目的采用IDEAL-IQ (iterative decomposition of water and fat with echo asymmetry and least-squares estimation quantitation)技术定量测定椎旁肌肉脂肪含量,评价椎旁肌肉脂肪含量与椎间盘退变之间的关系。材料与方法共招募临床以...目的采用IDEAL-IQ (iterative decomposition of water and fat with echo asymmetry and least-squares estimation quantitation)技术定量测定椎旁肌肉脂肪含量,评价椎旁肌肉脂肪含量与椎间盘退变之间的关系。材料与方法共招募临床以腰背痛或坐骨神经痛患者57名(其中男21例,女36例),行腰椎常规T2WI(T2-weightedimaging)序列矢状位及IDEAL-IQ序列轴位扫描,利用Pfirrmann分级标准对每个腰椎间盘进行分级,在AW 4.6工作站上利用IDEAL-IQ序列轴位图像测量所有L1~S1椎间盘中心层面椎旁肌的脂肪百分比(Fat fraction,FF)和横截面积(cross-sectional area,CSA);椎旁肌肉根据同层面椎间盘Pfirrmann分级相应的分为5组;用ANOVA (Analysis of Variance)单因素方差分析比较不同腰椎间盘Pfirrmann分级组间相应层面椎旁肌肉FF及CSA差异;用Spearman相关性分析评价腰椎间盘Pfirrmann退变程度与椎旁肌肉FF及CSA的相关性;采用多元线性回归分析以评估性别、年龄、腰椎间盘退变程度是否为腰大肌(psoasmuscle,PS)、竖脊肌(erectorspinaemuscle,ES)、多裂肌(multifidus muscle,MF)的FF和CSA的独立影像因素,以P<0.05为差异有统计学意义。结果不同腰椎间盘Pfirrmann分级组间椎旁肌肉FF及CSA的差异均有统计学意义;腰椎间盘Pfirrmann分级与PS、ES和MF的FF均存在相关性(相关系数分别为0.208,0.392,0.433,P值均小于0.001),腰椎间盘Pfirrmann分级与PS、ES和MF的CSA均存在相关性(相关系数分别为0.170,-0.284,0.240,P值分别为0.004、<0.001、<0.001);性别、年龄和腰椎间盘Pfirrmann分级是PS和MF的FF的独立影响因素,腰椎间盘Pfirrmann分级是ES的FF的独立影响因素;性别、年龄和腰椎间盘Pfirrmann分级是PS和MF的CSA独立影响因素,性别和腰椎间盘Pfirrmann分级是ES的CSA独立影响因素。结论 ES和MF的脂肪浸润程度与腰椎间盘退变程度存在中等程度相关性;腰椎间盘Pfirrmann分级是PS、ES和MF的FF和CSA独立的影响因素。展开更多

To provide a highly efficient adenov iral vector Ad CMV hTGFβ1 for the study of gene therapy for reversion of the intervertebral disc degeneration.Methods: A newly developed recombinant adenoviral vector constr uctio...To provide a highly efficient adenov iral vector Ad CMV hTGFβ1 for the study of gene therapy for reversion of the intervertebral disc degeneration.Methods: A newly developed recombinant adenoviral vector constr uction system was used in the study. The cDNA of hTGFβ1 was first subcloned into a shuttle plasmid pShuttle CMV. The resultant plasmid was linearized by d ig esting with restriction endonuclease PmeI, and subsequently transformed into E .coli. BJ5183 cells with an adenoviral backbone plasmid pAdEasy 1. Recombinants were selected by kanamycin resistance and confirmed by restriction endonuclease analysis. Finally, the recombinant plasmid linearized by PmeI was transfected in to 293 cells. Recombinant adenoviruses were generated within 2 weeks. Results: The recombinant adenoviral plasmids were cut by BamHI and PacI respectively, and the diagnostic fragments appeared in 0.8 % agarose electrophoresis. The infected 293 cells showed evident cytopathic effect (CPE). The productions of PCR confirmed the presence of recombinant adenovirus. The exp ression of hTGFβ1 was verified by immunohistochemical staining. Conclusions: The successful generation of the adenoviral vector Ad CMV hTGFβ1 and the confirmation of the interest gene expression make it p ossible for the experimental study of the reversion of the intervertebral disc d egeneration by gene therapy.展开更多
Objective:To investigate the influence of lumbar disc degeneration on the efficacy of lumbar fixed-point rotation manipulation in sitting position in treating lumbar intervertebral disc herniation(LIDH).Methods:We...Objective:To investigate the influence of lumbar disc degeneration on the efficacy of lumbar fixed-point rotation manipulation in sitting position in treating lumbar intervertebral disc herniation(LIDH).Methods:We simulated lumbar fixed-point rotation manipulation of sitting position using three finite element models including a normal model,a mild degeneration model and a moderate degeneration model of L3-5,in which the herniated disc was assumed at the left rear of L4 disc and the rotation manipulation was carried out on the right side.The displacement and stress at the left rear of L4 disc of the three models were analyzed.Results:When lumbar fixed-point rotation manipulation in sitting position was carried out,a displacement and stress were generated at the left rear of L4 intervertebral disc of the three models directing forward.The displacement and stress in degeneration models were less than those in the normal model,and the smallest values were found in the moderate degeneration model.From normal model to mild and then to the moderate degeneration model,the displacement decreased by 36% and 59%,and the stress decreased by 22.3% and 45.2%,respectively.Conclusion:The lumbar disc degeneration affects adversely the effectiveness of lumbar fixed-point rotation manipulation in sitting position in the treatment of LIDH.The severer the lumbar degeneration,the greater the influence.展开更多
