Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome in which ectopic production of fibroblast growth factor 23 (FGF23) by non-malignant mesenchymal tumors causes phosphate wasting and bone fractures. Recent studies have implicated the hypoxia-inducible factor-la (HIF-la) in other phosphate wasting disorders caused by elevated FGF23, including X-linked hypophosphatemic rickets and autosomal dominant hypophosphatemia. Here we provide evidence that HIF-la mediates aberrant FGF23 in TIO by transcriptionally activating its promoter. Immunohistochemical studies in phosphaturic mesenchymal tumors resected from patients with documented TIO showed that HIF-la and FGF23 were co-localized in spindle- shaped cells adjacent to blood vessels. Cultured tumor tissue produced high levels of intact FGF23 and demonstrated increased expression of HIF-la protein. Transfection of MC3T3-E1 and Saos-2 cells with a HIF-la expression construct induced the activity of a FGF23 reporter construct. Prior treatment of tumor organ cultures with HIF-la inhibitors decreased HIF-la and FGF23 protein accumulation and inhibited HIF-la-induced luciferase reporter activity in transfected cells. Chromatin immunoprecipitation assays confirmed binding to a HIF-la consensus sequence within the proximal FGF23 promoter, which was eliminated by treatment with a HIF-la inhibitor. These results show for the first time that HIF-la is a direct transcriptional activator of FGF23 and suggest that upregulation of HIF-la activity in TIO contributes to the aberrant FGF23 production in these patients. 展开更多

BACKGROUND The incidence and prevalence of atrial fibrillation are increasing each year,and this condition is one of the most common clinical arrhythmias.AIM To investigate the levels and significance of serum fibroblast growth factor 23(FGF-23)and miR-208 b in patients with atrial fibrillation and their relationship with prognosis.METHODS From May 2018 to October 2019,240 patients with atrial fibrillation were selected as an observation group,including 134 with paroxysmal atrial fibrillation and 106 with persistent atrial fibrillation;150 patients with healthy sinus rhythm were selected as a control group.The serum levels of FGF-23 and miR-208 b in the two groups were measured.In the observation group,cardiac parameters were determined by echocardiography.RESULTS The serum levels of FGF-23 and miR-208 b in the observation group were 210.20±89.60 ng/mL and 5.30±1.22 ng/mL,which were significantly higher than the corresponding values in the control group(P<0.05).In the observation group,the serum levels of FGF-23 and miR-208 b in patients with persistent atrial fibrillation were 234.22±70.05 ng/mL and 5.83±1.00 ng/mL,which were significantly higher than the corresponding values in patients with paroxysmal atrial fibrillation(P<0.05).The left atrial dimension(LAD)of patients with persistent atrial fibrillation was 38.81±5.11 mm,which was significantly higher than that of patients with paroxysmal atrial fibrillation(P>0.05).The serum levels of FGF-23and miR-208 b were positively correlated with the LAD(r=0.411 and 0.382,P<0.05).In the observation group,the serum levels of FGF-23 and miR-208 b in patients with a major cardiovascular event(MACE)were 243.30±72.29 ng/mL and 6.12±1.12 ng/mL,which were significantly higher than the corresponding values in patients without a MACE(P<0.05).CONCLUSION The serum levels of FGF-23 and miR-208 b are increased in patients with atrial fibrillation and are related to the type of disease,cardiac parameters,and prognosis. 展开更多

超声造影参数联合血清FGF23和FGFR1水平检测对乳腺癌的临床诊断价值研究

目的 探讨超声造影参数联合血清成纤维细胞生长因子-23(fibroblast growth factor-23,FGF23)和成纤维细胞生长因子受体1(fibroblast growth factor receptor 1,FGFR1)在诊断乳腺癌的临床应用价值。方法 选择2020年8月~2022年6月在新疆... 目的 探讨超声造影参数联合血清成纤维细胞生长因子-23(fibroblast growth factor-23,FGF23)和成纤维细胞生长因子受体1(fibroblast growth factor receptor 1,FGFR1)在诊断乳腺癌的临床应用价值。方法 选择2020年8月~2022年6月在新疆维吾尔自治区人民医院就诊的87例乳腺癌患者作为观察对象(恶性组),选取同期90例乳腺良性病变患者作为良性组,分析两组观察对象超声造影参数,酶联免疫吸附法(enzyme-linked immunosorbent assay,ELISA)检测血清FGF23和FGFR1表达水平,Pearson法分析乳腺癌患者血清FGF23和FGFR1水平与超声造影参数的相关性,ROC曲线分析超声造影参数、血清FGF23和FGFR1对乳腺癌的诊断价值。结果 与良性组比较,恶性组的超声造影峰值强度(peak intensity,PI)(15.36±3.47 dB vs 12.54±2.83 dB)、曲线下面积(area under curve,AUC)(382.69±90.51 dB/s vs 262.95±75.34 dB/s)及血清FGF23(119.28±26.41 pg/ml vs 86.41±20.35 pg/ml),FGFR1(11.36±3.42 ng/mL vs 7.24±2.26 ng/mL)水平升高,达峰时间(time to peak,TTP)(18.46±4.31 s vs 24.23±5.25 s)降低,差异均有统计学意义(t=5.934~9.579,均P=0.000)。相关性分析显示,乳腺癌患者血清FGF23水平与PI,AUC呈正相关(r=0.493,0.561,均P=0.000),与TTP呈负相关(r=-0.572,P=0.000);血清FGFR1水平与PI,AUC呈正相关(r=0.603,0.491,均P=0.000),与TTP呈负相关(r=-0.545,P=0.000)。超声造影参数联合血清FGF23和FGFR1诊断乳腺癌的曲线下面积为0.971,联合诊断效能明显高于单独诊断,差异均有统计学意义(Z=6.478,5.558,4.839,4.780,4.943,均P <0.05)。结论 超声造影参数联合血清FGF23和FGFR1对乳腺癌具有较高诊断价值,可为乳腺癌的临床诊治提供参考。 展开更多

阿尔茨海默病患者血清FGF23和ZAG水平表达与认知功能的相关性分析

目的 探究阿尔兹海默病(Alzheimer’s disease, AD)患者血清成纤维细胞生长因子23(fibroblast growth factor 23, FGF23)、锌-α-2-糖蛋白(zin-α-2 glycoprotein,ZAG)水平与认知功能损伤的相关性。方法 选取2021年4月~2022年4月在内江... 目的 探究阿尔兹海默病(Alzheimer’s disease, AD)患者血清成纤维细胞生长因子23(fibroblast growth factor 23, FGF23)、锌-α-2-糖蛋白(zin-α-2 glycoprotein,ZAG)水平与认知功能损伤的相关性。方法 选取2021年4月~2022年4月在内江市第二人民医院精神科收治的90例AD患者为研究对象(AD组),另外选取90例同期在该院体检健康人员作为对照组。酶联免疫吸附法(enzyme linked immunosorbent assay,ELISA)检测两组血清FGF23和ZAG水平。用简易精神状态量表(MMSE)对AD患者进行认知功能评估。Pearson法分析AD患者血清FGF23,ZAG表达水平与MMSE评分的相关性。受试者工作特征(receiver operating characteristic, ROC)曲线分析血清FGF23和ZAG对AD患者的诊断价值。结果 与对照组相比,AD组患者血清FGF23(28.67±7.64 ng/ml vs18.75±4.23 ng/ml),ZAG(22.31±3.26μg/ml vs 16.43±3.72μg/ml)表达水平较高,MMSE评分较低(13.67±1.72分vs 29.56±2.89分),差异具有统计学意义(t=10.776, 11.278, 44.823,均P <0.05)。Pearson分析显示,AD患者血清FGF23,ZAG水平与MMSE评分均呈负相关(r=-0.611,-0.480,均P <0.05)。ROC曲线分析显示,血清FGF23和ZAG水平诊断AD的敏感度分别为76.70%和75.60%,特异度分别为86.70%和83.30%,曲线下面积(area under curve, AUC)分别为0.870(95%CI:0.814~0.927)和0.849(95%CI:0.793~0.906),二者联合检测的敏感度、特异度和AUC分别为86.70%,88.90%,0.938(95%CI:0.905~0.972)。结论 AD患者血清FGF23和ZAG水平升高,二者的表达水平与认知功能障碍呈负相关,可能与该病的发生发展相关。 展开更多

目的 探讨2种不同血液透析方式对维持性血液透析(MHD)患者心脏舒张功能的影响.方法 60名无心力衰竭表现的MHD患者分别进入高通量血液透析(HFD)组及普通血液透析(HD)组,每组30例,观察12个月.分别于治疗前(0个月)和治疗后(12个... 目的 探讨2种不同血液透析方式对维持性血液透析(MHD)患者心脏舒张功能的影响.方法 60名无心力衰竭表现的MHD患者分别进入高通量血液透析(HFD)组及普通血液透析(HD)组,每组30例,观察12个月.分别于治疗前(0个月)和治疗后(12个月)测量2组患者左心房内径(LAD)、左心室舒张末期内径(LVDd)、左心室后壁厚度(LVPWT)、室间隔厚度(IVST)、射血分数(LVEF)、二尖瓣血流舒张早期和晚期速度峰值比值(E/A);二尖瓣环运动峰值早期和晚期速度比值(e'/a')、二尖瓣舒张早期血流峰值与瓣环运动峰值速度比值(E/e'),计算左室重量指数(LVMI).同期检测白蛋白(Alb)、血红蛋白(Hb)、血清钙、磷、甲状旁腺激素(PTH)、25羟维生素D(25-(OH)D3)、成纤维细胞生长因子-23 (FGF-23)、白介素-6(IL-6)、同型半胱氨酸(Hcy)及血压水平.观察2种透析方式对心脏舒张功能及临床指标的影响并进行相关性分析.结果 60例无心力衰竭表现的MHD患者心脏射血分数均正常,舒张功能障碍发生率为80%.钙磷乘积(95% CI 1.002~1.165,P=-0.048)、LVMI (95%CI 0.952~0.988,P-0.02)和收缩压水平(95% CI0.981~1.037,P=0.046)是E/e'的影响因素.HFD组治疗后E/e'下降(9.99±3.36比11.84±4.88,P<0.05),FGF-23 (56.07±26.63pg/ml比85.53±40.54pg/ml,P<0.01)、钙磷乘积(4.48±1.16mmol^2/L^2比4.96±1.03mmol^2/L^2,P<0.05)、IL-6 (3.37±2.48pg/ml比5.59±2.53pg/ml,P<0.05)、Hcy水平(21.13±6.95 μ mol/L比29.40±11.66 μ mol/L,P<0.01)下降,25-(OH) D3水平(27.3±10.26ng/ml比23.15±10.73ng/ml,P<0.05)升高,差异均有统计学意义,HD组治疗前后各指标无统计学差异.相关性分析显示,E/e'与钙磷乘积(r=0.359,P<0.05)、Hcy(r=0.378,P<0.05)呈正相关,与FGF-23无相关性.结论 HFD能有效改善MHD患者的左室舒张功能,可能与有效降低钙磷乘积、降低Hcy水平有关;与FGF-23水平降低无相关性. 展开更多

目的 探讨多模式组合透析对维持性血液透析(MHD)患者血Klotho蛋白、成纤维细胞生长因子23(FGF-23)、脑钠肽(BNP)水平的影响.方法 采用前瞻性随机对照研究(RCT)方法,选择2015年12月至2016年12月在贵阳市第二人民医院肾内科血液... 目的 探讨多模式组合透析对维持性血液透析(MHD)患者血Klotho蛋白、成纤维细胞生长因子23(FGF-23)、脑钠肽(BNP)水平的影响.方法 采用前瞻性随机对照研究(RCT)方法,选择2015年12月至2016年12月在贵阳市第二人民医院肾内科血液净化中心接受MHD〉3个月的120例慢性肾衰竭(CRF)尿毒症患者,按随机数字表法分为血液透析(HD)组(每月8次HD)、HD+血液滤过(HF)组(每月8次HD+每月1次HF)、HD+HF+血液灌流(HP)组(每月8次HD+每月4次HF+每月1次HP),每组40例.所有患者入组前及入组后6个月、12个月在体外循环管路静脉端采血,采用酶联免疫吸附试验(ELISA)检测血清Klotho蛋白及FGF-23水平,采用电化学发光法检测血清BNP水平.结果 120例MHD患者最终均纳入分析,无中途退出、撤出病例.3组患者入组前血清Klotho蛋白、FGF-23、BNP水平差异无统计学意义(均P〉0.05).与入组前比较:3组患者入组后血清Klotho水平均呈持续升高趋势,且HD+HF+HP组〉HD+HF组〉HD组(μg/L:6个月为2.59±0.61、1.63±0.35、1.13±0.26,F=119.374,P=0.000;12个月为6.98±1.21、3.57±1.03、2.12±0.43,F=275.675,P=0.000);而FGF-23水平均呈持续下降趋势,且HD+HF+HP组〈HD+HF组〈HD组(ng/L:6个月为69.22±38.26、132.28±61.18、178.50±74.64,F=33.509,P=0.000;12个月为32.81±17.32、87.93±43.27、146.33±69.28,F=55.466,P=0.000);3组间血清BNP水平也表现出与FGF-23相同的变化趋势(ng/L:6个月为4083.39±2864.53、7245.69±4643.81、7969.12±5360.85,F=8.758,P=0.000;12个月为1521.86±894.63、4554.32±1969.84、5013.89±2033.64,F=49.003,P=0.000).结论 多模式组合透析能提高接受MHD的CRF尿毒症患者体内Klotho蛋白水平,降低FGF-23、BNP水平. 展开更多
