The aim of this study was to determine the utilization of SRMD (stress related mucosal disease) prophylaxis in the ICU (intensive care unit) of a large teaching institution based on duration and defined adverse ef...The aim of this study was to determine the utilization of SRMD (stress related mucosal disease) prophylaxis in the ICU (intensive care unit) of a large teaching institution based on duration and defined adverse effects. Patients were reviewed for appropriateness of SRMD prophylactic therapy based on the presence of two independent risk factors (coagulopathy and mechanical ventilation greater than 48 hours) versus presence of any one risk factor from a list developed by the study investigator. Data was collected into spreadsheets and outcomes were analyzed using descriptive statistics. When evaluating patients based on the presence of any risk factor, 84 percent of patients had at least one risk factor present, while 16 percent did not have any. In patients who received famotidine, there was one occtLrrence of Clostridium difficile and 6 cases of electrolyte abnormalities. In patients who received a PPI, there was one documented case of HAP, two cases of ventilator-associated pneumonia, three cases of Clostridium difficile, and 14 patients who developed electrolyte abnormalities. Data does not show an association between acid suppression therapy and incidence of nosocomial infections. The number of patients whose therapy exceeded the appropriate stop-date compared with the number of patients in which SRMD prophylaxis was discontinued when risk factors diminished was greater when evaluating patients based upon the two independent risk factors.展开更多

It is based on data obtained from archives including the t...Approximately 15,000-17,000 thousand people die worldwide in traffic accidents as a consequence of all types of injuries. This study has a descriptive nature. It is based on data obtained from archives including the treatment database of the Emergency Center. The period January 2009 to December 2012 has been considered for this research. The aim of this study is to assess the prevalence of fatal road traffic accidents in Pristine, Kosovo, and describe their determinants. The number of injured in traffic accidents who have sought emergency medical assistance in the Emergency Center from January 2009 to December 2012 is increasing significantly. It is concluded that except reasons mentioned above and among others, there also are the following factors: enormous increase of number of persons equipped with driving license, driving speeds above limits which are considered to be the primary cause of the road traffic accidents. These findings can serve as a basis for health care professionals and policymakers to create preventive measures for traffic accidents.展开更多

In this paper, we propose and study a simple SI type model that considers the effect of various precaution- ary measures to control HIV epidemic. We show, unlike conventional epidemic models, that the basic reproduction number which essentially considered as the disease eradica- tion condition is no longer sufficient to eliminate HIV infection. In particular, we show that even when the basic reproduction number is made less than unity, the disease may persist if the initial outbreak is not low. Eradication of disease is however guaranteed if the ensemble control measure exceeds some upper critical value. It is also shown that an epidemic model with mass action incidence may exhibit backward bifurcation and bistability if density-dependent demography is considered. Our theoretical study thus indicates that extra attention should be given in controlling HIV epidemic to achieve the desired result.展开更多
