温督通脉隔药火疗法对腰椎间盘突出症患者腰椎功能、运动功能的影响

目的 探讨温督通脉隔药火疗法对腰椎间盘突出症(LDH)患者腰椎功能、运动功能的影响,以期为临床相关治疗提供参考。方法 选取2021年10月—2022年10月贵州省黔南州中医医院收治的LDH患者60例为研究对象。采用随机数字表法分为常规针刺组... 目的 探讨温督通脉隔药火疗法对腰椎间盘突出症(LDH)患者腰椎功能、运动功能的影响,以期为临床相关治疗提供参考。方法 选取2021年10月—2022年10月贵州省黔南州中医医院收治的LDH患者60例为研究对象。采用随机数字表法分为常规针刺组、隔药火疗组,每组30例。常规针刺组患者进行常规针刺治疗,隔药火疗组在常规针刺组基础上进行温督通脉隔药火疗法治疗。比较2组患者治疗前后(治疗后为治疗2周后)中医证候、视觉模拟量表(VAS)、腰椎功能评分表(JOA)、Oswestry功能障碍指数问卷(ODI)、Fugl-Meyer评定量表(FMA)评分及5-羟色胺(5-HT)、P物质(SP)、神经肽Y(NPY)水平;治疗结束后比较2组患者治疗效果。结果 治疗前,2组患者中医证候中腰部疼痛、运动受限、下肢麻木评分比较,差异无统计学意义(P>0.05);治疗后,2组患者中医证候中腰部疼痛、运动受限、下肢麻木评分均低于治疗前,且隔药火疗组低于常规针刺组,差异均有统计学意义(P<0.05)。治疗前,2组患者5-HT、SP、NPY水平比较,差异无统计学意义(P>0.05)。治疗后,2组患者5-HT、SP、NPY水平均低于治疗前,且隔药火疗组低于常规针刺组,差异均有统计学意义(P<0.05)。治疗前,2组患者JOA、ODI、VAS评分及腰部关节活动度比较,差异无统计学意义(P>0.05)。治疗后,2组患者JOA评分及腰部关节活动度大于治疗前,且隔药火疗组大于常规针刺组,差异均有统计学意义(P<0.05);治疗后,2组患者ODI、VAS评分小于治疗前,且隔药火疗组小于常规针刺组,差异均有统计学意义(P<0.05)。治疗前,2组患者FMA中旋转、屈伸、侧屈评分比较,差异无统计学意义(P>0.05);治疗后,2组患者FMA中旋转、屈伸、侧屈评分高于治疗前,且隔药火疗组高于常规针刺组,差异均有统计学意义(P<0.05)。隔药火疗组患者治疗总有效率高于常规针刺组,差异有统计学意义(P<0.05)。结论 使用常规针刺联合温督通脉隔药火疗法对LDH患者进行治疗,能够减轻患者中医证候,减轻患者疼痛,改善患者腰椎功能、腰部功能障碍、腰部关节活动度、下肢运动功能状况,为LDH的临床治疗提供一定的参考依据。 展开更多

CLINICAL OBSERVATION ON STAGE-AIDED TREATMENT OF 62 CASES OF PERIPHERAL FACIAL PARALYSIS WITH ACUPUNCTURE,MOXIBUSTION AND CUPPING

Objective: To probe the best therapy for peripheral facial paralysis. Objective: To probe the best therapy for peripheral facial paralysis. Methods: A total of 122 cases of facial paralysis patients were randomized into treatment group (n=62, treating the disease by stages) and control group (n=60). For patients at the acute stage in treatment group, main point Yifeng (TE 17) was pricked first, followed by performing cupping and moxibustion, for patients at the resting stage, main point Hegu (LI 4) was punctured with reducing needling method, combined with other acupoints in the light of the concrete situations. For patients at the restoration stage, main point Zusanli (ST 36) was punctured with reinforcing method in combination with seven-star-needle tapping at the local affected region. Patients of control group were treated with routine method by puncturing Fengchi (GB 20), Yifeng (TE 17), Jiache (ST 6), Hegu (LI 4), Taichong (LR 3), etc.. The treatment was conducted once daily, with 10 days being a therapeutic course. Results: Following 3 courses of treatment, of the 62 cases in treatment group, 44 (70.9%) were cured, 12 (19.4%) had remarkable improvement in their symptoms and signs, 6 (9.7%) had amelioration, with the cure plus markedly effective rate being 90.3%; of the 60 cases in control group, 30 (50.0%) were cured, 12 (20.0%) had apparent improvement, 16 (26.7%) had amelioration, and the rest 2 (3.3%) failed in the treatment, with the cure plus markedly effective rate being 70.0%. Ridit analysis showed that the cure rate and cure plus markedly effective rate of treatment group were significantly higher than those of control group (P<0.05). Conclusion: Stage-aided acupuncture treatment is superior to routine treatment for facial palsy. 展开更多

Is fire needle superior to Western medication for herpes zoster?A systematic review and meta-analysis 被引量:3

Objective:To compare the effectiveness of fire needle versus Western medicine in the treatment of herpes zoster.Methods:Randomized controlled trials comparing fire needle with Western medicine in the treatment of herpes zoster were identified using 8 databases.A meta-analysis was performed using RevMan 5.3 software.Results:Eight trials involving 569 patients were included in this meta-analysis,and the results showed that fire needle was superior to Western medicine comparing the effective rate[risk ratio(RR)=1.13,95%confidence interval(Cl):1.06 to 1.20;P=0.0002],the visual analog scale(VAS)score[mean difference(MD)=-7.95;95%Cl:-10.71 to-5.20;P<0.00001],time of pain disappearance(MD=-7.61;95%CI:-9.38 to-5.84;P<0.00001),time of blister-stop(MD=-1.34,95%CI:-1.51 to-1.18;P<0.00001),time of crusted scab(MD=-2.92,95%CI:-3.62 to-2.23;P<0.00001);and time of scab off(MD=-4.64,95%CI:-5.83 to-3.46;P<0.00001).In addition,a significantly lower incidence of postherpetic neuralgia was found in the fire needle group in 30 d(RR=0.23,95%CI:0.11 to 0.51;P=0.0002)and 60 d(RR=0.33,95%CI:0.12 to 0.91;P=0.03)after treatment.Conclusion:Fire needle has a favorable effect in increasing the effective rate,relieving pain,recovering skin lesions and decreasing incidence of postherpetic neuralgia in the treatment of herpes zoster.However,considering the limitations in this study,the findings should be interpreted cautiously. 展开更多
