Here, we report a case of giant hepatobiliary cystadenoma in a male patient with obvious convex papillate. On the basis of imaging examinations, the patient was diagnosed as hepatobiliary cystadenoma prior to operation. Left hepatectomy was performed and the patient was symptom-free during a 6-too follow-up period, suggesting that imaging examination is the major diagnostic method of hepatobiliary cystadenoma, and operation is its best treatment modality.展开更多

METHODS The results of ser...OBJECTIVE To investigate the diagnostic value of modified prostate specific antigen(PSA)parameters in the diagnosis of prostate cancer(PCA) when the serum PSAis in a grey zone of 4~10 ng/ml. METHODS The results of serum PSA determinations of the patients receiving a transrectal ultrasound-guided multiphase prostatic biopsy,were retrospectively analyzed.In the 88 patients with a serum PSA value of 4-10 ng/ml,the final diagnosis of PCA was made in 21,and that of benign prostate hyperplasia(BPH)in 67 patients.The percentage of the free-serum PSA([FPSA]/total-serum PSA[TPSA],F/T),PSA density(PSAD)and the sensitivity and specificity of the new PSA modified parameter(F/T)/PSAD in diagnosing PCA,within a set threshold value,was compared. RESULTS In the 88 patients with serum PSA in the grey zone of 4.0-10.0 ng/ml,there was no significant difference in comparing the TPSA between the 21 PCA patients and 67 BPH patients(P>0.05).However, there was a significant difference in the value of modified PSA parameters, such as F/T,PSAD and(F/T)/PSAD,between the PCA and the BPH groups (P<0.001).As the cut off point-value of the F/T,PSAD and(F/T)/PSAD was set at 0.16,0.15 and 0.8,the diagnostic sensitivity for PCA was 66.7%, 76.2%and 85.7%,and the specificity was 41.8%,43.3%and 68.7%,respectively.There was no significant difference in the sensitivity comparing the modified parameters for diagnosing PCA(P>0.05),whereas an overt predominance was present in the specificity of(F/T)/PSAD for PCAdiagnosis (P<0.05). CONCLUSION In the serum PSA grey zone of 4-10 ng/ml,a modified PSA parameter can improve the PCA diagnostic accuracy rate.With a considerably high sensitivity,application of the(F/T)/PSAD may effectively enhance the diagnostic specificity,which is superior to the F/T and PSAD, and can be expected to be one of the new indices derived from the PSA.展开更多
Two classification and identification methods based on pattern discrimination models and the majority-vote technique were investigated for implementing a World Wide Web-based system for the identification of rice dise...Two classification and identification methods based on pattern discrimination models and the majority-vote technique were investigated for implementing a World Wide Web-based system for the identification of rice diseases. The experiment was carried out using color and shape patterns in 425 images of three rice diseases, which were classified into four classes: two classes of leaf blast, and one class each of sheath blight and brown spot. A method consisting of two discrimination steps involving application of multiple discrimination models of a support vector machine gave the best result because of its capacity to evaluate the similarity of disease types. This accuracy of the method was 88% for leaf blast (A-type), 94% for sheath blight, and 80% for leaf blast (B-type) and brown spot; on average, the accuracy of this method was 5% greater than that of the other method when three classes were used in the model. Although the accuracy of both methods was inadequate, the results of this study show that it is possible to estimate the least number of possible or similar diseases from a large number of diseases. Therefore, we conclude that there is merit in grouping classes into subgroups rather than attempting to discriminate between all classes simultaneously and that these methods are effective in identifying diseases for web-based diagnosis.展开更多

Recently, a statistics pattern analysis (SPA) framework has been incorporated into PCA model to make full use of various statistics of data variables effectively. However, these methods omit the local information, which is also important for process monitoring and fault diagnosis. In this paper, a local and global statistics pattern analysis (LGSPA) method, which integrates SPA framework and locality pre- serving projections within the PCK is proposed to utilize various statistics and preserve both local and global in- formation in the observed data. For the purpose of fault detection, two monitoring indices are constructed based on the LGSPA model. In order to identify fault variables, an improved reconstruction based contribution (IRBC) plot based on LGSPA model is proposed to locate fault variables. The RBC of various statistics of original process variables to the monitoring indices is calculated with the proposed RBC method. Based on the calculated RBC of process variables' statistics, a new contribution of process variables is built to locate fault variables. The simula- tion results on a simple six-variable system and a continuous stirred tank reactor system demonstrate that the proposed fault diagnosis method can effectively detect fault and distinguish the fault variables from normal variables.展开更多

Methods: Between Fe...Objective: To summarize the experience of diagnosis and surgical treatment for a rare invasion manner of gallbladder carcinoma, which was combined with tumor thrombus in common bile duct (CBD). Methods: Between February 2003 and January 2005, 3 patients with gallbladder carcinoma who were identified of tumor thrombus in common bile duct in surgical procedure were retrospectively analyzed. Results: Abdominal ultrasound and magnetic resonance cholangiopancreatography (MRCP) were used for preoperative diagnosis. All 3 patients were given radical operations, which were composed of cholecystectomy, resection of the extrahepatic biliary duct, cuniform hepatectomy of gallbladder bed, skeletonization of the hepatoduodenal ligament, hilar choledochojejunostomy, and clearance of tumor thrombus from bile duct. Three patients were recovered well after surgery, which were respectively alive for 30 months, 17 months and 23 months without tumor recurrence,and 58 months, 41 months and 40 months for survival time after operation. Conclusion: Gallbladder carcinoma with tumor thrombus in common bile duct was very rare but with relatively special clinical manifestation and characteristic radiography manifestation. MRCP was one of the most potent diagnostic method. The prognosis of gallbladder carcinoma with tumor thrombus in common bile duct after surgical procedure was apparently better than gallbladder carcinoma with invasion of hilar tissues. Radical operation was feasible and safe for obtaining longer survival展开更多
