目的 分析2018年1月—2022年12月我院补益类中成药的应用现状及趋势,为此类药的合理使用提供参考。方法 统计2018年1月—2022年12月药品数据库中补益类中成药年销售金额和用量进行统计,分析补益类中成药的销售金额、用药频度(DDDs)、日... 目的 分析2018年1月—2022年12月我院补益类中成药的应用现状及趋势,为此类药的合理使用提供参考。方法 统计2018年1月—2022年12月药品数据库中补益类中成药年销售金额和用量进行统计,分析补益类中成药的销售金额、用药频度(DDDs)、日用药金额(DDC)、总金额排序号/DDDs排序号(DUI)值。结果 补益类中药口服剂型用药金额呈逐年上升趋势,注射剂构成比呈逐年下降趋势。口服剂型得到越来越广泛的应用,但是价格偏高。结论 我院补益药中成药品种结构基本合理,常用的补益药DDC值偏高,患者经济负担过重。补益药的不良反应发生率逐年增加,应注重合理用药。 展开更多

目的研究原发全面强直阵挛型癫痫(idiopathic generalized epilepsy with generalized tonic-colonic seizure,IGE-GTCS)合并睡眠障碍患者及未合并睡眠障碍IGE-GTCS患者的脑血流灌注特点。方法收集2021年12月至2022年12月就诊于宁夏医... 目的研究原发全面强直阵挛型癫痫(idiopathic generalized epilepsy with generalized tonic-colonic seizure,IGE-GTCS)合并睡眠障碍患者及未合并睡眠障碍IGE-GTCS患者的脑血流灌注特点。方法收集2021年12月至2022年12月就诊于宁夏医科大学总医院神经内科癫痫门诊并诊断为IGE-GTCS患者62例。根据有无睡眠障碍将患者分为IGE-GTCS合并睡眠障碍组和IGE-GTCS未合并睡眠障碍组。另收集年龄、性别、受教育程度与病例组相匹配、接受颅脑CT或磁共振检查的体检者及其他无神经系统疾病患者16名纳入对照组。采用手动勾画方法检测3组受试者脑部不同部位感兴趣区(region of interest,ROI)的脑血流量(cerebral blood flow,CBF)差异。结果IGE-GTCS合并睡眠障碍组、未合并睡眠障碍组及对照组左侧额叶、左侧丘脑及左侧海马结构CBF比较差异有统计学意义(均P<0.05)。两两比较结果显示,与对照组比较,IGE-GTCS合并睡眠障碍组左侧额叶、左侧丘脑及左侧海马ROI CBF降低(均P<0.05),未合并睡眠障碍组左侧额叶、左侧丘脑ROI CBF降低(均P<0.05);与IGE-GTCS未合并睡眠障碍组比较,合并睡眠障碍组左侧海马结构ROI CBF减少(P<0.05)。结论IGE-GTCS合并睡眠障碍患者及未合并睡眠障碍患者CBF异常存在差异,这将有助于进一步从脑功能角度理解IGE-GTCS合并睡眠障碍的病理生理机制。 展开更多

托吡酯联合拉莫三嗪治疗癫痫全身强直-阵挛性发作患者的效果

目的:观察托吡酯联合拉莫三嗪治疗癫痫全身强直-阵挛性发作患者的效果。方法:选取2021年4月至2023年1月商丘市第三人民医院收治的80例癫痫全身强直-阵挛性发作患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各40例。对照... 目的:观察托吡酯联合拉莫三嗪治疗癫痫全身强直-阵挛性发作患者的效果。方法:选取2021年4月至2023年1月商丘市第三人民医院收治的80例癫痫全身强直-阵挛性发作患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各40例。对照组予以拉莫三嗪治疗,观察组在对照组基础上联合托吡酯治疗,两组均连续治疗3个月。比较两组临床疗效,治疗前后认知功能[蒙特利尔认知评估量表(MoCA)]评分、神经损伤指标[神经元特异性烯醇化酶(NSE)、中枢神经特异性蛋白(S100β)、胶质纤维酸性蛋白(GFAP)、脑源性神经营养因子(BDNF)]水平,以及不良反应发生率。结果:观察组治疗总有效率为92.50%(37/40),高于对照组的75.00%(30/40),差异有统计学意义(P<0.05);治疗后,观察组MoCA评分高于对照组,差异有统计学意义(P<0.05);治疗后,观察组NSE、S100β、GFAP水平均低于对照组,BDNF水平高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:托吡酯联合拉莫三嗪治疗癫痫全身强直-阵挛性发作患者可提高治疗总有效率和认知功能评分,改善神经损伤指标水平,效果优于单纯拉莫三嗪治疗。 展开更多

目的:基于网络药理学探讨补体抗痨散对肺结核的作用机制。方法:通过中药系统药理数据库和分析平台(TCMSP)收集补体抗痨散的主要化学成分,根据药物分布、吸收、代谢、排泄(ADME)筛选出口服利用度筛选药物活性成分,并获取对应靶点;通过Dru... 目的:基于网络药理学探讨补体抗痨散对肺结核的作用机制。方法:通过中药系统药理数据库和分析平台(TCMSP)收集补体抗痨散的主要化学成分,根据药物分布、吸收、代谢、排泄(ADME)筛选出口服利用度筛选药物活性成分,并获取对应靶点;通过Drugbank、Genecards、OMIM数据库获得肺结核(pulmonary tuberculosis PTB)的重要靶点并去除重复值,利用String数据库进行蛋白质相互作用分析,构建PPI网络;使用Cytoscape3.7.1构建“补体抗痨散-肺结核靶点-通路”网络。并利用Cytoscape3.7.1软件筛选出排名靠前的核心靶点;使用微生信数据库对PPI中的靶点进行基因本体(GO)功能及京都基因和基因组(KEGG)信号通路富集分析。结果:筛选出补体抗痨散232个药物靶点,肺结核1452个非重复疾病靶点,两者取交集,共获得110个共同交集靶点。Cytoscape3.7.1筛选出排名前五的核心靶点:槲皮素、山奈酚、木犀草素、异鼠李素、常春藤皂苷元,PPI分析得到的RELA、TP53、TNF、AKT1、IL-6为核心的基因靶点。结论:补体抗痨散通过多靶点、多途径发挥其治疗肺结核的作用。 展开更多

When the training data are insufficient, especially when only a small sample size of data is available, domain knowledge will be taken into the process of learning parameters to improve the performance of the Bayesian networks. In this paper, a new monotonic constraint model is proposed to represent a type of common domain knowledge. And then, the monotonic constraint estimation algorithm is proposed to learn the parameters with the monotonic constraint model. In order to demonstrate the superiority of the proposed algorithm, series of experiments are carried out. The experiment results show that the proposed algorithm is able to obtain more accurate parameters compared to some existing algorithms while the complexity is not the highest. 展开更多

AIM: To study the contribution of tonicity response element binding protein(Ton EBP) in retinal ganglion cell(RGC) death of diabetic retinopathy(DR).METHODS: Diabetes was induced in C57BL/6 mice by five consecutive intraperitoneal injections of 55 mg/kg streptozotocin(STZ). Control mice received vehicle(phosphate-buffered saline). All mice were killed 2mo after injections, and the extent of cell death and the protein expression levels of Ton EBP and aldose reductase(AR) were examined.RESULTS: The Ton EBP and AR protein levels and the death of RGC were significantly increased in the retinas of diabetic mice compared with controls 2mo after the induction of diabetes. Terminal deoxynucleotidyl transferase(Td T)-mediated d UTP nick end labeling(TUNEL)-positive signals co-localized with Ton EBP immunoreactive RGC. These changes were increased in the diabetic retinas compared with controls.CONCLUSION: The present data show that AR and Ton EBP are upregulated in the DR and Ton EBP may contribute to apoptosis of RGC in the DR. 展开更多

Considering the fact that liver is one of the most important organs in our body,it deserves special attention and protection.Among various recommended supplements,complementary and alternative medicines particularly herbal remedies have received much attention owing to their truly healing properties.This review profits from Iranian traditional medicine and presents advantageous herbal guide directions for liver protection.According to credible Iranian medical literature such as Al Qanun Fil Tibb.Al-Havi and Makhzan-al-Aadvia.a wide spectrum of plants have been found to be useful for cleansing and protecting the liver.Some herbs such as ghafes(Agrimonia eupatoria),kasni(Cichorium intybus),anar(Punica granatum),darchin(Cinnamomum zeylanicum),za'feran(Crocus sativus),gole-sorkh(Rosa damascena) and zereshk(Berberis vulgaris) appeared to get strong consideration and were well documented as outstanding liver tonics.We conducted a comprehensive review of available Iranian medical resources such as scientific information database and medical sciences databases which cover all in vitro and in vivo studies of medicinal plants as liver tonics and hepatoprotcctive candidates.Literature survey was accomplished using multiple databases including PubMed,ISI web of knowledge,and Google Scholar. 展开更多

With exception of group D... The effects of A L tonic capsule on DNA content in rat experimental hepatocarcinogenesis induced by diethylnitrosamine (DENA) were observed. The experimental rats were divided into 4 groups. With exception of group D in which the rats were only administered with DENA, the rats in the groups A, B, C were previously, simultaneously and subsequently fed with A L tonic capsule respectively while they were administered with DENA. The DNA content of all rat livers was measured using automatic image analysis technique 20 weeks after administration of DENA. The results showed that the highest and lowest DNA contents were respectively seen in the groups D and A. There was significant difference in DNA contents between the groups A or B or C and D, and also between the groups A and B or C (both P <0.01). 4 components (4C) and ≥5C cells were predominant in the group D, while 2C cells were the minority. The number of 2C cells in the groups A, B, C was significantly higher than that in the group D, and the number of ≥5C cells in the groups A, B, C was markedly lower than that in the group D ( P <0.01). Also, there was very significant difference in the number of 2C and ≥5C cells between group A and B or C ( P <0.01). It was concluded that A L tonic capsule could effectively inhibit the increase of DNA content of hepatocytes and improve the distribution of DNA content in rat hepatocarcinogenesis, especially in group A. 展开更多

Airway hyperresponsiveness (AHR) is a characteristic feature of asthma, and generally correlates with severity of asthma. Understanding the protection mechanism against excessive airway narrowing and how it breaks down is fundamental to solving the problem of asthma. In this paper we have proposed a stochastic modeling the airway smooth muscle bundle for reproducing AHR such as an increased sensitivity of the airways to an inhaled constrictor agonist, a steeper slope of the dose-response curve, and a greater maximal response to agonist. A large number N of contractile muscle cells was assumed to repeat themselves in between contraction and relaxation asynchronously. Dynamic equilibrium of statistic physics was applied to the system of ASM bundle. Thus, the relation of dose to response of a piece of ASM bundle was described by Φ=tanh(βH) , where β was Boltzman factor and H represented energy of contraction induced by constrictor agents. Each of adjacent pair contractile cells was assumed to have Ising-type of antimagnetic interactions of preference energy J (for the condition of contraction-relaxation) between them. A motion equation for a piece of ASM bundle was described by Φ=N(H-zJΦ , which explained existence of combined tonic and phasic contractions. Based on observations of Venegas et al. [4], airway responsiveness was assumed to be assessable by total volume of the ventilation defects (TVD) of 13NN PET-CT images. Interactions via propagation of Ca ion waves between ASM bundles would cause percolation probability by PΦ=(1+tanh(βH))2/4 along the tree, then the relation of dose βH to TVD was described by TVD=PΦ[1-(1-PΦ)3/PΦ3]-TVD0. TVD0 represented the protection mechanism against excessive airway narrowing, which was determined by the ratio of amplitudes between tonic and phasic contractions, thus the balance of amplitudes between tonic and phasic contractions of peripheral lobular smooth muscles would be the determinant of AHR. 展开更多
