Radiotherapy is an important part of treatment for NPC patients, especially those with stage Ⅲ/Ⅳ disease. Subdural empyema is a rare but life-threatening complication in postradiotherapy NPC patients which should be paid more attention. Here, we present the case of a 64-year-old female postradiotherapy NPC patient with subdural empyema complicated with intracranial hemorrhage. She was treated by burr-hole surgery but unfortunately died because of recurrent intracranial hemorrhage. The mechanisms potentially underlying the formation of subdural empyema in postradiotherapy NPC patients and the surgical strategies that can be used in these patients are discussed in this report.展开更多

Therefore studying IMRT is important. CT and MRI are complementary, and their joint use is currently considered to be the optimal modality to delineate the extent of the primary spread of NPC. The key problem in delineation of the neck nodes is how to translate anatomic node regions into the CT boundaries. The consensus guideline which narrowed the gap among different cancer centers is recommended in delineating the boundary of the cervical lymph node regions. The definition of the NPC GTV is clear and almost the same among the main cancer centers in their IMRT planning protocols. The suggested biological dose to the GTV is close to or more than 80 Gy; the main differences are the definitions of the CTVs and their schemes for the prescribed dose, and also the dosage to the high cervical region is different among those centers. According to their long-term follow-up results, it is suggested that, besides adding 5-10 mm margins to the primary lesions, the immediate high-risk structures (including the entire nasopharyngeal cavity, retropharyngeal space, clivus, base of the skull, pterygoid plates and muscles, parapharyngeal space, the sphenoid and partial ethmoid sinuses, the posterior third of the maxillary sinuses and the nasal cavity) should also be included with a prescription of more than 60 Gy, and the bilateral Ib, II and Va node levels should be ranked as high-risk regions and differentially prescribed for treatment with no less than 60 Gy.展开更多

Methods: Cell cycle retardation, apoptosis and cell survival rate...Objective: To study cell cycle retardation, apoptosis and the expression of antioncogene p57kip2 by radioactive rays in nasopharyngeal carcinoma cells. Methods: Cell cycle retardation, apoptosis and cell survival rate induced by radioac- tive rays were tested by the methods of flow cytometry and MTT method. The expression of antioncogene p57kip2 was detected by immunohistochemistry and Western blot. Results: After irradiation, G1 phase had no obvious retardation, S phase showed transient delay. There was a positive correlation between irradiation dosage and retardation strength in G2/M phase (P < 0.01). Peak value appeared at 24 h after 12 Gy irradiation, then decreased. There was a positive correlation between apop- tosis incidence and irradiation dosage or after-irradiation time extention (P < 0.01). There was a negative correlation between cell survival rate and irradiation dosage or apoptosis incidence (P < 0.01). The expression of p57kip2 protein was up-regulated along with the prolongation of time and dosage after irradiation (P < 0.01). Conclusion: G2/M phase arrest, apoptosis and the up-regulation of the expression of p57kip2 protein all can reflect predict the radiosensitivity of nasopharyngeal carcinoma cells.展开更多

Moxibustion on herbs, a common complementary and alternative medicine approach, is frequently used for treating AR in clinical practice. Western medicine is good at quick symptomatic relief,while offer little or no sustainable and steady long-term effect. Little established evidence is available to support the long-term effectiveness of moxibustion on herbs for AR.Objective: This study is a randomized controlled trial to assess the long-term effectiveness of moxibustion on herbs in moderate-severe persistent AR population.Methods: The study sample size is 56 patients. Eligible patients with moderate-severe persistent AR will be randomized into a moxibustion on herbs combined with conventional treatment group(MOHCT group) and a conventional treatment group(CT group) in a 1:1 ratio. Patients in the MOHCT group will receive a 30-min moxibustion on herbs treatment on Zhiyang(至阳 GV 9), Dàzhuī(大椎 GV 14) and bilateral Shenshu(肾俞 BL 23), Fèishū(肺俞 BL 13) for a total of 12 times on the basis of conventional treatment, while those in the CT group will receive conventional treatment alone. The primary outcome measure is VAS score for total nasal symptoms, which will be obtained via a self-recorded AR diary. The secondary outcome measures include the average occurrence of symptoms per week, use of medication and Rhinoconjunctivitis Quality of Life Questionnaire(RQLQ).Discussion: The objectives of this study include(1) to evaluate the long-term effectiveness and safety of moxibustion on herbs for treating AR;(2) to evaluate whether moxibustion on herbs can reduce the frequency of AR symptoms in patients with moderate-severe persistent allergic rhinitis.
