Objective To observe the therapeutic effect of acupuncture and rehabilitation on poststroke hemiplegic spasticity, and probe into the indices of efficacy assessment of the disease. Methods According to the randomized, controlled and blind clinical trial design, 229 cases enrolled in accordance with the inclusive criteria were divided into 3 groups based on visiting sequence: treatment group 1 (74 cases), treated with acupuncture [Jianyu (肩髃 LI 15), Quchi (曲池 LI 11), Waiguan (外关 TE 5) etc. on the affected side]; treatment group 2 (75 cases), treated with acupuncture as treatment group 1 and combined with rehabilitation as control group; control group (80 cases), treated with rehabilitation (Bobath facilitation technique). 5 treatments were given each week, continuously for 4 weeks. The clinical efficacy on post-stroke hemiplegic spasticity was observed systematically after the treatment of acupuncture combined with rehabilitation. Results The research showed that the baseline situations of 3 groups were good, the group division was proportionate, indicating strong comparability (P〉0.05). Based on the modified Ashworth Spasticity Scale (modified ASS), the total effective rate was 63.01% (46/73) in treatment group 1, 66.67%(50/75) in treatment group 2 and 44.00% (33/75) in control group, indicating a statistical significant difference among 3 groups (P〈0.05). Based on MAS motor function scale, the function improvement in treatment group 2 was superior to that in treatment group 1 and control group, indicating statistical significant differences among 3 groups (both P〈0.05). Based on modified Barthel index, the daily life activity in treatment group 2 was superior in the improvement as compared with treatment group 1 and control group, but without statistical significant difference indicated (P〉0.05). Conclusion The efficacy of post-stroke hemiplegic spasticity is definite with acupuncture combined with rehabilitation, and the acupuncture program is safe and effective. 展开更多

Objective: To observe the effect of warm joint needling plus rehabilitation techniques on the balance function and quality of life (QOL) of patients with spastic hemiplegia after ischemic cerebral stroke. Methods: Ninety patients with spastic hemiplegia after ischemic cerebral stroke were randomized into a rehabilitation group, a warm joint needling group and an observation group, with 30 cases in each group. The rehabilitation group was intervened by Bobath therapy, the warm joint needling group was treated with joint needling on the affected side plus warm needling, and the observation group was given the same rehabilitation treatment as the rehabilitation group together with the same warm joint needling as the warm joint needling group. The three groups were treated once another day, 1 month as a treatment course for 6 months. Before the treatment, and respectively after 2-week, 1-month, 3-month, and 6-month treatment, the modified Ashworth scale (MAS) was used to measure the anti-spasm ability of the lower limb, the Berg balance scale (BBS) was adopted to evaluate the balance function, and the stroke-specific quality of life scale (SS-QOL) was employed to estimate the QOL. Results: After 3-month and 6-month treatment, the lower-limb MAS scores in the observation group were significantly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05). After 1-month, B-month and 6-month treatment, the BBS scores in the observation group were significantly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05). After 2-week, 1-month, 3-month and 6-month treatment, the SS-QOL scores in the observation group were markedly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05). Conclusion: Warm joint needling plus rehabilitation can effectively improve the lower-limb spasticity state, balance function and QOL in patients with spastic hemiplegia after ischemic cerebral stroke. 展开更多

Objective: To observe the difference in clinical efficacy between governor vessel-unblocking and mind- regulating acupuncture therapy combined with conventional rehabilitation therapy and simply conventional rehabilitation therapy for sensory and motor dysfunction of patients with spinal cord injury. Methods: Forty patients with spinal cord injury (SCI) were randomly assigned into rehabilitation combined with acupuncture group (group A) and rehabilitation group (group B), with 20 patients in each group. In group A, governor vessel-unblocking and mind-regulating acupuncture therapy combined with conventional rehabilitation therapy was adopted, and Baihui(百会 GV 20), Fengffu (风府GV 16), Dazhui (大椎GV 14), Zhiyang (至阳GV 9), Mingmen (命门 GV 4) and Yfioyangguan (腰阳关 GV 3) were adopted as the main acupoints. Conventional rehabilitation therapy was also applied, including the rehabilitation training of joint, motion, respiration, urinary bladder and intestinal tract. In group B, conventional rehabilitation therapy was adopted as same as the group A. Treatment in the two groups was conducted for once a day, 6 times a week, and 12 weeks in total. ASIA motor score (MS), ASIA sensory score (SS) and activity of daily living (ADL) score of patients in the two groups were observed before and after the treatment. Results: Before treatment, the differences in MS, SS and ADL score of patients in the two groups were not statistically significant (all P 〉 0.05), and the results were comparable. After treatment, MS, SS and ADL score of patients in the two groups were all higher than that before the treatment (all P 〈 0.05), and MS, SS and ADL score of the patients in group A were all higher than that in group B (all P 〈 0.05). Conclusion: The curative effect of governor vessel-unblocking and mind-regulating acupuncture therapy combined with conventional rehabilitation therapy was superior to that of simply conventional rehabilitation therapy in the treatment of sensory and motor dysfunction of the patients with SCI. 展开更多

Objective:To observe the clinical efficacy of mind-refreshing and balance-restoring needling method combined with Frenkel exercises in treating ataxia after cerebral stroke. Methods:The recruited 120 patients were randomized into an observation group and a control group, with 60 cases in each group. The control group was intervened by mind-refreshing and balance-restoring needling method, while the observation group was given additional lower-limb Frenkel exercises. Before and after treatment and at the follow-up,the ataxic lower-limb function was scored using Berg balance scale(BBS)and international cooperative ataxia rating scale(ICARS),and Barthel index(BI)was adopted to score the activities of daily living(ADL).Results:After treatment, the markedly effective rate was 70.2%and the total effective rate was 96.5%in the observation group, versus 39.7%and 87.9%in the control group, and the differences in the markedly effective rate and the total effective rate were statistically significant(P<0.01,P<0.05).The intra-group comparisons showed that the BBS,ICARS and BI scores after treatment and at the follow-up were significantly different from those before treatment in both groups(all P<0.01).There were significant differences in the BBS score between the two groups after treatment and at the follow-up(P<0.05,P<0.01);the between-group differences in the ICARS and BI scores were statistically insignificant after treatment(both P>0.05),while the between-group differences in the ICARS and BI scores were statistically significant at the follow-up(both P<0.05).The interaction effects between the scoring time of BBS and BI and the group factor were statistically significant(P<0.01,P<0.05).Conclusion:Mind-refreshing and balance-restoring needling can effectively improve the lower-limb ataxic symptoms and ADL after stroke;when combined with Fenkel exercises, this needling method can produce more significant efficacy. 展开更多

Objective: To investigate the effect of combined acupuncture and rehabilitation on intelligence and motor development of high risk infants with perinatal brain injuries. Methods: Ninety-seven survived cases in the neonatal intensive care unit (NICU) were allocated into a treatment group of 53 cases and a control group of 44 cases following the consent of the parents. Cases in the treatment group were treated by combined acupuncture and rehabilitation; whereas cases in the control group were treated by conventional child care methods. Systemic follow-up has been made for 2 years. Then the infants' intelligence was evaluated respectively in the 6th, 12th and 34th months. Results: Compared with the control group, the mental development indexes (MDI) and physical development indexes (PDI) in the treatment group were increased by 14.96 and 9.82 respectively. There was a significant difference (χ2=8.1659, P〈0.01 ) in the abnormal intelligence rates between the treatment group (15.1%) and the control group (40.9%,). Conclusion: Combined acupuncture and rehabilitation can effectively improve the mental and physical development of high-risk infants with perinatal brain injuries. Additionally, this therapy can decrease the disability rates and increase the infants' quality of life. 展开更多
