颂钵音疗在帕金森病康复治疗中的应用研究

目的研究颂钵音疗对帕金森病患者康复治疗的影响。方法纳入70例帕金森患者,随机分为对照组和观察组各35例。对照组予以常规健康教育,观察组在对照组基础上采取颂钵音疗的干预方式,共进行8周。干预前后,采用汉密顿抑郁量表(HAMD)及汉密... 目的研究颂钵音疗对帕金森病患者康复治疗的影响。方法纳入70例帕金森患者,随机分为对照组和观察组各35例。对照组予以常规健康教育,观察组在对照组基础上采取颂钵音疗的干预方式,共进行8周。干预前后,采用汉密顿抑郁量表(HAMD)及汉密尔顿焦虑量表(HAMA)评分评估患者焦虑抑郁情绪,采用蒙特利尔认知评估量表(MoCA)评估认知障碍程度、肌肉震颤评定量表(CRST)评估肌肉震颤状况,并进行肌张力评分。结果治疗后,观察组HAMD及HAMA评分较前显著降低,MoCA评分较前显著升高,且均优于对照组(P<0.05);治疗后观察组肌张力降低,且低于对照组(P<0.05);治疗前后两组肌肉震颤CRST评分比较,差异无统计学意义(P>0.05)。结论以颂钵音疗作为一种补充治疗干预手段,对改善帕金森病患者的焦虑抑郁、认知障碍及肌张力有积极的影响效果,可为此类患者提供一种安全、易学、有效的治疗康复保健模式。 展开更多

目的:探讨主被动音乐治疗对局麻下包皮环扎术儿童恐惧焦虑心理及手术配合满意度(依从性)的影响。方法:89例喜欢音乐的患儿随机分为干预组与对照组,对照组聆听轻音乐,干预组聆听并主动随着音乐吟唱自己选择的喜欢的歌曲。对两组儿童疼痛... 目的:探讨主被动音乐治疗对局麻下包皮环扎术儿童恐惧焦虑心理及手术配合满意度(依从性)的影响。方法:89例喜欢音乐的患儿随机分为干预组与对照组,对照组聆听轻音乐,干预组聆听并主动随着音乐吟唱自己选择的喜欢的歌曲。对两组儿童疼痛、恐惧程度、手术配合满意度等进行比较。结果:患儿术中VAS疼痛评分干预组为(4.96±1.11)分,低于对照组(5.84±1.24)分,两组差异有统计学意义(t=-3.55,P<0.01);手术前两组HAMA总分差异无统计学意义(P>0.05),手术后,干预组HAMA总分为(3.21±0.60)分,低于对照组(7.20±1.67)分,差异有统计学意义(t=17.87,P<0.05);护士测评的患儿手术配合满意度,干预组好于对照组,差异有统计学意义(t=6.75,P<0.01);术者测评的患儿手术配合满意度干预组好于对照组,差异有统计学意义(t=3.72,P<0.01)。结论:主被动音乐治疗能有效降低儿童术中疼痛程度及恐惧心理,提高手术配合满意度。 展开更多

Effect of music on procedure time and sedation during colonoscopy:A meta-analysis 被引量:13

AIM:To integrate results from different studies in examining the effectiveness of music in reducing the procedure time and the amount of sedation used during colonoscopic procedure. METHODS: An electronic search in various databases was performed to identify related articles. Study quality was evaluated by the Jadad’s scale. The random effect model was used to pool the effect from individual trials and the Cohen Q-statistic was used to determine heterogeneity. Egger’s regression was used to detect publication bias. RESULTS: Eight studies with 722 subjects were included in this meta-analysis. The combined mean difference for the time taken for the colonoscopy procedure between the music and control groups was -2.84 with 95% CI (-5.61 to -0.08), implying a short time for the music group. The combined mean difference for the use of sedation was -0.46 with 95%CI (-0.91 to -0.01), showing a significant reduction in the use of sedation in the music group. Heterogeneity was observed in both analyses but no publication bias was detected. CONCLUSION: Listening to music is effective in reducing procedure time and amount of sedation during colonoscopy and should be promoted. 展开更多

To elucidate the role of music therapy in gastrointestinal endoscopic procedures following the conflicting outcomes reported in two recent studies. The findings of our recent meta-analysis that examined this matter were discussed in the context of later studies. Our meta-analysis illustrated the beneficial effects of music therapy on patient anxiety levels when used as a single measure of relaxation and analgesia. Beneficial effects were also shown on analgesia and sedation requirements and procedure duration times when used as an adjunct to pharmacotherapy. These findings are in agreement with those of both studies excluded from analysis and those that followed it. Music therapy is an effective tool for stress relief and analgesia in patients undergoing gastrointestinal endoscopic procedures. 展开更多

Since its advent more than 20 years ago, endoscopic ultrasound (EUS) has undergone evolution from an experimental to a diagnostic instrument and is now established as a therapeutic tool for endoscopists. Endoscopic ultrasound cannot accurately distinguish benign from malignant changes in the primary lesion or lymph node on imaging alone. With the introduction of the curved linear array echoendoscope in the 2990s, the indications for EUS have expanded. The curved linear array echoendoscope enables the visualization of a needle as it exits from the biopsy channel in the same plane of ultrasound imaging in real time. This allows the endoscopist to perform a whole range of interventional applications ranging from fine needle aspiration (FNA) of lesions surrounding the gastrointestinal tract to celiac plexus block and drainage of pancreatic pseudocyst. The Effect of Music Therapy for Patients With Huntington's Disease: A Systematic Literature Review

Huntington's disease is a progressive, neurodegenerative, and autosomal dominant disease characterized by motor- and psychiatric disturbances, and cognitive decline. Since there is no cure, all treatment is aimed at improving quality of life. One form of non-drug therapy which is barely recognized, is music therapy. The literature on its use and effect in patients with Huntington's disease is limited. We therefore performed a detailed literature survey in order to delineate the different types of music therapy interventions and their aims. Music therapy studies were included when targeting: (1) motor disturbances; (2) cognitive disturbances; (3) psychiatric disturbances; and (4) emotional/social disturbances. A total of eight studies that met these criteria were analyzed. We found that music therapy interventions vary throughout the different stages of the disease and are not yet applied optimally. Not the stage of the disease but the individual treatment goals seem to be more determinative for the music therapy indications. In conclusion, precise aims and methods in relation to the stage of the disease are not well determined. There is a need for systematic study of this treatment option. 展开更多

Objective To observe the effects on the range of motion (ROM) of ankle joint and muscular spasm of the lower limbs treated with Chinese massage for promoting the Governor Vessel, benefiting the kidney and strengthening the spleen combined with music therapy in infantile cerebral palsy (CP). Methods In 286 cases of spasmodic CP, 7 kinds of Chinese massage were adopted, named massage on acupoints to promote circulation of the Governor Vessel, massage for strengthening the spleen and benefiting qi, spine-pinching therapy, segmental massage, foot massage, massage for promoting the recovery of muscle strength and massage for promoting ROM of joints. Chinese massage was applied in musical background. Buddha music and five-element music of Chinese medicine (music of the Heart, Liver and Kidney Meridians) were optioned. The treatment was given once or twice a day and 30 days made one course. The angles of femoral adductor and dorsal flexion of foot as well as the score of composite spasticity scale (CSS) were observed before and after treatment so that the therapeutic effect was assessed. Results In 30 days of treatment, 106 cases (37. 1%) were effective markedly, 172 cases (60. 1%) effective and 8 cases (2.8%) failed. The angle of dorsal flexion of foot was (105.64± 12.66)° and (76.58± 9.89)° before and after treatment respectively; the angle of femoral adductor was (119.89±25.47)° and (158.99± 18.66)° before and after treatment respectively; the score of CSS was (12.39±2.56) and (7. 069 ± 2.97) before and after treatment respectivel, there was significant difference before and after treatment (all P〈0.01). Conclusion Chinese massage for promoting the Governor Vessel, benefiting the kidney and strengthening the spleen combined with music therapy brings the extremely active effect into play on the recovery of sitting, kneeling, standing and walking for children with CP. 展开更多

Methods Sixty-one cases of dysarthria were randomly divided into two groups. The observation group (30 cases) was treated with speech therapy and acupuncture at major acupoints Lianquan (廉泉 CV 23), Jinjin (金津 EX-HN 12), Yuye (玉夜 EX-HN 13), Fengchi(风池 GB 20), Yifeng (翳风 TE 17) and Wangu(完骨 GB 12), and the control group (31 cases) was treated with speech therapy only. The changes of speech and acoustical indices were evaluated after 9 weeks treatment. Results The word articulation and the correct rate of writings in the two groups were significantly increased after treatment (both P〈0.01). The total effective rate of 96.7%(29/30) in the observation group was superior to that of 67.7%(21/31) in the control group (P〈0.01). The maximum phonation time (MPT) tested by a phonolaryngeal aeromechanical analyzer was significantly prolonged in both the observation group (12 cases) and the control group (11 cases) (both P〈0.01), the improvement in the observation group being more obvious (P〈0.01). Conclusion Acupuncture combined with speech therapy can improve the speech and acoustic level in the patient of dysarthria. 展开更多

Objective To observe the clinical efficacy of acupuncture combined with speech rehabilitation training for post-stroke spasmodic dysphonia and compare the differences in efficacy among the therapy of acupuncture combined with speech rehabilitation training and the monotherapy.Methods Two hundred and seventy cases of patients with post-stroke spasmodic dysarthria were randomly divided into three groups with the ratio of 1:1:1.Group A:acupuncture combined with speech rehabilitation training group(90 cases),group B:acupuncture group(90 cases),group C:speech rehabilitation training group(90 cases).In the group B,acupuncture treatment was given at Baihui(百会 GV 20),JTnjin(金津 EX-HN 12) and Yuye(玉液 EX-HN13) as well as tongue-three needles.In the group C,the treatment of speech rehabilitation training was provided.The two treatments mentioned above were combined in the group A.Patients were treated once a day for a month with improved Frenchay dysarthria rating scale as the indices of therapeutic effect evaluation.Results ① Group A:the cured and markedly effective rate was 88.7%,and total effective rate was 94.3%;group B:the cured and markedly effective rate was 44.2%and total effective rate was 81.4%;group C:the cured and markedly effective rate was 23.5%and total effective rate was 61.2%.Both the cured and markedly effective rate and the total effective rate in the group A were significantly higher than those in the group B or group C(all P0.05);both the cured and markedly effective rate and the total effective rate in the group B were higher than those of group C(both P0.05);② In comparison of functional recovery of tongue in accordance with the Frenchay dysarthria rating scale,the recovery rate of the tongue-stationary state was 71.74%in the group A,18.87%in the group B and 4.44%in the group C;the recovery rate of tongue lolling out was 66.23%in the group A,27.63%in the group B and 1.59%in the group C;the recovery rate of tongue up and down motion was 44.19%in the group A,4.94%in the group B and 1.35%in the group C;the recovery rate of lateral motion was 40.24%in the group A,7.59%in the group B and 0.00%in the group C;the recovery rate of alternating motion was 29.07%in the group A,7.14%in the group B and 1.23%in the group C;the recovery rate of speech was 29.07%in the group A,5.88%in the group B and 1.22%in the group C.In the three groups,the recovery rates of stationary state and tongue lolling out were superior to those of up and down movement,lateral movement,alternating movement and speech(all P0.05).Conclusion The clinical efficacy of acupuncture combined with speech rehabilitation training for patients with post-stroke spasmodic dysarthria is significant,and the efficacy of acupuncture is superior to that of speech rehabilitation training;as for functional recovery of tongue like stationary state and tongue out,the therapy of acupuncture combined with speech rehabilitation training is effective. 展开更多

Objective: To investigate the effects of low-intensity pulsed ultrasound in repairing injured articular cartilage. Methods: Ten adult New Zealand rabbits with bilateral full-thickness osteochondral defects on the cartilage surface of intercondylar fossas were used in this study. The wounds in the left knees were treated with low-intensity pulsed ultrasound as the experimental group. The right knees received no treatment as the control group. All the animals were killed at 8 weeks after injury and the tissues in the wounds were collected for gross appearance grading, histological grading and proteoglycan quantity. Results: The scores of the gross appearance grades, histological grades and the optical density of toluidine blue of the tissues in the experimental group were significantly higher than those of the controls at 8 weeks after injury (P<(0.05)). Conclusions: Low-intensity pulsed ultrasound can accelerate the repair of injured articular cartilage. 展开更多

Method: All of 286 cases that conformed to the diagnostic criteria of infantile cerebral palsy were treated with 7 types of tuina manipulations respectively to unblock the Governor Vessel, reinforce the kidney and strengthen the spleen, pinch along the spine, stimulate specific foot-reflex area and different spinal segments, accelerate recovery of muscle strength and increase joint range of motion, 25-30 min each treatment, once or twice a day, 30 d constitute a course of treatment. After this, the efficacy on femoral medial adduction and dorsiflexion angle and composite spasticity score (CSS) was evaluated. Result: The statistical analysis showed significant differences in dorsiflexion and femoral medial adduction angle and CCS scores (P〈0.01) after the treatments. Conclusion: Tuina plus music therapy can lubricate the joints, relax contraction of tendons, alleviate muscle spasm and improve scissors and toe-walking gaits, thereby benefiting the gross motor function of infants in sitting, kneeling, standing and walking. 展开更多

OBJECTIVE: To explore the effects of five-element music therapy on elderly patients with seasonal affective disorder in a Chinese nursing home.METHODS: The patients(n=50) were recruited from a Shijingshan district nursing home in Beijing,China. They were randomly assigned to two groups, a treatment group and a control group,with 25 participants in each group. The patients received music therapy for 1-2 h each week over an8-week period. The music therapy involved four phases: introduction, activities, listening to the Chinese five-element music, and a concluding phase.The participants in the control group did not listen to the five-element music. This study consisted of two parts:(a) a qualitative study that used focus groups to understand the feelings of the patients with seasonal affective disorder;(b) a quantitative study that involved administration of the self-rating depression scale(SDS) and Hamilton depression scale(HAMD) before and after treatment.RESULTS:(a) Qualitative analysis results: strength derived from the five-element group music therapy and emotional adjustment. The five-element group music therapy can reduce patients' psychological distress and let them feel inner peace and enhance their life satisfaction.(b) No significant difference in SDS and HAMD scores was found between the two groups(P>0.05) prior to treatment. After treatment,the mean SDS score of the control group was 49.9±18.8, while the treatment group's score was 40.2±18.1. The HAMD score of the control group was11.2±3.1, and the treatment group's score was 8.8±4.9. Following 8 weeks of music therapy, the SDS and HAMD scores of the treatment group were significantly lower than those for the control group(P<0.05).CONCLUSION: Five-element music therapy alleviated the symptoms of seasonal affective disorder in the elderly patients. 展开更多

Methods: By adopting a design of multi-centered randomized controlled trial (RCT), a total of 270 patients with anxiety were randomized into a treatment group and a medication group. The treatment group was intervened by music electric stimulation of points, while the medication group was intervened by oral administration of doxepin. The two groups were evaluated by using Hamilton anxiety rating scale (HAMA) and Chinese revised edition of self-rating anxiety scale (SAS-CR) before and after the intervention. The therapeutic efficacies were also compared. Results: The total effective rate was 93.6% in the treatment group versus 92.3% in the medication group, and the between-group difference was statistically insignificant (P〉O.05). After the treatment, the aggregate scores of HAMA and SAS-CR were significantly changed in both groups (both P〈0.001), and the inter-group differences were statistically insignificant (P〉O.05). Conclusion: Music electric stimulation of points can produce equivalent efficacy in treating anxiety compared to doxepin. Thus, it can be taken as a choice in the treatment of anxiety. 展开更多

OBJECTIVE: To evaluate the clinical efficacy and safety of five phase music therapy in patients with depression after ischemic stroke.METHODS: A total of 92 patients with post-stroke depression were randomly divided into the control group(32 cases), treatment group A(30 cases), and treatment group B(30 cases). All groups were given basic therapies for cerebral infarction. In addition,the control group was administerd 50 mg of oral sertraline hydrochloride daily, while treatment groups A and B received needling at Baihui(GV 20)plus acupoint injection at Yanglingquan(GB 34) daily; treatment group B also received music therapy derived from the five phases in Traditional ChineseMedicine theory twice daily. All treatments were administered for 5 d per treatment cycle for three cycles, with a 1 d interval between cycles. In all three groups, Hamilton's depression scale(HAMD-17)score and the activities of daily life(ADL) score were measured before and after treatment, and side effects were assessed with the treatment emergent symptom scale.RESULTS: The HAMD-17 score significantly decreased after treatment in all three groups, and the post-treatment reduction in HAMD-17 score was markedly greater in treatment group B than in treatment group A(P < 0.01). The ADL score significantly increased after treatment in all three groups,and the post-treatment increase in ADL score was significantly greater in treatment group B than in treatment group A(P < 0.01). The treatment emergent symptom scale score was highest in the control group, and lowest in group B, and significantly differed between the three groups(P < 0.01).CONCLUSION: Five phase music therapy plus acupoint needling and acupoint injection can improve the symptoms in patients with post-stroke depression. 展开更多
