铁调素、铁调素/铁蛋白比值对铁储量充足的维持性腹膜透析患者发生功能性缺铁的诊断价值 被引量:1

目的探讨铁调素、铁调素/铁蛋白比值诊断铁储量充足的维持性腹膜透析患者发生功能性缺铁的价值。方法选择74例铁储量充足的维持性腹膜透析患者,检测患者血清铁蛋白、血清β_(2)微球蛋白、血清铁、总铁结合力、血清铁调素水平,计算铁调素... 目的探讨铁调素、铁调素/铁蛋白比值诊断铁储量充足的维持性腹膜透析患者发生功能性缺铁的价值。方法选择74例铁储量充足的维持性腹膜透析患者,检测患者血清铁蛋白、血清β_(2)微球蛋白、血清铁、总铁结合力、血清铁调素水平,计算铁调素/铁蛋白比值及转铁蛋白饱和度(TSAT)。将TSAT≥20%的患者纳入无功能性缺铁组,TSAT<20%的患者纳入功能性缺铁组。采用多因素Logistic回归模型分析铁储量充足的维持性腹膜透析患者发生功能性缺铁的危险因素。采用受试者工作特征(ROC)曲线分析危险因素单独及联合诊断铁储量充足的维持性腹膜透析患者发生功能性缺铁的价值。结果74例患者中,无功能性缺铁32例(43.2%),功能性缺铁42例(56.8%)。与无功能性缺铁组相比,功能性缺铁组患者血清β_(2)微球蛋白水平、血清铁调素水平、铁调素/铁蛋白比值升高,血清铁水平、血清铁蛋白水平、TSAT降低(均P<0.05)。多因素Logistic回归分析结果显示,血清铁调素、β_(2)微球蛋白水平及铁调素/铁蛋白比值为铁储量充足的维持性腹膜透析患者发生功能性缺铁的独立危险因素(均P<0.05)。ROC曲线分析结果显示,血清铁调素水平、铁调素/铁蛋白比值、血清β_(2)微球蛋白水平单独诊断及三者联合诊断铁储量充足的维持性腹膜透析患者发生功能性缺铁的曲线下面积分别为0.955、0.874、0.645、0.976(均P<0.05),三者联合诊断的曲线下面积均大于单一指标(均P<0.05)。结论血清铁调素水平、铁调素/铁蛋白比值单独诊断维持性腹膜透析患者发生功能性缺铁的效能均较高,而二者联合β_(2)微球蛋白对这类患者发生功能性缺铁的辅助诊断价值更高。 展开更多

2016—2017年北京市6~17岁儿童铁储量状况

目的了解北京市6~17岁儿童铁储量(body iron,BI)水平和分布。方法于2016—2017年采用多阶段分层整群随机抽样方法对北京市1392名6~17岁儿童进行问卷调查和体格检查,采集空腹静脉血,采用免疫比浊法测定血清铁蛋白(serum ferritin,SF)和... 目的了解北京市6~17岁儿童铁储量(body iron,BI)水平和分布。方法于2016—2017年采用多阶段分层整群随机抽样方法对北京市1392名6~17岁儿童进行问卷调查和体格检查,采集空腹静脉血,采用免疫比浊法测定血清铁蛋白(serum ferritin,SF)和血清运铁蛋白受体(serum soluble transferrin receptor,sTfR),计算BI,评估6~17岁儿童BI状况。结果北京市6~17岁儿童BI水平为(5.49±2.94)mg/kg,男童高于女童,腹型肥胖者高于非腹型肥胖者(P<0.05)。BI浓度4~7.99 mg/kg占比最高(57.3%),<0 mg/kg占比最低(4.3%);不同性别、不同年龄及是否腹型肥胖者之间BI浓度构成比差异均有统计学意义(P<0.05)。贫血患病率为2.7%(95%CI 1.9~3.6),低铁蛋白(SF<25μg/L)率为22.5%(95%CI 20.0~24.8),铁缺乏(SF<15μg/L)率7.8%(95%CI 6.4~9.3),铁储量负态(BI<0 mg/kg)率为4.3%(95%CI 3.2~5.3);贫血率、低铁蛋白率、铁缺乏率和铁储量负态率均女童高于男童、12~17岁高于6~11岁(P<0.01)。结论2016—2017年北京市6~17岁儿童仍存在铁缺乏,女童和12~17岁儿童BI低水平比例较多。 展开更多

目的定量评估中国河南省农村地区贫血人群的铁储量(BIS)水平和分布特征,分析BIS与传统铁营养评估指标之间的相关性及可能存在的优势。方法于2010年1月对河南省南阳市宛城区10个行政村1 757名村民和学生进行贫血和铁缺乏状况调查,测定贫... 目的定量评估中国河南省农村地区贫血人群的铁储量(BIS)水平和分布特征,分析BIS与传统铁营养评估指标之间的相关性及可能存在的优势。方法于2010年1月对河南省南阳市宛城区10个行政村1 757名村民和学生进行贫血和铁缺乏状况调查,测定贫血个体的血清铁蛋白(SF)和转铁蛋白受体(sTfR)水平,定量估计个体BIS水平并分析该指标在不同人群中的分布特征,比较缺铁性贫血与非缺铁性个体BIS水平之间的差异及BIS、血红蛋白(Hb)、SF、sTFR 4个指标之间的相关性,探索BIS与Hb之间的数量依存关系。结果各研究人群BIS的平均水平分别为青春期女生(11.1±2.7)mg/kg、育龄期妇女(5.9±6.5)mg/kg、老年女性(12.8±4.1)mg/kg、老年男性(11.4±5.3)mg/kg;青春期女生缺铁性贫血组BIS平均水平为(0.02±5.43)mg/kg,非缺铁性贫血组为(11.39±1.99)mg/kg;育龄妇女缺铁性贫血组BIS平均水平为(0.42±4.66)mg/kg,非缺铁性贫血组为(10.82±2.93)mg/kg;老年男性缺铁性贫血组BIS平均水平为(-4.59±6.78)mg/kg,非缺铁性贫血组为(12.39±3.16)mg/kg;老年女性缺铁性贫血组BIS平均水平为(3.11±2.03)mg/kg,非缺铁性贫血组为(13.80±2.64)mg/kg;4个人群缺铁性贫血组与非缺铁性贫血组BIS平均水平差异均有统计学意义(P<0.01);Pearson相关分析表明,各人群BIS水平与Hb水平均呈正相关(P<0.05),BIS与Hb之间存在数量依存关系,可以建立拟合的线性方程。结论 BIS是一个综合评估贫血个体铁营养状况的良好指标。 展开更多

目的评估中国贫困农村地区女性的铁储量(BI)水平与分布。方法基于2010—2012年中国居民营养与健康状况监测贫困地区采血样本进行再抽样,采用免疫比浊法测定血清铁蛋白(SF)和血清运铁蛋白受体(s TfR),计算铁储量(BI),评估6岁以... 目的评估中国贫困农村地区女性的铁储量(BI)水平与分布。方法基于2010—2012年中国居民营养与健康状况监测贫困地区采血样本进行再抽样,采用免疫比浊法测定血清铁蛋白(SF)和血清运铁蛋白受体(s TfR),计算铁储量(BI),评估6岁以上不同年龄段女性的BI状况,比较不同BI水平的人群分布,分析BI与血红蛋白(Hb)、SF和s TfR的相关性。结果 6~11岁女童BI为(7.35±2.37)mg/kg,12~17岁女性为(6.50±3.59)mg/kg,18~44岁女性为(6.07±4.78)mg/kg,45~59岁中年女性为(8.60±4.45)mg/kg,≥60岁老年女性为(10.35±3.38)mg/kg。12~17岁和18~44岁女性在BI低浓度分布比例最高。除6~11岁外,其他所有年龄组BI与Hb、SF、s TfR均显著相关。BI〈0 mg/kg亚组的BI与Hb显著相关。结论 BI是可贯穿各个铁营养阶段综合反映铁营养状况的指标,贫困农村地区12~17岁青少年女性和18~44岁育龄妇女的铁营养状况需要关注。 展开更多

This paper is devoted to application of ordinary kriging method in Choghart north anomaly iron ore deposit in Yazd province, Iran. In order to estimate the deposit, 2329 input data gained from 26 boreholes were used. Fe grade was selected as the major regional variable on which the present research has focused. All of the available data were changed to 12.5 m composites so that statistical regularization could be reached. Studies indicated that iron grade input data had single-population characteristics. To carry out ordinary kriging, a spherical model was fitted over empirical variogram. Then the model was verified through cross validation method and proved to be valid with a coherence coefficient of 0.773 between the estimated and real data. Plotting the empirical variogram in different directions showed no geometric anisotropy for the deposit. To estimate the Iron grade, ordinary kriging method was used according to which, all of the exploitable blocks with dimensions 20 m x 20 m x 12.5 m were block esti- mated within the estimation space. Finally tonnage-grade curve has been drawn and reserve classified into measured, indicated and inferred. 展开更多

An attempt to estimate the reserves in the High Phosphorous stockpile (HP) at the Choghart Iron mine of Iran was carded out using geostatistical modeling. Grade and tonnage estimates of ore stockpiles can help convert them into a valuable ore reserve. This is valuable in consideration of increasing metal demand, and the depletion of in situ ore reserves, around the world. Estimation of reserves in stockpiles is difficult partly because of geological and grade discontinuities created during the dumping of the ore piles. Data input for the HP stockpile at Choghart was performed based upon pre-existing information gathered during extraction from the various mining benches. After establishing the input data files the reserve estimates were found using geostatistical methods aided by the international mining software SURPAC. The stockpile was divided in to three domains and the reserves in each domain were estimated separately. A grade block model was used to compute the reserve. Fe% and P% were estimated using the Ordinary Kriging method. The results showed that the total tonnage of the HP stockpile is 4.5 million tons with an average zrade of 55% Fe and 1.03% P. 展开更多

Uncertainty on the geological contacts and the block volumes of the models along boundaries is often a major part of the global uncertainty of reserve estimation.This work introduces a geostatistical technique that has been developed and tested in an iron ore deposit at Bafq mining district,in central Iran,and that,based on a probability criterion,helps to objectively model the geometry of this iron ore deposit.The main problem in reserve estimation of this ore body is its geometrical modeling and uncertainty in geological boundaries.This work deals with the geostatistical method of multiple indicator kriging,which is used to determine the real boundaries of ore body in different categories.This approach has potential to improve project performance and decrease operational risk.For this purpose,the ore body is separated into two categories including rich iron zone(w(Fe)>45%)and poor iron zone(20%<w(Fe)<45%).It significantly benefits to decrease the risk of reserve evaluation in the deposit.This case study also highlights the value of multiple indicator kriging as a tool for estimates the position of grade boundaries within the deposit.Comparison of the resultant probability maps with the real ore/waste contacts on the extracted levels shows that the first indicator model could separate the whole ore body(poor plus rich)from the waste zone by probability of more than 0.35,which concludes the total reserve of 53 million tons.The second indicator model applied to separate the rich and poor domains and the results show that the blocks with the estimated probability of equal to or more than 0.4 lay within the rich ore zone consisting of 15.8 million tons reserve. 展开更多

铁缺乏(iron deficiency,ID)是最常见的营养素缺乏症和全球性健康问题,铁缺乏的生物学客观指标及其检验方法是铁营养状况的个体诊断及人群筛查的核心技术,是铁缺乏观察研究、危害分析及营养干预的科学基础。本文综述了18项铁缺乏诊断指... 铁缺乏(iron deficiency,ID)是最常见的营养素缺乏症和全球性健康问题,铁缺乏的生物学客观指标及其检验方法是铁营养状况的个体诊断及人群筛查的核心技术,是铁缺乏观察研究、危害分析及营养干预的科学基础。本文综述了18项铁缺乏诊断指标包括近年来开始应用的低色素红细胞百分率、网织红细胞浓度、血清或尿铁调节素等指标的检验方法及特点。本文对血清铁蛋白-C-反应蛋白/α1-酸性糖蛋白指标体系(SF-CRP/AGP)、血清铁蛋白-可溶性运铁蛋白受体(SF-s TfR)两指标模型、血清铁蛋白-运铁蛋白饱和度-原卟啉/锌卟啉(SF-TS-FEP/ZPP)等三指标或多指标模型及铁储量(BI)模型研究进展进行综述,并探讨各指标体系的应用前景。综述表明SF-CRP/AGP研究较多,是目前推荐选择的铁缺乏指标,但SF值受炎症因子影响且其判断阈值存在争议。SF-s TfR和BI模型较为稳定,可减少炎症影响,s TfR阈值研究不足,还需积累更多数据。 展开更多
