AIM:To study the mechanism of cerebral pathological and pathophysiological cha nges after extremity gunshot wounds at molecular level.METHODS:M-193,5.56 mm bu llets were used to shoot the most plentiful part of the hi...AIM:To study the mechanism of cerebral pathological and pathophysiological cha nges after extremity gunshot wounds at molecular level.METHODS:M-193,5.56 mm bu llets were used to shoot the most plentiful part of the hind extremities of the dogs.C-fos gene expression in neurons of cerebral tissues of different time int ervals after gunshot in the subject and control groups were detected with immuno histochemistry.RESULTS:No C-fos gene expression was detected in the cerebral co rtical neurons in the control group.But in the subject group,C-fos expression i ncreased in the cerebral neurons 30 min after injury,reached the peak 6 hours(P< 0.05),and began to decrease 10 hours post-injury.CONCLUSION:After extremity gu nshot wound C-fos expression in neurons of distal parts is the early response o f the body to the injury,which might be caused by Leao’s spreading depression(SD ),and correlated to extracellular transduction and cell apoptosis.展开更多

Methods: Two to 4 courses of HBO therapy and/or medications were used to treat 320 patients who we...Objective: To evaluate the effects of hyperbaric oxygen (HBO) therapy on patients with postbrain injury neural status. Methods: Two to 4 courses of HBO therapy and/or medications were used to treat 320 patients who were randomly divided into two groups. Assessment was made with 99m Tc-ethyl cysteinate dimer ( 99m Tc-ECD) single photon emission computed tomography (SPECT) before and after treatment. Results: There was a significant difference between the HBO therapy group and the non-HBO therapy group. HBO therapy was superior to medication treatment alone in the recovery of clinical symptoms, control of epilepsy, and resolution of hydrocephalus (P< 0.01). Conclusions: HBO therapy has specific curative effects on patients with postbrain injury neural status, and 99m Tc-ECD SPECT could play an important role in diagnosing postbrain injury neural status and monitoring the therapeutic effects of HBO.展开更多
Solitary coronal shear fractures of femo- ral condyle, such as Hoffa's fracture, are usually associated with supracondylar or intercondylar fractures of the femur. These fractures are rare and seen in the context of ...Solitary coronal shear fractures of femo- ral condyle, such as Hoffa's fracture, are usually associated with supracondylar or intercondylar fractures of the femur. These fractures are rare and seen in the context of high energy mechanism leading to multiple injuries; therefore a thorough workup of the patient is required to rule out other significant injuries. Hoffa's fracture associated with femo- ral shaft and proximal tibial fractures is extremely rare and no such injury has been reported previously. We report two such cases which were managed with standard operativefixation techniques and demonstrated good to excellent func- tional outcome after a follow-up of one year. This report emphasizes that high index of suspicion is required for di- agnosis of these injuries and a thorough workup is manda- tory to rule out other associated injuries. Here we reported a case of compartment syndrome in the upper limb followi...Compartment syndrome after an adder bite is extremely rare, whose effects are only secondary to the cytotoxic and hemorrhagic effects of venom. Here we reported a case of compartment syndrome in the upper limb following an adder bite in the thenar eminence. Elevated compartment pressure was documented and immediate surgical fasciotomy was practiced. The patient achieved complete recovery with a good functional result. We discussed the controversies on fasciotomy and non-invasive measures in such a situation, and recommended intracompartmental pressure monitoring during the management of compartment syndrome following adder bites.展开更多
