目的探析肺动脉高压患者的右心室Tei指数应用组织多普勒超声心动图(TDI)进行检测,应用TDI方法检测Tei指数评估急性心肌梗死患者右心室功能的临床意义。方法入选该院肺动脉高压患者84例,按病情严重程度分为3组,同期入选健康体检者30例为... 目的探析肺动脉高压患者的右心室Tei指数应用组织多普勒超声心动图(TDI)进行检测,应用TDI方法检测Tei指数评估急性心肌梗死患者右心室功能的临床意义。方法入选该院肺动脉高压患者84例,按病情严重程度分为3组,同期入选健康体检者30例为对照组,运用超声心动图检测所有研究对象的右心室功能相关指标:正常对照组与不同程度肺动脉高压患者CDFI及PW-TDI检测右心室Tei指数;正常对照组与其他3组组织多普勒超声心动图检测三尖瓣环运动速度。结果肺动脉高压患者应用TDI检测右心室Tei指数显著高于正常对照组(P<0.05);右心室Tei指数检测的较理想测量点为心尖四腔切面前瓣瓣环处;肺动脉高压病人的后瓣三尖瓣环、隔瓣a低于对照组,其余各组病人a、e、s与正常组比较差异无统计学意义(P>0.05)。结论肺动脉高压患者右心室的Tei指数升高,提示病情程度严重;组织多普勒脉冲法可准确全面性评估不同程度的肺动脉高压患者的右心室功能,不受前负荷、心率、心脏形态等因素的影响,计算简便,准确性强,值得临床推广。 展开更多

简介:经食道超声心动描记法(TEE)近来被用于评估疑有外伤性主动脉撕裂(TAD)的患者。虽然传统的TAD治疗以尽快手术修补为基础,但对于有些患者可以将手术延迟。目前提出保守治疗浅表动脉损伤(SAL),可是这些患者的疗效还不清楚。... 简介:经食道超声心动描记法(TEE)近来被用于评估疑有外伤性主动脉撕裂(TAD)的患者。虽然传统的TAD治疗以尽快手术修补为基础,但对于有些患者可以将手术延迟。目前提出保守治疗浅表动脉损伤(SAL),可是这些患者的疗效还不清楚。我们力图描述高危外膜破裂的TEE表现,以选出必须尽快手术治疗的患者。 方法:我们回顾性研究了23例患者(平均年龄42±17岁),通过TEE,18例诊断为外膜下TAD,5例为SAL。所有的TAD患者都通过其他影像检查或手术得以证实,并被分成两组:11人接受了尽快手术治疗(≤12h),7人接受了延期手术或未行手术。所有的患者都被测定了如下数据(单位:mm):假性动脉瘤直径与胸主动脉降支直径的比值(FA/DA),中间瓣厚度,纵隔积血和左侧血胸量。也记录了缩窄综合征的出现情况。所有的SAL患者都采用保守治疗,并用TEE连续性随访。 展开更多

目的:应用斑点追踪显像(STI)技术定量分析肺动脉高压(PAH)患者右室游离壁及室间隔心肌收缩期峰值应变(S)和应变率(SR)及法舒地尔治疗对PAH患者右室收缩功能的影响。方法:PAH组17例,临床确诊为PAH且具有右心功能不全临床表现,超声心动图... 目的:应用斑点追踪显像(STI)技术定量分析肺动脉高压(PAH)患者右室游离壁及室间隔心肌收缩期峰值应变(S)和应变率(SR)及法舒地尔治疗对PAH患者右室收缩功能的影响。方法:PAH组17例,临床确诊为PAH且具有右心功能不全临床表现,超声心动图测量肺动脉收缩压(PASP)>35mmHg。对照组20例,为年龄、性别相匹配的健康体检者。常规二维超声测量右房左右径(RA)、右室左右径(RV)、主肺动脉内径(MPA)、右室壁厚度(RVWT)及右室射血分数(RVEF)。STI测量右室游离壁及室间隔基底段、中段收缩期峰值S、SR,比较上述参数在2组间有无差异,以及应用法舒地尔治疗前后PAH患者上述参数的变化。结果:(1)与对照组比较,PAH组RA、RV、MPA、RVWT均增大,RVEF减小(均P<0.01),PAH组右室游离壁基底段、中段收缩期峰值S、SR,室间隔基底段、中段收缩期峰值S和室间隔基底段收缩期峰值SR均低于对照组(P<0.05或P<0.01)。(2)PAH患者应用法舒地尔治疗2周后,PASP、RA、RV降低(P<0.01),RVEF较用药前增高(P<0.05),右室游离壁基底段、中段收缩期峰值S、SR及室间隔基底段、中段收缩期峰值S和室间隔基底段收缩期峰值SR较治疗前增加(P<0.01或P<0.05)。结论:通过STI技术能敏感、准确地反映法舒地尔治疗前后PAH患者右室心肌收缩功能的改变情况,是评价PAH患者药物治疗效果的新方法。 展开更多

To test the accuracy of real-time three-dimensional echocardiography (RT3DE) imaging system for evaluating left ventricular mass (LVM) in phantom and excised canine heart. Methods Ten left ventricular (LV) wall phantoms made of two rubber-bursas, ten excised canine hearts underwent RT3DE and two-dimensional echocardiography (2DE). In RT3DE "full volume" imaging, the myocardial volume was mea-sured using 2,4,8, and 16-plane method with the analysis software of RT3DE. Mass was then calculated by multiplying the resulting myocardial volume by specific density of myocardial tissue. In 2DE the masses were measured by area-length meth-od. The true LV wall phantom mass was measured by water displacement and the canine LVM was weighed by anatomy, which served as a reference standard. We compared RT3DE or 2DE with true mass. Results In LV wall phantoms, RT3DE correlated with true masses strongly (r = 0.813-0.994) and weakly correlated between 2DE and true masses (r = 0.628). In excised canine hearts, there is an excellent correlation between RT3DE and true masses (r = 0.764-0.991), while 2DE value showed a lesser correlation (r = 0.514). There are no difference between RT-3DE and true masses (P > 0.05) but different between 2DE and true masses (P < 0.05). In different planes, there was no difference between 8-plane and 16-plane (P > 0.05) but different between 8-plane and 2, 4-plane (P < 0.05). Conclusion RT3DE can accurately quantify LVM and provide a new tool to evaluate LV function. For LVM by RT3DE, 8-plane measurement method is the best choice for accuracy and convenience. 展开更多

Objective: To compare a new device (Innocor) for non-invasive measurement of cardiac output (CO) by foreign gas rebreathing method with conventional techniques used in the measurements of cardiac function. Methods: Cardiac outputs measured by Innocor (CORB) were compared with CO obtained by echocardiography (COEC), Swan-Ganz thermodilution (COTD), and left ventricle radiography (COLVR) in 34 patients subjected to cardiac catheterization. Values obtained from the four methods were analyzed by linear regression and paired values were compared by the method of Bland and Altman in SPSS. Results: There was strong positive correlation (r=0.94) between Innocor cardiac output values and the corresponding values obtained by ther-modilution and between COEC and COLVR values. Thermodilution appears to overestimate cardiac output when compared to the values obtained with Innocor by (0.66±0.22) L/min (P<0.0001). There was no correlation between data obtained by Innocor and the corresponding COEC and COLVR values. Conclusion: Innocor CORB is an easy, safe and well established method for non-invasive measurement of cardiac output with good prospects for clinical application in heart disease patients. 展开更多

Comparison study of harmonic imaging (HI) and fundamental imaging (FI) in fetal echocardiography

Objectives: To directly compare the quality of harmonic imaging (HI) and fundamental imaging (FI) in fetal echocardiography and to determine any differences in image quality between the two modalities. Methods: Fetal echocardiograms were performed with the use of FI and HI in 58 fetuses, image quality and visualization of left and right atria, left and right ventricles, mitral and tricuspid valves, aortic and pulmonary valves, left and right ventricular outflow tracts were evaluated and compared between FI and HI. Results: Mean HI scores were higher than mean FI scores (2.73±0.43 vs 2.16±0.69, P <0.001)for all the cardiovascular structures evaluated. Compared with FI, HI improved the image quality and visualization of fetal cardiac structures in subjects with both good (2.73±0.43 vs 2.88±0.32, P <0.001) and suboptimal (1.65±0.41 vs 2.58±0.47, P <0.001) echocardiographic windows. The interobserver correlation coefficient for the grading scores was 0.74 ( P <0.001). Conclusions: harmonic imaging enhances and improves the image quality of fetal echocardiography; and has important potential role in cardiac imaging in the fetus. 展开更多
