酮饮食方案结合脑功能区定... 目的探讨生酮饮食(KD)方案结合脑功能区定位电刺激对癫痫患儿的影响。方法选取2018年1—12月我院收治的94例癫痫患儿为研究对象,随机将其等分为对照组与观察组,对照组实施常规护理,观察组在对照组基础上实施生酮饮食方案结合脑功能区定位电刺激,比较两组血浆炎性因子水平以及认知功能改善情况。结果观察组白细胞介素-17A(IL-17A)、干扰素γ(IFN-γ)、环氧化酶-2(COX-2)以及前列腺素F2a(PGF2a)水平均低于对照组(P<0.05);观察组适应性行为、大运动行为、精细运动行为、语言行为以及个人-社交行为评分优于对照组(P<0.05)。结论对癫痫患儿实施生酮饮食方案结合脑功能区定位电刺激,可有效改善血浆炎性因子水平,改善脑发育情况,提高认知功能,值得临床推广应用。 展开更多

目的探索脑电立体定位系统引导伽玛刀治疗颞叶内侧癫痫的方法,评价治疗的效果。方法分析363医院头部伽玛刀治疗室2004年1月~2007年12月确诊并行伽玛刀治疗的54例颞叶内侧癫痫患者,男性患者32例,女性患者22例,年龄8~53岁,平均25岁。采... 目的探索脑电立体定位系统引导伽玛刀治疗颞叶内侧癫痫的方法,评价治疗的效果。方法分析363医院头部伽玛刀治疗室2004年1月~2007年12月确诊并行伽玛刀治疗的54例颞叶内侧癫痫患者,男性患者32例,女性患者22例,年龄8~53岁,平均25岁。采用前瞻性研究方法,进行随访观察,对治疗效果进行评价。结果本组共纳入病例54例,平均周边剂量12Gy(8~16Gy),随访1~6年,平均4.2年,疗效按Engel癫痫外科治疗结果分类及谭启富提出的癫痫外科手术结果评价标准分级:21例患者满意(占38.9%),13例显著改善(占24.1%),9例良好(占16.7%),11例效差(占20.3%);其中治疗部位与EEG、偶极子一致者43例,21例效果满意(占48.8%),10例显著改善(占23.3%),良好6例(占13.9%),6例效差(占13.9%);其余11例不一致的患者中,无一例达满意效果,3例显著改善(占27.3%),良好3例(占27.3%),5例效差(占45.4%);无死亡病例。结论长期随访结果显示良好的癫痫控制率及极低的并发症,脑电、偶极子提示位置一致,疗效好;当脑电图显示致痫灶没有局限于颞叶内侧结构,与偶极子定位结果不吻合时应首选开颅手术,术中皮层电极监测验证癫痫源灶部位。故伽玛刀治疗前必须严格选择病例,治疗靶点确定要有充分依据,严格控制剂量、容积,以提高疗效,降低并发症。 展开更多

Objective To explore the efficacy of target positioning by preoperative CT/MRI image fusion technique in deep brain stimulation.Methods We retrospectively analyzed the clinical data and images of 79 cases(68 with Parkinson's disease,11 with dystonia) who received preoperative CT/MRI image fusion in target positioning of subthalamic nucleus in deep brain stimulation.Deviation of implanted electrodes from the target nucleus of each patient were measured.Neurological evaluations of each patient before and after the treatment were performed and compared.Complications of the positioning and treatment were recorded.Results The mean deviations of the electrodes implanted on X,Y,and Z axis were 0.5 mm,0.6 mm,and 0.6 mm,respectively.Postoperative neurologic evaluations scores of unified Parkinson's disease rating scale(UPDRS) for Parkinson's disease and Burke-Fahn-Marsden Dystonia Rating Scale(BFMDRS) for dystonia patients improved significantly compared to the preoperative scores(P<0.001); Complications occurred in 10.1%(8/79) patients,and main side effects were dysarthria and diplopia.Conclusion Target positioning by preoperative CT/MRI image fusion technique in deep brain stimulation has high accuracy and good clinical outcomes. 展开更多

Television Programs for Teenagers and the Use of Multimodality in Knowledge Popularization

The aim of this work is to study the interaction of spoken words and images that are used within programs targeted to popularize knowledge among teenagers. We will look into the program Bit@bit developed by Television de Catalunya, which is intended to disseminate computer concepts, tools and procedures. In this kind of program, one can find spoken language (on/off screen voices, interviews, etc.), other oral material (sounds, music, songs), written language (descriptions, title sequences, inserts and Uniform Resource Locator, or electronic addresses), and other visual material (film cut images, advertising, other TV programs, cartoons, films, both location or studio images, depicting reality, or computer screens, or logotypes). The profusion of multimodal elements used in this program helps to keep the young audience's attention. The use of references to their world or knowledge, as a target group, is based on interplay among different semiotic strategies. At the same time, the entire program's multi-segmentation into short video clips, combined with a fast paced sound track helps its audience to understand the specialized explanations presented. Of all the multimodal wealth that the program presents, we will concentrate only on the relationship between the words and the images (leaving other secondary semiotics to one side), the main strategy for achieving the objective of transmitting knowledge and winning adolescent audiences. 展开更多

The conventional quality control method of core rocldill dam construction exhibit difficulty controlling compaction parameters accurately or ensuring construction quality. This is because it is easily influenced by human behavior or lack of adequate management. We therefore establish the timely monitoring indexes and control criteria of compaction processes by considering the characteristics and quality requirements of high core rockffll dam construction. Based on the established indexes and criteria, integrating GPS, GPRS and PDA technologies, a real-time compaction quality monitoring method is proposed. The relevant key techniques are proposed as well, including automatic collection of information and a graphic algorithm for rolling-process visualization. By the proposed method and techniques, a real-time monitoring system is provided to realize the precise automatic online entire-process monitoring of compaction parameters, including compaction pass, rolling trajectory, nmning speed of roller, vibration status and rolled pavement thickness. The application of the Nuozhadu project shows that the proposed system can control compaction parameters effectively and ensure better construction quality. Therefore, it might become a new way towards construction quality control of high core rockfill dam. 展开更多
