AIM: To assess the prevalence of mild gastrointestinal disorders in milk-fed infants in paediatric practice, and to evaluate the effectiveness and satisfaction with dietetic treatment.METHODS: A cross-sectional epidemiological study was first carried out. A total of 285 paediatricians included 3487 children seen during a period of one week. In a second phase an observational, prospective and multicentre study was conducted and 2069 milk-fed infants with mild gastrointestinal disorders (colic, constipation, regurgitation and diarrhoea) were included. There was a baseline visit (start of treatment) and a final visit four weeks later. The effectiveness of the various Novalac formulas, as well as the satisfaction of the parents/tutors and paediatricians with the dietetic treatment were assessed at the final visit.RESULTS: The prevalence of mild gastrointestinal disorders was 27.8% of all paediatrician consultations (9.2%, 7.8%, 6.1% and 4.6% in relation to colic, constipation, regurgitation and diarrhoea, respectively). The several Novalac adapted milk formulas resolved 88.4% of the mild gastrointestinal disorders. Depending on the type of disorder, differences in response rate were observed. The highest effectiveness was recorded with respect to diarrhoea (92.6%), followed by constipation (91.6%), colic (87.6%) and regurgitation (81%). Overall, 91% of the paediatricians and 88.8% of the parents/tutors were satisfied or very satisfied with the Novalac adapted milk formulas.CONCLUSION: Mild gastrointestinal disorders show a high prevalence in paediatric practice. The Novalac adapted milk formulas have been shown to be effective in treating mild gastrointestinal disorders in milk-fed infants in the context of routine clinical practice. 展开更多

A substantial proportion of patients with irritable bowel syndrome(IBS) associate their symptoms with the ingestion of specific foods. Therefore, in recent years, scientific research has increasingly focused on the role of diet in IBS and dietary management is now considered an important tool in IBS treatment. This article reviews the main dietary approaches in IBS emphasizing evidence from experimental and observational studies and summarizing the main diet and lifestyle recommendations provided by dietary guidelines and scientific literature. Despite the limited evidence for a beneficial role, general advice on healthy eating and lifestyle is recommended as the first-line approach in the dietary management of IBS. Standard recommendations include adhering to a regular meal pattern, reducing intake of insoluble fibers, alcohol, caffeine, spicy foods, and fat, as well as performing regular physical activity and ensuring a good hydration. Second-line dietary approach should be considered where IBS symptoms persist and recommendations include following a low FODMAP diet, to be delivered only by a healthcare professional with expertise in dietary management. The efficacy of this diet is supported by a growing body of evidence. In contrast, the role of lactose or gluten dietary restriction in the treatment of IBS remains subject to ongoing research with a lack of high-quality evidence. Likewise, further clinical trials are needed to conclude the efficacy of probiotics on IBS symptoms. 展开更多

Cow's milk constitutes a significant food for babies when mother's milk is insufficient. The purpose of this work is to offer a fermented milk, potentially hypoallergenic, using selected Lactobaeillus acidophilus and Bifidobacteria sp. for their proteolytic activities. The fermentation is evaluated by the rate of lactic acid production and by the bacterial enumeration at 45 ℃. The rate of acidification obtained by mixed cultures (La + B. longum, 63.8 ± 3.5 °D) and (La + B. breve, 43.4 ± 1.67 °D) compared with the control (milk) (18.6 ± 1.31 °D). The result of the best hydrolysis was obtained by (La+ B. longum) (154.88 ± 30.33 ug/mg) which corresponded to a better release of the a-NH2 functions (103.32 ± 12.81 umoles/mg) compared with control (307.2 ± 11.54 ug/mg and 10.25 ± 0.44 umoles/mg ). The best synergy was obtained by (La + B. bifidum) (13 × 10^6 cfu/mL and 60 × 10^6 cfu/mL) and reached 95 × 10^10 cfu/mL and 119 × 10^10 cfu/mg at the end. The electrophoresis of fermented milk revealed the presence of soluble proteins (a-Lactalbumin and β-Lactoglobulin). The Enzyme Linked Immtmo Sorbent Assay showed the results of residual antigenic activities of β-Lg (1.4 ± 0.23 ug/mg), a-La (0.012 ± 0.004 ug/mg) and bovin serum albumin (0.014 ± 0.005 ug/mg)by the associations (La + B. longum), (La + B. bifidum) and (La + B. bifidum) compared with the control (14.43 ± 5.91 ug/mg, 0.183 ± 0.062 gg/mg, 0.05 ± 0.008 ug/mg) respectively. Lactic fermentation reduced to a significant antigenic reactivity of principal whey milk proteins. 展开更多
