精细化营养支持结合支持性照护理论对食管癌术后患者体质指数血清白蛋白及总蛋白指标及特异性支持照护需求的影响

目的探究精细化营养支持结合支持性照护理论对食管癌术后患者的影响。方法选取2021年12月至2023年12月本院收治的食管癌术后患者72例,按照随机数字表法进行分组,各组均36例,对照组采取常规护理干预,观察组采取精细化营养支持结合支持性照护理论干预模式,对比2组术后恢复时间、体质指数(BMI)、血清白蛋白(ALB)、血清总蛋白(TP)水平及特异性支持照护需求量表(SCNS-ED)评分。结果实施后,观察组首次排气时间、首次排便时间、首次下床时间、住院时间均较对照组时间更短(t=3.33,P=0.001;t=3.01,P=0.003;t=3.37,P=0.001;t=3.24,P=0.002);实施后,观察组BMI较对照组更高(t=2.17,P=0.034),观察组ALB水平较对照组更高(t=2.82,P=0.007),观察组TP水平较对照组更高(t=2.32,P=0.024);实施后7 d、14 d、1个月,观察组SCNS-ED评分均低于对照组(t=3.10,P=0.003;t=2.82,P=0.005;t=5.03,P<0.001)。结论精细化营养支持结合支持性照护理论应用于食管癌术后患者,可加快患者的病情恢复,增强体质量与BMI,提高营养指标,降低特异性支持照护需求,具有一定推广价值。

妇科癌症病人特异性支持照护需求量表编制及信效度检验

[目的]编制妇科癌症病人特异性支持照护需求量表,并检验其信效度。[方法]通过文献回顾、质性访谈、参考相关量表,构建量表条目池,经过专家内容效度评定形成初测量表,对280例妇科癌症病人施测,检验量表的信效度。[结果]量表最终包含25个条目,内部一致性Cronbach'sα系数为0.951,折半信度系数为0.810。聚合效度分析显示,妇科癌症病人特异性支持照护需求量表得分与医院焦虑抑郁量表总分及各维度得分呈正相关(P<0.05),与癌症病人治疗功能评定量表的总分及各维度得分呈负相关(P<0.05)。区分效度分析显示,不同妇科癌症类型、不同年龄段及确诊时是否绝经的病人需求得分差异有统计学意义(P<0.05)。[结论]编制的妇科癌症病人特异性支持照护需求量表具有良好的信效度,可用于评估妇科癌症病人的特异性支持照护需求。

中国工业经济增长为何独立于国际油价波动?——基于油价波动来源和行业异质性的考察

国际油价波动是多种冲击(原油供给冲击、经济总需求冲击、特定性需求冲击)共同作用的结果,而不同冲击对中国工业产出增长的影响不同,且对不同工业行业的影响也不同,使得中国工业产出增长可能与国际油价波动同向变动或反向变动或没有相关性,由此产生中国工业产出增长独立于国际油价波动的现象。使用1998年7月至2023年9月的月度数据,运用SVAR模型,采用工具变量与符号约束相结合的识别方法,分析发现:不同冲击对国际油价的影响不同,原油供给冲击对中国工业产出增长的影响最大,3种冲击对中国工业产出增长的影响方向可能相反,原油供给冲击对低研发投入和中研发投入行业产出增长的影响较大,对高研发投入行业的影响较小,表明技术进步能够缓解国际油价波动对产出增长的冲击,经济总需求冲击和特定性需求冲击对高、低、中研发投入行业的影响依次减弱。因此,面对国际油价波动,应避免市场恐慌,坚持中国特色新型工业化的实践逻辑,并加强科技创新对经济发展的稳定作用。

Lived experiences of the disease journey among patients with idiopathic pulmonary fibrosis

Objective:This study aimed to explore the lived experiences of the disease journey and patients'care needs with idiopathic pulmonary fibrosis(IPF).Methods:Face-to-face semi-structured interviews were conducted with a purposive sampling of IPF patients admitted to the department of respiratory medicine in a tertiary hospital in Beijing.Interview data were analyzed using the thematic analysis method.In the end,16 patients were interviewed.Results:Four themes emerged from the qualitative data included the long and confusing journey to reach a diagnosis,living with the disease,understanding the disease and treatment and desire for continuity of care.A series of subthemes were also identified,including uncertainty of diagnosis,delaying the process,living with physical symptoms,living with emotional distress,loss of independence,uncertainty with the prognosis,questioning the cause of the disease,concerning the side effects of treatments,lacking continuity of care,and wanting a better quality ofhealthcare in community hospitals.Conclusions:Based on the findings,there is an urgent need to improve the care delivery to this vulnerable population in China.To meet their health needs,it is of paramount importance to develop effective education programs for health professionals and IPF patients and improve care models of healthcare systems,especially in remote areas,to enhance care continuity in the communities.

基于支持性照护理论食管癌患者术后延续护理方案的构建与应用

目的:构建食管癌患者术后延续护理方案并分析其临床应用效果,以期改善食管癌患者术后生活质量。方法:通过查阅文献、焦点小组讨论、德尔菲法等,在支持性照护理论指导下构建食管癌患者术后延续护理方案。便利抽取2019年1月—12月在某三级甲等医院胸外科进行手术治疗的60例食管癌患者作为对照组,采用常规护理干预;2020年1月—12月的60例患者作为干预组,采用基于微信小程序的延续护理方案。两组患者在出院前1天,术后1个月、3个月、6个月,采用食管癌患者特异性支持照护需求量表、家庭弹性量表、食管癌专用生活质量量表进行评估。结果:两组食管癌患者特异性支持照护需求总分、家庭弹性总分的组别、时间和交互效应差异均有统计学意义(P<0.05),生活质量评分的组别和时间效应差异有统计学意义(P<0.05)。结论:本研究构建的食管癌患者术后延续护理方案合理、可行,有利于患者的康复。

Parameters Affecting the Color Mechanism of Manganese Containing Colored Glasses

Color is an important tool for glass industry to let the product gain additional aesthetic and functional properties forced by the market demands. In this context, it is necessary to understand the color forming mechanism better in order to get target color and improve the yield. A detailed experimental study to investigate the relationship between the manganese content on color formation and the redox condition was carried out. All experiments were carried out at different level of iron and manganese oxides. Color parameters （dominant wavelength, brightness （%）, purity （%） and L-a-b values） were examined for large interval of batch redoxes. In this paper, the results of above mentioned experimental studies were discussed together.

Joint analysis of urban shopping destination and travel mode choice accounting for potential spatial correlation between alternatives

In recent years,there have been important developments in the joint analysis of the travel behavior based on discrete choice models as well as in the formulation of increasingly flexible closed-form models belonging to the generalized extreme value class.The objective of this work is to describe the simultaneous choice of shopping destination and travel-to-shop mode in downtown area by making use of the cross-nested logit(CNL) structure that allows for potential spatial correlation.The analysis uses data collected in the downtown areas of Maryland-Washington,D.C.region for shopping trips,considering household,individual,land use,and travel-related characteristics.The estimation results show that the dissimilarity parameter in the CNL model is 0.37 and significant at the 95% level,indicating that the alternatives have high spatial correlation for the short shopping distance.The results of analysis reveal detailed significant influences on travel behavior of joint choice shopping destination and travel mode.Moreover,a Monte Carlo simulation for a group of scenarios arising from transportation policies and parking fees in downtown area,was undertaken to examine the impact of a change in car travel cost on the shopping destination and travel mode switching.These findings have important implications for transportation demand management and urban planning.

Saudi Arabia＇s Growing Demand for Electricity： Some Strategic Recommendations

Demand for electricity in Saudi Arabia is growing at a significant annual rate of nearly 8%. It is expected that, by 2030, the demand will increase to about 120 GW per year, approximately three times the 2010 load. Satisfying this demand will require a significant investment in the power grid at an estimated cost, over the next 10 years, ofSAR （Saudi Arabia Riyal） 500 billion. Existing power plants rely on oil and natural gas, it is anticipated that meeting the demand in 2030 will consume 3 million barrels ofoil each day, which significantly impacting the economy by reducing the country＇s income from oil exports, which is a hot button for Saudi decision makers. This paper reviews the responses of various countries in meeting their loads, and therefore, draws recommendations for some resources that should, and should not, be considered best-candidate options for Saudi Arabia economically, technically and environmentally. The discussion primarily examines renewable and nuclear resources.

Detailed Study on Establishing a PV Power Plant to Generate Electricity in Saudi Arabia

Photvoltiacs （PV） power plants have beell built all over the world, and are successfully proven as one of the important substitutes of alternative energy sources. In the Kingdom of Saudi Arabia, the electricity demand will double in the next 15 years, dramatically increasing fuel consumption. To meet this huge load growth, there has been some series of movements to find out solutions. This research aims at studying the feasibility of design and construction of a solar power plant using photovoltaic cells in Saudi Arabia from the geographic, economic and technical perspectives. It＇s concluded that PV power is competitive with the conventional capacity required to meet ＂peak＂ power demand and represents an opportunity for reducing fuel consumption in Saudi Arabia.
