咽鼓管-鼓室气流动态图法与音响法检测咽鼓管开放功能比较 被引量:1

目的通过比较音响法和咽鼓管-鼓室气流动态图(tubo-tympano-aerography,TTAG)检查咽鼓管开放功能的差异,探讨咽鼓管开放功能的综合评估方法及意义。方法对129例(211耳)主诉耳闷胀不适、耳痛、耳鸣的患者(鼓膜完整且排除咽鼓管异常开放)... 目的通过比较音响法和咽鼓管-鼓室气流动态图(tubo-tympano-aerography,TTAG)检查咽鼓管开放功能的差异,探讨咽鼓管开放功能的综合评估方法及意义。方法对129例(211耳)主诉耳闷胀不适、耳痛、耳鸣的患者(鼓膜完整且排除咽鼓管异常开放)分别用音响法和TTAG法评估咽鼓管开放功能,依据检测的曲线特点及吞咽次数将TTAG法结果分为正常I型、正常II型及开放不良,音响法结果分为正常及开放不良,比较两种方法检查的结果。结果211耳音响法检测出延迟开放型咽鼓管功能障碍的阳性率为26.07%(55/211),TTAG法检测的阳性率为23.22%(49/211),前者略高于后者,但差异无统计学意义(P=0.498),两种检查的一致率为74.41%,Kapaa=0.312。在咽鼓管开放时TTAG法I型者鼻咽腔压强平均值为482.70±126.74 daPa,TTAG法II型者为418.84±152.70 daPa,两者差异无统计学意义(P=0.168);TTAG法I型者外耳道压强平均值为42.29±18.28 daPa,TTAG法II型者为36.88±16.25 daPa,前者大于后者,差异有统计学意义(P=0.004);TTAG法I型者音响法测得咽鼓管开放持续时间为220.48±94.28 ms,TTAG法II型者为423.99±207.13 ms,前者小于后者,差异有统计学意义(P<0.001)。结论音响法与TTAG法对延迟开放型咽鼓管功能障碍检查的敏感性基本相同;TTAG法I型者气流经咽鼓管进出较TTAG法II型者顺畅,提示TTAG法II型者形成延迟开放型咽鼓管功能障碍的倾向性更明显。 展开更多

目的通过比较咽鼓管测压(tubomanometry,TMM)结合咽鼓管评分(eustachian tube score,ETS)法和咽鼓管-鼓室气流动态图(tubotympano-aerodynamicgraphy,TTAG)评估咽鼓管被动开放功能的差异,为咽鼓管被动开放功能的综合评估提供依据。方法... 目的通过比较咽鼓管测压(tubomanometry,TMM)结合咽鼓管评分(eustachian tube score,ETS)法和咽鼓管-鼓室气流动态图(tubotympano-aerodynamicgraphy,TTAG)评估咽鼓管被动开放功能的差异,为咽鼓管被动开放功能的综合评估提供依据。方法对28例(50耳)主诉耳闷不适、耳痛、耳鸣的患者(耳镜检查显示鼓膜完整且排除咽鼓管异常开放)分别用不同鼻咽部压力(30、40、50 mbar)TMM法结合主观评分(Valsalva动作及吞咽时耳内是否有声音)和TTAG法评估咽鼓管被动开放功能,比较两种检查方法的阳性率及一致性。结果50耳ETS评估的阳性率为10.00%(5/50),TTAG法的阳性率为14.00%(7/50),前者低于后者(P<0.001),两种检查的一致率为96.00%(Kapaa=0.811)。30 mbar鼻咽部压力下TMM法的阳性率为26.00%(13/50),高于TTAG法(P<0.001),两种检查的一致率为90.00%(Kapaa=0.680);40 mbar鼻咽部压力下TMM法的阳性率为12.00%(6/50),低于TTAG法(P<0.001),两种检查的一致率为94.00%(Kapaa=0.694);50 mbar鼻咽部压力下TMM法的阳性率为4.00%(2/50),低于TTAG法(P=0.017),两种检查的一致率为90.00%(Kapaa=0.408);30 mbar鼻咽部压力下TMM法的阳性率高于40 mbar(P<0.001),两种鼻咽部压力下的一致率为86.00%(Kappa=0.559);30 mbar鼻咽部压力下TMM法的阳性率高于50 mbar(P=0.015),两种鼻咽部压力下的一致率为80.00%(Kappa=0.307);40 mbar鼻咽部压力下TMM法的阳性率高于50 mbar(P=0.008),两种鼻咽部压力下的一致率为94.00%(Kappa=0.645)。结论与ETS法相比,TTAG法对延迟开放型咽鼓管功能障碍检查的敏感性更高,且TTAG法与40 mbar鼻咽部压力下的TMM法的一致性最高;鼻咽部压力越高,TMM法的敏感性越低,且40、50 mbar鼻咽部压力下TMM法的一致性最高。 展开更多

A three-dimensional numerical model verified by previous experimental data is developed to simulate the fluidized bed gasification of refuse derived fuel (RDF). The CaO dechlorination model obtained by the thermal gravity analysis (TGA) is coupled to investigate the process of CaO dechlorination. An Eulerian-Eulerian method is adopted to simulate the gas-solid flow and self-developed chemical reaction modules are used to simulate chemical reactions. Flow patterns, gasification results and dechlorination efficiency are obtained by numerical simulation. Meanwhile, simulations are performed to evaluate the effects of Ca/Cl molar ratio and temperature on dechlorination efficiency. The simulation results show that the presence of bubbles in the gasifier lowers the CaO dechlorination efficiency. Increasing the Ca/Cl molar ratio can enhance the dechlorination efficiency. However, with the temperature increasing, the dechlorination efficiency increases initially and then decreases. The optimal Ca/Cl molar ratio is in the range of 3. 0 to 3. 5 and the optimal temperature is 923K. 展开更多

The bending moment acting on the overhung shaft of a gas-sparged vessel stirred by a Rushton turbine,as one of the results of fluid and structure interactions in stirred vessels,was measured using a moment sensor equipped with digital telemetry.An analysis of the shaft bending moment amplitude shows that the amplitude distribution of the bending moment,which indicates the elasticity nature of shaft material against bending deformation,follows the Weibull distribution.The trends of amplitude mean,standard deviation and peak deviation characteristics manifest an "S" shape versus gas flow.The "S" trend of the relative mean bending moment over gas flow rate,depending on the flow regime in gas-liquid stirred vessels,resulted from the competition among the nonuniformity of bubbly flow around the impeller,the formation of gas cavities behind the blades,and the gas direct impact on the impeller when gas is introduced.A further analysis of the bending moment power spectral density shows that the rather low frequency and speed frequency are evident.The low-frequency contribution to bending moment fluctuation peaks in the complete dispersion regime. 展开更多

Negative pressure plays a very important role in compact spinning system.To know airflow field and its distribution is helpful to look into the condensing principle of fiber bundle.Therefore,computational fluid dynamics(CFD)software was used to simulate airflow field in this paper.Airflow velocity distributions both in different fiber layers and under different negative pressures were discussed.The results indicate that airflow velocity in upper layer of the fiber bundle is greater than that in lower layer.Airflow velocities in both X and Y axis directions have a positive correlation with negative pressure.It can provide a theoretical base to make high quality compact yarns in productive practice. 展开更多

In order to identify potential wood substitutes for the production of energy by gasification, binary blends (wood/miscanthus, miscanthus/straw and wood/straw) and ternary blends (wood, miscanthus and organic residue) were systematic tested in a laboratory bubbling fluidized bed gasification system. The results of experiments were compared with results of wood gasification. Of the binary blends, wood and miscanthus exhibited great potential as a wood substitute in fluidized bed gasification in terms of process stability and product gas quality. Adding 10 wt. % of organic residues to form ternary blends further improved the product gas quality. Gasification of fuels blended with straw tended to agglomerate in the fluidized bed because of straw's low ash melting temperature. This can be counteracted by adding Ca(OH)2 to fuels. Nonetheless, fuels blended with straw with higher percentages of Ca(OH)2 need further study to establish the optimal additive ratio. 展开更多
