METHODS: PAIgG titers and immune markers were determined in 24 type C cirrhotic patients with an intact spleen, 17 type C cirrhotic patients submitted to splenectomy, and 21 non-C cirrhosis with an intact spleen. RESULTS: Thrombocytopenia (PLT〈15×10^4/μL) in type C cirrhosis was diagnosed in all patients with an intact spleen, 8 patients submitted to splenectomy, and in 19 non-C cirrhosis with intact spleen. Elevated titers of PAIgG at more than 25.0 ng/107cells were detected in all cirrhotic patients except for one splenectomized patient. PAIgG titers (ng/10^7cells) were significantly higher in the type C cirrhosis with an intact spleen (247.9 ± 197.0) compared with the splenectomized patients (125.6±87.8) or non-C cirrhosis (152.4± 127.4). PAIgG titers were negatively correlated with platelet counts in type C cirrhotic patients with an intact spleen. In comparison with the type C cirrhosis with an intact spleen, the splenectomized patients had a reduced CD4/CD8 ratio and serum neopterin levels. The spleen index (cm^2) was negatively correlated with platelet counts in the non-C cirrhosis, but not in the type C cirrhosis. CONCLUSION: Our data indicate that the autoimmune mechanism plays an important role in thrombocytosis complicated by HCV-positive cirrhosis. In addition, splenectomy may impair T cells function through, at least in part, a reduction of CD4/CD8 ratio, consequently suppressing PAIgG production.展开更多

Here, we report a case of ITP that developed during the f...Although both primary biliary cirrhosis (PBC) and idiopathic thrombocytopenic purpura (ITP) are autoimmune diseases, the association of the 2 diseases is rare. Here, we report a case of ITP that developed during the follow-up of PBC in a 74-year- old man. The patient had been diagnosed with PBC 12 years previously, and had received treatment with ursodeoxycholic acid. The platelet count decreased from approximately 60 × 109/L to 8 × 109/L, and the association of decompensated liver cirrhosis (PBC) with ITP was diagnosed. Steroid and immune gamma globulin therapy were successful in increasing the platelet count. Interestingly, human leukocyte antigen genotyping detected the alleles DQB10601 and DRB10803, which are related to both PBC and ITP in Japanese patients. This case suggests common immunogenetic factors might be involved in the development of PBC and ITP.展开更多

Alopecia areata is frequently associated with immune-medi- ated diseases with skin manifestations such as psoriasis and lichen planus, or without skin manifestations such as autoimmune thyroiditis and idiopathic thrombocytopenic purpura. Helicobacter py/ori (H. pylorl) infection is present in around 50% of the world's population and has been associated with a variety of immune-mediated extra-digestive disorders including autoimmune thyroid- itis, idiopathic thrombocytopenic purpura, and psoriasis. A case of a 43-year old man with an 8-too history of alopecia areata of the scalp and beard is presented. The patient was being treated by a dermatologist and had psychiatric support, without any improvement. He had a history of dyspepsia and the urea breath test confirmed H. pylori infection. The patient went into remission from alopecia areata after H. pylori eradication. If such an association is confirmed by epidemiological studies designed for this purpose, new therapeutic options could be available for these patients, especially in areas where infection with H, pylori is highly prevalent.展开更多

The results showed significant changes in some parameters in comparison with control animals started as early as 15 minutes and intensified by the continuous exposure time, leading to significant increase (p 〈 0.05) in the number of both WBC (white blood cells), PLT (blood platelets) and decrease in numbers of RBC (red blood corpuscles) while hemoglobin percentages (Hb%) showed an insignificant (p 〉 0.05) fluctuating rhythm. The percentages of both Neutrophil and Lymphocyt recorded a significant increase (p 〈 0.05) 1 hour post-exposure to the DEE while other WBCs showed less changes. It is concluded that blood components are so sensitive to DEE and do respond quicker to inhaled pollutants via changes in some components of WBC% but decrease in RBC% and the likelihood of blood clotting due to increase in blood platelets. These results indicate the acute toxic risks of the exposure to DEE of mechanics and technicians who work in vicinity of DEE sources or/and spend most daily hours in semi closed areas.展开更多
Objective:The aim of this study was to evaluate the response rate,time to progression(TTP),overall survival,and safety of the combination of capecitabine plus oxaliplatin in treatment of advanced gastric cancer(AGC).M...Objective:The aim of this study was to evaluate the response rate,time to progression(TTP),overall survival,and safety of the combination of capecitabine plus oxaliplatin in treatment of advanced gastric cancer(AGC).Methods:All the patients with advanced gastric cancer who were not received any prior chemotherapy or radiotherapy were treated with combination of capecitabine(1250 mg/m2 twice daily,days 1-14) plus oxaliplatin(130 mg/m2 as a 2-h intravenous infusion on day 1) every 3 weeks.Results:Two cases of complete response(CR) and 34 cases of partial response(PR) were confirmed,giving an overall response rate of 52.9%,of the 68 patients with advanced gastric cancer.The median TTP and overall survival for all patients were 7.3 and 11.9 months,respectively.Grade 3 leukopenia,thrombocytopenia,nausea/vomiting,and diarrhea were observed in 3,5,1,and 4 patients,respectively.Yet,no grade 4 toxicity was observed.Conclusion:Capecitabine/ oxaliplatin combination chemotherapy is active in patients with advanced gastric cancer.展开更多

Organoselenium immobilized polyethyleneimine as NO donor catalyst and sodium algi...A new biomimetic material for artificial blood vessel with in situ catalytic generation of nitric oxide(NO) was prepared in this study. Organoselenium immobilized polyethyleneimine as NO donor catalyst and sodium alginate were alternately loaded onto the surface of electrospun polycaprolactone matrix via electrostatic layer-by-layer self-assembly. This material revealed significant NO generation when contacting NO donor S-nitrosoglutathione(GSNO). Adhesion and spreading of smooth muscle cells were inhibited on this material in the presence of GSNO, while proliferation of endothelial cells was promoted. In vitro platelet adhesion and arteriovenous shunt experiments demonstrated good antithrombotic properties of this material, with inhibited platelet activation and aggregation, and prevention of acute thrombosis. This study may provide a new method of improving cellular function and antithrombotic property of vascular grafts.展开更多
