METHODS: Twenty-nine consecutive mechanically ventilated patients were inve...AIM: To investigate factors predicting failure of percutaneous endoscopic gastrostomy (PEG) to eliminate gastroesophageal reflux (GER). METHODS: Twenty-nine consecutive mechanically ventilated patients were investigated. Patients were evaluated for GER by pH-metry pre-PEG and on the 7th post-PEG day. Endoscopic and histologic evidence of reflux esophagitis was also carried out. A beneficial response to PEG was considered when pH-metry on the 7th post-PEG day showed that GER was below 4%. RESULTS: Seventeen patients responded (RESP group) and 12 did not respond (N-RESP) to PEG. The mean age, sex, weight and APACHE II score were similar in both groups. GER (%) values were similar in both groups at baseline, but were significantly reduced in the RESP group compared with the N-RESP group on the 7th post-PEG day [2.5 (0.6-3.8) vs 8.1 (7.4-9.2, P 〈 0.001)]. Reflux esophagitis and the gastroesophageal flap valve (GEFV) grading differed significantly between the two groups (P = 0.031 and P = 0.020, respectively). Histology revealed no significant differences between the two groups.CONCLUSION: Endoscopic grading of GEFV and the presence of severe reflux esophagitis are predisposing factors for failure of PEG to reduce GER in mechanically ventilated patients.展开更多

Through experimental analysis of the liquid flows in a flow channel instrument, prompt evaluation of the physically defined viscosity of non-Newtonian liquid foods was investigated for development into a structurally ...Through experimental analysis of the liquid flows in a flow channel instrument, prompt evaluation of the physically defined viscosity of non-Newtonian liquid foods was investigated for development into a structurally simple and easy-to-use viscometer. For Newtonian and non-Newtonian test liquids, a relation between the friction coefficient and Reynolds number, which was dimensionless as derived from an expression of analysis as a gravity current, indicated a condition under which the flow in the instrument was laminar and under which an average shear rate was on the order of less than 1 s~. Prediction organized based on this empirical relation reproduced practically the flow curve determined for the liquid foods using a rotor type viscometer. Utilization of the channel instrument as a viscometer was formulated in terms of physical meanings of measurements such as the flow length and elapsed time.展开更多
Objective: To analyze reflux parameters by means of combined multichannel intraluminal impedance and pH (MII-pH) monitoring in patients with gastroesophageal reflux disease (GERD) symptoms off medication, and to ...Objective: To analyze reflux parameters by means of combined multichannel intraluminal impedance and pH (MII-pH) monitoring in patients with gastroesophageal reflux disease (GERD) symptoms off medication, and to find the reflux characteristics of Chinese GERD patients and the influences of gender, age, body posture, and body mass index (BMI) on gastroesophageal reflux (GER). Methods: Between Dec. 2008 and May 2014, 125 patients with typical GERD symptoms were subjected to 24-h MII-pH monitoring. Twelve patients with normal MII-pH profiles were not considered for analysis. The reflux parameters of 113 GERD patients with abnormal Mll-pH results were analyzed. Results: (1) DeMeester scores were above the normal range in 46.90% (53/113) of GERD patients. Weakly acidic refluxes were prevalent in GERD patients, and the frequency of abnormal weakly acidic reflux was 75.22% (85/113). The frequencies of abnormal symptom index (SI) and symptom association probability (SAP) were 19.47% (22/113) and 14.16% (16/113), respectively. (2) The frequencies of DeMeester scores, the %time at pH〈4, and the numbers of reflux episodes and of long reflux episodes 〉5 min were significantly higher in male patients than in female patients. (3) The %time at pH〈4 was much higher during upright periods than during supine periods. During supine periods, 31.86% (36/113) of GERD patients had delayed bolus clearance time, compared with 19.47% (22/113) during upright periods. (4) The number of abnormal DeMeester scores, %time at pH〈4, and the number of acid refluxes during upright periods were significantly higher in obese GERD patients than in GERD patients with a normal BMI. Over- weight GERD patients also had many more acid refluxes during upright periods than GERD patients with a normal BMI Conclusions: Weakly acidic refluxes were prevalent in Chinese GERD patients.
