骨外型牙源性钙化上皮瘤极为罕见,通过复习1966~2011年英文MEDLINE与1994~2011年中文CHKD中收录的文献,仅有37例骨外型牙源性钙化上皮瘤的个案报告。根据上述资料分析,发现该瘤具有以下临床特点,就诊年龄3~71岁,平均35.8岁;男13例,... 骨外型牙源性钙化上皮瘤极为罕见,通过复习1966~2011年英文MEDLINE与1994~2011年中文CHKD中收录的文献,仅有37例骨外型牙源性钙化上皮瘤的个案报告。根据上述资料分析,发现该瘤具有以下临床特点,就诊年龄3~71岁,平均35.8岁;男13例,女19例,男女比例为1∶1.46;肿块直径0.5~4 cm,平均1.76 cm;好发部位主要是牙龈,累及下颌与上颌牙龈的比例为1∶1;组织学特征表现与骨内型牙源性钙化上皮瘤基本相同;复发率为5.4%。因该瘤具有浅表骨侵蚀能力,应行完整的肿瘤切除结合骨面刮治术。 展开更多

目的:探讨自攻型骨钉(以下简称骨钉)借助外固定器上的攻丝圆孔在松质骨和皮质骨上钻孔的生物力学性能。方法:采集8具新样成人尸体胫骨标本,制作骨折模型,以胫骨远、近骨折段的外固定钉间距p-p(25、50、75、100 mm)和中间的骨折断端间距... 目的:探讨自攻型骨钉(以下简称骨钉)借助外固定器上的攻丝圆孔在松质骨和皮质骨上钻孔的生物力学性能。方法:采集8具新样成人尸体胫骨标本,制作骨折模型,以胫骨远、近骨折段的外固定钉间距p-p(25、50、75、100 mm)和中间的骨折断端间距g(0、10 mm),依照2×4析因分组方法,共分为9组,测定骨钉旋入松质骨或皮质骨并穿透对侧骨皮质的扭力(穿透力F)、回弹力(G)和功耗(U)的大小,并行应力分析。结果:骨密度越高的骨质,螺钉旋入时力矩越大,反之,越小;用T字形套筒扳手可轻便地将骨钉旋入到骨质中并穿透对侧骨皮质。结论:骨钉对松质骨和皮质骨的钻孔扭力、回弹力和功耗存在差异,骨骼的骨密度(BMD)、个体差异等对骨钉钻孔的上述生物力学性能也存在差异;钉-钉间距的差异对上述生物力学性能无统计学意义(P>0.05),骨断端间距的差异对上述生物力学性能也无统计学意义(P>0.05)。 展开更多

目的观察正骨手法复位儿童旋后外旋型胫骨远端骨骺骨折的效果。方法该院2019年10月至2021年12月就诊的旋后外旋型胫骨远端骨骺骨折患儿34例,应用张氏提踵端提翻旋法骨折复位联合夹板外固定,术后观察骨折复位、愈合情况及踝关节功能恢复... 目的观察正骨手法复位儿童旋后外旋型胫骨远端骨骺骨折的效果。方法该院2019年10月至2021年12月就诊的旋后外旋型胫骨远端骨骺骨折患儿34例,应用张氏提踵端提翻旋法骨折复位联合夹板外固定,术后观察骨折复位、愈合情况及踝关节功能恢复情况。结果34例患儿均一次整复固定成功,骨折达解剖或近解剖复位,无血管、神经损伤。去除外固定时间35~48天,平均41天。治愈33例(97.1%),好转1例(2.9%)。结论应用张氏提踵端提翻旋法复位旋后外旋型胫骨远端骨骺骨折后行夹板外固定,功能锻炼贯穿治疗全程,踝关节功能恢复良好。 展开更多

AIM: To asses the expression of myeloid dendritic cells (CD11c+) subset during acute HCV hepatitis and its possible involvement in natural history of the infection.METHODS: We enrolled 11 patients with acute hepatitis C (AHC) (Group A), 10 patients with acute hepatitis A (AHA) (as infective control-Group B) and 10 healthy donors (group C) in this study. All patients underwent selective flow cytometry gating strategies to assess the peripheral number of the myeloid dendritic cells (mDCs) to understand the possible role and differences during acute hepatitis.RESULTS: Eight of 11 patients with acute HCV hepatitis did not show any increase of mDCs compared to healthy individuals, while a significant decrease of mDCs was found in absolute cell count (z=-2.37, P〈0.05) and percentage (z=-2.30; P〈 0.05) as compared with AHA. On the contrary, The remaining three patients of the group A had a higher mDCs number and percentage as occur in group B. Interestingly, after six months, those patients did not show any increase of mDCs subset were chronically infected, while the three subjects with an increase of peripheral mDCs, as in HAV acute infection, resolved the illness.CONCLUSION: The lack of increase of mDCs during acute hepatitis C might be an important factor involved in chronicization of the infection. 展开更多

In order to expand the engineering application of recycle aggregate mortars (RAM) with aggregates from demolished concretes, the models for the properties of RAM and the replacement rate of these recycled fine aggregates were proposed. First, different kinds of mathematical models for the basic properties (compressive strength, water retention rate, and consistency loss) of RAM with two kinds of admixtures, thickening powders (TP) and self-made powdery admixtures (SSCT) designed for RAM, and the replacement rates were established, while the average relative errors and relative standard errors of these models were calculated. Additionally, the models and their error analyses for the curves of drying shrinkage and curing time of RAM + SSCT at different replacement rates were put forward. The results show that polynomial functions should be used to calculate the basic properties of RAM + TP and RAM + SSCT at different replacement rates. In addition, polynonfial functions are the most optimal models for the sharp shrinkage sections in the curves of drying shrinkage-curing time of RAM + SSCT, while exponential functions should be used as the models for the slow shrinkage sections and steady shrinkage sections. 展开更多

The aim of the present study was to develop and validate a new marker model for optoelectronic systems adapted to wearable devices, in order to have an analysis tool for kinematic gait evaluation of reproduced patterns by exoskeletons. The marker model has a total of 36 retro-reflective markers attached bilaterally to anatomical landmarks during the static measures (without exoskeleton) and 28 markers at the dynamics measures (with exoskeleton). The main difference between others kinematic models and the described adapted model was the placement of the three markers in the back thigh and the other three in the back calf, what allowed removing the hip, thigh, knee, tibia and ankle markers. The proposed adapted marker model could be an effective tool to validate the joint movement and velocities of those wearable exoskeletons that at present have been developing. 展开更多

The most commonly accepted system of classification for tibia plateau fractures is that of Schatzker. Increasingly, both high energy injuries and atypical osteoporotic fragility failures have led to more complex, unusual and previously undescribed fracture patterns being recognized. We present a case of a patient with a previously unreported pattern of tibia plateau fracture and knee dislocation. We highlight the challenges confronted and present the management and the outcomes of his injury. A 28-year old male motorcyclist was involved in a head on collision with a truck and was transferred by helicopter to our level I major trauma centre emergency department. His injuries were a circumferential degloving injury to his left leg and a right lateral tibial plateau fracture/knee dislocation. The pattern of the lateral tibial plateau fracture was unique and did not fit any recognised classification system. The patient received a spanning external fixator initially and after latency of 12 days for soft tissue resuscitation he underwent definite fixation through an antero-lateral approach to the proximal tibia with two cannulated 6.5 mm partially threaded screws and an additional lateral proximal tibia plate in buttress mode. A hinged knee brace was applied with unrestricted range of motion post-operatively and free weight bearing were permitted post operatively. At the 6 months follow up, the patient walks without aids and with no limp. Examination revealed a stable joint and full range of motion. Plain radiographs revealed that the fracture healed with good alignment and the fixation remained stable. High energy injuries can lead to more complicated fracture patterns, which challenge the orthopaedic surgeons in their management. It is crucial to understand the individual fracture pattern and the possible challenges that may occur. This study reports a lateral tibia plateau fracture/dislocation which perhaps is best described as a reverse Schatzker IV type fracture. 展开更多

The purpose of this study was to establish a method for measuring the knee valgus angle from the ana- tomical and mechanical axes on three-dimensional reconstruction imaging models, and to use this method for estimating an average knee valgus angle value for northern Chinese adults. Computed tomographic angiography data in DICOM format for 128 normal femurs from 64 adult subjects were chosen for analysis. After the femur images were subjected to three-dimensional reconstruction, the deepest point in the intercondylar notch (point A), the midpoint of the medullary cavity 20 cm above the knee-joint line (point B), and the landmark of the femoral head rotation center (point C) were identified on each three-dimensional model. The knee valgus angle was defined as the angle enclosed by the distal femoral anatomical axis (line AB) and the femoral mechanical axis (line AC). The average (mean+SD) of knee valgus angle for the 128 femurs was 6.20°±1.20° (range, 3.05° to 10.64°). Significant positive correlations were found between the knee valgus angles of the right and left sides and between the knee valgus angle and age. During total knee arthroplasty, choosing a valgus cut angle of approximately 6° may achieve a good result in reestablishing the natural mechanical alignment of the lower extremity for patients of northern Chinese ethnicity.
