循环血量(CBV)作为主要的血流动力学参数,在心血管疾病的病情评估和手术监护中具有重要的临床应用价值。将吲哚菁绿色素(ICG)作为示踪剂的脉搏色素谱法,通过建立ICG稀释排泄的色素谱曲线,实现CBV的在体无创测量。在实际临床应用中,由于... 循环血量(CBV)作为主要的血流动力学参数,在心血管疾病的病情评估和手术监护中具有重要的临床应用价值。将吲哚菁绿色素(ICG)作为示踪剂的脉搏色素谱法,通过建立ICG稀释排泄的色素谱曲线,实现CBV的在体无创测量。在实际临床应用中,由于受到血氧波动和环境背景光等干扰因素的影响,脉搏色素谱法测量CBV的准确度低于预期值。为解决这一问题,研究了一种基于修正脉搏色素谱的循环血量检测方法。具体操作是,在患者的肘静脉处注入吲哚菁绿试剂,利用光电传感器分别采集特征波长点的透射光谱信号和背景光电信号,采用差分算法消除血氧波动和环境背景光的干扰影响,建立准确的ICG色素谱曲线,从而计算CBV等血流动力学参数。与^(131)I同位素"金标准法"相比较的试验结果表明,该研究提出的基于修正脉搏色素谱的循环血量检测方法,将CBV测量的平均相对误差从6.85%降低为4.53%,显著提高了其测量准确度。 展开更多

目的观察鸟巢式护理在新生儿护理中的应用效果。方法选择本院2017年10月—2018年10月接收的新生儿作为研究对象,一共60例,随机分组,各组为30例,对照组新生儿使用传统方式包裹新生儿,观察组新生儿在常规护理的前提下,使用鸟巢式护理措施... 目的观察鸟巢式护理在新生儿护理中的应用效果。方法选择本院2017年10月—2018年10月接收的新生儿作为研究对象,一共60例,随机分组,各组为30例,对照组新生儿使用传统方式包裹新生儿,观察组新生儿在常规护理的前提下,使用鸟巢式护理措施进行干预。比较两组新生儿的血氧饱和度、体温波动情况、睡眠时间、出暖箱时间、疼痛评分、体质量、身高、头围增长情况。结果和对照组对比,观察组新生儿的各项指标明显更优,数据差异存在统计学意义,P<0.05。结论鸟巢式护理在新生儿护理中的应用效果确切满意,可以有效地改善新生儿生长发育指标。 展开更多

Detrended fluctuation analysis (DFA) is fit for studies on the long-range exponential correlation of non-stationary time serial. In this paper, in order to find a hy- poxia adaptability evaluation criterion, the heart rate and SaO2 signals are analyzed by this method. The demarcate exponent about fit-good-group and fit-bad-group in hy- poxia and normal air are calculated and compared. The result shows a is different in different situation, the α in hypoxia is much higher than α of breath in normal air. And α of fit-good-group is higher than fit-bad-group. It shows that DFA could be a good criterion to analyze hypoxia adaptability, which is useful in the analysis of hypoxia phys- iology signal. 展开更多

Prediction of pulmonary arterial wedge pressure from arterial pressure or pulse oximetry plethysmographic waveform

Objective To assess the possibility of using arterial pressure waveform or pulse oximetry plethysmographic waveform variation to estimate the pulmonary arterial wedge pressure (PAWP) Methods Fourteen American Society of Anesthesiologists grade Ⅰ-Ⅱ patients aged 33-69 years and weighing 62 0±9 5 kg scheduled for elective abdominal tumor surgery were studied Their hemoglobin exceeded 120 g/L and hematocrit exceeded 35% Pre operative acute hypervolemic hemodilution was applied immediately after general anesthestic induction and tracheal intubation PAWP, systolic pressure variation (SPV), delta down (dDown), SPV plet , dDown plet and other hemodynamic parameters were measured and recorded when total fluid volume (crystalloid and colloid) infused reached 10 ml/kg and 20 ml/kg and again at the end of the operation Central venous pressure was maintained at 10-12 mm Hg during operation Systolic blood pressure at the end of Valsalva maneuver (airway pressure was kept at 22 mm Hg) and the systolic pressure before the Valsalva manoeuvre during apnea were used to calculate arterial pressure ratio (APR) Results APR, SPV, dDown, SPV plet and dDown plet all correlated well with PAWP ( r =0 717, -0 695, -0 680, -0 522 and -0 624 respectively, P <0 01) There was a closer linear correlation between APR and PAWP than between the other parameters The regression equation was PAWP (mm Hg)=0 207×APR (%)-0 382 Conclusion During positive pressure mechanical ventilation, APR, SPV, dDown, SPV plet and dDown plet can be used to estimate PAWP effectively 展开更多
