58例慢性硬膜下血肿的外科治疗效果研究

目的探讨慢性硬膜下血肿的外科治疗方法及效果。方法选择我院收治的58例慢性硬膜下血肿患者,予以钻孔扩大骨孔硬膜下血肿冲洗引流术进行治疗,同时对患者术后并发症发生因素进行分析,并总结如何使并发症得到避免及予以此种手术的手术效... 目的探讨慢性硬膜下血肿的外科治疗方法及效果。方法选择我院收治的58例慢性硬膜下血肿患者,予以钻孔扩大骨孔硬膜下血肿冲洗引流术进行治疗,同时对患者术后并发症发生因素进行分析,并总结如何使并发症得到避免及予以此种手术的手术效果。结果全部患者都被治愈,2例在术后出现继发性血肿,1例硬膜下血肿经骨瓣开颅血肿清除术之后被治愈,1例硬膜外血肿再采取保守治疗后血肿被吸收,术后2例患者出现气颅,予以调整高位引流管位置引流及取患侧卧位后,所积气体被吸收,1例泥砂样慢性硬膜下血肿,引流术后血肿残存,予以经骨瓣开颅血肿清除术后治愈。结论采取钻孔扩大骨孔冲洗引流术治疗慢性硬膜下血肿,术中术野暴露充分,对硬脑膜进行悬吊、术中冲洗干净、直视下打开血肿壁及术后通畅引流,能减少术后并发症的发生及发展,手术效果较好。 展开更多

Air-foam drilling has proved its efficiency in numerous situations where serious problems were encountered,such as in fractured formations and depleted or high permeable zones. However,the major disadvantage of air-foam drilling system is that the foam can only be used once,so that an extremely large pit is required to contain the foam to allow sufficient room for cuttings and for the foam to dissipate. Moreover,it needs enormous volume prepared,consuming abundance of water and ingredient additives,which results in the high cost of foam drilling. The recycling foam fluid by using foam breaking technology is the only effective method to solve these problems associated with the known foam drilling. Various types of equipment and technique have been employed to suppress foam formation in biological and process equipment in foam drilling. The study described various methods of foam breaking technology,and the trend of the foam breaking technology for foam drilling is discussed. 展开更多

Using traditional gas drainage methods, such as drilling upward holes to roof strata in tailgate or drilling inseam and cross-measure boreholes, could not meet methane drainage requirements in a gassy mine. The alternative is to drill boreholes from surface down to the Iongwall goaf area to drain the gas out. As soon as a coal seam is extracted out, the upper rock strata above the goaf start to collapse or become fractured depending upon the rock characteristics and the height above the coal seam. During overlying rock strata being fractured, boreholes in the area may be damaged due to ground movement after the passage of the Iongwall face. The sudden damage of a borehole may cause a Iongwall production halt or even a serious mine accident. A theoretical calculation of the stability of surface boreholes in mining affected area is introduced along with an example of determination of borehole and casing diameters is given for demonstration. By using this method for the drilling design, the damage of surface boreholes caused by excessive mining induced displacement can be effectively reduced if not totally avoided. Borehole and casing diameters as well as characteristics of filling materials can be determined using the proposed method by calculating the horizontal movement and vertical stain at different borehole depths. 展开更多

In order to expand the applications of the technology,the authors further studied the principle and analyzed the mechanism of reverse circulation drilling technique with air DTH hammer to get the perfect assembles of equipments by optimizing working parameters. No parameter seemed more important than the air volume because it could maintain the working performance stability. The minimum air volume is related to the parameters such as depth and pressure,which was calculated under the actual conditions. It was solved for the air injection flow tables of the air DTH Hammer working at the different pressures. According to the data tables,operators could adjust the air volume to meet the demand on this technique,which had a realistic guiding significance. Based on the evolution of geological dynamics and spatial chaos theory, we proposed the advanced prediction an advanced prediction method of a gas desorption index of drill cuttings to predict coal and gas outbursts. We investigated and verified the prediction method by a spatial series data of a gas desorption index of drill cuttings obtained from the 113112 coal roadway at the Shitai Mine. Our experimental results show that the spatial distribution of the gas desorption index of drill cuttings has some chaotic charac- teristics, which implies that the risk of coal and gas outbursts can be predicted by spatial chaos theory. We also found that a proper amount of sample data needs to be chosen in order to ensure the accuracy and practical maneuverability of prediction. The relative prediction error is small when the prediction pace is chosen carefully. In our experiments, it turned out that the optimum number of sample points is 80 and the optimum prediction pace 30. The corresponding advanced prediction pace basically meets the requirements of engineering applications. 展开更多

This paper reports the results of tests on the performances of a six row pneumatic precision drill in which the depression into each sowing unit is created by means of flexible pipes that do not start directly from the fan, as in most pneumatic drills, but from an air duct having the function of uniformly distributing the air flow along the working width. Field tests have been conducted, using three graded seeds, to evaluate the accuracy of the single seed metering system, according to ISO 7256/1 standard, and the uniformity of negative pressure, through the measurement of the distance between seeds in the furrow, under two speed conditions (5 and 8 km bl). 展开更多

Light-weight and high-strength materials have attracted considerable attention owing to their outstanding properties, such as weight-reducing, acoustic absorption, thermal insulation, shock and vibration damping. Diamond possesses specific stiffness and strength arising from its special crystal structure. In this work, inspired by the diamond crystal structure, hollow-tube nickel materials with the diamond structure were fabricated using a diamond structured polymer template based on the Stereo Lithography Appearance technology. The diamond structured template was coated with Ni-P by electroless plating. Finally, the template was removed by high temperature calcinations. The density of the hollow tube nickel materials is about 20 mg/cm3. The morphology and composition of the resultant materials were characterized by scanning electron microscope, energy-dispersive spectrometry, and X-ray diffraction. The results showed that the surface of the Ni film was uniform with the thickness of 4 gm. The mechanical property was also measured by stress and strain tester. The maximum compression stress can be reached to 40.6 KPa. 展开更多

We met such a dilemma in DHS fixation of ... Guide wire plays an important role in the fixation of femoral neck fracture with dynamic hip screw (DHS). Breakage of a guide wire during operation is a very rare condition. We met such a dilemma in DHS fixation of a 54-year-old male patient who sustained Garden type IV frac- ture of the right femoral neck. The distal end of the guide wire broke and was entrapped in the fractured femoral neck. We tried to get the broken part out by a cannulated drill. Reaming was started with the cannulated drill slowly rotat- ing arotmd the guide K-wire until the reamer fully contained the target under fluoroscope. A bone curette was used to get the broken wire out but failed, so we had to use the cannuated drill to dredge this bone tunnel. Finally the bro-ken wire end was taken out, mixed with blood and bone fragments. Through the existing drilling channel, DHS fixa- tion was easily finished. The patient had an uneventful re- covery without avascular necrosis of femoral head or non- union of the fracture at one year's follow-up. A few methods can be adopted to deal with the broken guide wire. The way used in our case is less invasive but technically challenging. When the guide wire is properly positioned, this method is very practical and useful. 展开更多
